Injured worker was working on hopper underneath the 6x16 screen. The job was welding patches. EE had EE's back leaning against the wall with one leg extended and the other one bent while welding. When the job was done, EE extended EE's legs, and EE's knee was numb. And started feeling like EE pulled something on EE's back.

The pin on the rental excavator was walking out the wishbone, operator was holding the pin in place while maintenance guy was trying to realign the bearings. The mechanic asked the operator to pull the pin out a little and they pulled it all the way and that caused fingers to stick out of the hole causing the very tip of the maintenance man's finger to get cut.

The employee was cleaning the under flow pipe while working in the mud tank. Allegedly developed rashes on hands 16 days after contact with the mud. Employee did not lose any time until 03/02/2015 when we received a letter from Arrowhead Orthopedics advising us about his restrictions.

Cleaning electric shaft with brake cleaner. Lighting the torch causing the fumes to flash. The flash caused minor burns to EE's face. (Looks like sunburn).

Standing over top of conveyor - bending over to remove troughing rollers - twisted back wrong. Reported as incident only on 8/15/12 (1st aid). Continued working until 8/22/12 - back hurting more.
The adjusting bar on the gate on the transfer box 15A caused a pinch point to the box - Pinched employees forth (ring) finger right hand.

Repairmen were changing out a crusher. The hand rail on the work platform had been removed so the the crane could lift out the crusher. EE stepped off the platform while the temporary rail was being prepared. He was hurt while trying to break his fall.
