Employee was helping another employee lift a plate of steel. The weight shifted and pulled the employee,s shoulder. He reported the incident to his Supervisor. It was handled as First Aid Only. He was notified on January 8 that he would be laid off on March 9. On January 9 he went to the doctor and was put on light duty.

Ee was using a flubber (air lance) to loosen compacted material in a cement silo. While lifting on the flubber he felt a pain in his shoulder. This accident was reported on 9/4. It was not until Friday, 9/18 that the employee requested to be seen by a doctor. Report filed on 9/24, three working days later.

Employee was bending over to pick up gloves off the ground and when he stood back up his right knee popped.

Employee was working in the Gravel Bed Filter System, cleaning screens. He suffered a chemical irritation where his shirt sleeve rubbed against his wrist. Cement clinker dust, has a high pH and can cause an alkali burn. He was seen by the company doctor. Rx ointment was administrated. No lost time or restricted work (other than keep wound clean).

Dust from inside the dust collector on top of the cement silo blew into the employees eyes. He was installing new bags at the time.

Employee was shoveling into a dumpster hopper and pushing the dumpster onto and off of an elevator car. Complained of back pain 24 hours after the incident.

Hearing loss - employee had a reportable standard threshold shift and was confirmed by retest.

Ee is alergic to bee stings. Ee self administered an epipen injection and was transported to Hosptital where he had a full recovery before the doctor could see him. No doctor report filled. There is some question as to whether or not this should be reported. We will report it as an "other" even though it was not medically treated.

Employee had complained about a lump in the palm of his right hand on 9/28 at 2:00. There was no problem at the time and he requested to see Dr. Joy who had seen him before. The doctor's next available date was October 6th. On Oct. 6 the doctor placed him on prescription drugs and refered him to a specialist. This is now a reportable accident as of October 6.

Employee had reported to work on September 6 stating that he had been seen by his personal physician and had been diagnosed with carpal tunnel syndrom. He had been working long hours, for serveral days, on an emergency overhaul and had aggrevated his wrist. He was sent to the company doctor who has placed him on light duty with PT and anti-inflamitory drugs.

Employee was checking the oil on the loader when he slipped. He was able to catch himself but wrenched his thumb. He reported the accident and was given first aid. On 10/2 he was seen by a doctor. The doctor released him back to his usual and customary. On 10/26 it was discovered that he had been given Rx drugs. This is reportable.

Employee was standing on concrete slab covered with broken rock and dirt. Turned and fell to the ground. Wrenched shoulder (dislocated) and bruised back. Was placed on light duty for about 4 weeks.

Employee was cutting bolts from a liner with an oxyacetylene torch when his pant leg caught fire. He sustained second degree burns to his shin in an area approximately 2 by 5 inches. He was wearing polyester coveralls which contributed to the severity of the burn.

Employee has been diagnosed with "trigger thumb? this condition could be the result of him using his right thumb to press the space bar. He performs clerical work. He received cortisone injections (medical treatment beyond first aid) but has not been restricted from doing his usual and customary work. This case is reporable because of the injections.

Employee was manually cycling tipping gate when cam drive pinched his finger against the actuating arm. He had removed guarding and had not stopped cam drive motor. He worked on tipping gates while gates were in operation.

When ee stepped off a dozer, he stood on an uneven piece of concrete, slipped and twisted his ankle. Injury reportable due to use of air splint & prescription pain relief.

Employee was removing a cover plate on the crusher when he slipped and fell landing on his left wrist.

EE was placing a cap on the kiln dust tank air slide when the material surged and blew dust into his right eye.

Employee was shoveling and sweeping coal into the unloading hopper when he stepped into some loose material. His foot slipped and he twisted his knee resulting in a torn meniscus.

employee was moving a lid on the 970 dust collector and stepped into the hole that the lid was covering. fell about 20 inches onto the tube sheet below. sufferred a strain to quadracept tendons.

Employee cut his hand with a sharp knife. He was attempting to score and break a plastic tray. The knife slipped and cut his hand. Sutures were required.

Employee strained his back while closing a gate under a finish mill tank. He reported the injury two days later, was treated on June 19th when he was released to return to light duty. Note: employee was terminated on June 21 for poor attendance.

Employee strained his groin on May 11, did not report to his supervisor until May 19, requested to see doctor on May 22, Ee was installing a traction wheel on a bucket elevator in the finish mill. He was carring a piece of plate when he stepped onto loose clinker. He says he felt a twinge, but did not report it at the time.

Ee was cleaning build up from the inside of a distribution manafold on the slurry line spray nozzle. She used compressed air to blow out the line and the plugged line blew material back into her face and into both eyes. This required medical care to remove the alkali material.

Employee slipped, grabbed hold of structure and twisted arm.

Lifting liner panels and back became sore

Employee was injecting compressed air into a discharge pipe beneath the drop out box when hot dust "spashed" into the opening around the gauntlet on his heat reflective gloves. This resulted in a second degree burn on his right hand. He was placed on light duty, to keep hand clean and dry.

Ee complained of sore arm about a week after the event (4/10/06). On 4/12 he went to see his own physician who told him that he had a strain and that it was work related. On 4/13 he was sent to our dr. who diagnosed a strain. PT and light duty prescribed.

Employee was placing a belt winder on B Belt. B-Belt is on an incline. The winder slid down hill pinching the Ee's hand between the winder and the open end of a 2" pipe handrailing. The railing had been removed to allow the winder to be placed into position. The resulting injury (an avulsion) required medical care (sutures)

Ee slipped on diesel spill while attempting to shut off hose nozzle. Placed on light duty. There is question as to whether or not the restriction (no working overhead) applies to his job. Will report to cover standards.

Ee worked a double shift starting at 3Pm on 3/7 thru 6AM 3/8. By the end of the shift he was stooped over and walking strange. He reported it to the shift forman and told him he was "stiff from the cold night air". He went home and could not rest. Called the plant about noon and requested to be seen by a doctor. No specific incident remarks. Ee placed on light duty

EE OPERATING SWEEPER MACHINE. LOOKING DOWN AT BRUSHES ALONG SIDE VEHICLE, RAN INTO A SUPPORT BEAM OVERHEAD. /EE SEEN AT BY DR. RELEASED TO WORK, TOLD SUPV. NO RESTRICTIONS, PAPERWORK SENT TO WRONG ADDRESS. / LATER EE COMPLAINED OF PAIN, RET.TO DR., CONTINUED ON LIGHT DUTY. REPORTED TO MSHA 2 WORKING DAYS LATER. / WHY REPORTABLE - LIGHT DUTY

EE was shoveling coal in the coal reclaim tunnel and felt pain in right shoulder.

was working on pre heater tower unplugging a fourth stage plug. was wearing a hood. pH dust blew under the hood and into his eye. The employee reported the injury to his supervisor the day it happened. The employee tolerated the inury for 17 days before he went to the doctor. Diagnosed as infalmation of eye.

Employee has been transfered from a Yard Foreman job to a desk job. He was used to walking around but the new job required him to sit at a PC and enter data. His back became sore after long periods of sitting.

At end of shift Ee complained to supervisor that his shoulder hurt. Said it had been hurting for weeks. Commented that when held water blaster lance over his shoulder that it became tender. Requested to be seen by a doctor to determine what was wrong. Restriction may or may not effect his performance - reaching or working above shoulders. Question reportability,

Employee was standing at the counter in the storeroom when he began to feel light headed. He says "I stepped outside to get some fresh air". Once outside he turned to close the door and fainted. NOTE: THIS IS NOT A WORK RELATED INJURY OR ILLNESS .

Employee caught his hand in a troughing roller on the 04-031 Cross Belt Analyzer feed belt. He was inpsecting the belt for alignment when he leaned out over the belt his right glove was caught between the roller and the conveyor belt. He pulled back on his hand and separated the thumb nail from the nail bed.

Employee bumped knee against a protruding angle iron bracket next to the roller mill. This resulted in a contusion to her knee. She had been off work for non-industrial knee surgery within the past year. Though the injury was minor, it was determined that a physician should evaluate her condition.

Breathing difficulties. EE provided first notification of this incident 7/26/05.

Employee was cutting with a torch and hot slag popped into his eye around loosely fitted welding goggles.

Employee has been tested and found to have a significant shift in his hearing. MSHA mandates that such hearing loss be reported.

Ee was using hand grinder cutting thin plate metal. strip of cut metal flipped off the edge of the grinder and cut his hand. Sutures required.

Ee operating a dozer inside the Clinker Shed. He was leaning on the right arm rest when he was jostled. His right arm slipped off the rest and banged against the metal plate under the arm rest resulting in a contusion. The Ee coming off swing shift, not required to return to work until following Wednesday, no light duty, Rx for pain and swelling.

EE WAS PUSHING A DUMPSTER ACROSS THE BURNER FLOOR & CAUGHT HAND BETWEEN DUMPSTER & I-BEAM.

Ee walking up stairs, tripped on top step and fell onto his hip. He was carrying a flashlight in his back pocket.

Ee developed callous on right hand from using hand tools. Callous is impinging on movement of right little finger.

Employee complained of having a sore back to his supervisor about 9:00. He attended safety/process training from 7:30 to 8:00, drove a sweeper from 8:00-9:00. His back was too sore to drive the water truck at 9:00. Says his back had a "twinge" when he bent over to pick up wheel chock.

CARPAL TUNNEL SYNDROME- EE FIRST COMPLAINED ABOUT PAIN IN 03/30/04, IT WAS REPORTED TO WORK. COMP ON MAY 17TH HE CALLED IN & DECLARED THAT HE WAS SCHEDULED FOR SURGERY & WOULD NOT RETURN TO WORK ONCE RELEASED HE WOULD RETIRE.

EMPLOYEE SLIPPED ON GRATING THAT HAD HYDRAULIC OIL SPILLED ON IT. EE REPORTED INCIDENT ON 5/28. ON 5/30 HIS DAY OFF, HE WENT TO THE DR. (FIRST KNOWLEDGE).

EE WAS OPENING A DOOR ON AN EXPANSION CHAMBER (1ST STAGE- 7TH HOUR). USING A PRY BAR, HE WAS PULLING AGAINST A STUCK HINGE. THE BAR SLIPPED, HE STEPPED BACKWARD & TWISTED HIS FOOT, FRACTURING A BONE IN HIS LEG.

EMPLOYEE WAS WORKING IN LIME MILL. AFTER A FULL SHIFT HE NOTICED ALKALI BURNS ON HIS HAND AND WRISTS.

EE LOOKED UP WHILE OPERATING A ROLL UP DOOR AND DIRT FELL INTO HIS RIGHT EYE.

EE WAS LOOSENING THE NUT ON A DRAW CYLINDER FOR THE GRAVEL BED FILTER SYSTEM. HE WAS TURNING A NUT WITH A WRENCH. THE NUT CAME TO THE END OF THE THREAD AND THE SHAFT FELL PULLING THE WRENCH DO WN, CRUSHING HIS FINGER.

EE WAS DUMPING A FORWARD DUMP CONTAINER. HE WAS ATTEMPTING TO SECURE A SPRING LOADED LATCH (HANDLE) WHEN IT SLIPPED FROM HIS HAND AND THE HANDLE HIT HIM IN THE MOUTH.

EE CUT THE BACK OF HIS HAND WITH A PUTTY KNIFE WHILE CLEANING THE WAX OFF OF A CEMENT MOLD IN THE LAB AREA.

EE WAS PLACING HIS SAFETY GLASS ONTO HIS FACE AFTER HAVING CLEANED THEM. HE POKED HIMSELF IN THE EYE WITH A TEMPLE ARM ON THE GLASSES.

EMPLOYEE STEPPED BACK OFF ELEVATED WORK AREA ONTO AN UNEVEN SURFACE AND TWISTED HIS FOOT.

EE CAUGHT LEFT LITTLE FINGER ON OCURLIED POST AT BOTTOM OF STAIRS. HE BENT HIS FINGER BACK CAUSING A MINER FRACTURE ABOVE CHIP. THIS ACCIDENT WAS REPORTED TO THE CO ON 8-4-03.

EE WAS ADJUSTING THE GUARD ON THE TIP OF THE VACTOR HOSE WHILE EQUIPMENT WAS OPERATING. HIS ARM WAS SUCKED INTO THE HOSE.

EMPLOYEE WAS ADJUSTING THE WIDTH OF THE FORKS ON A FORKLIFT TRUCK AND PINCHED HIS FINGER BETWEEN THE BACK REST BRACKET AND THE FORK.

EE CAUGHT HAND BETWEEN PALLET OF STEEL AND HAND RAILING. FORK LIFT OPERATOR WAS POSITIONING PALLET. SUPERVISOR ( INJURED WORKER) WAS DIRECTING HIM. INJURED WORKER PLACED HIS HAND ON THE RAILI NG WHEN THE STEEL STRUCK HIS HAND.

EMPLOYEE WAS HIT IN THE EYE BY A STREAM OF CEMENT DUST AS HE WAS ATTEMPTING TO CLEAR A PLUGGED AIR SLIDE.

EMPLOYEE WAS STANDING AT CUT OFF SAW, TURNED TO RIGHT AND FELT PAIN IN HIS LEFT KNEE. REPORTED TWO DAYS LATER. NO TREATMENT UNTIL MAY 23, 03.

EE COMPLAINED OF SORE BACK AFTER FINISHING SHIFT ON A HAUL TRUCK OPERATOR.

EMPLOYEE WAS INSTALLING A FAN IN THE ROLLER MILLAIR DUCT, LIFTED IRON ROD OUT OF DUST AND STRUCK HIS ARM, COMPLAINED OF SORENESS 1 MONTH LATER.

COATING FROM THE KILN FELL FROM THE BACK OF THE KILN STRIKING THE EE ON TEH HEAD, BACK AND ARM. THE MATERIAL BROKE LOOSE AFTER THE KILN COOLED ABOUT 30 HOURS.

EE STRAINED HIS BACK WHILE USING A HEAVY HAMMER OVERHEAD IN THE FINISH MILL.

DUST FROM WASTE DUST TANK SPRAYED INTO BOTH EYES WHEN CAP ON CLEAN OUT FELL OFF WHEN DRIVER APPLIED COMPRESSED AIR.

EMPLOYEE WAS PULLING A 4" RUBBER VACTOR HOSE OUT OF AN ELEVATOR PIT THROUGH A HOLE IN THE FLOOR. HE COMPLAINED OF PAIN IN HIS LEFT KNEE.

PINCHED FINGER BETWEEN A PIN & GATE.

EMPLOYEE REPORTED THAT THE CHAIR HE WAS SEATED IN COLLAPSED. I QUESTION THE VALIDITY OF THIS CLAIM. A NUMBER OF EMPLOYEES WITNESSED THE INCIDENT.

COMPLAINED OF SORE BACK AFTER BEING TREATED BY CHIROPRACTOR FOR MORE THAN A MONTH. NO LIGHT DUTY - SAME JOB.

EE WALKED INTO A LOW OVERHEAD BEAM. THIS SPRAINED HIS NECK.

EE WAS MOVING LARGE 6" DIAMETER VACTOR HOSE & FELT PAIN IN HIS SHOULDER.

CHANGING BUCKETS ON BUCKET ELEVATOR. CLIMBING IN AND OUT/UP AND DOWN. TOOK LARGE STEP AND FELT PULL IN GROIN.

EE WAS WALKING FROM PAINT SHOP TO LOCKER ROOM & RIGHT KNEE GAVE OUT.

EE WAS MOVING A SPEED REDUCER WHEN SHE PULLED IT (ROCKED IT) ONTO HER FOOT.

EE CUT THUMB W/ KNIFE WHILE CUTTING RUBBER BELT SHEET.

CUTTING THE VACTOR HOSE WITH A SHARP KNIFE.

EE WAS MOVING A HEAVY RUBBER VACTOR HOSE.

EE TWISTED KNEE WHILE SAND BLASTING. WHILE BLASTING ON UNEVEN SURFACE HE STEPPED ON A ROCK AND TURNED HIS KNEE.

EE WAS PULLING ON 6" VACTOR HOSE (HEAVY RUBBER HOSE) TO COUPLE IT WITH PIPE CONNECTION. HE FELT A "POP" IN HIS BACK.

EMPLOYEE WAS SHOVELING CLINKER AND CHUNKED MATERIAL FROM UNDER THE CRUSHER RECLAIM BELT. HE TWISTED AND STRAINED HIS BACK.

EE WAS CLOSING THE SLIDE DOORS ON THE BOTTOM OF A RAIL CAR. WHEN HIS CLIMBED OUT AND STARTED TO STAND UP HE FELT HIS BACK "GO OUT".

EE WAS USING A KNIFE TO SCRAPE MATERIAL OFF OF ABOLT. THE KNIFE FOLDED UP ON HIS FINGER CAUSING LACERATION.

EMPLOYEE EXPOSED TO POISON OAK, DEVELOPED RASH. SEEN BY DOCTOR AND PRESCRIBED PREDNISOVE.

EMPLOYEE LIFTED PLASTIC GARBAGE BAG FROM WASTE BASKET. BAG HAD DIRT IN BOTTOM. HE FELT A STRAIN AND REPORTED THE INCIDENT

DUST BLEW INTO RIGHT EYE. DID NOT REPORT UNTIL 5/14/01.

ORIGINAL INJURY OCCURRED 12/21/00. NOT REPORTED TO COMPANY UNTIL 5/7/01. PULLED GROIN MUSCLE WHILE OPENING DOOR ON DUST COLLECTOR.

EMPLOYEE WAS USING VACTOR MACHINE, SUCKING DUST FROM UNDER HOPPER DUST COLLECTOR. RECEIVE ELECTRIC SHOCK FROM STATIC ELECTRIC BUILD UP ON HOSE.

CEMENT DUST BLEW INTO EMPLOYEE'S EYES WHILE CLEANING VALVE ASSEMBLY.

EE WAS BURNED WHEN HOT DUST FROM RISER BLEW ONTO HIS BACK (SEE ATTACHED).

EE SLIPPED ON GRINDING BALLS, LOST FOOTING AND TWISTED HIS KNEE. ALTR @ 3 WEEKS RETURNED TO DR AND WAS DERGENRE WITH TORN CARTILAGE.

EE BANGED HIT ELBOW ON SAFETY CAGE ON LADDER. FIRST SEEN BY DR ON 2-26-01.

EE WAS CARRING A BOX OF BOLTS, TURNED TO GET AROUND A STAIR CAME. TWISTED BACK.

EE WAS INSTALLING A COVER OVER A FEEDER OPENING WHEN A SMAL ROCK (3" X 3" X 2") FELL APPROX. 40 FEET AND STRUCK HIS LEFT HAND. ROCK MAY HAVE FALLEN FROM A X-MEMBER IN THE ROCK STORAGE SHED.

EMPLOYEE WRENCHED BACK WHILE MOVING A MOTOR. HE FAILED TO USE CAUTION. HE DID NOT USE THE BACK SUPPORT PROVIDED. INCIDENT OCCURED IN EARLY MORNING AFTER CALL OUT. HE DID NOT USE TOOLS & EQUIP MENT PROVIDED. HE HAD PERFORMED THIS TASK BEFORE

EE WAS ASSISTING OTHER WORKERS TO REMOVE SCREW FROM PUMP. THE SCREW FELL AND CRUSHED HIS FINGER BETWEEN THE SCREW SHAFT AND FORK ON THE LIFT TRUCK. NO DAMAGE OR IMPAIRMENT TO THE MINING EQUI PMENT. THE PUP FAILED. DUST ACCUMLATED CAUSING ACCESS DIFFICULT. SCREW HAD TO BE MANHANDLED. THIS ACCIDENT DID NTO VILATE RULES OR COMPLAINCE ISSUES.

WHILE TRYING TO CLOSE OFF THE FLOW OF CEMENT POWDER FROM A "FLIBBER NOZZEL" DUST GOT INTO EYES.

EMPLOYEE WAS CLEANING A SIGHT GLASS AND SEDEMENTCONTAINER. GLASS BROKE AND CUT HAND.

EE MIXES CEMENT WITH KEROSINE IN DEVICE CALLED A WAGNER. A CYLINDER OF MIXTURE IS PLACED INTO A CLAMP MOUNTED ON THE WALL.

EE WAS EXPOSED TO WET CLINKER (CEMENT DUST) WHILE WORKING A JACK HAMMER. DEVELOPED A RASH. FIRST SEEN BY PHYSICIAN 3-28-00

EMPLOYEE WAS CLEANING ROTARY FEEDER (HAD BEEN LOCKED OUT) IMPELLER WAS BEING ROTATED BY EMPLOYEE WHEN HIS THUMB WAS CAUGHT BETWEEN IMPELLER AND HOUSING.

EMPLOYEE WAS TYING HIS SHOES, FELT PAIN IN HIS LOWER BACK WHEN HE PULLED ON THE LACES.

EE IS CLAIMING NORMAL WORK DUTIES AGGRAVATED AN OLD INJURY OF 4/98. HIS FEET ARE GOING NUMB & HIS BACKIS SORE

ASSISTING CONTRACT ELECTRICIANS REMOVE A SUPPORT INSULATOR. WHILE PUSHING THE SUPPORT INSULATOR TO BREAK SEAL ON LOWER GASKET, MY HAND GOT CAUGHT BETWEEN THE INSULATOR AND THREADS ON THE HIGH VOLTAGE HANGER ROD. WHEN SEAL SUDDENLY LOOSENED,THE THREADS CUT THE NAIL PULLING OUT THE ROOT BED.

EE WAS REMOVING SCOFFOLDING MATERIAL FROM HIGHER. WHEN HE PULLED A LADDER OUT OF THE RUN HE HOOKED HIS THUMB NAIL.

STOOD UP AND FELT PAIN IN BACK, DROVE TO PLANT AND PAIN PERSISTED.

EE WAS OPERATING A 988 FRONT END LOADER HE WAS IN AN AREA WHERE HE SPENT A LOT OF TIME LOOKING OVER HIS SHOULDER.
