Miner was changing segments on loader (Cat #4-11 988F) bucket when EE placed EE's palm on the carriage bolt head and applied the impact gun to the nut to remove. The bolt head spun and cut through the miner's glove and lacerated EE's hand.

The plant operator was working with EE replacing a bearing at the head pulley of the sand stacker when EE cut EE's index finger on EE's right hand.
