Employee was fueling his equipment from a tank at the mining site. Employee was walking down inside of fuel containment berm when he slipped on the exposed plastic that had frost on it from the cold weather. Employee fell onto back compressing/fracturing his vertebrate.

OTHER DID N OT KNOW THIS AND GAVE THE SIGNED TO ANOTHER TO BUMP THE BELT ON AND OFF. THE BELT TURNED ON AT THE TIME EE GRABED THE BELT.
