EE was installing rock bolts in rib and debris got into his eye. He worked through his entire shift. When waking up the next morning, he immediately noticed that the debris did not wash out, went to his eye doctor and had it removed, and reported the incident at the corporate office.

EE was drilling with jackleg drill when the steel shanked and caused his right hand to hit the D-Handle of the machine.
Employee was installing rock bolts in the ribs of the 2000 level past the Star switch when he got something in his eye. He continued to work through the end of shift. At shift end, his eye was still bothering him as the debris did not wash out. He reported the debris at the company's office on June 5, 2012 at 11:45 a.m.

Employee was tasked to weld. Employee misused a welding helmet. The helmet was for use with a hard had (required clips). The employee did not use it with a had had and instead held the helmet in place with one hand, thus making the equipment ineffective and causing flash burns.
