Fluid bed sand dryer was plugged with wet sand. Operator was checking the progress of unplugging efforts by opening an access door to the discharge chute. Operator did not stand aside when opening the door and was struck by hot sand. Injuries sustained were first and second degree burns to face, head, neck and upper arms.

Employee stopped to close and lock the gate at our highway entrance after EE's shift. While exiting the pickup, EE's weight was on the right foot which was on the step on the side of the pickup. Before EE's left foot contacted the ground EE felt a sharp pain in the right calf. EE didn't slip or fall. EE was able to secure the gate and go home.

Employee stepped into parking lot near the end of a sidewalk and slipped on a frosty permanent rubber wheel chock. Employee fell on left shoulder. Employee did not seek medical attention until 2/25/2020.

Employee was 7 hrs into 10 hr shift, loading the bulk truck for evening. Employee descended a set of stairs 7 steps, making their way over to the tunnel/conveyor to collect sample. Employee stepped down on the last step with left foot, continuing onto the concrete block step when they felt a stabbing/popping pain in the left calf muscle.
While rebuilding a valve a board the valve was resting on moved causing the valve to fall. One operator tried to catch it and the valve pinched EE's finger between it and the rim of a sump resulting in a cut to the middle finger of EE's right hand. EE was treated with seven stitches and released back to work with no restrictions. EE returned to work and finished EE's shift

Felt like I was having a heart attack and Unimin call the EMT
While using an angle grinder the grinder kicked back hard enough to come loose from the operators left hand and the cut off wheel nicked left thumb as it passed. This resulted in a minor cut between the first and second knuckles of the left thumb which was repaired with 3 stitches. The operator returned to work and finished shift with no restrictions from the doctor.

Employee was cutting steel with angle grinder. Steel was not supported properly and pinched cutting wheel. Cutting wheel disintegrated with debris reaching employee's eye despite dual eye PPE. Debris was removed by doctor and employee was returned to regular work

Back sprain caused by repetitive use of an 8 pound sledge hammer.
Cause is repetitive handling of bagged material from a belt conveyor to pallets. On 5/7, he complained of a sore right knee, saw a doctor and was diagnosed with no significant issues; and returned to work. After complaining of discomfort, a MRI was performed, the doctor diagnosed a torn meniscus on 5/18. He was placed on modified duty.

Operator struck thumb of left hand with hammer in right hand resulting in cut on thumb. Cut was repaired with two stitches and operator returned to work and finished his shift.

The operator was tightening nuts and bolts on the product chutes using a crescent wrench and a ratchet wrench with a deep socket. The ratchet was on the bolt head when it came loose, striking him in the teeth.

STANDING ON GRATED FLOORING ABOVE A PUMP, EE WAS USING A BREAKER BAR TO OPEN A FEED GATE. WHEN THE TENSION GAVE WAY, HE FELL BACK AND INJURED HIS SHOULDER UPON HITTING THE FLOOR.
