Examiner crossed belt, stepped down and hit his lower leg on rail for belt structure. This caused a laceration on his lower right leg, requiring stitches.

Man was changing clothes, fell against basket hanger in washhouse, lacerated his left ear. Cut was repaired 1" long, rec'd stitches.

Man was taking bolt off of Roof Bolter to install. Bolt hung up. He pulled, pin broke loose pinching his right hand against rib. Mashed right little finger. Stitches. Accident was under investigation.

Man was walking main north belt linem slipped in mud, fell striking right knee cap on mine floor. Off for surgery 8-3-07.

Checking feeder under raw coal silo, wheel used to open gate on feeder fell off shaft and hit employee on left leg. Stitches received on left lower leg.

Man was putting up 10' pins, rotated hand and wrist around pin and hurt ring finger and little finger of left hand. Laceration and dislocation of ring finger.

Man was moving rock from the top of the miner. Rock pinched his right ring finger against the frame, causing a laceration to end of finger, received 4 stitches.

Slate broke loose and fell striking brent in the left facial area, He recieved stitches in his left ear.

Man was riding in mantrip; mantrip rubbed against rib and caught his lamp pouch, pulled man around straining his right shoulder. He started missing 3-30-07.

EE was cutting channel iron on the clean coal conveyor when a large amount of coal/water fell onto his head and into his eyes. Employee was working in rainy conditions.

Man was cutting band on pallet of header boards. Cut into his left upper leg, cut approx 1" long, received stitches at Memorial Hospital.

Man roofbolting, rock fell and hit tray, bounced and landed on his right foot. Contusion on foot, later took MRI, found broken bone. Started missing on 8/17/06.

EMPLOYEE WAS RECOVERING HIGH VOLTAGE CABLE, HE PULLED ON THE CABLE & IT TIGHTENED UP, SWUNG OVER & HIT HIS RIGHT ANKLE. ANKLE SWOLLEN WITH CONTUSION.

CHANGING #4 HOSE, THE FITTING WAS BROKEN. WHEN EMPLOYEE WENT TO PULL HOSE OUT THE BROKEN FITTING CUT HIS LEFT HAND, RECEIVED STITCHES.

EE SLIPPED ON SLATE AND COAL WHILE FILLING WATER TANK ON THE MAINTENANCE VEHICLE FOR FILLING BATTERIES. EE FELL DOWN UNDER THE VEHICLE AND HURT HIS RIGHT HIP AND RIGHT SHOULDER - LOWER BACK PAIN. INITIAL X-RAYS NEG. 1ST DAY MISSED WAS 1/6/06 (MINE WAS SHUT DOWN 1 WEEK AT CHRISTMAS). X-RAY & MRI BY ORTHO. SPECIALIST. DIAG. VERTEBRA PROBLEM, SURGICAL REPAIR REQUIRED.

Was roof bolting #5 entry 5th East, tripped over rock and fell on left knee, pain in left knee also strain to groin. Started missing 1-30-06 for procedure on knee.

Unintentional roof fall in the 5th east panel occurred at crosscut located at 1380 ft survey station between entries 3 and 4. The fallen material measured 20 x 13 x 6 feet. The support methaod used was 6 ft tension rebar roof bolts.

Unintentional roof fall in the 5th east panel. Fall occurred in crosscut by survey station 1620. It was between entries 6 and 7. The loose material measured 20 x 18 x 6 feet. The method of support was 6' tension rebar roof bolts.

Unintentional roof fall in 5th east panel occurred in crosscut 23 between the #8 and #9 entries. The area had been supported with 6' tension rebar bolts. The fallen material measured 30 x 18 x 6 feet.

Unintentional roof fall occurred in the 2nd main east #3 entry in intersection nine crosscut 85. The rock consisted of slate and layered rock measuring 30' x 18' x 9'. The date of development is 7/10/93. 6' and 8' TR bolts were the support method in the fall area.

Changing top roller on clean coal collecting belt, man had roller dropped on his right hand. Injured right ring finger, fracture.

Working on roof bolting loading bolts, mashed his right little finger. Received laceration, went to hospital, received 3 stitches.

At the 3EP filter bank man slipped in mud and twisted left knee. Started missing 10-11-05.

Employee was driving down 2nd main east travelway xcut 170. A piece of clod fell down between pins and struck his left shoulder. Started missing on 10/01/05.

Unintentional roof fall took place in the third east panel between #2 and #3 entries at the 3630' mark. 6' tension bolts were the support in the fall area. The fall dimensions are 15' x 12' x 6' 1/2"

Employee was pushing cable through shieve. Cable was twisted and his left shoulder was twisted. He started missing on 9/29/05.

Fall occurred in the 30th crosscut in the #8 entry of the third east panel. Supported by 8' and 6' tension rebar pins. The fall blocked the travelway. Trafic was diverted and the area propped. Fall dimensions are 70' x 18' x 8'.

Man bolting in the 1 EP unit. Slate slid down the bolter area hitting him on the forehead causing a laceration about 1" long. No lost time, he reveived stitches.

Employee was bolting to reinforce intersection 3rd East Panel, #4 entry 2970'. Using spiral auger pulling it out of the hole, he hit rotation, broke his left arm just above his wrist & dislocated his finger.

Worker was on the bottom lifting a 5 gallon can of oil. He felt pain in his left arm & shoulder. Torn rotator cuff, started missing on 5/14/05.

UNINTENTIONAL ROOF FALL, #2 ENTRY 3 EP 2850' INBY THE LOADING POINT MMU 050-0 25'L X 35'W X 8', 4 WAY INTERSECTION. PARTIALLY CLEANED UP TO ALLOW GAS CHECKS FOR X-CUT TO GO THROUGH THEN COMPLETELY PROPED OFF.

UNINTENTIONAL ROOF FALL 3EP 1830' #8 TO #9. 20' LONG X 18' WIDE X 6' HIGH. DATE OF DEVELOPMENT 4/4/05. 4-WAY INTERSECTION ADVANCING. BOLTED WITH 6' TR BOLTS. AREA PROPPED OFF.

After cleaning the electrical boxen for the man and material hoist and while putting power back on. A transformer tunnel a wire and malfauctoned slops hoist and to transport non undergorund til 4-27-05 at 8 am.

UNINTENTIONAL ROOF FALL IN BY LOADING POINT 3RD EAST PANEL 2378 #6 ENTRY MMU 010-0. 21' X 21' X 8' BOLTED WITH 8 ' BOLTS 4 WAY INTERSECTION PROPPED AROUND

EMPLOYEE WAS WALKING TOWARDS LAST OPEN XCUT & WAS BETWEEN 4-5 ENTRY IN XCUT WHEN A ROCK FELL & HIT HIS RT. HAND & LT. FOOT. EE WENT TO HOSPITAL & HAS A FRACTURE TO LEFT FOOT.

AN EMPLOYEE WAS OPERATING END LOADER IN AREA OF HOIST ROOM AND CAUGHT OVERHANGING MESSENGER WIRE WITH BUCKET AND PULLED DOWN. SEVERAL WIRES LOOSE IN YARD AND HOIST ROOM. AT 5:30 PM POWER WAS TURNED OFF AND WIRES OUTSIDE RE-HUNG. WHILE OFF, USED SLOPE HOIST. POWER BACK ON 11:30 PM. NOTIFIED MSHA & STATE OF IL BY PHONE 4/5/05.

Taking screw jacks out for cut when jack fell and hit him on the right forearm and hand. The result was a contusion. Started missing 9/27/05.

EE was rock dusting on top of overcast, coming down stepped in hole hitting shin on water fittings.

REPLACED NW DOG MOUNTING BRACKET COMPLETE SHINED TOP WEST GUIDES 1/8", OILED GUIDES ON CAGE AND COUNTER, WEIGHTS USED SLOPE HOIST TILL NOON 2/5/05.

EE WAS SCALING DOWN LOOSE ROCK PIECE. ROCK HIT LEFT THUMB PINCHING IT AGAINST PRY BAR.

HOISTING ENGINEER WAS LOWERING ON MANUAL. DOG ON SOUTH WEST CORNER OF CAGE CAUGHT, WHEN RAISED IT DID NOT RELEASE PROPERLY. BROKE THE DOG & LINKAGE. REPLACED 2 DOGS AND LINKAGE.

ROOF FALL 1ST EAST XCUT #14, #5 ENTRY, 4-WAY INTERSECTION, 20'LONG X 20'WIDE X 8.5'. BOLTED WITH 8'TR. AREA WAS MINED 9/25/04.
