Miner was walking away from the miner while they were changing bits when a piece of the bit or hammer hit the employee walking away in the side of the head. The employee received 5 sutures.

Employee had Carpal Tunnel Surgery on 1/17/18.

Employee was straightening roof bolt plates when a piece of rock fell and smashed right ring finger. EE received 7 sutures and broke the bone at the end of finger

Elevator had an electrical problem

Employee had carpal tunnel surgery on wrist on 4/9/2019.

Employee was scaling the top when a piece of rock fell and hit ee in the head. EE received 15 staples.

Employee had Carpal Tunnel Surgery on wrist on 4/4/2019.

Stepped on a piece of coal and twisted ankle. The employee never missed a day of work until 11/02/2017. The ankle never really healed and now the doctor has put EE in a boot and told EE to stay off it for 2 weeks.

Truck driver dumping fly ash got dizzy fell into a pile of fly ash. Employee received burns to employee's eyes which employee had to get antibiotics for employee's eyes.

A piece of wood off of a wood plank was blown into employees left eye by the fan on a bolter. Employee went to eye doctor the next day and received medication in the eye.

Elevator went down due to a bad capacitor.

Brakes on Brake car locked up.

Employee had Carpal Tunnel Surgery on both right and left wrist. Surgery was on 6/15/17.

Cage went down wouldn't work on automatic. Bent a magnet on cage.

Repairmen doing perm on scoop was removing pump motor lids. When ee went to lift and move cover ee tore bicep muscle in right arm

Fell on grating and caught self and tore left rotator cuff. Employee returned to work until surgery on March 2, 2017. Never missed work waited 2 weeks before going to the doctor. Returned back to work with no restrictions until surgery date.

We found a loose connection on the elevator doors.

Receiver on elevator doors malfunctioned.

Drill steel popped out of chuck and hit EE in the left thumb. EE had to get 8 sutures.

Steel pipe fell and broke finger.

Getting off a mantrip and pant leg got caught on the ride causing EE to fall. EE torn EE's left bicep when EE grabbed the mantrip to keep EE from falling.

VFD drive went bad

EE was putting in the frame for equipment doors when the header of the door frame slid down pinching index finger between the header and the post.

Had a bad control board on the DC drive.

Employee was pulling a drill steel out of EE's tray when the steel smashed EE's finger against the drill pod. EE received 4 sutures.

They were moving a feeder when it hit the rib. They stopped the feeder and had to move the cable and when they move the feeder again the rib rolled out and hit an employee in the left leg.

Hitting a connecting link for a conveyor chain when a piece of metal flew off and hit employee in the left arm. Employee received 1 suture.

Employee was running a miner and cutting when a piece of metal broke off a bit and flew back hitting the operator on the right hand.

Electrical faults resulting in blown fuses, changed fuses.

Employee was using a pry bar to pry up a valve bank on a continuous miner when the bar slipped causing the employee to fall. When the employee fell their feet stayed planted to the ground causing EE's body to roll over right ankle causing a fracture to EE's ankle

Employee was using a hammer and a pit punch to knock out a broken bit cone on a miner when a piece of metal flew off and cut EE's middle finger on left hand. The employee received four sutures to repair the wound.

Wear and tear over several years and repetitive movements. This is an occupational illness. Employee had repairs done on 4/12/17 and will be off work for several weeks.

Elevator trailing cable required splicing and lubricate door switches.

While roof bolting, employee turned to retrieve a roof bolt wrench from the material tray and stuck his left elbow on roof plates that were stacked on the tray. The result was a laceration requiring 5 sutures to close.

The elevator door became out of adjustment and would not allow the elevator to function properly. The doors were adjusted and the elevator placed back into service at 8:40 am.

Employee went home from work following 3 pm to 11 pm shift. When he went to bed his left eye started burning. He went to eye doctor and doctor removed small sliver of metal that had scratched his eye. Employee does not know how or when foreign body entered his eye.

A door roller on the inner door fell off preventing the doors from operating properly. The roller was replaced and the elevator returned to service at 4:10 pm.

The outside doors on the bottom landing of the personnel elevator shaft would not function properly. A defective door roller was found and replaced and the doors adjusted. The elevator was back in service at 2:10 am.

Employee was operating a skid steer cleaning debris from the railroad loadout tracks. Bucket contacted a rail joint that had an elevated rail, causing the skid steer to come to an abrupt halt. This in turn caused the employee's hard hat to strike the windshield. The hard hat cut his forehead, and he required 9 sutures to close the wound.

Employee was raising lid on battery pack on scoop tractor and before he could secure it in the upright position, he dropped it on his right hand, fracturing his right middle finger and also requiring sutures to close a laceration to the same finger.

The elevator door limit switch became dislodged from its mounting bracket resulting in door not closing. Switch was repositioned in mounting bracket and elevator resumed operation at 3:03 am.

Employee was helping another employee change a bottom belt roller. The employee's right middle finger was caught between the belt roller bracket and belt frame resulting in a fracture and laceration to the tip of his finger. The employee received 7 sutures.

Employee was descending steep narrow spiral staircase while carrying parts and felt pain in left knee. Employee tried to work thru pain but had arthroscopic knee surgery on 09/10/2015 and returned to work on 09/14/2015 with a full medical release. Employee had no lost work days.

The elevator doors buffer switch on the bottom was found to be defective and was replaced.

The elevator car buffer switch on the bottom landing needed repaired.

Fire was observed in the Reclaim Belt Tunnel in the vicinity of No. 3 Feeder, which is located under the Stacking Tube. The coal and belt fire was extinguished with water, the concrete stacking tube and surrounding area was inspected and no structural damage was identified. The fire was deemed extinguished by approximately 1:30 pm.

The elevator doors would not open when the elevator reached the bottom landing. The elevator doors were adjusted and the elevator was back in service at 8:05 am.

While using a razor knife to cut excess tape from the end of a water line, the employee cut his left middle finger and required 3 sutures to close the wound.

The bottom outer doors of the elevator would not close. A problem with the door linkage was discovered and repaired. The elevator was back in operation at 1:30 am.

The doors on the elevator would not open and close properly.

The hoisting elevator would not operate. Troubleshooting revealed a short in the electrical system.

Troubleshooting revealed that the elevator experienced a short circuit in the electrical system.

Elevator sensor, roller and bracket on door broke.

On 10/23/2014 the section supervisor was walking between entries 7 & 8 in the last open x-cut when he was hit on top of the head from a piece of roof. This causing his teeth to come together abruptly breaking his back lower two molars on the left side.

Employee was constructing scaffolding and got finger pinched in cross brace. The employee was diagnosed with a fracture in the 5th digit finger of the right hand

At approximately 5:30am the Nordberg slope hoist became inoperable. Due to heavy rains the sump for the hoist drum filled with water shorting out the brake coil. Water was pumped down, the brake coil was dried out, and the hoist was back in operation.

At approximately 2:25pm the Schindler Elevator became inoperable. The approach and landing switches were cleaned, re-set the drive, and the elevator was back in operation.

At approximately 9:30pm the Shindler Elevator became inoperable. Switches and sensors were cleaned due to high humidity and condensation. The drive was then re-booted/re-set and the elevator was back in operation.

At approximately 5:22am, the Shindler Elevator became inoperable. The elevator was intermittently stopping in the shaft with multiple fault codes. Numerous connections on the elevator were cleaned and tightened. The elevator was back in operation at 9:20am.

At approximately 8:55am, the Shindler Elevator became inoperable. The VFD drive was indicating a low line power fault. A loose connection was found on the drive transformer and was repaired. The elevator was back in operation around 2pm.

At approximately 12:05am, the Shindler Elevator became inoperable. The elevator doors would not open properly at the bottom landing. Bolts on the outer door mechanism at the bottom landing were tightened and the elevator was back in operation around 1:15am.

This is an occupational illness that happened over time. There wasn't and accident really. The report date is just the day he first mentioned he couldn't do the job. He was a roof bolter that developed a shoulder issue. He had his left shoulder surgically repaired.

Employee had Carpal Tunnel surgery on both right and left wrists.

While on the Main West travel way, the examiner was trying to pry down a piece of rock that was resting on a curtain board. Another piece of rock fell striking him across the bridge on the nose causing a small fracture and eight sutures.

At approximately 5:05 a.m. the Shindler Elevator became inoperable. The door safety switch on the top landing was readjusted and the elevator was back in service.

Tuesday afternoon at approximately 3:10pm, the mine lost all power due to a problem with the incoming power from the power company. The mine was evacuated and power was restored at approximately 6:00pm.

At approximately 10:00pm the employee was stuck by falling debris on top of his head. This caused his teeth to come together abruptly and a crowned tooth was broken out. The remainder of the tooth will need to be removed and replaced in the future.

He was taking structure apart hitting it with his hammer and hit his picky finger and fracture it.

Employee was pulling down a curtain when he felt a tear/pain in his right bicep.

While trying to drive out a broken bit on the head of the continuous miner, the mechanic was impaled by a piece of flying metal in the left cheek. This resulted in four sutures.

At approximately 6:50am the Schindler elevator became inoperable. Dirt was found on the door re-opener photo eyes therefore the automatic doors would not shut. The safe edge was cleaned and the elevator was back in operation.

At approximately 11:30pm the slope hoist car became inoperable. The batteries had to be replaced for the braking system and the brake car was back in operation.

At approximately 3:15am the Shindler Elevator became inoperable because the automatic doors would open. The processor was rebooted and the Shindler Elevator was back in operation.

At approximately 2:30pm the Schindler elevator became inoperable. The Schindler Company was called on site, electrical repairs were made, and the elevator was back in operation.

At approximately 12:45am the slope break car became inoperable. Electrical problem was corrected and the hoist was back in operation at 3am.

At approximately 4:45am the Schindler elevator became inoperable. Electrical components were changed out and the Schindler elevator was again in operation at approximately 7:15am.

At approximately 12:45pm the Slope Hoist became inoperable due to an electrical malfunction. Parts were ordered and delivered. At approximately 4:10pm the hoist was back in operation.

While trimming top with the continuous miner, the operator was struck in front of the right ear by a small piece of flying metal. The small piece of metal had to be removed and sutured.

Employee had Carpal Tunnel surgery on right and left wrists.

At approximately 6:15pm the Shindler became inoperable. The door switch was adjusted and the elevator was back in operation.

While installing a grade 5 fully grouted 8ft roof bolt, the operator injured his shoulder when straightening the bolt during the installation process.

Pinched the tip of the right ring finger while installing belt structure.

At approximately 3:30AM on Tuesday Feb. 26, 2013 a heating event was discovered in the 4L1RMS panel at xc19. Employees were removed from the mine and the proper personnel were notified to investigate elevated CO levels.

At approximately 11:45PM the Shindler elevator became inoperable. A shorted door switch was found and the Shindler was again in operation.

At approximately 6:00am the Shindler elevator became inoperable. The oil level was found low on the rope gripper causing the fuse to blow. Oil was added and the fuse was replaced.

Employee pinched left ring finger between roof bolt pod and the winch causing a cut and fracture to the end of the finger.

At approximately 11:45am the Shindler elevator became inoperable. The processor was rebooted and back in operation.

At approximately 5:00pm the Shindler elevator became inoperable due to an electrical malfunction. The Shindler elevator company was notified to resolve the issue.

At approximately 10:00am the Shindler elevator became inoperable. Repairs were made to the elevator and operating by 11:30am.

At approximately 6:40 pm on Tuesday Oct. 16, 2012 a possible ignition occurred at the face in Unit 1. The unit was immediately shut down, power removed and the proper persons notified. The scene was preserved for the company and agencies to conduct an investigation.

Employee caught his right index finger between the staionary sideboard and false bottom sideboard, causing a fracture to the end of the finger.

Employee was struck on the left thumb while his hand was resting on the shop bench causing a fracture to the thumb.

Working to put track back together on D8N Cat beating pin back in track with sledge hammer while EE held sledge on other side of link, missed pin swinging hammer into his handle smashing finger (first digit of left index finger smashed & torn). Received stitches.

The employee was struck with a piece of rock from the top, hitting him on the side of his right foot, causing a small fracture to his little toe.

At around 10:30 am on July 9, 2012 the Schindler Elevator became in-operable. The door switch bracket on the door was broken. Repairs began immediately and the elevator was back in full service in 90 min. The failure did not entrap or cause injury to persons.

On June 11, 20212 an event occurred at #3 face of Unit 2 & was allegedly witnessed by the CM & FCT operators. Based on initial accounts, the operator did not believe an accident occurred. Subsequent info made available to the operator revealed that a probability of an even that occurred was an ignition of an unknown substance which was extinguished in less than 30 sec.

Employee smashed his left thumb between a 2" pipe and a guard while removing the pipe. He received 4 stitches on the side of the thumb.

At approximately 8:15 am on April 30, 2012 an unintentional roof fall was discovered in the South Entries off the Main West (Unit 1)in entry #7 from SS# 1350 to SS# 1420. The fall measured 50' long x 19' wide x 8' high. The fall did not effect passage of men, disrupt ventilation, affect equipment nor cause injury to persons. The fall will NOT be cleaned up.

Employee stepped on a rock twisting his left ankle while carrying a hydraulic cylinder. He received no medical treatment nor missed any work. On 6/28/2013 the employee had surgery to repair damage do to the accident.

At approximately 8:15am on April 4, 2012 the Shielder elevator became inoperable. The solenoid on the brake mechanism failed and would not allow the brake to release. Parts were ordered and repairs began immediately. No one was trapped or injured. The elevator was back in full operation later in the day.

An unintentional roof fall occurred in our Unit 1 at SS# 7+80 in entry # 7. The fall measured approximately 19'x19'x8'. The fall did not affect ventilation, passage of men nor did it damage any equipment. The fall will not be cleaned up.

At approximately 6:45am the man elevator became inoperable. A door switch was out of adjustment and repaired immediately. No one was trapped or injured.

The Shiendler elevator became inoperable at approximately 10:50PM on Jan. 18, 2012. No one was injured or trapped. Troubleshooting began immediately and a faulty bottom door switch was found and repaired.

Employee was struck in the right index finger with a piece of metal that broke off a gear pulling device causing a small laceration.

At 7:15 am on Nov 14 2011, the elevator became inoperable. After troubleshooting it was discovered that a door switch bracket has broken. The piece was repaired and the elevator was back in service within the hour.

Employee was lifting a scoop drive line brake and felt pain in his lower back.

At approximately 6:35 pm on September 15, 2011 a failure of the Shiendler elevator occurred. A elevator door switch was found to be slightly out of adjustment not allowing the doors to close. The adjustment was made and the elevator was fully functional in less than 1 hour.

A failure of the Shiendler elevator occurred at approximately 4:00 pm on Sept. 14, 2011. The controller had a control power issue. The problem was found and the elevator was back in operation within 45 minutes.

The Shiendler elevator located in the primary escapeway became inoperable at approximately 12:53pm. A control power issue was discovered. Work began immediately to correct the problem.

A roof fall occurred in the Main West entry #8 at 37 xcut. The fall measured 20'x20'x 3' to 6' thick. This fall did not effect ventilation, passage of men nor did it damage any equipment. The fall will not be cleaned up.

EE was struck in the mouth with a piece of chain causing 2 broken teeth and laceration to inside of bottom lip requiring 2 sutures.

The secondary escape way was blocked for a short period in which interfered with the operation of the brake car.

Operator was roof bolting shale fell striking on right forearm.

Employees ankle was pinched between belt roller and a 25' roll of belt causing a hairline fracture.

A roof fall occurred in the Main West entries at Survey Station 51+75 between #4 & #5 entries including a portion of the intersection. It did not affect ventilation, passage of men nor did it cause damage to any machinery. The fall will not be cleaned up. The fall measured approx. 20' x 20' x 5'.

At approximately 3:30am the man/material elevator became inoperable. After inspection it was determined that a power fuse had blown. The use was replaced and elevator returned to normal operation.

Employee was struck by a small coal rib which caused a sprain to his right ankle.

Employee was hit on top of head with piece of rock, knocking off hard hat and lacerating his head.

At approximately 1:05PM, the slope car and hoist became inoperable. The drum brake on the Nordberg slope hoist would not hold. The hoist was taken out of service. The hoist was repaired.

At approximately 11:55 the elevator became in operable. After investigation was determined that a control cable had been severed. The cable was repaired immediately.

At approximately 8:45 the man elevator became in operable after investigation it was determined that a door safety switch had failed. The switch was replaced on the elevator.

EE slipped and fell on right elbow fracturing off a small chip of bone off the end of his elbow.

Employee was reaching for a shovel when a rock fell out from between two longwall shields and struck him on his left shoulder, knocking his hard hat off his head.

Employee was walking down steps of trailer when he missed a step and came down hard on his left knee, causing pain in his knee.

A roof fall occurred at 13th East Gateroad, Unit 1, Crosscut 69, Entry 3. The fall measured approximately 8' long x 16' wide by 4'-6' thick. The fall did not result in any damage to equipment or property.

A roof fall occurred at 1st North Main East, Crosscut 56, Entry 6. The fall measured approximately 22' long x 24' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.

Employee states that years of working with both hands as roof bolter, mover, laborer and repairman have caused pain in both wrists.

Employee was hanging curtain when he blacked out for a second and fell backwards, injuring his back.

Employee states that over the course of roof bolting and repetitive motion he has experienced pain in his left shoulder area. On 12/13/07 medical verification was received on work-relatedness of this claim.

A roof fall occurred at Main East Travelway, Entry #4, Crosscut 156. The fall measured approximately 20' long x 20' wide x greater than 14' thick. The fall did not result in any damage to equipment or property, but did affect passage of miners.

On 11/1/07 employee was aligning a crawler assembly when he felt pain in his left shoulder. He continued working until 11/21/07 when he sought medical attention and was prescribed restricted duty.

A roof fall occurred at 1st North Entry #8, Crosscut 20. The fall measured approximately 20' long x 20' wide x 10' thick. The fall did not result in any damage to equipment or property.

Surface employee was leaving building when gusty winds blew dust into his eyes behind his safety glasses. Employee used eyewash several times. He saw optometrist and was given prescription medication for abrasion and inflammation. He returned to work.

Employee was struck on the shoulder by a water shut off valve that was being removed off the end of the water line.

Employee was using pry bar to raise a cover when the pry bar slipped causing him to lose his balance and strain his neck. Employee had major neck surgery earlier this year (a few months prior to this accident date). On 10/31/07 medical verification was received regarding work-relatedness of this injury.

Employee was pulling wire out from under rocks when he felt pull in his left shoulder. He continued working until 12/18/07 when he sought medical attention and started missing work.

On 10/1/07 employee reported that over time he has experienced pain and numbness in both hands, wrists and left elbow. Employee continued working until 12/28/07 when he had surgery for carpal tunnel syndrome and started missing work.
