Employee sought medical attention from EE's personal physician for shoulder pain. Compensable determination for WC was made on March 10, 2026--making it reportable at that date. Employee uses pole to poke feed through feeder and has performed this task over many years of employment

THE IE REPORTED BACK STRAIN ON 8/18/25 AT THE START OF SHIFT. IE STATED THAT THE INCIDENT OCCURRED ON 8/13/25. THE IE WAS REMOVING A BROKEN PALLET FROM THE BAGGING SYSTEM WHEN THEY FELT A STRAIN IN BACK. IE WAS SEEN BY PHYSICIAN AND TAKEN OFF WORK.

IE was rolling the tarp off of McNay rental trailer #118 when the handle detached from the trailer causing the IE to fall to the ground, same level injuring left lower back. The IE did not seek medical attention until 11/15, when they were taken off work.

IE was lifting a mop bucket to dump in a sink and felt a pop in EE's back. IE refused treatment on 10/21/24. On 10/22/2024 IE sought medical treatment and was given two injections and was taken off work...unable to accommodate restrictions.

THE IE TRIPPED OVER A PLASTIC BAND AND FELL STRIKING EE'S LEFT HAND AGAINST A PIECE OF METAL STRUCTURE. THE IE WAS TAKEN TO BLESSING HOSPITAL ER WHERE AFTER X-RAYS EE HAD A FRACTURE TO EE'S LEFT MIDDLE FINGER.

IE WAS CLIMBING DOWN A LADDER AND EE'S RIGHT ARM "GAVE WAY". REPORTED INCIDENT AND FINISHED SHIFT. IE DEVELOPED A PERSONNEL MEDICAL ISSUE (NON RELATED) THAT TOOK EE OUT OF WORK. WHILE OFF ON PERSONNEL MEDICAL HAD MRI ON RIGHT ELBOW, REVEALED TORN R BICEP TENDON TEAR. IE RETURNED FROM PERSONAL MEDICAL, WORKED UNTIL SURGURY. FIRST SCHEDULED WORK DAY MISSED 8/26/2024.

The IE was struck by a shard of glass from a UV light that broke. The IE required 3 sutures. The incident occurred in our Nutrition Department / Mill 34.

2nd Shift Scale House Clerk was giving paperwork to an over the road driver when EE suddenly collapsed and went unresponsive. CPR was performed until the ambulance arrived and EE was transported to the Hospital where EE was pronounced deceased.

IE was loading a 6" vacuum line into the back of a pick-up truck when they felt a slight "pull" or "pop" in lower back. The IE reported incident to supervisor on 6/22/2021.

IE was helping to free a bucket elevator leg. The leg, locked out at the time of the incident. Another employee using a pipe wrench to try and turn the shaft while the IE was trying to pull on the belt, when the pipe wrench slipped causing three of the IE's fingers to get pinched between the belt and a sheave. X-rays revealed a fracture to the tip of IE's middle finger.

The IE was moving a stack of pallets through the "speed door" in the Packaging Plant when saw dust came off of one of the pallets and into the IE's right eye. The IE was seen the following day by an OD who removed debris from the eye and prescribed antibiotic drops.

On 1/30/21 the IE was trying to free up material in the Mill 13 bin using a long rod to poke the material. EE felt a strain in EE's lower back and reported the incident to management. The IE continued to work until EE's doctor took EE off work on 5/18/2021

EE was filling super sacks with product in the packaging plant. As the super sack was going down the conveyor, EE noticed the sack was leaning. EE stopped and tried to push back on the sack and felt a pop in EE's left knee so EE was taken to ER. EE can't get into a Dr. to be released due to COVID-19 policies so EE has missed too many days of work.

IE was attempting to hook up a mobile air compressor by unhooking a stationary air compressor so that the stationary air compressor could be serviced. As they started to hook the 3" hose up, they tripped over the excess hose on the ground and fell. As they fell, left thumb hit the framework and caused a fracture to left thumb.

Employee lifted arm to work on air lock and as employee raised arm employee felt a pop. Employee reported the incident two days later on Sat. May 18th 2019. Sent employee to hospital and has restrictions

Removing belt from a motor and did not relieve tension on belt first. When right hand pulled the belt, left hand holding a screwdriver slipped and two fingers were pulled between sheave and belt. Two fingers were lacerated and very tips were fractured. Employee was retrained to loosen tension first to remove belt.

Employee was stepping out of rail car mover and when EE stepped onto the ground EE twisted EE's ankle. Went to be seen at clinic. X-rays were taken and EE was diagnosed with a slight sprain. Missed two shifts then back to work normally. Ground was level and no work conditions were noted as cause.

Dropped a piece metal on left foot while loading onto a roller.

Employee was grinding off a bearing and a piece of debris flew into left eye. Not first reported Friday because employee believed it was just a piece of stone, not steel. Employee was wearing safety glasses and grinding shield.

Bulk bag fell over while employee was placing strap onto forks of fork truck. Employee lost balance and fell forward. Employee later in the day noticed back was painful.

Employee was getting out of EE's personal truck and when EE stepped onto the ground felt EE's hip pop. EE's truck is lifted and is very high off the ground. Employee was in awkward position and has had a previous hip surgery on that hip.

Employee tripped on stretchwrap and fell. Employee reached out with a hand to catch themselves and hurt a wrist. After a few days employee went to the doctor who determined it to be a fracture.

An operator was weighing a railcar, while an employee was spotting the railcar by track 6 and 8 during the early morning. The employee stepped an uneven ground, and twisted knee.

While re-installing a guard, the employee let go of guard with one hand to re-position. Before ee could grab the guard again ee dropped it. The guard fell onto right index finger resulting in a fracture.

The employee was walking outside of the mill 18 area by the tramway of the ball mills/NTR area. There was a defined clean walkway, but it appears that the employee walked through a buildup of material that was not on the walkway. Twisted right knee.

Lab employee was opening a sample jar and felt a "pop" in wrist. Did not feel pain at that time but over a few days began to experience pain. Being seen by a doctor for possible cartilage or repetitive.

Employee was stepping off of a super sack cart and misjudged landing. EE twisted ankle resulting in a very mild sprain.

Employee's foot slipped off rung of fixed ladder and employee lost balance. When landing on floor employee rolled employee's ankle. Sprain resulted. No coast, surgery required.

Employee was in the process of stacking empty bags on the conveyor for the robotics system. Employee was standing at a pallet that was on a spring assist table so the bags were waist high. Employee collected a hand full of empty bags and turned full body to place the bags on the conveyor and at that time felt a sharp pain shoot from left rib cage area.

The employee was trying to remove a 8' dust collector bag that was full of material. While trying to remove the dust collector bag, EE tried to open the top door on the dust collector with a clamp left on. At this point, the employee felt a pull in lower back. The employee was taken to a doctor, and was diagnosed with a muscle strain.

Employee started walking up a set of office steps. While on the 3rd step employee began to lose grip of coffee cup, by trying to catch it lost footing. Left foot slipped off the step and down onto the 2nd step hard rolling ankle.

This employee and another employee were on the belt cleaning out a rock jam. The other employee was pushing the rock up the belt to this employee who was lifting the rocks out and throwing them over the side. They were just about done when this employee encountered lower back pain. Employee was taken by ambulance to the hospital.

Employee was scrapping/scooping oil dry that was used to absorb oil spillage in the Mill 7 area. Employee noticed discomfort in their side and pain in their groin area.

Employee from temp agency was scooping a limestone pile with a shovel. As went down in a scooping motion, felt something "pop" in shoulder.

Employee ran a jack hammer to break up the dried up/hardened sludge that had built up on the floor of the mill on February 1, 2016. On February 9, 2016, employee reported to supervisor that employee had sought medical treatment the previous day for swelling in the groin area.

Employee rolled their ankle on a rock while cleaning rock off the belt that feeds Mill 5.

A passenger vehicle was traveling south on Illinois 57 when the vehicle veered out of the lane and hit a retaining wall on mine property. The vehicle burst into flames, killing both the driver and a passenger. Neither of the individuals had any affiliation with the mine. Aside from proximity to the road, the mine had no involvement in the accident.

Employee was struck in the right ankle by the end of a pneumatic deliver hose while attempting to clear the hose with compressed air prior to use. Employee was bruised and the attending physician prescribed time away from work to allow for healing.

Operator was doing maintenance on mill and losing bolt and felt pop in back.

Employee was walking down mill #1 ramp, looking down at his 2-way radio, when he stepped onto a big rock, slipped and caught himself, he felt his back "pop".

Operator was assisting maintenance on Mill 19 reel chain that had broke. Couldn't get chain on rotary splitter. Operator came up on steps to hold splitter to position to install. Maintenance employee walked down steps to get tools. Operator stated he was rotating rotary splitter to position & it bound up and he had sharp pain in back.

Icy surfaces -Employee was walking from building to truck and fell and hit his head on ground.

mill operator was finishing poking down the agstone in the outside system hopper. Operator got it all poked down and turned to put the poking rod up and slipped on some loose rock strained knee

Felt twinge in back during manual bagging

Employee stepped down off forklift and felt knee pop.

Employee was loading truck at Silo 33. Employee lowered load out chute into hole. Chute contacted lip of the hole rather than going down into it. Employee reached down with his left hand and pulled on chute to allow it in the hole. The steel rings of the chute pinched the employees left palm resulting in laceration approximately 2 inches long. First Aid was administered.

He explained that he fell while attaching the sack loops to the forklift. Access to forks on tow motor, and sack spout was being gained by a stack of 3 empty pallets, 4x4 boards, and a piece of plywood on top of the floor scale. As he stepped up onto the pallet configuration the plywood on top shifted causing him to fall.

Employee's knee gave out stepping up on packaging cart and fell back hitting shoulder on pallet.

Employee started descending from the packaging cart while carrying his large, personal portable radio in his right hand. The employee did not maintain the required 3-point contact as he descended and stumbled, chipping his upper left front tooth. Employee was taken to the dentist. The employee's mouth was x-rayed, there was no damage to the root or nerve and the tooth was filled.

The employee was using a pipe and a hammer to break up clumps of material in an outside hopper. The employee put one end of the pipe on a clump and went to strike the other end of the pipe. The hammer head glanced off the end of the pipe and the employee's hand that was holding the hammer hit the end of the pipe. The employee broke the second digit on his right index finger.

EE WAS PUSHING SNOW OFF THE ROADWAY WITH THE BUCKET OF THE CAT 980 F LOADER. HE CAME TO THE RAILROAD TRACKS AND LIFTED THE BUCKET TO CLEAR THE RAILS. THE CUTTING EDGE OF THE BUCKET CLEARED THE RAIL BUT THE SKID PLATE UNDER THE BUCKET CAUGHT THE INSIDE RAIL CAUSING EE TO STRIKE THE LEFT SIDE CAB WINDOW WITH HIS HEAD AND SHOULDER BREAKING OUT THE WINDOW. INJURY RESULTED IN THREE SUTU

EE WAS HELPING ANOTHER EE UNLOAD A SHEET OF FLOOR GRADING (36" WIDE X 20' LONG) OFF THE BED OF A PALLET TRUCK. EE LOST CONTROL OF THE GRADING WITH HIS LEFT ARM AND THE FULL WEIGHT OF THE GRADI NG CAME OFF THE TRUCK AND, ACCORDING TO EE, HIS RIGHT ARM ABSORBED THE FULL WEIGHT OF THE GRADING. THE OTHER EE STILL HAD HIS END OF THE GRADING SUPPORTED. EE DID NOT REPORT THE INCIDENT UNTI

EMPLOYEE HIT HEAD ON LOW CLEARANCE AUGER. HE WAS WEARING HIS HARD HAT.

ATH EMPLOYEE ENTERED DOCK #6 AREA TO LOCATE TRUCK DRIVER TO REPORT PROBLEM WITH HIS TRUCK. WHILEINFORMING THE DRIVER OF THE PROBLEM, A UWI EMPLOYEE WAS IN THE PROCESS OF RE-ENTERING THE TRUCK TO COMPLETE UNLOADING THE FINAL TWO PALLETS OF PRODUCT WHEN THE ATH EMPLOYEE WAS STRUCK ON THE RIGHT FRONT BY THE TOWMOTOR. NEITHER EMPLOYEE SAW THE OTHER UP TO THE TIME OF THE INCIDENT.

THIS EMPLOYEE WAS UNDER MEDICAL TREATMENT FROM HIS PERSONAL DOCTOR FOR HEALTH CONDITIONS UNRELATED TO WORK. THE EE CAME TO WORK AND WAS IN THE PROCESS OF LOADING TRUCKLOAD OF 50 LB BAGS ON PAL LETS INTO A VAN TYPE OVER-THE-ROAD TRUCK. BEFORE LOADING COULD TAKE PLACE THE EE HAD TROUBLE BREATHING. THE EE WAS TAKEN TO THE ER FOR TREATMENT. DR STATEMENT ON REPORT LISTED AS AN ILLNESS.

EE WENT OUT OF DOOR NOT REALIZING THERE WAS A STEP DOWN SHE STEPPED DOWN & TWISTED & BROKE HER ANKLE

EE WAS REMVONG SHAFT BEARING HOUSING FROM SCREENWHEN SHAFT SLIPEED CATCHING EE'S HAND BETWEEN ELECTRICAL PANEL & END OF SHAFT EE HAD HIS HAND ON END OF SHAFT TO HELP GUIDE IT OUT INJURY REQU IRED 5 STITCHES T

EE WAS TRYING TO GET GROUND LIMESTONE MATERIAL OUT OF AIR SLIDE UNDER TANK 1 AT PACKAGING PLT. HE WAS PRYING ON A VALVE ARM ON AIR SLIDE WHEN PIPE SLIPPED AND HIT HIM IN THE LIP. UPPER LIP WAS LACERATED AND REQUIRED FIVE STITCHES.

EE WAS REMOVING AN AIRLOCK USING A HYDRAULIC PLATFORM. AIRLOCK AND PLATFORM FELL BACK PINNING EE'S ANKLE TO FLOOR.
