New Future Mine Coal

A contract worker for Midwest Vulcanizing Company was being supervised by their own people was injured. EE claims a steel belt block slid down the rib and hit EE in the head. Cause of injury was steel block was not secured against the rib. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor.

Employee was walking around the Longwall drive area slipped and fell on right arm causing fracture.

Employee was placing base lift back on the shield feet, when EE felt a strain in the neck and back. Employee did not start missing work until 5/18/2017.

MMU 058 experienced a roof fall at approximately 51 crosscut on the longwall headgate drive. The fall was approximately 20'L x 16'W x 6'T. The fall will be mined thru as part of the longwall cycle according to the MSHA approved plan. The roof fall reference number is 8.

Employee was filling pod duster. Once pod was filled employee removed hose striking leg causing laceration.

Temporary mine employee was capping crib and a rock struck employee's mouth breaking a tooth. Medical Treatment.

Employee was recovering bad rams on the longwall face. One side of the ram shifted smashing right thumb.

Temporary mine employee was changing water coupler it came apart and struck employee's face. Restricted Duty.

Employee was stacking concrete block and smashed finger between two concrete blocks.

Employee was installing cribs and had a hole in one of employee's boots made contact with bed ash causing injury.

Employee was a passenger on mantrip 45 traveling out of the mine at end of shift. The mantrip inadvertently struck a pot hole, located on the inner seam slope, in travel road injuring employee.

Employee was walking and slipped while handling hog panel at the face in the First East Headgate. Employee braced themself with right hand straining wrist.

Employee was carrying a broken roller down the slope to be taken out. Employee slipped on runoff, causing the roller to slip off and land on employee's pinky finger.

Temporary mine employee while shoveling in the slope slipped reached out for belt structure not to fall and injured left thumb. Lost Time.

Employee was preparing to leave at the end of EE's shift of the 1AE Longwall. Employee was in the air crosscut, bent over to pick up EE's lunch box and a small piece of roof debris struck employee in the back of the head causing laceration. Medical Treatment.

Temporary mine employee was carrying a concrete block and dropped it on left foot causing injury. Restricted Duty.

Employee was walking around the roof bolter on the 1st East Headgate slipped braced self with right arm on the coal rib injuring right shoulder. Restricted duty. *Amended to lost time as of 1/19/2017. Employee has not returned to work.

Employee was removing cover off rear of road grader and felt pain in right shoulder. The road grader was parked in the supply/air shaft area. Restricted Duty as of 12/27/2016. *Amended to lost time as of 1/12/2017. Employee has not returned to work.

Employee was working on the face of the 1A East Long Wall, pulling shield and right foot was struck by crib block causing contusion. Restricted Duty.

Employee was performing duty as an examiner walking beltline. Employee lost footing straining right ankle. Restricted Duty.

Employee was struck by rock on right hand while standing at the Main North 2 Belt Head. Restricted Duty.

Employee slipped while carrying crib block twisted and injured lower back.

Was shoveling longwall feet & was using left foot to brace self. Foot slipped causing to fall backwards on a piece of steel. Cause of injury wet conditions, body positioning. Employee was handling material and heard something crack behind EE and a rock fell from the roof striking EE on the lower back and left leg causing fracture to leg. Lost time as of 11/28/2016

Death occurred from natural causes. Person suffered a fatal heart attack.

Employee was knocking out block from brattice and bent over to pick up block when a block fell from above striking the second and fourth digit on left hand causing fracture and laceration sutures required. Restricted as of 11/27/16.

Employee was assisting mechanic putting chain back together when mechanic struck piece of roller chain and a piece of metal struck employee in the right arm. As of 11/09/2016 restricted duty sutures required.

Employee states while employee was installing roof bolts in the Main North 2 travelway the roof bolter machine was bumped by a ram car causing injury to right hip. Restricted Duty.

He was cutting a flap off a piece of belt when knife blade broke cutting his wrist and received 9 sutures.

Employee opened the right side of equipment door. He turned to open the left side equipment door and the door that was open closed on his left ankle. Causing fracture to left ankle. Employee was wearing gloves pulling a loose crete plank off of an overcast. When the plank came off of the back end and hit the ground causing the end he was holding to smash his left fingers against a block that was on the rib. Laceration to left fingers and sutures. Employee was trying to break a tail gearbox loose when a bolt from the lifting eye broke striking him above the left eye causing laceration. Employee had sutures above left eye (Medical treatment). Employee was dropping bolter cable, when a rock fell from the edge of the screen wire striking his upper right arm causing a laceration. Injured employee was placing a crib underneath a shield foot, when he smashed his left middle finger and index finger between the shield foot and the crib. Injured employee was working at the bottom of the airshaft when he was struck on the head by a piece of grout. The employee suffered a fractured skull and a laceration that required 5 staples. Employee states he was walking backwards into a crosscut, stepped into a hole, and fractured his left lower leg. Compliance with rules and regulations was not a factor. Miners proficiency and job skills may have been a factor.

Employee was loading belt structure when he pinched his left small finger between a roller and a stand. The employee received 4 stitches. Miners proficiency may be a factor.

The injury occurred when the contractor was struck on the left hand by a crib block that had fallen from an adjacent set of cribs that were being erected. The contractor received a fracture to the left hand. Cause of the injury was working below areas where objects could fall from above.

The injury occurred when the contractor was carrying a crib block through water, and tripped over another block in the water. The contractor dropped the one he was carrying and was struck in the mouth, chipping two teeth. Caused of the injury was not removing tripping hazards out of the work area.

An unintentional roof fall occurred at the 5WLW tailgate, entry #3, crosscut 42. The fall measured approximately 18 feet in length, 18 foot in width, and 20-25 feet high. The fall will be mined through, during longwall mining, and the fall will become part of the gob line.

An unintentional roof fall occured on the Main West Travelway, crosscut 25, entry 3. The fall measured approximately 25 feet in length, 18 feet in width, and 11 feet in height.

An unintentional roof fall occurred on the 5 West Long Wall, Crosscut 56, in the intersection of number 2 entry. The fall measured approximately 18 feet wide, by 18 feet long, by 16 feet high.

Injured employee was filling the rock duster at the rock dust hole, and unhooked the dust hose before the pressure had been bled off. The hose struck the employee in the right forearm and in the lower abdomen/groin area. Job skills and miner' s proficiency may be a factor.

Employee was loading damaged loose cable bolts into a battery ramcar, when one of the bolts struck him in the right eyelid causing a laceration that required 6 stitches. Job skill' s and miners proficiency may be a factor.

An unintentional roof fall occurred on the 5 West Long Wall at crosscut number 60, entry number 2. The fall measured 22 feet in length, 16-18 feet in width and 20-25 feet in height. The area was supported with 12 and 16 foot cable bolts, T-3 channel, and wire mesh.

Employee states his hand was outside of the cab of the scoop, and when turning 3 of his fingers were caught between the cab of the scoop and the rear section of the scoop. Employee was splicing a shuttle car cable and the knife slipped, causing a laceration to the employee's right leg that required 1 continuous stitch. Employee was struck in the forehead by a cutting wheel that had broken in half. The employee received 1 stitch as a result of a laceration. Employee was assisting in connecting chain together when the sheave wheel broke and hit the employee in the lower right leg fracturing the tibia and fibia bones.

The affected employee was removing covers from the tail drive, hooking up a come along, and the spragging plate fell on his right hand causing a laceration, a fractured small finger, and a fractured ring finger. Job skills and miner's proficiency may be a factor.

At approximately 2:55 a.m. an incident occurred involving a dozer operator who inadvertently entered the number 4 feeder located at the raw coal pile. The employee safely exited the dozer uninjured. Job skills and miner's proficiency may be a factor.

Employee was removing a cutter bit from the longwall shearer, and a piece of metal debris entered his left eye.

Employee was changing a universal joint on a battery scoop and his left finger was caught between the driveshaft and the frame of the battery. Employee was pulling an existing 90 foot utility pole with a digger derrick truck. The employee was rotating his boom to the drivers side of the truck when the weight started causing the passenger side outriggers to come off the ground. He then boomed down & winched off lowering the pole but it flipped the truck throwing the employee off.

Employee was carrying a crib tie, when he stepped in a hole and slipped into a built crib, striking his left index finger between crib and crib tie. EE was putting bridges through a doghouse with both return doors open when the pressure caused the first door to shut on the EE's right foot. Medical treatment injury.

An unintentional roof fall occurred at the New Future Mine 4W Headgate Entry #3, 5627W, Crosscut 41. The fall was approximately 25 feet in length, approximately 25 feet in width and approximately 20 feet thick. The fall did not affect ventilation. The fall will be cleaned up. Fall #29E

The fall was approximately 20 feet in length, approximately 18 feet in width and of undetermined thickness. The fall did not affect ventilation or passage of workers. The fall will be cleaned up. Employee stated he slipped and fell while walking in wet conditions. Reportable lost time injury as of December 7, 2012 due to surgery.

Employee was cleaning rock on the tailgate. When he attempted to move a rock that was wedged, it shifted and mashed his finger against the gear case. Medical treatment injury

EE states he was riding in a mantrip hit a bump, causing him to strike his head on the roof of the mantrip and bounce back down into the seat, causing pain in his lower back.Reportable lost time injury 10/30/12 due to surgery

An unintentional roof fall occurred at the 4W Headgate Entry #2, 4840 W. The fall was approx. 27' L. x approx 18' W. and of undetermined thickness. The fall did not affect ventilation. The fall will be cleaned up. Fall#29C

The crane was being moved into position to make a lift on a containment tub when the cranes boom came into contact with some overhead power lines. Employee stayed in the crane until it was confirmed that the power was off the lines, he was not injured and was checked out at a local medical facility to confirm there were no injuries to him.

An unintentional roof fall occurred at the New Future Mine 3W Head gate belt entry crosscut 14. The fall was approximately 42 feet in length, approximately 18 feet in width and approximately 10 - 12 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up. Employee state he was removing a motor from a golf cart when the motor slipped and fell, puncturing EE's right forearm. Medical treatment injury.

EE stated that while bolting, he extended the boom and it caught on the track and threw him upward. When he used his right arm to brace himself he felt pain in his right shoulder and elbow.Reportable lost time injury as of 8/15/12 due to surgery.

Employee stated that while lifting high voltage cable and strained his lower back. Reportable lost time injury 9/28/2012 due to surgery.

EE was taking down high voltage cable when his left thumb became pinned between the cable and cable hook. Medical treatment injury.

Employee was taking down monorail when a rail fell, striking employee's right foot. Original x-rays taken at the time of the injury were negative for fractures. Re-examination and additional x-rays on July 5 indicated a fracture of ankle. Reportable medical treatment injury 7/6/12.

Employee was helping put a steel beam on top of a longwall shield when a rock fell on the beam, causing the beam to fall and strike employee on the right arm. Lost time injury.
