The employee was working at the base of the crusher ladder and placed their left hand on the frame and started turning the pin with their right hand to collapse the ladder. The employee failed to notice that their hand was placed in a pinch point area and when the ladder folded in, it crushed their left hand.

An employee was changing the screen at the dry plant located at Pit 115 WP. The employee stated EE was coming down a fixed ladder and rolled EE's ankle when stepping off the bottom rung resulting in a sprained ankle. Employer incident investigation revealed employee did not follow written company policy and work rules.

Operating the haul Truck on the haul road near the new hopper and conveyer, there was water and mud down near the bottom of the ramp. While driving empty the employee hit a hole and this caused the seat in the truck to suddenly slam down and caused employee's back to compress, this caused pain in back.
Was using a razor knife to cut conveyor belt and cut left thumb on top.

Had some packed snow on the ground. EE did not notified anyone that he had slipped. He continued to work until 2/5/2014 when I received a call from the hsp that he had been admitted because he slipped and hit his head. I then filed a claim with our Workers Comp Carrier. We are still not sure if he fell at work, that is being investigated by the Workers comp Carrier

Miner left vehicle running and in drive. Miner exited the vehicle. Vehicle started rolling. Miner opened door to stop vehicle. Vehicle hit concrete block. Steering wheel spun, miner's right forearm/wrist got entangled in the steering wheel. Sprained right wrist.
Lifting screens from squeeze sreen, he strained his lower back. (x-ray only, restrictions, anti-inflammatory muscle relaxant)
During regular work-wrist started hurting-tried to lift large dump of mud.
Using hammer and chisel to remove bearing race from shaft. Race shattered causing metal fragments to puncture finger and palm.
Operating scraper - frozen ridges - causing operator to bounce (1/18/08 reported - Doctor 1/23/08).
