IMI Huntington Quarry Metal/Non-Metal

EE was cutting an old belt to use as skirting boards for the tail end of the number 5 belt. The EE was using a box cutter to perform this task. The EE was cutting towards themself when the knife slipped and cut the EEs right mid-thigh (approximately 8 inches above the knee) leaving a 2 cm laceration.

Employee's right hand was caught between pulley and the drive belt while rolling a belt on the crusher.

Employee was trying to open the gate to a bin with a sledge hammer and missed and struck finger.

While thawing ice on a bin pulley, a propane thawing device became entangled, causing the employee to become entangled. Investigation continues.

EE drove the cat skidloader to the shop to take bucket off and put the forks on. EE put front of bucket on the ground, could not get pins out to release the bucket. EE used a hammer to hit the pin. Once EE did that, the back of the bucket fell onto EE's foot.

The employee was operating Cat 775D Haul Unit stocking out. The right front tire drove over a pile of stone. The unit bounced the seat bottomed out and the employee felt pain in lower back. Depending on the healing, surgery may be required.

EE was cutting the outer jacket off of a piece of wire, holding the wire with his right hand and the utility knife in his left hand slicing back towards his right hand. The utility knife slipped and continued through his right thumb. Resulting in a cut requiring 11 stitches.

EE and another EE were working @ the #3 bin. They had a piece of steel on sawhorses. Injured EE went to flip the steel over and the steel hit a 2' level causing it to fly up and hit EE in his left cheek.

We were pulling a belt back up on conveyor with a rope tied off onto a loader. The rope broke and we don't know whether the rope, chain, pulley, clevice or what came up and hit his arm.

DRIVER HAD BACKED UP TO STOCK PILE AND WAS WAITING TO BE LOADED. HE HAD BEEN ASLEEP, WOKE UP AND TOLD A FELLOW DRIVER HE COULD NO SEE, & HE WAS NOT FEELING WELL. CO-WORKER SAID THAT HE WOULD GO & GET HELP BUT EE SAID NOT TO & THAT HE WOULD BE ALL RIGHT, WHEN EMS ARRIVED THEY PUT MONITOR ON EE & SAID HE WAS GONE.

TWO EMPLOYEE'S WERE CHANGING RETURN IDLERS ON A SECTION OF CONVEYOR. THEY HAD THE CONVEYOR BLOCKED UP 6" OFF THE GROUND, WHEN THE BLOCK SLIPPED AND FELL ON INJURED'S FOOT.

EE WAS HELPING CO-WORKER READ GROUND RESISTANCE. HE WAS WALKING ACROSS THE QUARRY FLOOR, WHEN HE STEPPED ON A ROCK. THE ROCK ROLLED, CAUSING HIM TO FALL ON TO ANOTHER ROCK.
