Speed Cement Plant Metal/Non-Metal

Injured employee was changing a 10" line blind valve. The change bar slipped out of the knuckle and struck IE over the right eye. IE lost consciousness momentarily. IE was taken for medical evaluation & returned to work with no complications or restrictions

Employee was working on #2 Clinker Cooler and hot dust fell in EE's glove causing burns to wrist / arm area. Employee went to ER (time of day), was treated and was cleared to return to work.

Injured employee assisted another employee picking up a section of floor grate (70lb) felt a pain in shoulder. Reported it to supv., was taken for medical eval. Employee placed on restricted duty till follow-up, eventually diagnosed as rotator cuff tear. Surgery scheduled 12/3, lost time as of 12/6, potential RTW 1/25/22

Electrical manager was moving on foot in the finish building when tripped on floor grating and fell on left side. EE received a medical evaluation, an X-ray confirmed EE had cracked three ribs. As EE's job is typically office work, EE was released to full duty. Medical treatment due to fractured rib.

Outside truck driver stepped on a scale access cover which shifted under EE's weight, allowing EE's legs to fall into hole. EE notified scale operator of the incident & went to an immediate care center-diagnosed with lumbar, knee strain, & abrasions & given prescription pain meds & a muscle relaxer. Instructed to follow up if no improvement. NOT an employee or contractor of Lehigh

Employee was in process of in-choking a chute. Employee was using an 8' air lance when they extracted the lance, the weight of the air hose and lance made it top heavy and the lance slipped from hands. the lance fell, striking them on the back of the head, causing a laceration, requiring 6 staples to close. Employee returned to work with no restrictions.

While climbing down from the Vac Truck, employee rolled right ankle. Ground condition was fairly level. Reported to supervisor, got ice pack and was sent to OMP for further medical evaluation. X-ray showed fracture to 5th metatarsal. was placed in a protective boot and returned to work on Restricted Duty. Ortho ruled out need for surgery

Employee was walking behind BIC packer when employee slipped and caught self on the handrail. When employee caught self, employee injured R arm. Reported to supervisor & was sent to clinic for med evaluation. Suspect tear to distal bicep. Placed in sling & returned to work with restricted duty. Scheduled for out patient surgery. Restricted duty only.

Employee was moving pallets of empty bags up to the bag floor using the 506 freight elevator. EE made three loads with no incident. When EE closed the interior gate, EE felt a pop in the R elbow w/pain. Reported to supv. taken to clinic, diagnosed w/elbow strain. Placed on restricted duty (1 armed) until follow up 3/24.

Operator trainee reported to supervisor EE was reaching overhead and felt a sharp pain in EE's left shoulder. Employee thought it might be a re-occurrence of an earlier injury sustained in Nov of 2019. Employee was referred to clinic, who recommended 2wks of physical therapy. Employee continues to work EE's regular job with no restrictions. Medical treatment only.

Employee was loading a 5 gal bucket(approx. 40#) into the back of an SUV. When EE did, EE felt a sharp pain in the right shoulder. Reported to supv., contacted Workcare spoke to an RN, advised treatment follow up 2/9 - slight improvement - follow up 2/10 at clinic, who recommended physical therapy and place on restrictions (one arm).

Removing liner plates inside mill. While using a hammer to free one of the plates a piece of steel hit employee in the forehead. Employee received 4 stitches to the head.

Employee was removing a taper lock bushing from a drive sprocket. Employee was using a pry bar to remove the taper lock, when the bar slipped, striking the tip of EE's right ring finger between the bar and an I-beam. Employee suffered a fracture to finger tip. Employee taken to medical professional, finger was splinted & employee placed on restricted (one-handed duty)

Employee was un-choking an airslide. When material started to flow suddenly, cement got behind EE's glasses & in R eye. Eyes were flushed, & EE was taken to a medical professional, given a prescription antibiotic. EE worked next scheduled day on restricted duty (clean environment) & was released to full duty at follow-up appointment the next day. (1 day restricted)

Employee was installing an inspection cover on #2 Plattco valve when EE got clinker dust in EE's left eye. Flushed eye onsite, taken to medical professional, given prescription antibiotics as a precaution, returned to work on restricted duty (clean environment) until follow-up appointment. Returned to regular duty at follow-up. 2 days restricted

Employee was preparing to start EE's shift. As EE was stepping over a curb, EE tripped & landed on EE's right knee. Employee was taken for medical evaluation, placed on Restricted Duty 5/10, & scheduled for MRI. MRI revealed torn quadriceps, scheduled for surgery. First day lost time 5/29

IE. was shoveling clay fines from tailwheel of B-11 belt. IE was working parallel to wall, throwing material in front of EE, so EE was not twisting EE's back, nor was material wet or in large chunks. EE was working at a moderate pace about 15 mins, when EE felt a sharp pain in EE's lower back. Taken to phys., placed on restricted duty, released to full duty 2 days later.

employee was removing end cap bolts on a kiln trunnion w/ a 2-1/4" box end wrench. As EE tugged on wrench, it came off the bolt head, pinching employee's R hand between the wrench and a pipe. The wound was cared for onsite, employee taken to ER & received 10 stitches. RTW on restricted duty. IE was wearing heavy leather gloves, wrench & bolt head were in good condition.

Employee was using an Oxytherm torch to cut a section of pipe. Unbeknownst to employee, the trigger was leaking oxygen, saturating the glove. EE's glove caught fire, and employee received 2nd degree burn to the right hand and finger. Employee seen by Dr., placed on restricted duty (one handed) until hand heals.

Injured employee was using an air lance to get material feeding. When material broke loose, it flooded out, knocking glasses off employee. Employee got dust in eyes and flushed them out thoroughly. Employee was taken to ER where eyes were flushed again and employee was given prescription eye drops as a precautionary measure. Return to work at full duty- Medical treatment only

Employee was returning a piece of expanded metal to the steel rack. As EE slid it in, the metal hung momentarily, then sprung back, striking EE's R forearm. EE received a shallow cut to EE's arm. EE was taken to OMP where EE received a few stitches to close wound & returned to work at full duty. Medical Treatment only.

Supervisor observing the cleaning operation of an air compressor radiator when dust got behind EE's safety glasses and in the right eye. Eye was flushed out and EE was sent to clinic for evaluation. Given a prescription for Ethromycin ointment as a precaution. Return to work with no restrictions. Medical treatment only.

Injured employee was troubleshooting a high differential pressure on a dust collector- when IE closed the door, caught the tip of right middle finger between the door & the frame, cutting the tip.(did NOT get bone)IE was wearing leather gloves at the time. Reported incident, taken for medical treatment, placed off work 1 day, & returned to work on restricted duty.

Employee walked out of building into the rain and slipped on wet and muddy pavement and broke lower leg

Employee was working on a dust collector, unchoking a hopper when material started flowing out the poke hole as they attempted to close the lid, dust was ejected and got in their eyes. Eyes were flushed on site, employee taken to Dr and given prescription antibiotics as a precautionary measure. Employee returned to work with no restrictions- MEDICAL TREATMENT ONLY.

Employee was operating a forklift, moving refractory material in and out of a kiln. While backing out of the kiln, the wheels slipped at the transition of the brick ramp. Employee made several attempts to back the lift up and on one of them, jarred their back. Reported incident to Supervisor, taken to DR, diagnosed W back strain, placed on RD 2/16, Rtn to full duty 3/5.

Injured employee was removing a 2" pipe plug from a line. The wrench slipped, and the employee fell backwards. EE threw EE's hand out, striking the backside of EE's left ring finger on the corner of an piece of angle iron. I.E. was seen by physician, treated for a broken finger and placed on restricted duty.

IE was rolling a piece of switchgear on steel casters. When IE wanted to turn it 90 degrees, IE shoved against the unit with IE's right knee. IE felt a pain & was taken to ER seen & released. On a follow-up visit the following day, IE was diagnosed with knee strain & placed on restricted duty until a follow up on 10/17 when employee was released to full duty

Employee was walking up steps to the OH Crane. (steps were 4 flights of 12 steps ea.) Employee stated when ee got to the top ee felt a "pop" in R. calf. Employee was taken to physician, diagnosed w/a possible strain of the gastrocnemius muscle, placed on restricted (sit down duty until follow up. Employee stated: didn't trip or stumble & there were no problems w/ the stairs.

Employee was removing a nut holding a crossbar for a ball mill hatch. When EE loosened the nut EE's finger got pinched between the bar and the nut. Employee was taken to physician, received 4 stitches and placed on restricted duty.(No forcible grip with right hand, no use of vibrating tools with right hand)

Employee was working on fuel transfer pumps when EE started to feel weak, dizzy & nauseated. EE went to cool down, took a sip of water & started vomiting. Employee was taken to medical professionals, received treatment for heat exhaustion & was instructed not to report to work the next day. Employee returned to work EE's next scheduled day with no restrictions. 1 day lost time

3 employees were to clean up an outdoor parts storage area. Prior to the job, all employees were cautioned to stay away from poison ivy in one portion of the area. Employee developed a light rash on forearms, neck and face. Employee taken to MD, was given a one-time prescription Prednisone and returned to full duty. First Aid Only, No days lost or restricted

EE was cleaning preheater riser duct. Some of the material blew back, hit EE's face shield, & got under the collar of EE's PPE, causing a burn on EE's collar bone. First aid was administered, employee was taken to OMP for medical evaluation, where EE was giving a prescription for Silvadene and advice to keep wound clean dry and covered - Returned to regular duty. Medical treatment.

Supv. was showing contractor details of work to be done later in the day. When Supv. looked up & to the right, crews removing dust collector bags knocked material down, getting cement dust in Supv. eyes. Eyes were flushed at nearby eyewash station & employee was taken to medical professional for evaluation. Placed on restricted duty (non-dusty environment) for 7 days

Employee reported at 11:30, ee bent over to pick up a 14" pipe wrench. When ee straightened up, ee had some back pain. Employee stated that ee had a pre-existing back pain issue (that was NOT work related) and this may have aggravated it. Employee was taken to medical provider, examined, and placed on restricted duty (no lift, push, pull>10 lbs) until re-evaluation (4 days)

Employee was working under 2D stone hopper, breaking up lumps to get material flowing with a poke bar. A large chunk broke free and struck the bar, causing the end of the bar to strike employee's chin. Employee was taken to physician, received 7 stitches under chin and returned to work with no restrictions - Medical treatment only.

Lab tech was taking out trash at the end of the shift. When EE threw a 2 gallon bucket of clinker(<20 lbs.) in the side door of an eight yard dumpster, EE felt a pull in the left side of EE's stomach. EE was taken to the ER for medical evaluation, placed off work until a follow up with regular physician, when EE was placed on restricted duty. 1 day LT, 10 day RD

Employee locked out disconnect, removed guard and was removing belts from a dust collector fan by rolling them off the sheave. As was rolling a belt from the sheave EE caught glove. When EE jerked their hand back by reflex, found the belt had sheared the tip of left index finger off. EE was taken to physician, had the tip sewn up, and placed on restricted duty.

A Rock was kicked up from under a moving forklift resulting in employee being struck in the face/nose.

Employee stepped down from 773B water truck access ladder and stated when their foot hit the ground it felt like they had strained their right knee. Ground was level and smooth in the area. Employee was taken for medical evaluation and placed on restricted duty on 9/3, released after follow up visit 9/14

Employee injured when boom lift overturned. Employee was coming down for break when EE drove the lift too close to sloped area causing the lift to overturn. EE sustained serious cut to facial area and other unknown trauma.

Employee was unchoking silo 27 and got cement dust in eye. Flushed eyes thoroughly at the plant and was taken to physician where employee was given a anti-biotic ointment as a precautionary measure. Employee returned to work with no restrictions. Medical treatment only.

Employee was fueling an air compressor when employee slipped off of the second step of a folding ladder, landing on left hip. Rainy conditions may have contributed to the incident. Employee was taken to physician diagnosed with a bruised hip and possible lumbar strain placed on restricted duty.

Employee was clearing lumps from a rotary feeder. In order to have manual control of the feeder the linkage was disconnected, when the employee reconnected the linkage ee caught left thumb in the clevis, causing a laceration. EE received stitches to close the wound, and was placed on restricted duty

Lifting oil cooler with crane through engine compartment - collar on the cable became hooked on tin ware, slipped allowing part to swing striking technician right below the right ear causing contusions

Electrician was checking grounding and rotation on a submersible pump. When employee jogged the pump, the reaction caused the pump to start to fall. When employee reached to stabilize pump, pinched hand between the pump and the discharge pipe. Employee received a hairline fracture to a bone in hand, and was placed on restricted duty

Employee was using a chain fall to rotate a plate on a ball mill. EE had put the hook on bolt & used left hand to hold the hook in place. As ee put a strain on the chain fall, the hook slid up bolt, pinching palm between the bolt head and the hook. Employee was taken to a phys. who cleaned and dressed wound, and placed employee on restricted duty

Employee was unchoking a vessel, when material fell down on the inside striking the bar EE was holding. The bar kicked up striking the nail of EE's little finger. Employee reported incident and was taken to OMP where a subungal hematoma was drained and an antibiotic was prescribed. Employee returned to work with no restrictions

Employee was operating a walk-behind skid steer cleaning material under a dust collector. EE ran the machine under a screw conveyor housing and when EE raised the bucket, the machine lurched striking EE's hand between the control bar and the conveyor housing. Employee was taken to OMP for medical evaluation, no fracture, but splinted thumb and placed on restricted duty

Employee was using a 40 lb electric jackhammer to break up material in the cooler. EE bent over to move some material, and as EE stood up, EE felt a pain in lower back. Employee reported incident to supervisor, was taken to OMP for medical evaluation, placed on restricted duty( no lift push pull>15 lbs, no jackhammering until re-evaluation.

Employee was traveling down an interior stairwell, when EE missed a step and rolled ankle. Employee was taken to Physician for medical evaluation, diagnosed w/ a sprained ankle placed on restricted duty. EE was released to full duty 7 days later. Stairs were clean, dry, well lit, and in good condition. Not a contributing factor. Employee stated EE just missed a step.

Employee was passing refractory brick to the bricklayers for approximately 4 hours when EE started to experience pain in right forearm. EE reported the incident to their supervisor, and was taken to OMP for medical evaluation. Employee was placed on restricted duty (no lift/ push/ pull with right hand until recheck) Returned to regular duty 4/6/2016

employee was using an 18" adjustable wrench to remove bolts from an 8" pipe flange. As EE pulled on the wrench to loosen a bolt, EE felt a pull/ sprain in shoulder. the wrench did not slip. Employee reported incident to supervisor, was taken to OMP for medical evaluation, and placed on restricted duty (no lift, push, pull>10lbs, no prolonged overhead work

Employee was exiting passenger elevator while holding a 6 ft stepladder, when EE stumbled and fell, landing on left knee and side. Employee reported incident to supervisor, was taken to OMP for medical evaluation, was placed on restricted duty (no lift push, pull,15lbs, no squatting or kneeling until re-evaluated, returned to full duty 2/29

During a clinker breaker choke, employee climbed fixed ladder to check load on cooler. When EE stepped down, he stumbled & fell against railing, hyperextending his little finger. Was checked by physician-possibly flexor tendon rupture. Regular duty initially, expect restricted duty after surgery-2 days restricted prior to surgery, 8 days lost after surgery, RTW w/restrictions.

Employee was walking beside a railing, looking up when he struck his knee on the mid rail. Did not think it was significant and continued to work. Reported the incident the next day and was sent to Dr. for medical evaluation- placed on restricted duty. Worked 2 days & knee retained fluid, causing employee to miss 3 days work. Employee back to work under restrictions- doing well now

Employee was in change house at the start of the shift, tripped over a bench, stumbled & fell. Didn't report incident to supv. Later that day, he mentioned it in passing, but no incident report taken. He was off the next day, worked the next day & then went to ER -Employee was placed on restricted duty 9/16(no OH work, climbing or twisting until recheck)Rtrn to reg duty 9/29

Employee left control room and went down the stairs. At the bottom of the stairs, he stumbled and turned his ankle. Steps were clear, no trip hazards present. Employee reported incident, was taken to dr. placed on restricted duty (limited standing and walking) 6/24.

Employee was troubleshooting palletizer unit feeder forks. He locked out unit and when he released the brake on the mechanism, the feeder fork frame struck the side of his head. Employee received a cut to the scalp requiring stitches, was placed on restricted duty 6/23 returned to regular duty 7/2.

Employee had just reported to work. As she pushed open the door, she felt a pain in her R middle finger. Reported incident to supervisor. Wound was cleaned, employee sent to dr. who removed a small metal sliver and gave her 2 stitches. Employee placed on restricted duty (left handed)6/22 Door was inspected and no sharp edges found. Employee released to reg duty 7/1

Employee was packing cement on the grayline packer when a problem downstream stopped the Ashworth belt. This created a pile-up of cement bags. In the process of straightening the bags out, the back of the employee's hand struck a saddle, causing a small laceration on the edge of his hand. Employee was sent to Dr- he received 2 stitches and returned to work with no restrictions.

The employee was working outside pulling wire near an SMCR Pump skid. A bee snuck into the arm pit of the employee's shirt and stung the employee. Employee had to use a prescription epiPen for allergy.

Employee was attempting to take apart a quick-release fitting on a 3/8" airline. Employee depressed lock collar and pulled on tubing. Tubing released unexpectedly and employee struck bag saddle creating a small laceration to right little finger. Employee taken to Dr, received 3 stitches & placed on restricted duty(limit use of right hand) Released to full duty 4/29.

Employee experienced sharp pain in stomach while handling 50# bags mixing pigment. pain was short term and faded quickly. employee was taken to Dr. who placed employee on restricted duty (no lifting >10#,bending,twisting, squatting) as a precautionary measure until recheck 4/6 - Employee was released to full duty 4/6

Employee was climbing ladder to access 988F loader when he lost his grip and fell backwards to the ground. Ladder was in good condition, but employees boots had mud on them. Employee was taken to Dr and placed on restricted duty (No climbing, lift, push, pull > 5#) Employee released to full duty 4/6.

Employee was lifting 55# buckets of refractory into a cement mixer. While lifting a bucket up to dump in the mixer, he felt a pain on his left side between his mid side and back. Employee was sent to Dr and placed on restricted duty (no lift, push, pull > 10#) until recheck on 3/17. Released to reg duty 3/17.

Employee was removing refractory inside the riser duct. he was standing on decking supported by scaffolding installed by an outside scaffolding contractor. when he handed a tool to the next level he stepped through a hole in the decking, scraped his leg and hit his R thumb on the scaffolding. Employee was taken to Dr. placed on Restricted duty(no gripping w/R hand) Released 3/31

Employee was watching for frozen chunks of clay to prevent them from getting into the clay crusher. He tried to lift a large chunk and felt a pull in his abdomen. Was taken to Dr.& diagnosed with a umbilical hernia. Returned to work on light duty until surgery could be scheduled. Date of injury 2/26,Restricted duty2/27-3/2, (3dys RD)First lost time day 3/2

Employee was attempting to loosen a 1/2 pipe from a valve. He braced the pipe on his thigh in order to apply pressure with a crescent wrench. As he applied force, he felt a pain in his lower right side of his abdomen. He was taken to Dr., who placed him on restricted duty until follow up

Employee was working inside of a dust collector hopper repairing a conveyor screw. When he went to turn around, his foot was caught in the flights of the screw. As a result, he twisted his right knee. He was taken to a Dr. for evaluation, and placed on restricted duty (on 2/5)(no repetitive kneeling, squatting or bending) until follow up appt. (2/13)

Employee was standing in the area around a ball mill when the mill started, dust fell from above, getting behind employees safety glasses and in his eye. Employee went to a nearby eyewash station, rinsed his eyes out thoroughly, and was sent to a Dr. for evaluation. There he received prescription antibiotic eye drops.

EE was working around the Clinker Cooler Dust Collector Heat Exchanger screws. Winds were 10-15 mph with gusts to 20mph. As the employee turned, a gust of wind blew dust in his eye. Employee was wearing prescription safety glasses with side shields. His eyes were flushed immediately, he was taken to dr. as a precautionary measure. There he was given prescription eye drops.

Employee was removing the front panel on a heating register in an office. when he removed the cover, he felt a "pop" in his right arm. He reported it the next day, was sent to physician, who placed him on restricted duty (No lifting w/ R arm, may do light work w hand) until follow-up appt. Employee was released to full duty at follow-up appt.

Employee was operating a 988F loader. As he was cleaning the floor of the clinker building, the bucket hit an obstruction, causing the loader to come to an abrupt stop, throwing employee against his seat belt. Employee reported it the next day, was seen by a physician, & placed on restricted duty over the weekend. Employee was released to full duty on Monday

Employee was cutting a piece of corrugated plastic hose, holding it horizontal with his body and one hand. The other hand was making a cut around the plastic hose and when he came around to the other side it cut quicker than he anticipated. The knife cut through his leather glove and gave him approx. a half inch cut between his thumb and index finger.

Employee was pumping lime and pipe got choked up. He shut blower off and noticed one ear was loose on a cam lock fitting. When he tried to secure ear that was loose the other ear came loose and hose came off covering employee and area in lime dust. Employee got lime dust in eye, immediately flushed eyes for 15 min and went to opthomologist for further treatment.

Employee was working in the undergrate compartment of #2 cooler. Access to this area is thru a 4' square door. When employee entered the compartment, as he stepped forward and stood up, he hit his head on an X brace. He was wearing his hardhat, but felt a "pinch" in his neck. EE was taken to Dr.,who placed him on restricted duty (no lift,push,pull>10lbs)until recheck on 7/28/14

Employee was re-stacking 74lb bags of masonry cement that had shifted on a pallet. Employee started to notice a pain in his right shoulder, reported it to his supervisor. Employee was taken to physician, who paced him on restricted duty (no push, pull or lift >10lbs) until re-check.

Employee was using a skid steer loader to clean material from ground in finish room. As he was scraping the bucket contacted hardened material, stopping machine and throwing employee against safety bar. Employee was wearing seat belt. Employee was diagnosed w/bruised ribs and placed on restricted duty.(No lift, push pull>10lbs.

Garage mechanic was working on Cat 988G front end loader, he was using a two-foot step ladder for access. When he stepped down from the ladder, he stepped on a rock and twisted his right knee. He was taken to physician for evaluation, with a follow up after the weekend, after the follow-up visit he was placed on restricted duty-No climbing, squatting or kneeling

Employee was attempting to get material flowing from CKD tank. He was using an air lance to dislodge material. The material broke loose, employee was struck in the top of hardhat & chest with dust. Employee was wearing safety glasses, dust mask & face shield. Employee was given antibiotic eye drops & instructed not to report to work the following day. Employee was off work two days

Employee had just made a splice to a conveyor belt. As he was tapering the edges of the belt at the splice, his hand slipped and he cut his left middle finger with a utility knife. He was taken to a physician where he received a few stitches to close the wound. Employee returned to work with no restrictions.

Employee was passing brick in the kiln. As he reached down to get one of the last bricks on the pallet he pulled his stomach muscle. This employee has a medical condition (rectus diastasis- a separation of the abdominal muscles) which likely contributed to the outcome of this incident. (The condition was unknown to Essroc Management prior to the injured being seen at OMP

was NOT an accident. Bricklayer laying brick in kiln, reached for anchor brick and dislocated shoulder. He did not twist or bend to cause an injury, it is a chronic condition he has had happen previously. He went to ER to get shoulder reset, returned to work with no restrictions.

Employee was passing refractory brick to bricklayers during a kiln outage. As the day wore on, he started to experience pain in his abdominal muscles. He reported it to his supervisor, was sent for medical evaluation. Employee placed on restricted duty (No lift,push,pull>10lbs) until re-evaluated 3/31/2014

Employee was using a hammer and chisel to clean material build-up from the flights of a pump screw. He missed the chisel and struck his left index finger with the hammer. Employee was wearing leather gloves at the time.Was sent to physician, x-rays were taken, no broken bones, no torn skin. Physician placed on restricted duty until recheck 1/20, then released to full duty

Employee was using a knife to slice air hose lengthwise. Rather than laying hose on a bench to slice it, he held it in his left hand; the utility knife cut his palm. The wound was cleaned and dressed, & employee received several stitches at the base of his thumb. Returned to work-no restrictions. Note: discovered incident n/reported in July after review of incidents in Oct.

Affected employee was holding a ladder for another employee. When the first employee handed some tools down to the employee on the ground, dust got behind the employees safety glasses, and in his eye. Employee flushed his eyes out and was fine. Sent employee to company physician and he was given antibiotic eye drops as a precautionary measure. Returned to work with no restrictions.

Employee was handing brick to bricklayers. Later that day, he noticed a knot on his right wrist. He reported the condition to his supv.when he came in for his next shift, was sent to OMP for medical evaluation, placed on restricted duty (no right hand work for 2 days,evaluate,light right hand duty for 9 days)11 calendar days total restricted-

Employee was operating forklift, dumping a skidbox. When he exited forklift, he stepped down on a crack in the concrete pavement, spraining his right ankle. He was seen by physician, diagnosed with a sprain with a fibular fracture, and will likely be off work for 10 working days, with some restricted duty after that.

Employee was using an adjustable wrench to loosen a 3/4" bolt. The bolt came loose suddenly, allowing the wrench to strike the employee's right thumb and wrist. No broken skin or bruising, but employee was seen by physician and placed on restricted duty-no lifting over 15lb w/ right hand

Lubricator had just checked oil on vent fan bearings. He climbed down the vertical access ladder and was standing on an 11" high concrete pad at the base of the ladder. He turned to leave, his foot rolled off the corner of the concrete, causing him to fall, landing on his elbow. He received a cut which required 3 stitches, was placed on restricted duty for 6 days

Employee was attempting to remove an actuating arm from a shaft on the triple gate feeder for the raw mill. He was prying the arm off with a 5' pry bar. During the course of removing the arm, the employee felt a slight pain in his right shoulder. Reported it to his supv., was taken to OMP, evaluated and placed on restricted duty (one armed)

Employee was at water filter plant, priming the water pump. The bleed valve is about 7' overhead. Employee chose to stand on motor to access valve. He lost his footing, fell, striking his arm on the motor, cutting his forearm. Employee was taken to OMP & received stitches on his arm & placed on restricted duty. Employee was disciplined for violating Co safety policy on WAH

Employee traveling on foot, stumbled over 1/4" checker plate covering opening in floor. When he stumbled, he pulled or strained muscle in lower back. Plate was flat, but had thin layer of dust on it making it harder to see. Employee sent to physician, placed on restricted duty for 5 days, released to full duty.

Employee was cutting nylon ties securing electrical cable to beam. Due to location, employee was cutting in a downward motion towards opposite hand. When cable tie severed, tip of knife struck employees left thumb. Wound was cleaned, dressed. Employee received two stiches. Put on restricted duty (no squeezing-l hand).

Employee was attempting to clear a bag of portland cement from in front of a photo eye that controlled the belt conveyor feeding the palletizer. LOTO procedures were followed and when employee lifted bag, he felt a twinge in his lower back. He reported it to his supervisor, was sent to physician, diagnosed with lumbar strain and placed on restricted duty.

Injured employee was holding an extension ladder for another employee.(rung latches were not fully engaged) When employee put his weight on the ladder, the ladder started to collapse, pinching injured employees right thumb between the first section and the fly section of the ladder. Employee suffered laceration to right thumb, received stitches, and placed on restricted duty.

EE using 16' extension ladder(extended out two rungs-10'overall)to access a cover plate on mill feed pipe. EE was on 2 or 3rd rung of ladder (approx 2'-3' high) Rung lock wasn't fully engaged, ladder slipped, EE fell to ground. Sent employee to phys. for evaluation-was placed on restricted duty-no ladder climbing, heavy gripping with index finger. will re-evaluate 3-23

Monday, March 12, employee was experiencing lower back pain attributed to heavy lifting & pulling he had been doing installing electrical conduit and wiring over the past few months. Employee was sent to physician, diagnosed w/lumbar strain & placed on restricted duty-no lifting pulling, pushing>20lbs. No specific incident to cause pain, rather a cumulative effect of exertion.

Employee was cutting a steel plate. While he was cutting the flame of the torch stirred up dust or scale, which got behind his cutting goggles and in his right eye. eye was flushed on site, employee was sent to physician to be sure we got it all out. physician confirmed all clear and prescribed antibiotic eye drops. employee returned to work at regular duty.

Employee was cutting drag bars out of a chute. Space was small and cramped, so he was bent at the waist to gain access. He felt a pain along his belt line and reported it to his supervisor. Employee was sent to physician-diagnosed as a possible pulled muscle. Placed on restricted duty 2-23(no lifting over 30 lbs)Rechecked on 2-28: released to full duty

Over a period of days, employee was jack hammering hard material to remove it. Employee experienced pain in his right elbow area.

Employee was welding 2D cement line at Silo 59. Employee was holding welding hood in his left hand (hard hat did not have clips to attach hood) and started welding with his right hand. He was having trouble getting the weld to start because pipe was dirty. While trying to get the weld to start the employee flashed himself at least 2 times.

Employee tripped over a pallet of material in the Brick Shed and fell backwards to the ground, striking his left elbow on the ground, resulting in a laceration to his elbow requiring 4 stitches.

Climbing into mill from ground, employee reached the top few ladder rungs when the ladder slipped out from underneath him. Employee caught himself from falling through the hole by having his arms out to the side, bracing himself. He felt pain in his left shoulder as a result of the sudden impact.

Employee was completing a job using the vacuum truck. As she was returning to the truck she caught her foot on the vacuum truck hose and safety valve resulting in her falling to the ground, striking her right hand and right knee and landing on her right side.

Employee was crossing a belt to access a work platform on the other side of the belt. This platform is rarely used; work on this belt is normally done annually. As employee was stepping down, his coveralls caught causing him to readjust his landing and roll his right ankle resulting in a sprain to his right ankle and foot. Belt was locked out at time of incident.

Employee was preparing to remove a large piece of metal from a compressor room and removed the safety bar across the back of the door in order to open the door and removed the metal. The employee proceeded to step through the doorway without recognizing the 2 foot drop to ground level and fell through the opening landing on the ground below on his right side.

Employee was walking from his vehicle to the locker room before the start of his shift. He slipped on ice and fell backwards, striking his head, neck and shoulder on the ground.

Employee was working in clinker cooler area and dust blew into his right eye.

Employee struck left shoulder on stationary piping resulting in shoulder, cervical and taperzius strain.

Employee stepped down from a 19" work platform and twisted his left ankle.

Employee tripped over the leg of another employee and injured his right knee.

Employee was working overhead cleaning out 2D dust collector pipe, when dust/material blew into his left eye.

While replacing bearings and buffing a fan shaft, employee felt something in his left eye. Prior to that the ee had been using a bench grinder on part of the fan

Employee was packing cement bags on packer when one of the bags burst resulting in the employee getting cement in his eyes.

Employee was exiting a Bobcat loader when she slipped on ice and snow on the foot step of the Bobcat, resulting in a sprain to her right knee.

Employee was welding from an awkward working position inside a feed chute. Employee took a scheduled break for lunch and when he was returning to the job site he felt a sharp pain in his hip and leg.

Employee was in the process of removing the center differential from a 988 Loader. As he removed the guide bolts, the section moved forward slightly, catching his hand between the impact wrench and the frame of the machine.

Employee was prepping trunnions for installation on Kiln #2. While walking in the work area the employee tripped and fell over a cross tie that had been put in place to keep the trunnion from moving. The fall resulted in injury to his left wrist.

Two employees were welding on the screw in the shop. The next morning the employee had pain in his eyes. Diagnosed with welder's flash burn by physician.

Employee welding at the 495 belt chute, working off a ladder with steel beam above him. He raised up on ladder and hit the beam, compressing his neck.

Employee was pulling up a 36" by 24" piece of 1" used grating from the incline belt walkway and he felt a pain in his right hand.

Employee walking from parking lot to locker room for start of his shift. As he tried to step across a stream of flowing water with his left foot he felt a pain in his right knee and was unable to walk without keeping his knee straight.

Employee installing muffler on gas engine of service truck. Truck bed is about 4 ft off ground, employee used set of rolling stairs to access the truck bed. After changing muffler, employee was descending stairs, slipped and slid down bottom 2 steps grabbing the right side handrail and hitting elbow and knee on handrail.

Employee repairing door at East 2D FK Compressor. While kneeling on left knee he was scraping cement debris off hinge and frame. When he stood up he felt pain in his knee and could feel stiffness from the swelling.

Employee was tying up sagging cable under catwalk along #1 kiln. While tightening the wire, some material flaked off and fell behind his safety glasses into his left eye.

Employee trying to remove large chunk from 2D gypsum hopper above scale belt. He was using a sledge hammer and jackhammer bit as a bar. While hitting the bit to break up the chunk, a fragment broke off the bit penetrating the employee's right arm. *THIS INJURY NOT REPORTABLE UNTIL FOREIGN PARTICLE SURGICALLY REMOVED ON 7/28/09 AND EMPLOYEE PLACED ON RESTRICTIONS.*

Employee doing cleanup of accumulated piles of material at #2 Cooler Fan area using skid steer. When he was exiting the skid steer, he stepped into bucket and felt pain in ankle. He commented that he had strained his ankle but did not report incident. Ankle started swelling overnight and incident was reported the following morning.

Employee removing rock from screen deck in Quarry, when rock from hopper above the screen came down and hit thumb on left hand, catching it between two rocks and cutting it.

Employee replacing glass inspection window on horizontal drive of #2 cooler. While trying to separate glass from the metal frame, employee tapped glass with wrench causing glass to flip out of frame and cutting him on tip of finger.

Employee using air lance to clear material from 2C feed chute & was in process of using sledge hammer to get clinker to move. The head of sledge hammer slipped on the chute, causing hand to come in contact with feed throat of mill. Smashed right index finger taking off nail, splitting end of finger and breaking bone at tip of finger.

Employee using 12' ladder to remove bags on AAF #3 room. Ladder was against 2 bags. Ladder spun left and employee fell on left side 2 rungs up ladder. Dust was knocked onto employee, hardhat and glasses fell resulting in dust getting in employee's eyes (primarily left eye).

Employee exiting backhoe, as he was stepping to the ground he slipped on ice, falling and hitting his left shoulder and right thigh.

Ice had built up on steps and handrails at west side of warehouse. Salt had been put down earlier, but with the amount of ice and sleet it was hard to keep up with removal. Employee fell and hit his right elbow causing swelling and soreness. Employee did not seek medical treatment until 1/30/09.

Employee using 2 pipe wrenches to loosen coupling on frozen pipe line. Working overhead on ladder and felt pain in his stomach area.

Employee was tightening a lateral line on North Filter Bed using a pipe wrench. The wrench slipped, smashing his left index finger. He broke the tip of the finger and required 6 stitches on underside of his finger to close laceration.

Employee was working overhead jackhammering out old refractory in No. 2 Cooler. Discharge air from hammer blew material back toward employee and dust got past safety glasses and faceshield and into left eye.

Employee raised bed of CAT 777 to allow access to fender. Raising the bed causes the front of truck to sit 4-6" higher. When employee stepped down he felt twinge in his right knee. *THIS INCIDENT WAS NOT REPORTABLE UNTIL 11/19/2008 WHEN EMPLOYEE ASKED TO SEE DOCTOR FOR PAIN IN RIGHT KNEE. HE WAS PLACED ON RESTRICTED DUTY AS OF 11/19/2008 WITH RIGHT KNEE STRAIN.*

Employee was greasing loader at start of shift, grease gun stuck on one of the grease fittings and when he pulled it off his elbow hit the edge of the bucket. ***** THIS INCIDENT WAS NOT REPORTABLE UNTIL 11/12/08 WHEN EMPLOYEE ASKED TO SEE DOCTOR FOR PAIN IN HIS ELBOW. HE WAS PUT ON RESTRICTED DUTY AS OF 11/12/08 WITH LEFT ELBOW STRAIN.****

While employee was troubleshooting 2D clinker scale, he laid drawings on frame metal & slide gate panel. Attempting to move the drawings with his left hand he hit a sharp piece of protruding metal causing laceration on left palm and outside edge of hand.

Employee punching lumps at Silo 47 and had installed check valve on one of the air lines of silo. He looked up towards the cone and bumped something to cause dust to fall. Dust fell between glasses and his face. Employee wears hard hat backwards because of welding and burning.

Employee was removing pipe from roll press oil cooler. He had wrench on fitting and when he applied pressure the nut broke loose and he hit his thumb on the pump base. Employee did not report incident until 8/29/08 and did not seek medical treatment until 9/8/08.

EMPLOYEE WAS STACKING BAGS IN THE COLOR PLANT. HE TURNED TO PLACE A BAG ON THE PALLET AND FELT A PULL IN HIS GROIN AREA.

EMPLOYEE LOOSENING A NUT WITH RATCHING BOX END WRENCH. WHILE USING A STEADY PULLING MOTION, THE NUT BROKE LOOSE AND HE FELT SHARP PAIN IN HIS LEFT MIDDLE FINGER AT THE JOINT.

Employee trying to close bulkhead on loaded boxcar. He was using drift bar to align holes with bulkhead and receiving bracket. Another employee using forklift to push bulkhead in. Drift bar slipped and struck employee's thumb on left hand.

EMPLOYEE ATTEMPTING TO CLOSE BULKHEAD ON LOADED BOXCAR. HE WAS USING DRIFT BAR TO ALIGN HOLE IN THE BULKHEAD WITH THE HOLE IN RECEIVING BRACKET. OTHER EMPLOYEE BUMPING BULKHEAD WITH HIS FORKLIFT TO PUSH BULKHEAD IN, DRIFT BAR SLIPPED CATCHING EMPLOYEE'S FINGER ON RIGHT HAND BETWEEN BAR AND BRACKET.

Employee was working in BIC stacking bags and strained his wrist.

#1 Can elevator chain had broken, using crane to pull chain out. When chain was out a counterweight was being pulled and employee's feet slipped and jerked his back and legs. Employee had cleaned grating down to flat surface prior to starting job.

EMPLOYEE CLIMBED ONTO BACK OF FLATBED TRUCK, INSTALLED PUMP AND WAS TRANSFERRING OIL FROM 2 55-GALLON BARRELS TO 24K CRUSHER. WHEN HE WENT TO STEP DOWN OFF TRUCK, HIS FOOT SLIPPED AND HE FELL INTO TRAILER HITCH ON BACK OF TRUCK. THE HITCH CAUGHT HIM ON HIS RIGHT SIDE RIB AREA AND RIGHT WRIST.

While working on south reclaim belt, employee knelt down and felt pain in left knee.

EMPLOYEE WAS UNCOUPLING A PUMP HOSE TO PUT WATER IN A DISCHARGE HOSE, WHEN HOSE CAME UNCOUPLED, HITTING EMPLOYEE IN HIS MOUTH BREAKING OFF PART OF FRONT TOOTH. ALTHOUGH THIS ACCIDENT OCCURRED ON 3/18/2008, WE DID NOT REALIZE IT WAS REPORTABLE UNTIL TODAY.

EMPLOYEE WAS WORKING IN A, B AND C MILLS DRIVING HEADS WITH SLEDGE HAMMER ALL DAY. PULLED MUSCLE IN HIS NECK.

Employee was welding and suffered welder's flash. Other employees welding behind him caused a flash from their side as well. Employee wearing welding hood which was not automatic and did not drop fast enough. Did not know he was flashed until during the night and reported incident at start of shift the following day.

EMPLOYEE UNHOOKED A PNEUMATIC CYLINDER AND IT FELL DOWN ON HIS LEFT INDEX FINGER, PINCHING IT BETWEEN A STEEL ARM AND THE CYLINDER.

EMPLOYEE USING SLEDGE HAMMER, WHEN SWINGING HAMMER LEFT HAND SLID DOWN HANDLE AND SPLINTER WENT THROUGH GLOVE AND INTO INDEX FINGER.

Employee was handling actuator around CKD system. This requires an awkward position in order to install. Employee strained his groin.

Employee picked up piece of plywood on top of stack approximately 5' off ground. He pulled plywood off stack and attempted to pick it up twisting his upper body at the same time. He then felt sharp pain in middle of his back.

Employee manually trying to adjust packing machine and pulled his back.

EE dumping the trash hopper from color mix floor. After dumping hopper, he was in the process of latching it back in place. Hopper will not latch without help. He was trying to latch the hopper back in place & missed on the first try. Hopper came back up & smashed his finger between hopper and the mast of forklift.

Employee was dumping pigment into the pigment tank and his right shoulder started hurting him.

EMPLOYEE ADDING TEMPORARY SUMP PUMP TO PUMP WATER OUT OF COAL SILO MCC ROOM. HE WAS STANDING ON PALLET TO AVOID STANDING IN WATER, SLIPPED OFF PALLET AND STRAINED HIS RIGHT KNEE.

Employee pulled neck and upper back muscles while stacking bags in BIC.

WHILE CHANGING LP TANK ON FORKLIFT, EMPLOYEE LIFTED THE TANK AND FELT A TWINGE IN HER BACK.

Working on the vac truck, hose sucked up chunk of rock and stuck to floor. Hose tangled up employee and threw her to the ground twisting her ankle causing her to land on her wrist.

EMPLOYEE EXITING OVERHEAD CRANE, LOST FOOTING ON STEPS, HIT KNEE ON GUARD RAIL.

EE WAS USING VAC TRUCK IN THE ABC TUNNEL. HOSE CAUGHT IN PILE AND JERKED FORWARD PUSHING HER THUMB BACKWARDS.

Employee was taking bolts out of the hopper door when his wrench slipped off of the bolt. When the wrench slipped the back of his left hand hit the barb and it went through his glove and through his knuckle.

Randy was moving a skid of spring plates, all but one were banded, and that one slid off. After setting the skid down he then took the lift and picked up the single one, while sitting it down on the others it bound up. He then got off the lift and went to free it, when it freed, it slid off the forks and pinned his right ankle between the plate and another skid.

Employee was removing a hose on the Cat 777D in order to change a leaking O-ring of the steering system. When he removed the line residual pressure in the steering tank caused oil to come out of the line covering him with hydraulic fluid.

Employee was climbing out of Loesche Mill and tripped, he tried to catch himself and bent his fingers backwards. **** This was not reportable until he was seen by the Dr. on 05/24/2007 and put on restricted duty.****

Passing buckets up & down to make a small pour.

Employee was using a hammer and chisel to remove an air pad from the simplex coal silo. A piece of the chisel broke off and went into his right arm.

Employee was working in the Loesche Mill area. A long steel choker and a metal rack were on the ground plus some loose rock and he slipped and fell.

While servicing a pick-up truck employee hit her left elbow on the lower control arm of the vehicle.

While putting clay and flyash into the clay grates some dust flew into employee's left eye. He washed his eye out and thought that he had removed the dust but on Wed. morning when he returned to work his eye was reddened and partially swollen so he was sent to the company doctor.

Employee was cleaning up around #2 cooler and clinker dust flew in her left eye. It was very windy conditions.

Employee was stacking bags in the BIC and felt a burning sensation in his back, he then felt a tightness in his groin area.

Employee was getting into the skidsteer loader and his foot slipped on the bucket. As he slipped he felt a pain in his back. ***This was not reportable until he was scheduled for physical therapy on 3/23/2007.***

EE was coming down the ladder on the loader, he slipped and hit his back and his right side on the ladder.

Employee was cleaning clinker off of the reclaim grates when the lip of the loader bucket struck the concrete around the edge of the grates. The loader came to a sudden stop and her right hand struck the gear shift jamming her right shoulder. ***THIS IS REPORTABLE BECAUSE SHE WAS SCHEDULED FOR PHYS. THERAPY.**

Employee was holding a grate for the clay grates while another employee used a 2lb hammer. The employee missed his mark and hit EE's right thumb with the hammer.

EE was dumping bags of pigment into the tank. He felt strain in his left shoulder and neck.

EE was using the Vac Truck in the apron. As he was pulling the hose around the tail wheel, the hose went into a pile snapping his wrist and thumb forward. He had a hold of the stick attached to the hose. ***This was not reportable until 05/30/07 when he had surgery.***

Employee was stacking bags in the Color Plant; as he turned to stack a bag he felt a pull in his left shoulder. ***This was not reportable until 12/26/2006 when he was scheduled for Physical T herapy.***

Employee hurt his right shoulder while pulling dust collector cages and bags out of #2 cooler dust collector. ***This was not reportable until he was given Physical Therapy on 12/05/2006.***

Employee was walking down the steps on the east side of the Finish Room at the apron conveyor. The steps had clinker on them and she slipped and fell breaking her right ankle.

Employee was assisting in taking guard off tailwheel of #1 apron conveyor. He was straddling a water puddle to cross over when the guard came loose and landed on top of his left foot.

EE was hauling clay in the CAT 777 haul truck. He took the truck on top of the clay pile and backed towards the southwest corner of the clay pile. Either when the truck stopped or when he went to raise the bed the face sloughed off under the weight of the rear wheels causing the truck to go down the face tail first overturning anc coming to rest upside down.

Employee was trying to get water on #1 water spray. The system has been worked on during shut down and some changes were made. They couldn't get water to flow through the system and had closed all the valves and were going to bleed the air out. The pipe plug was rusted and ee had injured unscrew the valve. The water sprayed on his arm and burned it.

Employee was inside #2 kiln removing the lower analyzer pipe. He and a co-worker picked up the pipe and his foot slipped and he felt pain in the middle of his back.

EE was removing tape from wires so that they could be reconnected to circuit breaker. He was using a knife to cut and remove the tape. In this process his knife slipped as he was prying on the tape and cut his left forearm.

EE was preparing to do a preshift inspection on a plant mobile man lift unit. As he was raising the lid on the engine compartment he felt a sharp pain and pop in his right shoulder.

Employee was in the process of emptying a sediment trap for the #2 kiln exit analyzer. Employee was extending his reach to open a dump valve and lost his footing straining his lower back and left side.

Employee was climbing the west fly ash tank ladder to measure the tank level, about half way up the ladder his feet slipped off and he caught himself straining his right arm and shoulder. ***WAS NOT REPORTABLE UNTIL HE WAS SCHEDULED FOR PHYSICAL THERAPY ON 06/26/2006.***

Employee was vacuuming out the 160 dust collector when she fell off the ladder, which was approx. 2 foot from the ground, and hurt her right arm and back. ***THIS WAS NOT REPORTABLE UNTIL SHE WAS SCHEDULED FOR PHYSICAL THERAPY ON 06/21/2006.***
