Employee was installing Roof Bolts when he strained his back pulling on the spinner.
Employee was working on a conveyor when a back plate that he was removing slipped and he tried to catch it injuring his shoulder. This injury was not reportable until after he had surgery to determine if it was in fact a work related diagnosis. His Surgery was performed on 3/10/06.
Employee was using a 2 lb sledge hammer to loosen a bolt when he hit his knee!
Employee was tightening bolts on a fan blade guard when he contacted the alternator with his wrist watch causing burns to his wrist. Employee did not see a Dr. until 2/28/04
Employee was rigging a head pulley for a lift when he was contacted by the boom of the lifting device!

Employee was walking under the boom of his drill while doing preshift inspection and bumped his head. He then stumbled catching himself and straining his shoulder.
Employees at Noblesville Stone pumped water out of floor section and found a large pile of rocks that had appearantly fallen out of the back section above it. To date, an investigation has been impaired by the water and mud and it is unknown whether or not the fall occurred above the anchorage zone. No airflow or escapeways are inhibited as a result.
Employee was walking around the end of the maintenance truck when he fell and cut himself on the belt knife blade that was open.
Employee was using a pry bar to remove blockage from a feeder. The bar slipped and struck him on his left knee.
Employee was using a Flexco Belt cutter when his hand slipped of the handle and he punctured his Left forearm on a nail head, requiring 3 sutures.

EMPLOYEE STEPPED ON A ROCK AND TWISTED HIS ANKLE.
Roof fall occurred near the west portal resting on the canopy in place to protect from such an event. Once the Canopy was removed the roof came down indicating that the fall had occurred above the full resin bolt anchorage zone!
EMPLOYEE WAS ASSEMBLING A HYDRAULIC HOSE IN THE HOSE ROOM WHEN HE STEPPED ON A BAD SPOT IN THE FLOOR AND SPRIANED HIS KNEE.
EMPLOYEE WAS USING A PRY BAR AGAINST HIS SHOULDER. HE OVEREXERTED CAUSING A CONTUSION.

EE WAS WALKING INTO EE LOCKER ROOM WHE HE SLIPPED AND FELL, STRAINING A MUSCLE IN HIS NECK.

EE WAS PASSING BY THE FACE OF N12 & Z WHEN HE NOTICED A 35 FT DIAMETER, 20 FT THICK SECTION OF THE BACK HAD FALLEN. HE IMMEDIATELY NOTIFIED MANAGEMENT AND IN TURN MSHA WAS CALLED IMMEDIATELY. THE AREA HAS BEEN BEAMED OFF AND A GEOLOGICAL ENGINEER HAS BEEN CALLED TO EVALUATE.
EMPLOYEE WAS WORKING ON A LARGE PIECE OF EQUIPMENT WHEN IT FELL OVER ON HIS LEG AND ROLLED OFF. HE WENT TO THE DOCTOR ON THE 14TH.
PULLING A FUEL TANK INTO THE MINE WHEN HE LOST CONTROL OF THE VEHICLE AND RAN INTO A PARKED CAT 773 HAUL TRUCK.
