Employee was walking an old beltline counting waterline joints when EE lost EE's footing and caught self by grabbing a belt chain causing a sharp pain in EE's shoulder. EE was first seen by a doctor on 11/06/2025 and continued working without restrictions until 02/04/2026 when EE was placed off work for an exploratory surgery to have EE's shoulder repaired.

Employee was cutting a piece of scrape belt to make DTI pads when the blade slipped contacting the employees left forearm resulting in a laceration. (1) lace suture was required to close the wound.

Employee was loading a shuttle car in the first turn of 7 right. They went to take a step back and lost footing and fell backwards striking their head, neck and upper back against the shuttle car resulting in an acute fracture of the T6 vertebrae. Employee is expected to make a full recovery without surgery being required. They started missing work on 08/27/2025.

While handing an angle grinder to EE's supervisor, EE inadvertently contacted the rotating grinding wheel with EE's left hand resulting in a laceration. (8) sutures were required to close the wound.

Employee was picking up a hollow block while building an intake stopping and felt a sharp pain in EE's lower back. EE was placed off work beginning on 08/14/2025 for a lower back strain.

Employee was moving miner cable over in order to hang when EE felt a sharp pain in EE's lower abdomen. EE was later seen by a doctor and diagnosed with a hernia. EE underwent surgery on 8/11/2025 and is expected to make a full recovery.

Employee inadvertently contacted the corner of the rib with the mast of the mini-trac causing a large chunk of coal to fall, striking the employee in the right hand. The employee sustained fractures and lacerations to EE's right hand. EE began missing work on 08/04/2025.

Employee was attempting to remove a rock that had become lodged in the tailpiece when they felt a sharp pain in lower back. Employee was seen by a doctor and placed off work for a lower back strain. They began missing work on 07/29/2025.

Contractor bent over to pick up EE's drill steel and when EE did EE's right forearm contacted a pin plate that was sitting on EE's tray resulting in a laceration. (14) sutures were required to close the wound.

Employee stepped awkwardly into a hole while hanging the feeder cable and felt a sharp pain in the back of their right knee. Employee was taken to OA and placed off work for a possible torn meniscus. They began missing work on 06/26/2025.

EE rolled EE's right ankle on a piece of loose coal while walking to the face to help the miner move. EE was taken to OA the next day where EE's x-rays revealed a hairline fracture. EE is expected to make a full recovery without surgery being required. EE began missing work on 6/24/2025.

The injured miner was struck in the right ankle by a shuttle car trailing cable as it was being wound onto the cable reel resulting in a fractured ankle.

Employee was using an axe to separate a coupler from a waterline when a piece of the coupler broke off and struck employee in the safety glasses resulting in a bruise to their left eye. Employee was seen by a doctor and prescribed eye drops.

While dragging a piece of line curtain, employee twisted right knee and felt something pop. Employee was seen at OA and was diagnosed with a torn meniscus. They continued working for several weeks with no restrictions. However, it was later determined that surgery would be required to repair the knee. They began missing work on 7/7/25.

While bolting a header hole, was retrieving EE's drill steels from the mine roof when EE's starter steel fell out of the hole, striking EE in the left middle finger causing a laceration. (5) sutures were required to close the wound.

While adding cable to a roof bolter a piece of wood from the wooden spool the cable was one got embedded in EE's left ring finger and needed 5 stitches to get out.

While reclaiming waterline, EE felt a sharp pain in lower back when EE was attempting to remove one of the couplers from the waterline. EE was seen at OA the following day and placed off work for a lower back strain. EE began missing work on 1/21/2025.

While on overtime, EE was throwing the miner cable over out of the haulage way when EE felt something pop in EE's right knee. EE continued working until EE was placed off by a doctor on 12/12/2024 for a possible torn meniscus.

EE was installing EE's middle roof bolt when a rock fell from the mine roof and struck EE in the lower left leg resulting in a small laceration. (2) sutures were required to close the wound.

The employee was cutting a bolt off a pump using a grinding wheel when the wheel slipped and contacted the employee's left ring finger resulting in a small laceration. (5) sutures were required to close the wound.

Employee was replacing a blown hose on a roof bolter when a rock fell from the mine roof and struck EE in the right shoulder resulting in a dislocation. EE began missing work on 11/25/2024.

While carrying a stack of pin boards to the bolter, EE felt a sharp pain in EE's back. EE continued working and was later seen by a doctor on 10/17/2024 where EE was placed off work for a back strain.

While installing a roof bolt in the corner of #5 entry on unit #3, a rock came out and struck the contractor on the wrist causing a small laceration. (4) sutures were required to close the wound.

EE was walking in the intersection of #4 entry on #3 unit when a rock fell from the mine roof and struck EE in the right leg and knee. EE was placed off work by a doctor beginning 7/24/24 for a sprained knee.

EE slipped, lost footing and hit chin on the handrail of the scissor lift resulting in a small laceration. (3) sutures were required to close the wound.

During the weekly hoist exam, a broken strand was discovered on the Man/Material cage hoist rope. The hoist was immediately removed from service and the condition was reported via phone call to the hotline at approximately 9:47am on 7/12/2024.

While walking back to the scoop, the employee stuck EE's head on a drill steel that was stuck in the mine roof. EE continued working but began having headaches and was taken off work on 7/11/2024 by a doctor for a possible concussion.

While pulling up miner cable with the mini trac, a rock fell from the mine roof and struck the employee in the right hand causing a fracture to EE's right index finger. EE was seen by a doctor at OA and placed in a splint with a full release.

EE was getting out of EE's mantrip when EE felt something pop in right knee. EE was seen at OA on 6/10/24 and placed off work for a possible torn meniscus/pending an MRI.

While reclaiming 4" airline off the NE road, a small rock fell from the mine roof and struck the employee on the back of the right hand causing a small laceration. (3) sutures were required to close the wound. Employee started missing work on 5/24/24.

While waiting on the miner, the roof bolter operator went to reposition the bolter in the cross cut so that the opposite operator would have room to load the tray. Unaware of the other operator's position, the RB operator slewed the bolter which pinned the opposite operator's leg between the bumper and the rib resulting in a fractured tibia. EE started missing work on 5/7/24.

While changing out belt on the 1st SE, EE was reaching into the diesel scoop bucket to re-attach the belt and asked the operator of the scoop to lower the bucket. Unaware that the foot was under the bucket, EE's foot was pinned to the mine floor resulting in a fracture to the left foot. EE began missing work on 4/30/24.

EE was walking alongside the bolter in 6R and tripped over some gob in the face. EE caught right arm on a stack of roof bolt plates that was sitting on the bolter resulting in a laceration on the forearm. (6) sutures were required to close the wound.

Employee was attempting to release the winch from the mechanics ride and inadvertently hit the switch the wrong direction. Their right index finger was caught between the winch rope and the hook resulting in a laceration. (10) sutures were required to close the wound.

While cutting an underpass on overtime, the cm operator struck the back of their hand against the shuttle car. X-rays were negative but the employee was placed off work for a contusion. Employee returned to work on 04/16/2024.

EE was changing a tire on an outby scoop using the hoist winch on the back of EE's service ride when the winch rope broke. The scoop tire sat down on EE's right ankle causing a fracture. EE started missing work on 4/9/2024.

EE stepped over some gob and twisted right knee resulting in a torn meniscus. EE started missing work on 03/12/2024.

EE was stepping down out of the back of a mantrip and awkwardly stepped onto a piece of a hollow block resulting in a fractured tibia. EE began missing work on 01/23/2024.

EE was cutting a metal plate out of the dust cart. The cutting wheel kicked off the plate and came in contact with EE's right knee causing a small laceration. (4) sutures were required to close the wound.

EE got out of EE's shuttle car to throw the miner cable over on in entry 4 on #2 unit. As EE bent over EE felt a sharp pain in EE's lower back. EE finished EE's shift and was seen the next morning at OA. EE missed work on 11/28/2023 and 11/29/2023 but was able to return to work on 11/30/2023 and was able to perform EE's normal job duties.

Contractor was drilling the inside hole when a small rock slung off EE's drill steel striking EE in the right side of the face causing a laceration on EE's cheek. (8) sutures were required to close the wound.

While building an overcast, the employee was pushing one of the panels onto the forks of the mini trac when EE felt a pop in EE's right arm. EE was placed off work by a doctor on 11/14/2023 for a suspected muscle strain.

While mounting a new hose reel in the prep plant, the employee's left hand got caught between the hose reel and the unistrut frame resulting in a small laceration. (3) sutures were required to close the wound.

EE was standing at the front of the bolter as EE's partner was backing the bolter up. The bolter slid/shifted towards the side EE was standing when the mine floor busted apart causing the ATRS pad to contact EE's right leg. X-rays revealed a small hairline fracture in EE's tibia. EE started missing work on 10/13/2023.

While cleaning around the 1A head drive, employee slipped/fell and caught self on the guarding with EE's left arm, injuring EE's left shoulder. EE was seen by a doctor on 10/05/2023 and was diagnosed with a small tear in EE's left shoulder. EE continued working with a full release until EE's surgery date which was on 11/15/2023.

Employee was re-entering a miner on #3 unit when a small rock fell from the mine roof causing a laceration. The employee received (6) sutures to close the wound. They started missing work on 09/15/2023.

Employee was relocating the shuttle car anchor. While lifting the anchor off the anchor plate EE felt a sharp pain in the left shoulder. EE finished the shift and worked the next regularly schedule shift. On 09/13/23 EE was seen by a doctor and diagnosed with a shoulder strain. EE started missing work on 09/13/23 due to restrictions.

EE was installing discharge pipe in the haul road when the pipe came apart and struck EE under left eye on the cheek causing a small laceration. (2) sutures were required to close the wound.

Employee was removing a drill steel from the mine roof and felt a sharp pain in their right shoulder. EE was seen by a doctor for a possible torn rotator cuff on 08/29/2023 where they were taken off work due to the restrictions. EE began missing work on 08/30/2023.

EE was placing EE's drill steels back in EE's drill tray when EE's right forearm came into contact with the corner of a roof bolt plate causing a small laceration. (8) sutures were required to close the wound.

An unplanned roof fall occurred in the intersection of entry #6, crosscut #163 on the 1NE3 beltline. The fall measured approximately 18 feet in length, 18 feet in width and 8 feet in height.

A contractor was attempting to connect the drill steels together which became cross threaded and struck the contractor in the left hand. EE began missing work for a sprained thumb on 06/21/2023.

While connecting the drill steels together, the bottom steel cross threaded into the starter steel which popped the operator in the left hand causing a small laceration in the webbing between the thumb and index finger. (3) sutures were required to close the wound.

The employees drill steels became cross-threaded and popped EE in the left hand causing a laceration in the webbing between EE's index finger and thumb. (6) sutures were required to close the wound.

EE was attempting to remove a loose rock that was laying on top of EE's canopy when the rock slid off striking EE in the left shoulder. X-rays were negative. EE was taken off work starting on 5/17/2023 for a contusion to EE's shoulder.

While checking a splice on EE's shuttle car cable, a piece of rock fell from above the coal seam that was on the rib/mine roof and struck the employee's left leg causing a fracture to EE's tibia. EE began missing work on 5/17/2023.

EE was installing the first row of bolts in entry 3 right on Unit #1, cross cut 37, when a rock fell out from the corner and struck EE's right foot causing a bruise. X-rays were negative. EE began missing work on 4/11/2023.

Employee was hanging tie wire and stepped off in a mudhole awkwardly, rolling right ankle. X-rays revealed a fracture in EE's right ankle. EE began missing work on 04/10/2023.

As EE was exiting the low trac operator's compartment during a unit power move, another miner was placing a cathead on the low trac's boom and smashed EE's right pinky finger with the cathead. X-rays revealed a small fracture. EE is expected to make a full recovery without requiring surgery. EE began missing work on 4/6/2023.

EE was inserting a roof bolt into the mine roof and while bending the bolt at the stamp, a loose piece of rock fell off the operator's canopy striking EE in the back of the hard hat causing a cervical strain. EE began missing work on 03/31/2023.

EE was installing a roof bolt into the mine roof and felt a pop followed by a sharp pain in EE's right shoulder. EE was taken off work on 3/30/2023 for a muscle strain in EE's right shoulder.

EE was picking up a stack of roof bolt plates off EE's bolter and strained EE's lower back. EE started missing work on 3/23/2023.

EE was attaching the probe to the side of the roof bolter when a rock fell from the mine roof in between roof bolts and struck EE in the side/lower back causing a small contusion. EE began missing work on 03/21/2023.

EE was attempting to connect EE's drill steels together and popped right thumb causing a sprain. EE began missing work on 03/17/2023.

EE was installing first bolt in a slider when a rock fell from the mine roof and struck EE on top of the right foot. X-rays were negative but EE was placed off work on 3/9/2023 due to restrictions.

EE was helping the roof bolt operators and cut EE's left arm on a roof bolt plate. (5) sutures were required to close the wound. EE returned to work on 2/3/23 with no restrictions.

EE was trying to get EE's drill steel out of the mine roof with EE's drill pot. The steel popped out of the chuck and struck EE on EE's right thumb causing a fracture. EE began missing work on 2/1/2023.

EE was hanging high voltage ropes in the intake and walked into one of the ropes. The "S" hook on the end of the rope got caught on one of EE's nostrils and tore EE's nose open. EE was treated at the ER where EE received (5) sutures.

EE tripped/fell while dropping cable/putting ropes on the lo-trac during power move. EE's left hand/arm landed on the tire of the lo-trac. The operator did not see EE fall and trammed through the intersection which caused EE's hand to become lodged in between the tire and fender. No fractures. EE was treated for Compartment Syndrome. EE began missing work on 12/15/22.

During a power move on 3rd shift, EE was attempting to reel up the slack pile on a shuttle car by manually actuating the reel valve. The cable started reeling up quickly and struck EE in the left forearm. X-rays revealed (2) fractures in the forearm. EE began missing work on 12/5/2022.

EE was standing #4 entry on Unit #3 beside the right middle car anchor when the car cable hung from the mine roof was pulled down by the car, striking EE in the left rib cage and back, knocking EE to the ground. EE began missing work on 11/23/22 for a strained lower back.

EE was pulling glue out of the box in order to load EE's tray and felt a sharp pain in EE's right shoulder. EE was taken off work on 11/22/2022 for a possible torn rotator cuff.

EE was drilling a hole into the mine roof when EE lost suction. EE's starter steel remained in the mine roof while EE was checking the suction. EE's hand was on the chuck when the starter steel broke free from the mine roof and struck EE on the left index and middle finger causing a laceration which required (5) sutures. EE began missing work on 11/15/2022.

While reaching for a roof bolt in EE's tray, EE grazed EE's right forearm against a stack of plates which resulted in a laceration. (14) sutures were required to close the wound.

While marking up entry #3 on #2 unit, the section foreman stepped in a hole awkwardly and twisted left knee. They started missing work on 11/2/2022 and is being treated for a possibly torn meniscus.

EE was carrying a top roller and felt a sharp pain in the left bicep. An MRI revealed a partial tear in EE's bicep. EE is expected to make a full recovery without surgery being required.

EE was bending a 12' cable bolt to install into the mine roof and the end of the bolt slipped out of EE's hand and struck EE in the mouth, fracturing EE's front tooth.

While stepping on to the road grader, EE slipped and felt a sharp pain in EE's lower back. An MRI later revealed a bulged disc. EE continued working without restrictions until the date of EE's surgery. EE began missing work on 12/01/2022.

EE was pulling supplies into the mine with a supply tractor along the Main NE Haul Rd at xc 60 and unknowingly got too close to the overhanging discharge waterline. EE hit head on a waterline coupler causing a small laceration. (1) staple was required to close the wound.

EE was backing a battery scoop into a crosscut in order to clean out some mud in an intersection when a belt rail that was lying under the mud against the pillar popped up as the back tire ran over it. The rail entered the cab compartment of the scoop striking EE in left side and lower back. The injury resulted in a muscle strain along with cuts/contusions to EE's back.

EE was shoveling the walkway at the plant feed platform outside of the breaker building. As EE was attempting to throw a shovel full onto the plant feed belt, EE struck the back of EE's left hand against the railing causing a laceration. (7) sutures were required to close the wound.

EE was cleaning rocks off the miner when a rock measuring approximately 1.5' x 1.5' by 2" thick slid off the miner and struck EE in the left foot. EE was taken off work for a contusion on 9/20/2022.

An unplanned roof fall occurred in the pillar line of entry #8 between cross cuts #70 and #71 on the 2A5 panel. This entry is in the return air course. The fall was found at approximately 1:50 AM on September 16, 2022. The fall measured approximately 60 feet in length, 17 feet in width and 8 feet in height.

A contractor was inserting the 2nd drill steel as EE was drilling the inside hole on Unit #1, entry #7, crosscut 17. A rock measuring approximately 2' x 1.5' x 2" thick fell from the mine roof and struck EE on top of the left foot. EE was taken off work on 9/19/2022 for a contusion and an unrelated lower back strain.

EE was greasing the 6043 roof bolter on #3 unit. EE was working on the operator side boom and when EE swung the boom out to access the inside section of the boom EE's left ankle became caught in between the drill pot and the coal rib. X-rays revealed an avulsion fracture in EE's ankle. EE is expected to make a full recovery without surgery being required.

A contractor was installing a stamped roof bolt into the mine roof. As they were inserting the bolt with the boom they inadvertently engaged the rotation causing the bolt to spin around and struck them on the right hand. X-rays were negative and they continued working until 08/18/2022 when they were taken off work for a bruised hand.

Contractor was attempting to connect EE's drill steels together and pinched the skin between EE's thumb and index finger on EE's left hand causing a laceration. (5) sutures were required to close the wound.

While carrying an 8' timber to set under a bridge, employee slipped on the slick edge they had just scooped. On 7/25/2022 employee was seen by a doctor and was diagnosed with an inguinal hernia and was given no restrictions. They went off work on 11/3/2022 to have the hernia surgically repaired.

At approximately 9:50 PM on 7/20/2022 a non-injury accident occurred when the Caterpillar D10 company number 002 dozer slid into the #7 raw coal feeder on stacking tube #2.

Contractor was swinging the boom out to install the outside bolt when a rock fell from the mine roof and struck the contractor on top of left foot. X-rays revealed a fracture in the 2nd metatarsal. Contractor began missing work on 07/21/2022.

EE was unloading a busted bundle of crib block by hand off of a supply trailer. One of the crib block fell and landed on EE's left index finger causing a laceration. (11) sutures were required to close the wound.

Employee was installing cable bolts with a walk thru truss bolter in an outby area. While inserting the glue and bolt up into the mine roof EE felt a pain in EE's right shoulder. EE continued working until 6/15/2022 when the doctor placed EE off work. An MRI has been scheduled for a later date.

Employee had just finished installing the first row of bolts in the 1st cut of 4 left and was standing beside the tray when a rock fell from the mine roof striking EE in the head and shoulder. EE fell to the mine floor and sprained left ankle. EE began missing work on 06/07/2022.

EE was dragging a 4" waterline and felt a sharp pain in the left shoulder. An MRI later revealed a partial rotator cuff tear. EE continued working without restrictions until EE's surgery which took place on 11/18/2022. EE began missing work on 11/18/2022.

Employee was standing in between a golf cart and the waterline, greasing the NE header. The golf cart slid off the concrete pad and pinned their left leg against the waterline causing a fracture in their fibula. Employee is expected to make a full recovery without surgery being required.

Employee was reaching for a hammer on the back of the scoop when a rock fell from the mine roof and cut left middle finger. (9) sutures were required to close the wound.

As the employee was inserting 2nd drill steel, a small flat rock fell from the mine roof, penetrating their sleeve and contacted left forearm causing a laceration. (4) sutures were required to close the wound.

A contractor was bending a stamped roof bolt prior to installation. As the contractor pushed down on the roof bolt, it rolled in the contractor's tray causing wrist to hyperextend. X-rays later revealed a small chip fracture in left wrist. The contractor is expected to make a full recovery without surgery being required. The contractor began missing work on 4/7/2022.

The employee was running a CM. As EE was finishing punching a crosscut, a sharp rock fell out between the rib and roof bolt and struck EE on the back of EE's left hand which was on the remote box causing a laceration. (9) sutures were required to close the wound.

An employee was attempting to bend a roof bolt and sprained wrist in the process. Employee continued to work on light duty for the next 3 days and then began missing work on 1/12/2022.

An unplanned roof fall occurred in the pillar line of entry #5 between cross cuts #7 and #8 on the 2A4 Panel. This entry is the Unit's belt entry. The fall was found at approximately 12:30 am on 11/11/2021. The fall is estimated to be approximately 7 feet high, 18 feet wide and 30 feet long.

Unplanned roof falls occurred in the cross cuts between entries #6 and #7 and entries #7 and #8 at cross cut 10 on the 1H3 Panel. The falls were found at approximately 2:15 am on October 2, 2021. The roof falls are estimated to be 6 feet high. A shuttle car was located beneath the fall that occurred in the cross cut between entries #7 and #8.

The contractor accidently scratched EE's right bicep with the bearing plate as EE was putting on the roof bolt prior to installation. (2) sutures were used to close the wound.

The contractor had cross threaded a drill steel in the drill pot. As contractor was pulling the steel out it broke free from the drill pot and struck the contractor in the left knee causing a small laceration. (2) sutures were required to close the wound.

The employee was attempting to remove a bent drill steel from the drill pot as it had been cross threaded and become stuck in the drill pot. As they lowered the boom they accidently engaged the rotation. The bent drill steel came around and struck them in the left arm causing a laceration. (13) sutures were required to close the wound.

EE was reclaiming belt structure off the old 3B belt line and as EE went to put a top roller in the bucket of the ram car EE felt a pop in left shoulder. An MRI later revealed a tear in left shoulder. EE continued to work until 9/17/2021 when EE was taken off work for surgery. EE is expected to make a full recovery.

The employee was placing the belt bar back onto the belt winder. The mini track operator went to pick up the roll of belt and as EE did the belt rolled off the forks into the employee's legs and pinched EE's legs between the roll of belt and the belt winder bar. The employee began missing work on 05/27/2021 due to possible torn ligament in the right leg.

Employee's glove got caught in between pinner steel and chuck unknowingly. When employee engaged the rotation it twisted hand causing a small fracture. Surgery was completed on 5/26/2021 and employee is expected to make a full recovery. The employee started missing work on 5/24/2021.

The employee was reclaiming belt structure and pulled something in EE's left leg lifting a 42" top roller. EE continued working but was still experiencing pain and discomfort on the back of EE's leg. An MRI later revealed a partial tear in EE's hamstring. EE is expected to make a full recovery without requiring any surgery. EE began missing work on 01/26/2021.

While the employee was working on a roof bolter, a rock (measuring approximately 3 feet by 2 feet by 3 inches thick) fell from the mine roof and struck the mechanic in the back of right knee causing a small tear in knee and calf muscles. The employee started missing work on 1/4/2021 and is expected to make a full recovery. No surgery is expected to be required.

Employee was replacing broken drive shaft when EE slipped off the wrench and hit right forearm against the mantrip. (8 stitches)

The employee was using a cutting wheel to remove a section of hog panel from overhead in order to install arches. In the process a small metal sliver went over EE's brow, behind the glasses and into EE's right eye. EE was treated by an ophthalmologist where the sliver was removed.

An unplanned roof fall occurred in the intersection of cross-cut #78, entry #7, of the 1st NE Main at an unknown time on August 30, 2020. The roof fall is approximately 8' high, 15' in length and 15' wide.

On August 3, 2020 at 9:47AM, a proud strand was discovered during the regular inspection of the hoist. The hoist was immediately taken out of service and reported to MSHA via hotline.

Sampler was monitoring the mechanical sampler on company property. EE went into the control room to get another bag when EE turned and tripped over EE's feet, fell out the open door and hit left arm on edge of concrete.

As the employee was removing the utility knife from EE's pouch, the knife blade came out and cut the back of EE's right thumb causing a laceration. (4) sutures were required to close the wound.

An unplanned inundation occurred while mining in the 4F1 panel at approximately 2:40 pm on January 13, 2020. The inundation occurred in the face of #2 entry inby cross cut #4 when an abandoned well was intercepted while mining.

Employee was opening a box with employee's utility knife when the knife slipped cutting employee's left thumb. (4) sutures were required to close the wound.

Smoke was observed on the west side of the raw coal pile. We have continued our efforts to extinguish all known hot spots using dozers and an excavator.

Employee just finished bolting 2nd cut in 4 left on Unit #3. As EE was lowering the canopy to back the bolter out of the entry, a rock fell and struck EE on the back of the head, pushing the operator's face into the tray on the boom. This caused a small laceration on EE's nose requiring (5) sutures to close the wound.

As the employee was sliding a section of metal on the rollers in the shop, a piece of a metal strap cut left ring and pinky finger causing a small laceration. (7) sutures were required to close the wounds.

As the employee was nipping a set of batteries to the scoop and piece of loose material fell from the mine roof striking the employee on EE's right hand. X-rays revealed a hairline fracture which should not require any surgery. EE started missing work on 08/06/2019.

Employee was hand dusting rib when the roof bolt operator swung the boom in to install a middle bolt. A loose roof bolt in the tray caught on the bolter light housing and popped out of the tray striking the employee in the forehead causing a laceration. (14) sutures were required to close the wound.

Employee was sliding a piece of T3 channel across the booms of the roof bolter. Employee's partner retracted boom to install 2nd bolt and the boom pushed the channel that the employee was holding into the rib which smashed the injured employee's right index finger in between the rib and the channel causing a fracture of right index finger.

Employee was cleaning future seal construction area with a scoop. As employee was pushing gob in the old beltline, the bucket unintentionally caught a section of old waterline that the operator was not aware of. As employee continued pushing the gob the waterline broke and came up striking the operator in the back of the head causing a hairline fracture to skull.

The employee was on the opposite operator side of the roof bolter about to begin bolting the 1st cut of entry #1. As the operator turned the roof bolter towards the unbolted corner the opposite operator's right foot was caught underneath the rear tire causing a fracture in first and second toes. The operator began missing work on 06/13/2019.

Employee was hooking up the pod duster to the battery jeep and got EE's right index finger caught in the pin and receiver of the battery jeep causing a small laceration on the side of EE's finger. (13) Sutures were required to close the wound. The employee began missing work on 06/04/2019.

A belt mechanic was changing out a top roller when the belt unexpectedly started and put tension on the come-a-long being used to raise the belt. The come-a-long got wrapped around the injured miners hand causing lacerations to EE's right hand and thumb, and a fracture to EE's index finger.

Employee was participating in a belt splice and started feeling ill. Employee was transported to the hospital and died later from a heart attack.

An unplanned roof fall occurred in the intersection of cross-cut #12, entry #4, of the 3B10 panel at an unknown time on May 18, 2019. The roof fall is estimated to be 20 feet high. Entries approaching the fall measure approximately 18.5 feet wide.

A mechanic was repositioning a crib block under a shuttle car when the shuttle car slid down inside the bucket of scoop approximately 1" causing the car to pinch the employees thumb on the crib block. The employee lost approximately 1/2 inch of the tip of right thumb due to the accident.

As the employee was opening a bundle of glue, employee's knife slipped and made a small laceration in right wrist. (3) sutures were required to close the wound. The employee returned to work and completed shift.

A contractor had just bolted the corner of a first cut and walked around the bolter to gather supplies off the top of the canopy. The operator assumed ee was handling the cable so ee could reposition the bolter and could not see thru the curtain. When ee did this the contractor's left arm was pinched in between the rib and the canopy causing a fracture to wrist.

As the roof bolt operator was reaching for a bearing plate to put on the roof bolt, EE scraped arm on the plate causing a small laceration which required (4) sutures to close the wound.

As the contractor was turning around inside the cab of shuttle car, contractor scraped right pinky finger against a piece of sharp metal on a lid inside the cab causing a small laceration. (6) sutures were required to close the wound.

As the employee was hand loading cable bolts onto the roof bolter, ee strained neck. EE finished shift but when ee woke up the next morning ee was unable to turn ee neck left or right. The doctor diagnosed ee with a pinched nerve in ee neck. EE began missing work on 1/30/2019.

As the employee was removing crib blocks from under a shuttle car in which they were finished making repairs to. As the employee reached in to pull a crib block out, a crib block on which the service jack was planted on split in two and allowed the car to sit down on the employee's right hand causing a fracture to right index finger and multiple fractures to thumb.

As the employee was preparing to turn around inside the cab of shuttle car on the dump, a loose rock that was lying on the top of the canopy slid off and struck employee in the back of the left arm causing a small laceration. (2) sutures were required to close the wound.

As the contractor was drilling a hole to install a roof bolt, a small rock fell from the mine roof and landed on drill pot while it was rotating. A small, sharp piece from the rock then hit the contractor in the chin causing a small laceration. (2) sutures were required to close the wound.

The contractor was building a brattice on unit #3. The average mine height on this section is 9 foot. As ee was stepping down off a ladder and inadvertently pulled one of the hollow cinder blocks down and it struck ee on top off left foot. X-rays revealed that ee had (4) fractured metatarsals in left foot.

An unplanned roof fall occurred in the intersection of cross #25, entry #7, of the 2nd Southeast Parallel at an unknown time on December 1, 2018. The roof fall is approximately 12 feet high. Entries approaching the fall measure 17.6 feet, 19.0 feet, 17.8 feet and 17.2 feet wide.

An unplanned roof fall occurred in the intersection of cross-cut #7, entry #6, of the 3B7 Panel at an unknown time on November 14, 2018. The roof fall is approximately 11 feet high. Entries approaching the fall measure 18.4 feet, 17.5 feet, 17.9 feet and 17.6 feet wide.

As the employee was bending over to lift a jack pipe on the NE road, xc 104, ee felt a pain in lower back and down left leg. The doctor's assessment was a lumbar strain which should be treatable with therapy. EE began missing work on 11/13/2018. EE is expected to make a full recovery.

An unplanned roof fall occurred in the intersection of cross-cut #17, entry #7 of the 2nd Southeast Parallel at an unknown time on November 11, 2018. The roof fall is approximately 11 feet high. Entries approaching the fall measure 17.78 feet, 17.82 feet, 17.88 feet and 17.50 feet wide.

As the employee was hand loading bundles of glue onto a roof bolter, EE felt a sharp pain in the lower back and shoot down the left leg. EE's doctor believes EE has a pinched nerve and is expected to make a full recovery without requiring surgery. EE began missing work on 10/30/2018.

While the employee was reclaiming high voltage, ee lost footing and cut left arm with utility knife. (3) sutures were required to close the wound.

The employee was in the process of cutting off a corner in the #6 entry on #1 unit. As they sheared down, a small flat rock, that was resting on the boom of the miner, slid off the miner and struck them in the left foot. X-rays revealed a small fracture in the 2nd toe on the left foot. EE is expected to make a full recovery without any surgery being required.

A contractor was loading the tray on roof bolter with supplies and as ee was putting resin in the tray left elbow contacted a stack of roof bolt plates. The Kevlar sleeve ee was wearing had slid down arm exposing elbow which resulted in a small laceration which required (9) sutures to close the wound. EE returned to work the same day with no restrictions. restrictions.

Contractor was assisting with rotating a screenbowl when a piece of metal cut a hand causing a small laceration. (4) sutures were required to close the wound.

As the employee was hanging a piece of curtain on unit #4, entry 9, xc 27-28, a small rock measuring approximately 4" x 6" x 1/8" thick, fell from the mine roof in between a section of hog panel and T-3 channel, striking the employee in the mouth causing a small laceration. (4) sutures were required to close the wound.

While an employee was loading a shuttle car on #4 unit, entry 4L xc 19, a rock being loaded out of the tail of the miner onto the shuttle car slid out of the car and struck the employee in the back of the right knee. The employee continued working until August 3rd. EE started missing work on August 6th when an MRI revealed a tear in the ACL.

While closing the first door on the inby set of equipment doors located on the NE road xc 59, the second door began to close and caught the employee's left arm in between the two doors. This caused a small laceration which required 6 sutures to close the wound.

As the employee was walking,tripped on some gob on the mine floor and twisted their ankle. This caused a tear in a ligament in the left ankle.

Employee was installing permanent roof support when a piece of loose material fell from the mine roof striking the employee in the right ankle. This caused a hairline fracture to EE's ankle.

While the contractor was removing the steels from the mine roof, the bottom steel bowed and popped out of the chuck striking the contractor in the right hand causing a fracture. Contractor began missing work on July 23, 2018.

Employee was assisting a mechanic work on a shuttle car when the lid covering the panel began to fall. The employee attempted to catch the lid and had EE's finger caught under it causing a small laceration. (6) sutures were required to close the wound.

A roof fall occurred in the 3B7 panel at cross cut #12-13 in entry #6. The dimensions measured approximately 10' in height, 18' wide and 32' in length.

A roof fall occurred in the 1E2 panel at cross cut 17 in entry #6. The dimensions measured approximately 10' in height, 18' wide and 40' in length.

Contractor was drilling a hole to install a roof bolt when the steel popped and struck the contractor on the right hand. This caused an acute fracture in the 1st metacarpal of the right hand.

Employee tripped over a secured I-beam, causing EE to lose balance and land on hip and strained lower back.

Employee raked left bicep against a roof bolt plate resting on a tray which caused a small laceration. (3) sutures were required to close the wound.

A contractor was drilling a hole to install a roof bolt, EE dropped EE's starter steel which bounced off EE's drill pod and stuck the contractor in the mouth. (6) sutures were required to close the wound.

While employee was reclaiming curtain, a piece of draw rock fell from the mine roof measuring 1'x1'x1". This caused a minor contusion to EE's left hand.

The employee was attempting to straighten roof bolt plate and pin board. While doing this the employee inadvertently contacted moved the joystick on the roof bolter, causing finger to get pinched between the mine roof and pin board.

While operating the roof bolter, the employees drill steel fell to the mine floor while the drill boom was being lowered. The drill steel got caught underneath the drill boom, pushing it into the employees right foot, causing a non displaced fracture to the 3rd metatarsal.

While roof bolting the employee dropped drill boom and a roof bolt plate got wedged between the side of the boom and the tray. The plate popped out and hit right hand causing a laceration. (5) sutures were required to close the wound.

A contract employee was roof bolting. While reaching for a roof bolt EE bumped left elbow, causing a laceration. Sutures were required to close the wound.

The employee was unloading a belt trailer so maintenance could be performed on the trailer. While unloading the trailer the tongue slipped off the material used to block the tongue. This caused the front of the trailer to fall, and struck the employees right foot.

The employee was preparing to roof bolt the crosscut of #5 entry when a rock fell from the unsupported roof on top of the canopy of the roof bolter. The rock broke and cantilevered off the canopy. It landed vertical and when it fell to the side, the employee got caught between the rock and the tray of the roof bolter. This caused a non-displaced fracture to one rib.

A roof fall occurred in the 1D3 Panel at cross cut 23 in #5 entry. The dimensions of the fall measured approximately 18' wide, 40' in length, and 10' in height.

The employee drilled starter steel into the mine roof and dropped the boom. Employee's partner was trying to communicate with employee so employee covered the second drill steel in the chuck with hand to hear better. When employee did this, the first drill steel fell out of the mine roof onto the employee's hand causing a fracture.

A contract employee was roof bolting. EE bent drill steel, and while trying to correct the problem the drill steel hit EE's left hand. EE suffered a contusion.

The employee bent over to move the miner cable and strained left knee.

The employees drill steels cross threaded. While trying to correct the problem the drill steels popped and struck the employees left hand. This caused a laceration to the left thumb. (2) sutures were used to close the wound.

The employee was adjusting a bench and strained back.

While using a pry bar to scale down some loose draw rock a piece rolled back causing a laceration to employee's left thumb. (3) sutures were required to close the wound.

The employee was in the process of making a splice on a continuous miner trailing cable. While using a utility knife EE cut through the protective gloves causing a laceration to hand. (4) sutures were required to close the wound.

The contract employee was hanging miner cable during a power move and felt a pain in shoulder. EE was diagnosed with a strain and was not able to fulfill normal work activities.

While trying to flop the gate for the slope belt from the reclaim belt to the ROM belt, the employee had the pipe wrench slip the employee was using and hit the employee in the mouth. (5) sutures were required to close the wound.

An unplanned roof fall occurred in a regularly traveled area above the anchorage zone in the 3B6 Panel at cross cut #12, entry #5.

The employee stepped off a supply trailer and strained hip.

The employee bumped elbow on the head of a roof bolt while loading it in tray at drill station. (2) sutures were required to close the wound.

While operating the continuous miner a piece of draw rock fell from the roof, measuring 6" wide X 6" long X 1/2 inch thick. It struck the employee on the back of the neck. 4 sutures were used to close the wound.

The employee had a pry bar slip while replacing a tram motor in a shuttle car, causing a laceration to left index finger. 2 sutures were required to close the wound.

The employee hit a bump while operating a shuttle car and chipped employee's upper left lateral incisor.

The employee got foot caught between the mine floor and the rear bumper of the truss bolter, causing a fracture to the 3rd metatarsal on right foot.

The employee cut arm on a roof bolt plate. 1 suture was used to close the laceration.

A contract employee was roof bolting. The bolter was moving when the employee thought the cable was going to get run over. They flagged and yelled at the operator to stop. When they bent over the operator thought they flagged to resume tramming. When tramming resumed the contract employee got caught between the rib and the ATRS pad.

The employee was sliding the forks over on the mini-trac and pinched finger, causing a non-displaced fracture of distal phalanx of the left index finger.

A contract employee was operating a mini-trac and a piece of loose material fell from the mine roof hitting the employee in the groin.

A contract employee was roof bolting, and while bending a roof bolt felt a pain in left elbow.
