Miner was jacking up a haul truck to make repairs when the hydraulic ram EE was using fell from the jack stand onto EE's left thumb. The thumb received a cut that required stitches.
The miner was using a walk behind skid steer to clean out a tunnel. While moving under a conveyor tail pulley the skid steer ran over a rock throwing the control end up into a guard. The miner broke their left arm near the wrist.
The miner was performing maintenance on a crusher removing liners. While lifting one of the liners employee felt right elbow pop.

Clearing a rock chute when a rock fell smashing their finger causing a laceration.

Employee was securing a chain to a piece of sheet metal. As EE was pulling on the chain, the piece of sheet metal tipped over striking the employee on the left ear causing a laceration to EE's left ear. The injury required stitches.

Employee and witness were completing repairs using a hand-held hydraulic jack. Prior to its use, the jack was placed on a ladder. The employee bumped into the ladder, and the jack fell off the ladder striking the employee's foot. Employee suffered fracture of great toe. Employee was wearing protective shoes.
Employee was being trained on the torque multiplier and had right hand resting on the end support of the stabilizer bar. When co-worker torqued the multiplier it caused the stabilizer bar to move and pinch right middle finger against hammer resulting in the partial amputation and open fracture.
Employee notified employer on 5-9-16 at 5 pm. Employee was jacking trailer up off hitch of the company vehicle EE was driving to transfer to another pickup upon release, the trailer rolled back approximately 6" and the employee tried to hold trailer and strained their shoulder.

Employee was removing a bushing from a phasing arm of a horizontal screen (P-009). Employee apparently slipped and fell 20 feet to the ground. Employee was not wearing fall protection provided by the Employer. Injuries are unknown to Employer at this time, but appear to be not life threatening, nor permanently disabling.

EE was assisting with repairs. Using a wrench, the wrench slipped, ee slipped and fell backwards from a sitting position onto a rock on concrete. EE incurred bruised muscled in back. EE had a preexisting herniated disk in back, He was currently completing physical therapy so ee was taken to hsp for observation. EE was instructed to rest and stretch the muscles.

While working with a air powered hand drill, the employee dropped the drill and in an attempt to catch it a piece of metal entered his left hand palm, causing a pencil sized laceration that required medical attention to release pressure from infection that developed.

While hammering on a screen bearing, a piece of metal broke off striking the employee in the upper left arm, causing a 2cm laceration that required three (3) stitches to close. At the time, the employee was wearing a short sleeved shirt with unprotected arms.

While walking from the Corporate Office to the dumpster a shard of glass fell out of a garbage bag striking the employee on his left lower leg (calf) causing a laceration requiring 25 stiches to close
While assisting welder work on rollmill employee exposed eyes to welders flash causing burns to his eyes.
At the surface shop our mechanic EE was removing an hydraulic pump from a haul truck (belonging to a subsidiary of River Product), when the pump slipped and struck the 5th finger (pinky finger) of his right hand, requiring three (3) sutures.

Employee was reaching into the bed of a truck to retrieve his tool belt and the exposed blade of a box cutter/utility knife caused a laceration of the right forearm, requiring 8 stitshes.

While repairing the guard on a conveyor tail pulley a C-Clamp being used to secure the guard came loose. The tension in the guard threw the clamp striking the employee just above the right eye. This resulted in a laceration about 2.8 cm in length that required stitches to close.
While doing maintenance on Dryer Plant auger was using an air hose to clear the auger shaft of material. Blowback from the auger caused material to be blown into the eyes of the employee. The primary injury was to the Right Eye, caustic material in the dust caused eye irratation. Safety glasses were being worn at the time of the injury.

STAFF MEMBER WAS WALKING OVER PAVEMENT COVERED WITH ICE SLIPPED, BUT DID NOT FALL. TWISTED HIS KNEE CAUSING SWELLING.
COMPANY TRUCK WAS BACKING OUT OF SHOP WASHBAY. EE WAS PREP'G CASE 1825B LOADER TO WASH. THE FORD TRUCK WHILE BACKING UP STRUCK & PINNED EE IN BUCKET OF CASE 1825B LOADER. THIS CAUSED COMP RESSION INJURIES (PUNCTURE & LACERATIONS)TO EE'S RIGHT LEG AT MID-SHIN.
WHILE WORKING ON A BEARING RACE THE EE WAS USING A HAMMER TO LOOSEN THE BEARING. WHILE STRIKING THE BEARING RACE THE METAL ON METAL ACTION CAUSED A PIECE OF METAL APPROX.1/4" IN SIZE TO BREAK AWAY & STRIKE THE EE IN HIS RT. FOREARM, PIERCING THE SKIN. THIS REQUIRED 4-5 STITCHES.

WHILE INSTALLING A BEARING ON A TAIL PULLEY THE COMEALONG BROKE LETTING THE PULLEY COME AHEAD STRIKING EE IN THE MOUTH AND HAND. SUFFERING A CHIPPED TOOTH, LACERATION LOWER LIP (9 STITCHES), A ND LACERATED RIGHT HAND LITTLE FINGER (SPLINTED).
EE WAS WALKING FROM CONTROL HOUSE TO THE CONVEYOR WHEN HE TWISTED HIS RIGHT KNEE ON UNEVEN SURFACE.
EE WAS DOING LOADER REPAIR, WHILE PUSHING PINS OUT OF BUCKET HYDRAULIC RAM LET LOOSE ALLOWING METAL SPACER TO FALL STRIKING HIS HEAD. THIS RESULTED IN A LACERATION THAT REQUIRED EIGHT STITCHES TO CLOSE. EE RETURNED TO WORK SAME DAY.
EE WAS COMING OUT OF SCALEHOUSE AFTER PUNCHING TIMECARD AT SHIFT END TO LEAVE QUARRY PREMISES- STEPPING OFF OF A CONCRETE SIDEWALK DOWN TO A ROCK PARKING AREA- HE MISJUDGED THE DROP OFF AND CAME DOWN ON HIS HEEL THE WRONG WAY CAUSING INJ
WHILE ACCOMPANYING AN MSHA INSPECTOR ON A ROUTE INSPECTION OF AREA, I SLIPPED ON GROUND COVER AND FELL, STRIKING MY LEFT SHOULDER/ARM ON THE GROUND. NO INJURY NOTED AT THAT TIME. IN JULY I HAD AN M.R.I. AND IT WAS DISCOVERED THAT I HAD A TORN ROTATOR CUFF. THIS AFTER 3 MONTHS HAD PASSED.

WHILE CHANGING DOWN A LOAD ON FLATBED TRAILER WITH A BREAKOVER BINDER, WAS RE-CHECKING ALL THE DOWNS. WHEN BREAKOVER BINDER CAME LOOSE (POOSIBLE MECHANIC'S FAILURE) STRIKING EMPLOYEE ON THE PA LM OF HIS RIGHT HAND.
WHILE WORKING ON DRILL IN QUARRY SHOP, EE WAS CHANGING STEELS IN THE CAROUSEL. HE WAS HOLDING A STEEL WHEN THE JAWS WERE OPENED. THE LOCK RELEASED PINCHING HIS FINGER, RESULTING IN A CUT TO TH E MIDDLE FINGER OF RIGHT HAND, REQUIRING SIX STITCHES TO CLOSE.
EMPLOYEE FAILED TO USE PROPER FALL PROTECTION. WHILE HELPING REPLACE A BURNT OUT ELECTRIC MOTOR, HE FELL STRIKING THE BED OF MACK DUMP TRUCK, INFLICTING MULTIPLE FACIAL LACERATIONS THAT REQUIRED STITCHES.

WHILE MOVING A FORK ON THE FORKLIFT EE STRAINED HIS LOWER BACK.
WHILE PLACING A PIN IN A BUCKET CYLINDER, HE GOT HIS HANDS COVERED WITH GREASE. HE THEN PICKED UP 18 LB SLEDGE HAMMER. WHILE USING HTE SLEDGE TO HAMMER IN THE PIN, THE SLEDGE SLID DOWN THRU HI S HAND AND STRUCK HIS INDEX FINGER. THIS RESULTED IN THE NEED TO STITCH UP A CUT ON THIS FINGER.
WHILE CUTTING CARDBOARD FLAT FOR PALLETS THE EE CUT HIS LEFT THUMB WITH A UTILITY KNIFE. HE WAS PULLING THE KNIFE TOWARDS HIS BODY. NO PERMANATE IMPAIRMENT ANTICIPATED.
