EE was reported missing and was later discovered under a rib failure several hours later. The miner was found in an abandoned section of the mine beyond a barricade berm, along the top of a 20 foot high, waste material spoil pile. The victim was recovered on 1/26/17 at approximately 3:30am. and officially pronounce dead by the local medical examiner.

Miner was loading a room with explosives from the man basket of the powder rig, when a softball sized rock fell and hit EE on the hand that was resting on the handrail, resulting in injury.

Miner was climbing into powder rig man lift basket and hit EE's left knee on the basket.
Operator was parking equipment at the end of their shift and as EE was climbing out of the frontend loader EE stepped on a rock and rolled EE's ankle.

Miner discovered during their workplace exam a section of ceiling had fallen out on an escapeway. the area has been rescaled and roof bolted to prevent further issue.
An electrician was removing some damaged cable between 2 junction boxes. They de-energized and locked out the power going between the boxes and went to cut the wires to be removed, but they got confused and accidentally cut into the wires coming into the junction box from a transformer that had not been de-energized resulting in arc flash.
During the second shift workplace exam it was discovered that a portion of secondary escapeway out of a portion of the mine had a section of ceiling fall out. Initially all portions of the mine affected by the escapeway being compromised had been barricaded off and shut down, until we were able to rehab the area that was compromised.

During the first workplace exam after the weekend, it was discovered that a portion of ceiling had fallen out on an escapeway. The intersection has been barricaded off, escapeways have been rerouted and miners trained to account for the change.
As the miner was operating a skid steer the drive belt malfunctioned causing the cab to fill with smoke resulting in the miner getting smoke inhalation.
As miner was readjusting EE's lines at barge load out they got snagged on a rock causing EE to jerk EE's arm and shoulder resulting in them hearing a pop and instant pain.
The production loader was digging in a pile of material to load a haul truck, when EE's loader bucket caught a hump in the road that was hidden by the pile of material. Hitting the hump in the road caused EE to get jarred in the cab resulting in injury.
Miner was exiting the man basket of the powder rig and accidentally stepped in a hole and twisted ankle causing a severe sprain.

Miner was scaling the ceiling in a room, when EE pulled a piece loose from the ceiling, as the piece came loose it jarred the miner in the cab causing the seat to break and jar EE even more. Violently shaking EE in the cab.
Miner was removing a handrail from a piece of equipment. The handrail was supported using an overhead hoist. The strap on the handrail came loose causing the handrail to shift and pinch the miner's finger causing a laceration. The miner was wearing gloves at the time of the incident.
Development miner had checked the ceiling to be bad using a machine scaler. After coming to the conclusion that the ceiling was good they proceeded to scale past that point when they had a chunk fall from the ceiling onto the scaler boom. The material hitting the boom teetered the machine forward causing the miner to hit their head inside the cab.
Miner had a hatch door open on a hopper as EE was was attempting to clean an overloaded conveyor belt. As the miner was attempting to clean out the hopper, the door shut on EE's hands causing a laceration to the left index finger. Miner was not wearing gloves at the time of the incident.
Annual Audiometric testing showed a potential reportable shift. Miner was sent for follow-up examination. The follow-up examination confirmed the reportable shift.

Miner was moving a water hose out of the path of mobile equipment travel when miner caught a finger between the hose and a support beam causing a laceration to the left index finger.
Miner was working as a mechanic and noticed hands would break out in a rash. Still not 100 percent determined to be work related.
Truck driver was jarred in the cab of a haul truck while getting loaded by a front-end loader.
While assisting a coworker in a work task not related to plant operation improper handling of a chainsaw led to the injury of a miner resulting on the injured miner's left leg.
Miner put index-finger (left hand) in vehicles heater vent and tip of finger came in contact with blower fan-wheel causing partial traumatic amputation.
Miner reported back strain caused due to improper lifting technique when lifting water hose.

Annual audiometric testing results indicated a potentially reportable shift from current baseline readings. This report is prior to a follow-up examination and review which is to be scheduled. This report will be modified accordingly pending results of the follow-up examination and review.
While seated in cab of haul truck, injured person closed cab door by not firmly gripping handle, but with an open palm. As the door closed, a finger extended into the area of, and got caught in the door jam causing laceration/crushing injury to finger(middle)-left hand.
Injured Employee attempted to unspool rope from barge rope winch without releasing tension at winch when rope slack backlashed. Employee's left arm that was holding slack was thrust upward due to force of backlash resulting in fractured wrist.
Miner had left hand resting near conveyor structure being moved into place, when structure (impact bed) moved out of place onto miner`s hand causing laceration/crushing injury to finger(index)-left hand.
While bent over cleaning bottom drill holes (post drilling) injured miner was hit in back with rock that fell from face.

While shoveling material around belt tail-pulley guarding, injured person hit hard spot in floor with shovel and fell into edge of belt guarding. Injured person's hand hit edge of guard and cause injury (cut/laceration) to right hand - index finger knuckle.
Employee was putting a bearing on a mixer with hand. The bearing moved quicker then expected and ee smashed finger between the bearing and the mixer. The tip of left index finger was amputated by dr.
Injured person (operator) was backing up water-truck up to ramp which led to a water sump at the base of ramp. Operator backed up truck onto ramp when water-truck began to slide down ramp. Operator ejected self out of vehicle. Operator met water-truck at base of ramp & became pinned between the front bumper assembly of the water-truck & loose material on the ground.

While hooking up caps in the mine, a rock fell from the ceiling, hitting hard hat of employee, knocking it off, the rock edge then caused a laceration on the back of his head, requiring stitches. Also causing a neck strain, joint pain in the right shoulder and pain to the right mandible.

Felt pain under left armpit and elbow while carrying a 35 lbs. ladder up stairs.
While cutting ties off of a hose, his hand slipped from oil on hose and cut palm of left hand.

While backing up the JLG his foot slipped off the gas pedal causing the JLG to stop abruptly and slammed his left ankle into the back cage causing a sprain to his ankle.
EE was conducting routine daily walk-around inspection of the primary crusher plant and conveyors. Supervisor came to check on EE and told him to take a break based on his appearance. EE then complained of chest and arm pains. 911 was called and EE was transported to hospital where he passed away later that night.

Ee was pickup up a load & the operator "slammed" the load into the bed of the truck on the right side & ee was thrown to the right straining his neck and back.
fracture left thumb; replacing the crusher hammer. EE's thumb was pinched between the head of the bolt and the pin hole while placing the bolt that is used to keep the hammer rotor from moving.
Lip laceration & fracture of teeth; cutting drill steel from #1 belt on crusher; once tension had been released steel piece flung and hit ee in the mouth.
Open wound left index finger; manually rotating the adjustment ring into position on the crusher; finger was pinched/pulled through the guide roller.

EE's left elbow has bursitis; struck elbow on door while opening it.
Foreign body in left eye; cutting a piece of handrail with a cutoff wheel, without wearing a face shield; the wheel broke and a piece of metal went behind the safety glasses.
Small chip fracture right elbow; backing up haul truck, struck elbow on "suicide" knob on steering wheel.
Ee had just dumped a load of concrete stone; he thought the bed was down and he took off. The bed was up and he hit the cam span. An alarm is to sound when the truck is in gear and the bed is up. The alarm did not sound.
Laceration to lip; replacing steering pins and bushing on #7 drill, pin slipped out of employee's hand and hit him in the lip. Received 3 stitches.
The Hitachi Scaler caught fire at 10:50am. A fire extinguisher was used and was ineffective. The mine was evacuated. The fire burned itself out at 2:30pm.
RT knee strain; slipped on rocks getting into truck, hyper extended right knee
Employee was driving a haul truck underground; went around a curve and there was a pillar that he collided with. Employee was driving to fast and when he attempted to steer to avoid hitting the pillar the truck didn't react. Employee sustained a laceration to the shin.
Right shoulder strain. Employee was laying on his left side replacing feed wear plate. He reached overhead to position plate and heard a pop and felt a sharp pain in his right shoulder.
Shoulder strain from lifting.
Employee was disassembling the wash screen; while cutting off the foot pads dirt and rust build up on bolts blew into his eye. Corneal abrasion.

Employee positioned a platform ladder along side a ventilation fan to clean with pressure washer. Water valve was opened and the pressure was more than expected knocking the ladder over sideways. Multiple contusions, right shoulder, arm, ribs and hip.
Backing the haul truck under the loader the employee was watching the front of the truck and the loader and hit the wall. Jarring motion caused a strain to his neck.
Employee was using a utility knife to open a cardboard box containing parts. As he was holding the box, trying to cut it open, it began to slip out of his hands. When he went to catch it, he pushed the blade of the knife into his lower right leg, causing a laceration.
Employee failed to lower the bed of the haul truck and hit the cam span. He was thrown from the cab of the truck. The fall resulted in a broken nose with a laceration, 2 front teeth broken out, broken great toe as well as contusions and abrasions.
While climbing down a ladder to the sugar beet belt in order to unplug the chute employee was struck by 2 sugar beet rocks. One on the shoulder and one on the left wrist. Resulting in cuts to the wrist treated with first aid. The next day his wrist was swollen and he requested medical attention. X-rays were negative he was diagnosed with contusion to the wrist.
Employee was cleaning frozen material out of the chute to the screen on the sugar beet plant. The material was covered with snow and frozen together making it impossible to pass through the chute. Employee, using an 8 foot metal bar to break up material, sustained an injury to his left elbow.

Employee states that he was cleaning the chutes at the lower plant and as he was walking back to the control room he slipped and fell on ice over gravel from the weekend ice storm. He landed on his right elbow and jammed his arm up into his shoulder. Employee is experiencing extremely limited range of motion. Diganosed with dislocated shoulder.
Ee was using a torch to burn off the bolts of the take up bearing on the tail pulley of chips cross belt of the Nordberg. He reached behind the bearing, while torch was still lit, to clear mud off of the bolts and his left wrist came in contact with the flame.
Employee states that he was chiseling with a hollow bar frozen chunks out of chute and main screen deck at lower plant for approx. 10 hours and wrists became sore. He requested medical attention on 3-6-07 as he was experiencing burning pain popping in his wrists and wrists & forearm were swollen.
Employee was dumping a load of rock into the primary crusher when loader operator, backing up from the stockpile, hit right side of EUC causing the EUC operator's left shoulder to hit side of EUC.

After dumping, employee went to lower bed; he pushed down on the lever and released. He had forgotten this particular truck requires the lever to be held down until bed is completely down. Unaware that bed was still up, he returned to pick up another load and drove under cam span wedging the truck with the front wheels off the ground.
Employee was driving haul truck down the haul road when he came upon a boulder in the middle of the road. The boulder had fallen from a previous truck; not being able to drive around the boulder, employee moved it by hand. Employee took a vacation day for dental work.
As employee was changing screens he twisted wrong as he lifted one. Two days later his back continued to be sore so he sought medical treatment.
Employee was hauling rock in a Euc when he hit a pillar. As he was turning he injured his left ring finger and left leg.
Employee was coming from the office attempting to cross over the highway to his truck to get over to the mine, when he was hit by another vehicle causing neck and shoulder injuries.
EE was putting a screen back in at the top plant and had to lift the flip screen door in order to put the screen in it. The door caught his leg and pinched the top of the back of his right thigh.

EE REPORTED HE WAS PERFORMING MAINTENANCE WORK ON A DRILL; ATTEMPTING TO LOCATE AN HYDRAULIC LEAK, WHEN AN "O" RING BLEW HYDRAULIC OIL AND DIRT INTO HIS LEFT EYE.
EE REPORTED TO SUPERVISOR THAT ONE OF THE WHEELS OF THE ENDLOADER HE WAS OPERATING DROPPED INTO A HOLE AND TOSSED HIM SIDEWAYS IN THE LOADER, CAUSING HIM TO FEEL PAIN IN HIS NECK.
EMPLOYEE REPORTED HE WAS PULLING A BARGE LINE TO MOVE BARGE DOWN RIVER AND FELT PAIN IN HIS LEFTFOREARM AND ELBOW.
EMPLOYEE WAS DRIVING TRUCK DOWN THE HAUL ROAD INTO THE MINE TO GET A LOAD OF ROCK AND AS HE HIT BUMPS HIS BACK STARTED HURTING.
EMPLOYEE WAS CLEARING ROCK OFF THE ROTOR OF THE CRUSHER AND CAUGHT HIS HAND BETWEEN ROCK AND CORNER OF OPENING CUTTING HIS LEFT INDEX FINGER.
EE WAS MARKING HEADINGS WHEN ROCK FELL FROMT HE MINE'S CEILING STRIKING EE ON HIS LEFT SHOULDER, ELBOW, AND FOREARM.
EMPLOYEE WAS PERFORMING CLEANUP WORK IN THE PLANT. HE WAS PICKING UP BRICKS WHEN HE SMASHED HIS LEFT INDEX FINGER WITH A BRICK AGAINST THE RAILING. TREATED WITH ANTI-INFLAMMATORIES AND MIMIMAL RESTRICTIONS.
EMPLOYEE REPORTED THAT HE WAS SCHEDULED FOR LEFT SHOULDER SURGERY THE END OF THE MONTH WITH PERSONAL PHYSICIAN, BUT NOW BELIEVED HIS SHOULDER WAS WORK RELATED.
