Cleaning Rock off of the Continuous Miner.

Employee and Co-worker were removing rock off continuous miner. As EE was removing the rock, the rock slipped pinning EE's finger against another rock which resulted in a laceration to a finger on EE's left hand.

Employee was rock dusting the face of the entry by hand. The employee was in a bent over position, as they moved backwards they came to an area where the roof was higher. When the employee stood upright they felt pain in lower back.

The employee was stepping off the mantrip and twisted Rt. Knee.

The roof fall occurred in the No. 5 entry between X-cut No. 26 and x-cut No. 27 on the DS-2 mains. The roof fall is confined to the No. 5 entry. The fall is approximately 12' long, 10' wide, and 5' thick/high. Ventilation was not effected and the fall was cleaned up and re-supported.

The roof fall happened in the No.4 intake entry at x-cut No. 28 on the DS-2 mains. The roof fall is approximately 30' Long, 19' Wide, and 8' thick/high. Ventilation was not affected. The primary escapeway has been re-routed to the No. 5 entry starting at x-cut No. 26 on the DS-2 mains.

On 9/15/2021 a roof fall occurred in the No. 3 beltline entry at x-cut No. 56 on the DS-1 mains. The roof fall is confined to the No. 3 entry. The roof fall is approximately 30' in length X 19' wide and approximately 8' thick or high.

At 8:20 pm section foreman encountered a roof fall outby the MMU 005 in No. 5 Return Air Course Entry between x-cut No. 28 and x-cut 27 on the DS-2 Mains where the No. 5 entry of the DS-7 Panel was started.

EE was installing a roof bolt and when EE raised the drill pot, to attach the wrench to the bolt head, the bolt caught and bent, mashing EE's pinky finger to the frame of the drill head.

Employee was patching a hole in a brattice. While packing brattice block employee slipped on the mine floor and felt a sharp pain in lower back. Employee did not go to the doctor the date of the accident.

The employee was shoveling around the section tailpiece and felt a sharp pain in lower back.

Employee was removing a guard from the transfer jack off of a ram car, and when they lifted the guard they strained their lower back. Employee finished the shift and went to a local clinic for medical treatment.

Employee was replacing the guard over the tail shaft on a feeder. The guard slipped and caught the employee's left hand resulting in a simple fracture on EE's index finger. The employee did not go to the hospital until 01/22/2021 and was released the same day.

An unplanned roof fall occurred at the intersection of crosscut 27 on the DS-5 Panel of the 005 MMU in the #4 intake entry. The fall was approximately 30' in length, 20' in width, and 8' to 10' in thickness.

A Non-Injury Roof Fall occurred in the outby area of the mine. The Roof Fall is located in the No. 1 entry between x-cuts No. 9 & 11 of the DS-1 Panel. The Roof Fall is approximately 20'ft. wide x 70'ft. long x 6'-9 ft. thick. MSHA was notified about this Roof Fall.

A Non-Injury Roof Fall occurred between the No. 4 & No. 5 entries at crosscut No. 22 on the 005-0 MMU on the DS-5 Panel. The roof fall was found during the weekly exam. MSHA was notified about the roof fall.

EMPLOYEE WAS DROPPING WORK BASKET IN THE LOCKER ROOM WHEN IT SLIPPED CUTTING RIGHT PINKY FINGER REQUIRING 4 STITCHES.

EMPLOYEE WAS WORKING ON A RAM JACK PIN. WHEN EE WAS LINING UP THE PIN EE'S RIGHT INDEX FINGER WAS AMPUTATED JUST ABOVE EE'S MIDDLE KNUCKLE ON EE'S RIGHT HAND.

employee was picking up a box of glue off of the scoop stepped in a hole and felt pain in EE's lower back

A non-injury roof fall occurred in the No. 1 entry at x-cut No.21 on the 005-0 MMU. Roof Fall size approx.20'ft. x 20'ft x 6'-7'ft. thick. The fall will be cleaned up and roof re-supported. (No injuries associated with this Roof Fall). See attached Roof Fall Map.

EMPLOYEE WAS PUTTING A LID BACK ON THE MINER WHEN THEY FELT PAIN IN RIGHT ELBOW.

THE ROCK FALL OCCURRED ON THE 005 SECTION AROUND 11:45 AM IN THE #5 HEADING CROSS CUT (16) 20 FOOT OUT BY THE FACE FALLING ON THE ROOF BOLTER. NO ONE WAS INJURED. THE FALL MEASURED 18 X 30 X 6 FOOT THICK.

EMPLOYEE WAS CLEANING ROCK OFF THE MINER WITH A SHOVEL WHEN LEFT FOREARM CAME IN CONTACT WITH A SHARP PIECE OF ROCK CAUSING A LACERATION THAT REQUIRED 7 STITCHES, NO LOST WORK DAYS.

The accident was an unplanned roof fall

EE was riding in mantrip & hit a bump on a section of muddy roadway jarring ee neck. EE started missing work on 5/15/18.

Employee was installing straps & bolting in #1 entry. When a rock fell striking employee in face & head. Employee had stitches on the nose & scratch on the cornea.

Battery end operator side tire was coming apart, tramming car to repairman & a flapper tore fender off & came in deck, hit left arm in bicep area.

While moving cut ends from timbers, the employee picked up a timber end and a piece of draw rock fell striking EE on the right forearm and wrist causing a laceration on the arm requiring sutures.

While employee was repositioning the roof bolter head to install the next bolt ee stepped over toward the drill station when right knee popped. EE was diagnosed with a torn meniscus that will require surgery.

While backing up a 4 man ride, the employee caught ee left foot between the ride and a crib resulting in a fracture to the left heel.

Employee was walking back from the feeder to their car when they twisted their right knee. Employee was diagnosed with a torn meniscus.

Employee was taking bottom belt structure loose while the belt was running. A belt splice hit a bottom hanger and moved it striking the employee on the left arm and wrist causing a contusion. The employee began missing work on 7/20/2017 due to the injury.

While moving the roof bolter, a piece of roof rock measuring 13" x 15" by 2" in thickness fell striking employee's hard hat causing the hat to slide down employee's face. Employee sought medical attention on a later date and began missing work on 2/9/2016 when employee had surgery on their neck.

A roof fall was discovered on this date in a bleeder entry. The fall was located in the wall entry between the L4 and L5 panels in the bleeder. The area was previously supported with 6' torque tension bolts, super bolts, and truss bolts. The area was dangered off and a BEP was established.

A roof fall was discovered on this date. The fall was located in the #1 entry extending into the crosscut at crosscut 16 of the 2-Left Panel off 6-West. The fall measured 18' by 30' and was 10' in thickness. The fall area was dangered off. There were no persons affected. The area was previously supported with 6' torque tension bolts and 12' tensionable cable bolts.

Employee sought medical attention on this date after months of reoccurring pain in lower back. An MRI was obtained and the results shows that Employee has degenerative disc disease from L3-L4 to L5-S1. Under the advise of the physician, employee began missing work on 2/21/2017 due to the condition.

A roof fall was found on this date during weekly exams. The fall was located on the 2-North Mains at crosscut #50 in the #6(Intake) entry. The fall measured 20' by 20' and was 5' in thickness. The area was originally supported with 36" resin bolts and timbers. All persons were moved outby until the escape way was re-routed. No persons were affected.

A roof fall was discovered on this date. The fall was located in the #1 entry of the 2-Left Panel on MMU 003. The fall measured 20' by 30' and was 8' in thickness. The area was previously supported with 6' torque tension and 12' cable tension bolts. The area was dangered off and the return entry rerouted. No persons were affected.

A roof fall was discovered on this date. The fall was located on the 2 Left panel at crosscut #22 in the #1 entry. The fall measured 70' by 18' and was 8' in thickness. The areas was previously supported with 6' torque tension bolts and 12' cable tension bolts. The return entry was rerouted and no persons were affected.

A roof fall was discovered on this date during weekly exams. The fall was located in the #1 entry of the Lower 3-Left Bleeder. The fall measured 18' by 20' and was 6-8' in thickness. The area was dangered off and ventilation was not affected.

As worker was helping align a feeder during a set up, went around the feeder to look at the alignment. As EE walked around the feeder, slipped and fell striking chest on a crib block that caused a contusion to chest. EE began missing work on 5/11/2016 when EE had the contusion drained by a physician.

A roof fall was discovered on this date. The fall was located in the #3 entry of 3-North-2 roadway at cc #73. The fall measured 20' by 50' and was 10' in thickness. The area was originally supported with 5' Hy-tech bolts. The secondary escape way was re-routed and the fall will be cleaned up. No persons or ventilation were affected.

On this date, employee reported that he had been suffering back pain. He sought medical advice and began missing work on 12/7/2015 to seek treatment. Results of a MRI shows anterolisthesis and degenerative changes to his L4-L5 disc. He remains off work to continue treatment.

Employee was taking a belt spreader our during a belt pull back when he felt pain in his right bicep. He continued working and sought medical treatment on 11/03/2015 for a surgical procedure on a tendon in the arm. He returned to work his next scheduled shift.

A roof fall was discovered on this date. The fall was located in a crosscut at break #10 between the #4 and #5 entries, on L-3 left panel. The fall measured 20' by 20' and was 8' in thickness. The area was originally supported with 6' torque tension bolts. The fall was cleaned up and re-supported. There were no workers involved nor was ventilation affected.

A roof fall was discovered in the #5 entry of 3-Left paned on 003 section between crosscuts 17 and 18. The fall measured 20' by 40' and was 8' in thickness. The area was originally supported with 6' torque tension bolts, and 12' super bolts. There were no injuries and ventilation was not affected.

A roof fall was discovered on this date. The fall measured 20' by 30' and was 10' in thickness. The fall was located on 3-Left Panel in the #1 Entry at Crosscut #3. The area was originally supported by 6' torque tension bolts and 12' super bolts. There were no injuries and ventilation was not affected.

A roof fall occurred on this date. The fall was located on the 005 section while retreat mining was taking place. The fall measured 20' by 20' and was 8' in thickness and was located in the #4 entry inby the #44 crosscut. Two mobile roof supports were located inby the fall and had to be recovered. No injuries occurred due to this event.

A roof fall was discovered on this date. The fall occurred on 3-North-2 main return at crosscut #75 in the #2 entry. The fall measured 25' by 25' and was 8' in thickness. The fall area was previously supported with 5' Hy-tech bolts and cribs. The fall area was cleaned up and resupported. There were no injuries or equipment affected with this fall nor was ventilation affected.

The roof bolter was backed out into the #3 entry at crosscut #12 and employee was on inby side of bolter changing a coupling of a piece of drill steel when a loaded ram car came out of the heading and pinned employee between the car and roof bolter resulting in multiple injuries to his pelvis.

Employee was assisting another worker install a roof bolt. When the mast was lowered, it caught his right hand between the drill pot and mast base causing multiple fractures to the hand and the loss of the ends of his index and middle fingers which were removed during later surgery.

While operating a ram car during loading from the face, an undetermined object was propelled into the employee's left eye, resulting in the loss of sight in the eye. The employee was wearing eye protection, but had the protection lowered.

As employee was traveling through a water hole he twisted his right knee. On 5/9/2013 he had surgery on the knee to repair a torn meniscus and it was determined that it was related to the work injury making it reportable.

While employee was tramming the miner, a piece of draw rock (25" X 18" X 0-7") fell striking the employee on the left foot causing a hairline fracture to the second metatarsal.

A roof fall was discovered on this date. The fall was located on the Slope beltline from break 43 to 44. The fall measured 20' wide by 100' long and was 2-5 feet in thickness. The fall was cleaned up and re-supported. The area was originally supported with 4' fully grouted rods. There were no persons involved.

The injured employee was using a slate bar while working with a crew to repair a breaker car. The bar slipped and struck him in his right forehead. This resulted in a laceration that required medical treatment.

A roof fall was discovered on this date. The fall was located in the L8-Right Panel in the #1 entry, #28 crosscut. The fall measured 15' by 20' and was 8' in thickness. The area was originally supported with 5' Hy-tech bolts and 12' cable bolts. Ventilation or persons were not affected.

The fall was originally supported with 5' Hi-tech bolts and 12' cable bolts. There were no persons involved and ventilation was not affected.

The area was originally supported with 5' Hi-tech bolts and 12' cable bolts. Ventilation was not affected and there was no persons involved.

While locking down a band while splicing a water line, the employee hit is right index finger with a hammer resulting in a fractured finger.

The employee had his right foot partially out of the mantrip when his foot struck a timber on the roadway. He suffered a laceration to the right big toe and the tip of the toe had a non-displaced fracture.

A roof fall was discovered on this date. The fall was located in the #1 return entry on 7-Right at crosscuts #37-38. The fall measured 100' by 20' and was 10-12' in thickness. The fall was dangered off and no persons were affected. The area was originally supported with 5' Hy-tech bolts and 12' cable bolts.

A roof fall occurred on this date. The fall was located on 3-North Mains at break #70 in the #6 entry. The fall measured 25' by 20' and was 10' in thickness. The area was previously supported with 5' Hy-Tech bolts. The area has been dangered off with cribs set in the area. The primary escapeway was rerouted. There was no persons affected.

The foreman was watching production activities when a piece of rib rock fell striking him causing him to strike his nose against a mobile bridge unit. The employee continued working and sought medical treatment for his nose on 2/4/2011. The nose was determined to be fractured on that date.

A roof fall occurred while retreat mining was taking place. The fall was located just outby the MRS units on the 005 working section. The fall was located inby crosscut # 54 in the #4 entry and measured 20' by 15' and was 5' in thickness. No equipment or persons were involved.

While he was tightening a roof jack, a piece of draw rock fell striking the employee on the left arm, causing a laceration that required 3 sutures.

While bolting, a small piece of draw rock fell striking the employee on the left ear, causing a laceration requiring sutures.

Employee reported that while traveling on man trip ride with other crew members, he bumped his head on a low area of the top. There was no visible injury but he c/o dizziness and was taken to Harlan ARH where he was treated and released.

A roof fall was discovered on this date. The fall was located on MMU-005 in the #3 entry at break #49. The fall measured 20' by 20' and was 8' in thickness. The area was previously supported with 5' Hy-Tech Bolts and 12' Superbolts. The fall occurred during an idle shift, with no personnel present.

A roof fall ocurred on 003 section during retreat mining. The fall ocurred inby the 65th x-cut in the pillar block extending out over the continuous miner. The size of the fall was undetermined and the fall pulled 2 roof bolts. MSHA was notified immediately.

A roof fall has occurred on the 005 Section. The fall measured 50' by 20' and was 5' in thickness. The fall in the number 3 entry at crosscut #72. The area was previously supported with 5' Hy-Tech bolts and superbolts.

A roof fall was discovered on this date. The fall measured 60' by 20' and was 6' in thickness. The fall was located on the 3-North Return in the #1 entry between crosscuts #51 and #52. The fall did not affect any persons or ventilation. The fall was dangered off.

Employee had been lifting belt structure during his shift on 8-18-09. At approx. 5:00 A.M. he complained of severe pain to his lower back and was taken to Lonesome Pine Hospital for evaluation. He was treated and released to the care of his regular physician.

Employee tripped over a rock and fell at approx. 1:30 A.M. Later that day he went to a local clinic for an x-ray & treatment due to pain in his index & middle fingers of his right hand. The x-ray was neg. for fracture & he was diagnosed with sprain to his fingers & put off work for 10 days.

A roof fall occurred on the Slope Roadway at break #58. The fall measured 20' by 20' and was 4' in thickness. The fall was cleaned up and re-supported. No equipment or employees were involved.

A rock fall was discovered on this date on 005 section. The fall started near the gob line during retreat mining and extended outby into the intersection at CC #51 in the #4 entry and extended into the crosscut toward the #3 entry. The fall caught the tail of the continuous miner that was parked in the crosscut. The fall ocurred during an idle shift with no one present.

Employee was pulling a fork on a Lo-Trac when the fork came off and struck the employee's left ankle resulting in a fracture.

While roof bolting, the employee was adding the second piece of steel to the starter steel when the bolter head moved, catching his right thumb between the two pieces of steel. This resulted in a laceration and fracture to the thumb.

While tramming a roof bolter a piece of rib fell, striking the employee on the head and neck, causing a laceration to his forehead that requiried sutures. The rib material measured 3' by 2' and was 4" in thickness.

While pulling a piece of conveyor belt with a pair of vice grips, the vice grips slipped off the belt causing the employee to fall backwards onto his right foot resulting in a fracture to his lower right leg.

While crossing a bridge while carrying some roof bolt plates, employee felt something pop in his left shoulder. He started missing work on 2/16/2009 to have surgury on the shoulder.

A roof fall occurred in the 6-East Intake # 7 Break # 5. The fall in # 7 Entry was 60' long and 20' wide and 7' thick. There was no equipment involved or any injuries. The fall was cribbed and dangered off. The area was originally supported with 5' Hi-Tech roof bolts.

A roof fall occurred in the 6-East intake, Entry # 8, Inby Break # 5. The fall in #8 Entry was 30' long and 20' wide and 6' thick. There was no equipment involved or any injuries. The fall was cribbed and dangered off. This area was originally supported with 5' Hi-Tech roof bolts.

As the injured EE was hooking up a supply car to a scoop, he raised up, striking his right ear on a sign that was hanging from the mine roof and caused a laceration to the ear which required sutures.

Employee was loading retriever links from a flat car to a battery ride when he felt a sharp pain in his lower back. He started missing work for treatment on 5-23-08.

A roof fall occurred in the 6-East #1 Entry at break #23. The fall was 25' long by 20' wide, and 6' in thickness. There was no equipment involved, or any injuries. The fall cleaned up and the roof re-supported. The area was originally supported with 5' hi-tech roof bolts.

EE was cleaning out to access the crawler take up on the miner, a small piece of rib/coal fell from the lower rib striking him in the lower leg. EE suffered a fracured tibia and a laceration to the leg.

As retreat mining was taking place the left side MRS unit was being trammed forward after the #9 cut. A fall occurred in the #9 cut, coming out into the entry trapping the MRS units. The area was originally supported with 5' Hy-Tech bolts. There were no injuries. The fall occurred on the 1 Right Panel 005 unit in the #1 entry.

A roof fall was discovered on this date. The fall is located in 8-Right, Break #9, in 2-Left crosscut. The fall measured 18' X 20' and was 6' in thickness. The roof was previously supported with 5' Hy-Tech bolts. The fall will be cleaned up and supported with super bolts. Ventilation was no affected and there were no injuries.

A welder cable fell on the tram pedal of a mechanic's buggy causing the buggy to move forward. The buggy struck the injured employee pinning his leg against a coal rib. No one was operating the buggy at the time. The injured employee suffered a fractured lower right leg.

Employee was bolting top when a bolt he was attempting to anchor dropped approx. 9 feet and struck his right thumb. He was treated and released at the local ER. He required stitches to close a laceration. His thumb was fractured as well.

A roof fall was discovered on this date in the #6 entry on the 3-North Intake at break #67. The fall measured 20' by 20' and was 8 feet in thickness. The roof was previously supported with 5 foot Hy-Tech bolts. The fall is not impeding ventilation.

After the employee cut a timber with a hydraulic saw, he took a step and tripped over the piece of wood he had cut from the timber, causing him to fall onto the timber and the saw lacerating his left ring finger on the saw blade requiring sutures. The saw was not energized when his hand hit the blade.

Employee stated that while disassembling belt structure, he reached too high & his left shoulder "popped out of place". He said he was able to "pop it back in place" and continued work until his shift ended. Later that evening he had pain & sought treatment at the local emergency room.

A roof fall was discovered on this date. The fall was located in the #2 entry of the 7-right panel off 3-North #2 in the #5 crosscut. The fall measured 20' x 20' by 5'-6' in thickness. The area was orginally supported with 5' high tech bolts and 5' torque tension bolts.

As employee was helping move a pump to re-install the flexible water line, he slipped and fell straining his back. The conditions were wet and muddy. He sought medical treatment after his shift on 4-9-07 and was diagnosed with a disc herniation and a degenerative disc. He began missing work on 3-10-2007.

As employee was installing a boom jack on a mobile bridge unit, the jack fell off the jack bracket pinching his left ring finger between the jack and the bracket. He suffered a laceration and a fracture to the finger. The employee continues to work restricted duty.

A roof fall occurred on the 005-0 section of the 6 East Panel, in the #4 entry intersection & extending into the 4 left crosscut at #18cc. The fall was approx. 7'high X 20'long X 20'wide. The area was originally bolted with 5'hi-tech bolts.

Employee was cleaning a rock off the continuous miner. While flipping the rock into the conveyor of the miner his left ring finger was caught between the rock and the mine roof.

A roof fall was discovered on the 3 north main at the #21 crosscut in the #4 entry. The fall measured 20' by 20' and was approximately 12' in thickness.

Apparently, a small confined methane face ignition occurred on the 003-0 section in #3 entry right crosscut of the 9 Right Panel off of 5 East. No injuries occurred. After the investigation of the incident by MSHA and company personnel, no signs of ignition, burning or charring were observed.

Employee was cutting a piece of drill steel. When he turned to hand the piece of steel to a co-worker, he felt his lower back tighten up. The EE continued to work & began missing work because of the injury on 11/6/06.

As employee was walking to his bridge carrier, a piece of draw rock fell from between roof bolts striking him on the right leg.

A roof of mine roof occurred on the 005-0 section, 3 right panel, #4 entry at #9 crosscut. The fall was approximately 19' wide, 35' long and 6' thick. The fall occurred during retreat mining, catching the Joy continuous miner and 2 MRS's. The material was removed and the equipment removed safely.

A roof fall was discovered in the # 5 entry at #21 crosscut of the 3 North Intake. Thae fall was approximately 20' wide, 20' long and 5' high. The area was originally bolted with 5' high tech bolts.

He was watching the miner cut coal on the 005 section when a piece of rock fell from between the roof bolts and struck his right leg. The rock was approximately 26" x 67" x 15" thick.

He was operating the continuous miner and as he was loading material, a rock flew from the tail of the conveyor and struck him in the face. The rock was small and he was located approx. 10' to 15' from the machine.

He was drilling a roof bolt hole when a piece of rock approximately 6" x 6" x 1" thick fell and struck his chin.

He was on a ladder taking down a charger cable and lost balance and fell.

EE was backing the miner out of #5 entry and walked between the #4 mobile bridge unit and a rock that was laying against the coal rib. The mobile bridge unit swung around and caught his left leg against the rock.

EE was cleaning the mine floor in preparation of setting the next timber when a co-worker accidently dislodged a previously installed timber. The timber fell and struck him in the back.

He was operating a lo-trac and hauling timbers when he ran over something in the roadway, causing him to be jarred around.

A roof fall occurred in the belt entry #3 ont he 005 section 5 west mains. The fall was located in an enter section 1 crosscut inby survey staiton. 24576. The fall measured 18'l x 14'w x 8'h. The fall will be cleaned up. superbolt will be installed prior to starting clean up operators.

He was drilling a roof bolt hole when a piece of rock fell and struck him on the arm.

HE WAS WALKING IN A STOOPED MANNER DUE TO HEIGHT RESTRICTIONS AND FELT PAIN IN HIS BACK.

He was operating a lo-trac diesel loader while removing a belt drive head roller. He bumped the lo-trac into the rib & a piece of the rib rolled off & struck him in the back & hip. He has since complained of pain in his back & hip. on 11-30-05, he was diagnosed with a compression fracture at T6 & severe arthritis of the hip. He began missing work on 12-9-05.

He was drilling a roof bolt hole when a piece of rock approx. 12" x 12" x 1" thick fell and struck him on the arm resulting in a laceration.

HE WAS OPERATING A MOBILE BRIDGE UNIT & WHILE BACK UP, HE RAN OVER SOMETHING IN THE ROAD CAUSING HIM TO BE JARRED. DRS REPORT ON 9/27/05

He was putting a pin in a rub rail guard on a continuous miner and got his thumb caught between the rail and the frame of the miner.

He was crossing over a mobile bridge system and tripped and fell. He continued to work and started missing work on 8/30/05. The original doctor's report showed his absence due to rheumatiod arthritis but on 9/30/05 we received a MRI report showing a small rotator cuff tear in his shoulder.

HE WAS WALKING UP AN ENTRY AND STEPPED IN A HOLE AND TURNED HIS ANKLE.

EE WAS TRYING TO LOOSEN A BOLT BY PUSHING ON THE WRENCH WITH HIS FOOT. HE STATED THAT HE FELT SOMETHING POP IN HIS BACK. HE CONTINUED TO WORK AND BEGAN MISSING WORK ON AUGUST 1 2005

HE WAS PREPARING TO SET TIMBERS ON THE CORNER OF #4 ENTRY WHEN A RIB ROLLED OUT AND STRUCK HIM ACROSS THE LEGS.

EE WAS WORKING ON THE CHAIN GUIDE ON THE CONTINUOUS MINER WHAN A RIB ROLL OCCURRED. EE WAS STRUCK ON THE LOWER RIGHT LEG AND FOOT.

A coal outburst occurred on the 005 section, 7 left panel in #3 entry inby #28 crosscut. The outburst was triggered by the continuous miner while mining the "B" run to the right.

A ROOF FALL OCCURRED ON THE 005 PILLAR SECTION IN THE #2 ENTRY STARTING AT THE PILLAR LINE AND EXTENDING OUTBY APPROXIMATELY 70 FEET. THE FALL WAS OUTBY CROSSCUT #57 AND WAS APPROXIMATELY 20' X 70' X 9' THICK. FIVE FOOT HIGH TECH ROOF BOLTS HAD BEEN INSTALLED IN THE AREA AND 6' AND 9' SUPPLEMENTARY SUPER BOLTS HAD BEEN INSTALLED ALSO.

A ROOF ALL OCCURED AT #50 CROSSCUT ON THE 7 LEFT PANEL IN #2 ENTRY IN THE INTERSECTION. THE FALL WAS APPROXIMATELY 25'X25'X6' THICK. 5' HIGH TECH BOLTS HAD BEEN INSTALLED IN THE AREA.

HE HAS EXPERIENCED PAIN IN HIS BACK FOR A LONG TIME & HAS BEEN DIAGNOSED WITH DEGENERATIVE CHANGES, SPONDYLOSIS & AGE-RELATED FACET DISEASE AT MULTIPLE LEVELS. NO RECENT INCIDENT HAS OCCURED. HIS IS STILL WORKING.

HE TWISTED HIS KNEE AS HE STEPPED DOWN FROM A SCOOP. HE CONTINUED TO WORK & BEGAN MISSING ON 10/22/04 TO REPAIR TORN CARTILAGE IN HIS KNEE.

He was installing roof bolt into previously drilled hole. As he pushed bolt into hole, it caught on lip of hole & bent the bolt. He struck bolt with tightening wrench, in hopes that bolt would jump into the hole, but instead, the bolt came loose & struck him on lt wrist. He was instructed to follow up w/his dr on next day & returned to work on his next scheduled shift.

He was in the process of drilling a roof bolt hole, when a piece of rock approximately 12" x 30" x 0" to 7" thick fell and struck him on the right forearm.

EE WAS OPERATING A BATTERY-POWERED SCOOP. HE WAS SCOOPING THE #4 ENTRY WHEN THE SCOOP BUCKET DROPPED OFF OF A LEDGE APPROX. 10" DEEP, CAUSING EE TO BE JARRED IN THE DECK OF THE SCOOP. (EE CONTINUED TO WORK AND BEGAN MISSING ON 12/28/04.)

A ROOF FALL WAS FOUND DURING THE WEEKLY EXAM ON 3 WEST RETURN, BRK. #62, IN FRONT OF THE #2 SEAL. THE FALL WAS APPROX. 20' X 20' X 6' THICK. THIS AREA WAS BOLTED WITH 60" HIGH-TECH BOLTS AND CRIBS.

He was drilling a roof bolt hole when a piece of rock approx 12" by 15" by 1" thick fell from the roof and struck the drill head and broke into pieces. A small piece of rock bounced up from the drill head and struck him under his nose.

HE & CO-WORKERS WERE SETTING BREAKER POSTS BETWEEN #4 & #5 ENTRIES AT #34 CROSSCUT ON THE 8 LEFT PANEL (005) SECTION. HE WAS ON HIS LEFT KNEE AND CLEANING OUT A SPOT TO PLACE A TIMBER WHEN A PIECE OF ROCK APPROX. 33"X 35"X 2"THICK FELL FROM BETWEEN BOLTS AND STRUCK HIM ON THE LEFT LEG, ABOVE THE ANKLE.

Roof fall occurred in #2 entry face on 8 left panel (005 section) at #38 crosscut. Fall was approx 25' long x 18' wide x 6' thick & fell outby the pillar line & entrapped 2 mobile roof supports. 5' high tech bolts had been installed in the area, supplemented with 12' super bolts.

HE WAS OPERATING THE CONTINUOUS MINER AND MINING A PILLAR LIFT WHEN A PIECE OF ROCK APPROXIMATELY 12" X 20" X 1" THICK FELL FROM BETWEEN THE BOLTS AND STRUCK HIM ON THE LEFT HAND.

A roof fall occurred in the #4 entry face on the 8 left panel (005 section) at #49 crosscut. the fall was approx 40' long x 18' wide x 7' thick and fell outby the pillar line and entrapped two mobile roof supports. 5' high tech bolts had been installed in the area, supplemented with 12' super bolts.

A ROOF FALL OCCURRED ON THE 003 SECTION FROM THE INTERSECTION IN #3 ENTRY AT #65 CROSSCUT TO THE OUTBY CORNER OF #66 CROSSCUT. THE FALL WAS APPROXIMATELY 20' WIDE X 60' LONG X 8' THICK. 5' HI- TECH BOLTS HAD BEEN USED IN THIS AREA.

AFTER DRILLING A HOLE, HE WAS REMOVING THE DRILL STEEL FROM THE HOLE WHEN A PIECE OF ROCK APPROXIMATELY 2'X2'X7" THICK FELL, PUSHING HIS HAND AGAINST THE HEAD OF THE DRILL.

HE AND A CO-WORKER WERE BUILDING THE LAST OF 3 CRIBS IN THE LEFT CROSSCUT OF #2 ENTRY, ON THE 5 NORTH SECTION (003 MMU), WHEN A PIECE OF ROCK FELL FROM BETWEEN THE ROOF BOLTS AND THE COAL RIB STRIKING HIM ON THE SIDE OF THE LEFT FOOT. THE ROCK WAS APPROXIMATELY 3' WIDE X 4' LONG X 0" TO 8" THICK.

PILLAR MINING WAS BEING DONE ON THE 9 LEFT PANEL 003 SECTION. A ROOF FALL OCCURRED IN THE #3 ENTRY AND FELL APPROXIMATELY 30' OUTBY THE MOBILE ROOF SUPPORTS. THE FALL WAS APPROX. 5' THICK X 18 ' WIDE. A MECHANICAL MALFUNCTION HAD OCCURRED TO THE MOBILE ROOF SUPPORT WHICH DELAYED IT IN BEING MOVED. THE AREA HAD BEEN SUPPORTED WITH 5' HIGH TECH BOLTS & HAD BEEN SUPPLEMENTED W/12' SUP

HE STATED THAT HE RAN OVER A ROCK IN THE ROADWAY AND JARRED HIS BACK. HE CONTINUED TO WORK AND BEGAN MISSING ON 1-28-04.

A ROOF FALL OCCURRED IN THE 5 WEST BLEEDER ENTRIES THAT WAS APPROX 200' LONG BY 40' WIDE IN THE INTERSECTION BY 10' THICK. THE FALL WAS LOCATED IN THE #3 ENTRY FROM #68 CROSSCUT. THE AREA HAD BEEN SUPPORTED WITH 5' HIGH TECH BOLTS AND SUPPLEMENTED WITH 9' SUPER BOLTS.

A ROOF FALL OCCURRED AT #7 CROSSCUT ON THE 10 LEFT BELT LINE ON 005 SECTION. THE FALL WAS APPROX 40' LONG BY 18' WIDE BY 5-10' HIGH AND FELL IN THE INTERSECTION AND ABOUT 20' IN BY TOWARD #8 CROSSCUT. THE AREA HAD BEEN SUPPORTED WITH 5' HIGH TECH BOLTS AND SUPPLEMENTED WITH 9' SUPER BOLTS, AND SEVERAL CRIBS.

WHILE DRILLING A HOLE FOR A ROOF BOLT, A SHARP EDGED PIECE OF ROCK (APPROX. 6"DIAMETER BY ZERO TO 1/2"THICK) FELL FROM THE ROOF AND STRUCK THE ROTATING DRILL HEAD THROWING THE PIECE OF ROCK INTO HIS CHEST.

HE FELT PAIN IN HIS LOWER BACK WITHOUT SPECIFIC INCIDENT AND BEGAN MISSING WORK ON 8/28/03. HE HAS BEEN DIAGNOSED WITH LUMBAR SPONDYLOSIS IN THE LUMBAR SPINE.

MINING HAD BEEN COMPLETED IN THE NO. 1 ENTRY ON THE 005 SECTION 10 LEFT PANEL AT CROSSCUT NO.46 AS THE MOBILE ROOF SUPPORTS WERE BEING MOVED OUTOF THE NO. 1 ENTRY A ROOF FALL OCCURRED THAT FEL L APPROXIMATELY 10' OUTBY THE SUPPORTS. THE FALL WAS6'THICK. THE AREA HAD BEEN SUPPORTED WITH 5' HI TECH BOLTS.

HE ORIGINALLY INJURED HIS BACK ON 2/26/96 & HAD SURGERY IN APRIL OF 1996. HE HAS CONTINUED TO HAVE PROBLEMS WITH HIS BACK AND BEGAN MISSING WORK AGAIN ON 8/14/03. HE HAS BEEN DIAGNOSED WITH DE GENERATIVE DISC DISEASE.

HE HAS A HISTORY OF BACK PAIN AND HAS BEEN DIAGNOSED WITH SPONDYLOLISTHESIS AND DEGENERATIVE OF THE DISC. THIS WAS REPORTED TO MANAGEMENT ON 08-12-2003. HE CONTINUES TO WORK.

A ROOF FALL OCCURRED ON THE 3 NORTH BELT AT #59 CROSSCUT. THE FALL WAS APPROX. 30' WIDE X 40' LONG X 8' THICK. THE AREA HAD BEEN SUPPORTED WITH 5' FULLY GROUTED ROOF BOLTS AND SUPPLEMENTED WIT H SUPER BOLTS AND CRIBS.

HE WAS OPERATING THE CONT MINER, LOADING COAL INTO THE #4 MOBILE BRIDGE CONVEYER WHEN THE CONVEYER CHAIN HUNG ON THE BRIDGE. HE WENT BACK TO SEE WHAT HAPPENED, AND A RIB ROLLED AND STRUCK HIM FROM BEHIND, PUSHING HIM INTO THE MOBILE BRIDGE.

WHILE CHANGING DRILL STEEL FROM THE STARTER STEEL TO THE FINISHER STEEL, A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIM ON THE LEFT HAND.

A COAL OUTBURST OCCURED ON THE 003 RETREAT SECTION IN THE #3 ENTRY, IN BY CROSSCUT #45. THE NOS 28A AND 28B CUTS HAD BEEN MINED ACCORDING TO THE APPROVED PLAN AND THE NO. 29A CUT WAS IN THE PR OCESS OF BEING MINED WHEN THE OUTBURST OCCURED. THE ROOF IN THE AREA CONSISTED OF MASSIVE SANDSTONE. TWO EE'S RECEIVED MINOR INJURIES IN THE INCIDENT.

HE WAS OPERATING THE CONTINUOUS MINER & MINING THE "A" RUN TO THE RIGHT IN #3 ENTRY WHEN A COAL BUMP OCCURRED. HE WAS PUSHED IN TO THE MOBILE BRIDGE CARRIER BY THE FORCE OF THE BUMP. (HE WENT TO THE ER & WAS RELEASED TO WORK & CONTINUED TO WORK UNTIL JUNE 30, 2003, WHEN HE BEGAN MISSING TO HAVE SURGERY ON A DISC IN HIS NECK.)

HE WAS OPERATING THE CONTINUOUS MINER IN #2 ENTRY, IN BY #11 CROSSCUT, MINING THE #36-A LIFT PER THE APPROVED PILLAR PLAN, WHEN A COAL OUTBURST OCCURRED. EE WAS KNOCKED DOWN WITH COAL WHICH ST RUCK HIM FROM BEHIND.

A COAL OUTBURST OCCURED ON THE 005 SECTION WHILE RETREAT MINING. THE MINER WAS CUTTING THE 36-A LIFT PER THE APPROVED PILLAR PLAN IN THE #2 ENTRY WHEN THE OUTBURST OCCURED. THE OUTBURST OCCURE D IN AN AREA WHERE MASSIVE SANDSTONE LIES DIRECTLY ON THE COAL SEAM.

AN IGNITION OCCURED ON THE 003 SECTION WHILE RETREAT MINING. THE CONTINUOUS MINER WAS CUTTING THE NO.42 LIFT (IN ACCORDANCE WITH THE PLAN) AND HOLED THROUGH INTO A PREVIOUSLY MINED LIFT WHEN THE IGNITION OCCURED. EE'S IN THE AREA STATED THAT THEY SAW A FLASH IN THE AREA OF THE MINER HEAD. THE ROOF CONSISTED OF MASSIVE SANDSTONE, & BITS STRIKING THE ROOF MAY HAVE CREATED A SPARK.

A COAL THUMP OCCURRED ON THE 003 SECTION 1 LEFT PANEL THAT THREW COAL OFF OF THE RIGHT RIB AND KNOCKED THE MINER OPERATOR AND HELPER DOWN, RESULTING IN MINOR INJURIES ONLY. THE THUMP OCCURRED IN THE #3 ENTRY WHILE RETREAT MINING WAS BEING PERFORMED.

HE WAS SETTING BREAKER TIMBERS BETWEEN #4 AND #5ENTRIES ON THE 005 SECTION WHEN A PIECE OF ROCK APPROX 20" X 36" X 2" THICK FELL FROM BETWEEN THE ROOF BOLTS AND STRUCK HIM ON THE LOWER LEG.

HE HAD JUST DRILLED A NINE FOOT HOLE IN THE ROOFUSING 3 SECTIONS OF DRILL STEEL. AS HE WAS RETREATING THE STEEL OUT OF THE HOLE, THE TOP SECTIONOF STEEL GOT HUNG IN THE HOLE. HE HAD HIS HAND ON THE BOTTOM SECTION OF STEEL TO GUIDE IT AS HEUSED THE FEED TO RECONNECT THE STEEL, AND THE TOP SECTION FELL FROM THE HOLE STRIKING HIS FINGER.

HE WAS USING A SPANNER WRENCH TO UNCOUPLE A HI-VOLTAGE PLUG, AND THE WRENCH SLIPPED, CAUSING HIM TO STRAIN HIS BACK.

A ROOF FALL OCCURED ONT HE 003 PILLAR SECTION THAT MEASURED APPROX. 20'X40'X5' IT FELL OUTBY THE MOBILE ROOF SUPPORTS APPROX. 20'. THE FALL WAS INBY #47 CROSSCUT OF THE 3LEFT PANEL OF NORTH MA INS. THIS AREA HAD BEEN SUPPORTED WITH 5' RESINGBOLTS, AND SUPPLEMENTED WITH 9' SUPER BOLTS.

HE WAS LIFTING A 5 GALLON CAN OF HYDRAULIC OIL AND STRAINED HIS BACK.

A ROOF FALL OCCURED INT HE #2 ENTRY INBY #59 CROSSCUT ON THE 003 PILLAR SECTION AND FELL APPROX. 6' HIGH X 18' WIDE X 40' LONG CATCHING TWO MOBILE ROOF SUPPORTS AND THE HEAD OF THE CONTINUOUS MINER. THE AREA HAD BEEN BOLTED WITH 5' TORQUE TTENSION BOLTS, AND TIMBERS HAD BEEN SET AS SUPPLEMENTARY SUPPORT.

HE WAS WALKING DOWN THE BELTLINE AND A METAL ROOF STRAP RUBBED ACROSS THE SIDE OF HIS HEAD, BELOW THE HAT LINE.

HE WAS PULLING THE COUPLING PIN OUT OF A SUPPLY TRAILER, THAT WAS HOOKED TO A SCOOP, AND THE TRAILER ROLLED AS THE PIN CAME OUT A PINCHED HIS FINGER BETWEEN THE PIN AND THE FRAME OF THE SCOOP.

HE WAS ASSISTING IN STARTING A LOW TRAC FORKLIFT. THE HOOD OF THE LOW TRAC WAS RAISED AND HE PLACED HIS HAND ON THE FRAME. THE LID FELL AND STRUCK HIS FINGER.

HE HAD DRILLED A HOLE IN THE ROOF FOR A BOLT AND THE DRILL STEEL HAD HUNG IN THE HOLE. HE PLACED HIS RIGHT HAND ON THE DRILL HEAD AND THE STEEL FELL FROM THE HOLE AND STRUCK HIS LEFT HAND.

HE WAS TRAMMING A SCOOP LOADED WITH ROCK DUST & THE CENTER SECTION OF THE SCOOP BROKE, BOUNCING HIM INTO THE CANOPY.

HE WAS CUTTING STRIPS OUT OF A PLASTIC BUCKET IN ORDER TO USE IT FOR A PUMP STRAINER. AS HE CUT A STRIP THE KNIFE SLIPPED & CUT HIS RIGHT THIGH.

DURING THE WEEKLY EXAM, A ROOF FALL WAS FOUND AT#56 CROSSCUT IN #2 ENTRY IN 1 NORTH MAINS. THE FALL IS APPROX. 20'X 20'X 8'THICK. 42"FULLY GROUTED BOLTS HAD BEEN USED IN THIS AREA.

HE KNELT DOWN AND HIS KNEE HIT A PIECE OF ROCK SURGERY CAUSING PAIN. HE CONTINUED TO WORK AND HAD CATILAGE SURGERY ON 12-20-2002. THE MINE WAS DOWN FOR VACATION FROM 12-2-2002 UNTIL 1-2-2003.

HE WAS SCALING ROOF AND AFTER PULLING ONE PIECE OF ROCK DOWN, HE PULLED ANOTHER LARGER PIECE AND AS IT FELL, IT BOUNCED OFF OF THE FIRST PIECE &STRUCK HIM ON THE LEG, CAUSING HIM TO FALL AND TWIST HIS KNEE. (HE CONTINUED TO WORK & BEGAN TOMISS WORK ON 9-20-02.)

HE WAS UNLOADING A CONVEYOR SPEED REDUCER FROM A STAMLER BRIDGE SYSTEM FROM A PERSONNEL CARRIER, AND IT SLIPPED FROM ONE HAND, CAUSING THE OTHER HAND TO SUPPORT ALL THE WEIGHT. AS A RESULT, HI S LEFT MIDDLE FINGER WAS MASHED BETWEEN THE REDUCER AND THE FRAME OF THE PERSONNEL CARRIER.

HE AND A CO-WORKER WERE UNLOADING A ACETYLENE TANK FROM A RUNNER. THE TANK SLIPPED FROM THE CO-WORKER'S HANDS WHEN INJURED JERKED, AND IT DROPPED ON HIS HAND.

HE STARTED MISSING WORK ON 6/3/02 COMPLAINING OF BACK PAIN. THERE WAS NO INCIDENT PRIOR TO HIS MISSING WORK. HE HAS BEEN DIAGNOSED WITH DEGENERATIVE ARTHRITIS.

HE WAS LIFTING A TIRE ON THE SUPPLY YARD AND FELT PAIN IN HIS BACK.

HE WAS WALKING UP TO THE PILLAR LINE WHEN COAL SHOT OFF OF THE RIGHT RIB AND STRUCK HIM.

HE BRUSHED HIS HAND AGAINST A BURR ON A METAL CONVEYOR AND CUT HIS THUMB.

HE WAS DRILLING A HOLE FOR A ROOF BOLT AND AS HE STARTED THE HOLE, A SMALL PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIS FOREARM.

HE WAS HELPING THE CONTINUOUS MINER ON A PILLAR SECTION WHEN THE POWER WENT OFF OF THE MACHINE. HE WENT TO THE REAR END OF THE MINER TO SEE IF THE BREAKER HAD KNOCKED AND A ROCK (APPROX. 57" X 78" X 15" THICK) FELL TRAPPING HIM BETWEEN THE MINER AND THE ROCK.

HE WAS ASSISTING CO-WORKERS IN LIFTING A ROCK WHEN HE STRAINED HIS BACK.

HE WAS SITTING BESIDE THE COAL RIB NEAR THE FACE ON A PILLAR SECTION WHEN A SMALL PRESSURE BUMP OCCURRED. HE WAS STRUCK IN THE BACK WITH COAL OR ROCK.

HE WAS PULLING ON A ROOF BOLT TO BEND IT AND FELT PAIN IN HIS BACK. (HE CONTINUED TO WORK AND BEGAN MISSING ON 4-24-02).

EE WAS LIFTING AN OXYGEN TANK FROM A MANTRIP TO WORK SITE WHEN EE FELT A SHARP PAIN IN HIS STOMACH. EE WENT TO THE DR. AND HAS A PULLED STOMACH MUSCLE.

HE WAS DRILLING A HOLE IN THE FLOOR WITH A HAND DRILL. THE DRILL STEEL BECAME STUCK AND HE AND A CO-WORKER PULLED ON THE DRILL IN ORDER TO FREE THE STEEL. THE DRILL SEPARATED FROM THE STEEL AN D THE DRILL HANDLE STRUCK HIM IN THE MOUTH.

HE AND A CO-WORKER WERE RE-INSTALLING A COUPLING ON A WATER LINE. WHEN THE JOB WAS COMPLETED, THE WATER WAS TURNED BACK ON AND THE COUPLING BLEW OFF OF THE LINE AND STRUCK HIM ON THE HAND.

HE WAS SPOT BOLTING A CORNER IN THE #2 INTERSECTION, AND THE RIB STARTED POPPING AND CRACKING. HE TURNED TO RUN AWAY FROM THE AREA AND STRUCK THE CANOPY WITH HIS FACE, WHICH RESULTED IN A LACE RATION ON THE BRIDGE OF HIS NOSE.

A ROOF FALL WAS FOUND AT #4 CROSSCUT IN #3 ENTRYIN THE 3 NORTH MAINS DURING THE WEEKLY EXAM. THEFALL WAS APPROX. 20' X 25' X 6' THICK. THE AREA HAD BEEN BOLTED WITH 5' BOLTS.

A ROOF FALL WAS FOUND AT #67 CROSSCUT IN THE #4 ENTRY ON 3 NORTH MAINS DURING THE WEEKLY EXAM. THE FALL WAS APPROX. 20' X 25' X 6' THICK. AREA HAD BEEN BOLTED WITH 5' BOLTS.

A ROOF FALL WAS FOUND AT #71 CROSSCUT IN #4 ENTRY IN 3 NORTH MAINS DURING THE WEEKLY EXAM. FALL APPROX. 20' X 35' X 6' THICK. AREA HAD BEEN BOLTED WITH 5' BOLTS.

HE WAS BENDING A ROOF BOLT BY PLACING IT OVER THE ATRS AND APPLYING PRESSURE. THE ROOF BOLT SLIPPED OUT OF HIS HAND AND STRUCK HIM ON THE CHIN.

HE HAD SAT DOWN BESIDE THE RIB TO EAT A SANDWICHAND A PIECE OF DRAW ROCK APPROX 52" LONG BY 26" WIDE AND 5" THICK FELL FROM BETWEEN THE ROOF BOLTS AND THE RIB, AND STRUCK HIM ON THE SHOULDERS AND BACK.

HE WAS OPERATING A CONTINUOUS MINER AND THE RIB POPPED, DUE TO OVERBURDEN PRESSURE, AND A PIECE OF COAL STRUCK HIM ON THE ARM.

HE STATED THAT WHILE CRAWLING ALONGSIDE A ROOF DRILL HE TWISTED HIS KNEE. HE CONTINUED TO WORK AND BEGAN MISSING ON 12-21-01.

HE WAS LOADING ROOF BOLTS INTO A SUPPLY TRAY WHEN HE CUT HIS LEF ON A METAL BAND THAT WAS USED TO BIND THE BOLTS TOGETHER.
