EE WAS AWARDED RET RAINING INCENTIVE BENEFITS ON THE ABOVE MENTIONED DATE FOR COAL MINERS PNEUMOCONIOSIS
EMPLOYEE HAS BEEN AWARDED A RIB CLAIM FOR COAL MINERS PNUMOCONIOSIS.
INJURED SUFFERS FROM COAL MINER'S PNEUMOCON- OSIS CATEGORY 1/0 OR 1/1. HE WAS AWARDED A RETRAINING INCENTIVE BENEFITS.
EE STATED THAT THE ROOF BOLT MACHINE HAD GOTTEN STUCK IN 1 ENTRY HE WALKED ABOUT 21/2 BREAKS TO THE SECTION POWER CENTER TO GET TIMBERS TO PUT UNDER TIRES ON THE BOLT MACHINE. HE GOT 2 SPLIT T IMBERS AND CARRIED THEM BACK TO BOLT MACHINE ANDWHEN HE DROPPED THEM HE COULD NOT STRAIGHTER UP HIS RIGHT LOWER BACK STARTED TO HURT.

INJURED WAS BENT FORWARD MOVING CONTINUOUS MINER CABLE OUT OF THE SHUTTLE CAR ROADWAY WHEN HE FELT PAIN IN HIS LOWER BACK AREA.
HE STATED THAT HE WAS TURNING OVER A ROOF BOLTER CANOPY SO THAT THEY COULD PUT IT BACK ON THE ROOF BOLTER. HIS FEET SLIPPED OUT FROM UNDER HIM AND HE FELL FACE FIRST INTO THE CANOPY STRIKING H IS MOUTH AND NOSE, CAUSING LOOSE TEETH IN FRONT OF MOUTH, A LACERATED UPPER LIP TAKING UNKNOWN STITCHES TO CLOSE, AND A BUSTED NOSE.

A ROOF FALL IN #2 ENTRY (RETURN) WAS FOUND ON PRE-SHIFT EXAMINATION 14 BREAKS INBY PORTAL. ROOF FALL MEASURED 20'X20'X3 1/2'.
EE STATED THAT HE GOT OUT OF PERSONNEL CARRIER TO OPEN AIR LOCK DOORS TO LET PERSONNEL CARRIER THRU. AFTER THE PERSONNEL CARRIER WENT THRU HE WAS CLOSING THE AIR LOCK DOORS WHEN THE DOORS CLOS ED QUICKLY CATCHING HIS RIGHT HAND BETWEEN THE DOORS HE WENT TO DR AND NO TREATMENT RENDERED ON 12-6-91. HE CONTINUED TO HAVE PAIN, AND SAW ANOTHER DR ON 1-31-92 AND DISCOVERED FRACTURE.

ROOF FALLS FOUND IN #1 AND #2 ENTRIES(RETURNS).AT BREAK 9 IN #1 ENTRY 6'X30' X30'.IN #2 ENTRY BETWEEN BREAK 10 AND BREAK 11 IS 3'X30'X20'. IN #2 ENTRY AT BREAK 13 IS 3'X30'X50'.THESE FALLS WER E FOUND BY (NAME AND NAME,MSHA AND NAME WITH SAFETY DEPT).
INJURED EMPLOYEE STATED THAT THE #3 ENTRY RIGHT BREAK HAD JUST BEEN MINED AND CONTINUOUS MINER WAS BEING BROUGHT OUT AND MOVED TO NEXT PLACE. INJURED THEN STATED THAT HE PICKED THE MINER CABLE UP AND WHEN HE DID SO THAT A VERY SHARP PAIN CAME TO HIS LOWER BACK AND DOWN INTO HIS LEFT LEG.
CARRYING BAG OF ROCK DUST DOWN POWER CENTER TIPPED OVER POWER CABLE STRAINING LOWER BACK
HE STATED THAT HE WAS TRAMMING THE MINER FROM ONE ENTRY TO #1 ENTRY AND A PIECE OF DRAW ROCK APPROX. 1 FOOT FELL AND STRUCK THE MINER LEVER BOX AND CUT HIS RIGHT INDEX FINGER.
INJURED STATES THAT HE AND CO WORKER WERE PREPARING TO THREAD BELT THRU BELT DRIVE. ONE END OF BELT HAD BEEN PLACED UNDER SCOOP BATTERIES WHEN BATTERIES WERE RAISED TO RELEASE BELT INJURED RIG HT THUMB BECAME CAUGHT BETWEEN BATTERY LIFT JACK AND BATTERY HOUSING ACCIDENT RESULTED IN A COMPOUND FRACTURE OF RIGHT THUMB.
INJURED STATED THAT WHILE INSTALLING BOTTOM STANDS UNDER BELT ROPES, DURING A BELT SET-UP ON THE 001 ACTIVE SECTION, HE ALLEGEDLY HAD A PAIN IN HIS LEFT THIGH AND A KNOT FORMED IN HIS LOWER BA CK. NOTE: DUE TO SOEM QUESTIONS THAT WERE BROUGHT UP DURING INVESTIGATION, THIS WAS NOT RULED TO BE WORK RELATED UNTIL 2-21-90.

WHILE OPERATING THE STANDARD SIDE SHUTTLE CAR, HE WAS WAITING FOR CONTINUOUS MINER TO MOVE INTO NEXT PLACE TO BE MINED, WHEN A LOOSE SECTION OF COAL RIB STARTED TO BREAK AWAY, INJURED JUMPED F ROM DECK AREA OF SHUTTLE CAR AND WHEN HE DID SO, HE STRUCK HIS LEFT HAND AGAINST A PILE OF MINE TIMBERS, RESULTING IN A FRACTURE TO THE RING FINGER ON LEFT HAND.
HE WAS OPERATING STANDARD SHUTTLE CAR IN ENTRY APPROX. 2 BREWKAS IN FEEDER GOING TOMINER AFTER A CAR OF COAL. HE HIT A HOLE IN THE ROADWAY, AND WAS BOUNCED OUT OF HIS SEAT AND WHEN HE CAME BA CK DOWN, HE LANDED ON HIS BACK ON OUTSIDE RAIL OF OPERATORS DECK.
ROOF FALL APPROXIMATELY 20' X 40' X 4O" FELL ACROSS INTERSECTION IN THE #3 ENTRY (BELTLINE) APPROXIMATELY 660FEET INBY PORTAL. ROOF FALL IN BREAK 11.
WHILE UNLOADING BAGS OF ROCKDUST (APPROX. 50 LBS PER BAG) FROM THE #5 BELT CONVEYOR, INJURED ALLEGEDLY STRAINED A MUSCLE IN THE UPPER PART OF HIS LEFT HIP.

SCOOP HAD BELTHEAD RAISED UP TO PUT IN OIL BATHS SCOOP LET THE BELTHEAD BACK DOWN BUT FRAME OF BELTHEAD GOT CAUGHT ON OIL BATH BELTHEAD WAS RAISED BACK UP AND VICTIM WAS PUTTING OIL BATH IN PL ACE WHEN BELTHEAD FELL CATCHING VICTIMS LEFT AHDND BETWEEN BELTHEAD FRAME AND OIL BATH
