Mine #1 Coal

Employee was helping to remove the feeder. Material around the feeder broke loose and caused employee to stumble and fall. Employee continued working until 10/29/12 when he began missing work. On 1/28/13 we were notified that this was an occupational illness due to mental stress.

While racking conveyor belt EE's pinky finger was caught between a pulley on a sheave wheel and the rope attached to the conveyor belt, resulting in a fracture to the tip of his pinky finger.

Claim of cumulative trauma. Neck, shoulder, low back and knee pain. Diagnosed date of 1/21/2014.

Employee was removing a pin to unhook the motor from a roof bolter it was towing. When he pulled on the pin the bolter shifted causing the pin to shift smashing his middle and ring finger on his left hand

Employee was hanging miner cable when he felt a pain in his lower back. He has continued to work his regular scheduled shifts. On 4-27-12 he visited his Dr. and was given an immobilization device and continues to work his scheduled shifts.

A non-injury roof fall occurred at this mine 2 breaks inby the #3 intake portal. The fall was approximately 22'W X 20'L X 4'H. The area has been breakered off.

Employee was using a slate bar to remove a rock from around the head jack of the continuous miner, when the bar slipped striking him in the ribs resulting in two fractured ribs. Employee continued to work until 9-17-11 when he was diagnosed with the fracture.

Employees had scaled some loose ribs in an old overcast area and was in the process of spot bolting the area. As he was putting the drill steel against the roof a piece of rib fell out knocking employee into the roof bolter controls causing a laceration to his face.

While installing 4" water line injured employee was positioned behind another employee, while other employee was hitting water line between his feet the hammer slipped striking injured employee in the mouth.

Employee was loading a joint of 6" waterline when he felt a sharp pain in his lower back. He has continued to work his scheduled shifts and has started physical therapy on 10/11/2011.

EE was removing a section of belt from continuous miner head using a utility knife, knife slipped and cut his ring finger. The laceration required 10 stitches to close the wound.

EE was welding on a parts box with his back to another parts box. Two other employees were using an overhead hoist to move the second box and it flipped over striking EE fracturing his right lower leg.

Member was pulling 5'roof bolts out of a bundle to load onto his tray and his back started hurting .He was seen by a specialist and told to remain away from work because of a sprain or a herniated disc in lower back.

Employee was loading an old drill pot onto flat car, by hand, when he felt a pain in his lower back and down his left leg. He started missing work on 5-31-11.

Member was bolting in 2 left putting up the 2nd bolt he was putting glue and bolt into hole when a small rock hit the canopy and then causing a small cut to his face. He worked his next shift and then missed 2 days because he sprained his neck when he seen small rock falling

Roof Fall, Parrell 2nd northwest mains area was already dangered off prior to it falling in fall was Apprx 100'l x 5' H in between #4 and #5 enrty . MSHA has checked the area it won't be cleaned up.

EE had been shoveling on LF 1 belt as he twisted to put material on the belt with a shovel he felt pain in lower back and his left leg went numb. EE had back surgery in the past and his symptoms were the same as before the back surgery. I ask this be classified as an occupational illness because of his past history with his back.

EE was in the process of taking the wheel unit bolts out of a shuttle car he was using a impact wrench and 3/4" Allen plug to loosen the bolts someone had placed a nail were the dow pin went to hold the plug in the socket and he started the impact wrench and it caught his glove on his right hand and caused a laceration requiring 4 sutures to close the wound.

EE was standing beside the forklift as they were loading the scoop bucket with a bag of bulk dust. He reached down to get the pallet and had his right foot there also as the forklift pulled up, when he did this, he ran over his right foot causing a contusion with the forklift.

EE was placing a wrecked flatcar back on track and had hooked a chain ratchet into the eye of a roof bolt plate and the eye broke out allowing the chain ratchet to fall and hit EE in the head causing a mild concussion. He had asked off prior to the accident for a funeral on 1-28-11 and he wasn't scheduled on 1-29-11 He returned on 1-31-21.

Employee was removing a 60" top roller 2 bks out by #3 tailpiece. He had raised the belt with a chain ratchet and was pulling the top structure out when his back started hurting. He finished his shift then called in the following day stating he was going to the doctor who prescribed him physical therapy for 2 weeks because of a strain.

EE wrecked the mantrip at bk-50 on Rh 1 track he was using a 10 ton railroad jack when the bar struck him on left side of chin at his jawline he lost consciousness for a few minutes. He was taken to hospital and x-rays were negative and he was released back to work on his next shift he did return to his normal job.

Roof Fall in the brushy mains between survey stations 14081 & 14085 in between #1 and #2 entries. MSHA inspector found this fall during his examination.

Fall was found on 11/30/10 MSHA was present and notified about roof fall. It was located #1 entry between bk 12&13 on the 4th west mains. Area will be dangered off, fall will not be cleaned up MSHA has completed their investigation of the area.

Roof Fall 3rd west submains 1 bk outby spad number 31423 goes into #2 entry fall is 6' high x 19'w x 300' long area is dangered off and checked by MSHA during routine inspection and will not be cleaned up.

EE was tightening turnbuckle on tailpiece when his hands slipped off hammer allowing him to fall into the metal tailpiece on section striking his facial area.He had stitches to repair his punctured tongue and chipped his front tooth.

Fall in 4th North West mains in # 2 entry 1 Bk inby spad number 30660 Apprx. 12' High x 20' Wide x 40' Long, this area is dangered off and MSHA has been notified of this rockfall.The fall will not be cleaned up at this time

Member was taking down a roof bolter cable he was standing on top of the machine he lost his balance falling to the ground landing on his rt side fracturing his ribs. He never quit working went to hospital where he was diagnoised with 2 broken ribs and allowed to return back to his job

Roof Fall between #6 &7 entry 1ST NW Mains spad no 318& 319 fall was found by MSHA inspector and our examiner, it will be dangered off. Fall is 30' Long x 18 ' wide x5' High. Area will not be rehabilitated at this time.

Cleaning walkway on 6 belt bk.26 and tripped on a rock and his finger went into a top roller pushing his finger nail back, he had 1 stitch to repair the injury. This is reported late because I was unaware that we were responsible for reporting contractor accidents and have been checking msha data base to see if it has been reported by the contractor company.

Hitting insert in the continuous miner with a hammer piece of metal went into index finger. Had to be removed by a doctor embedded object. The piece of metal was removed and he returned back on his next shift without restrictions.

Member was bolting in the # 3 heading they had installed 2 rows of bolts he installed outside bolt and swung over to install inside bolt drawrock fell striking rt forearm causing laceration requiring stitches. He returned back on his next shift.

EE was cutting the plastic wrapping off of a bearing when he cut his right hand with a utility knife requiring 4 stitches to close the wound.

#1 entry 003 section at spad 31450 fall was 20"w x 20'l X 8 " high fall was checked by MSHA and will not be cleaned up area was dangered off.

Roof Fall found on 3/3/10 on 2 east panel@bk56 between 3 and 4 x-cut it was 20'w x40' lx 6'high investigated by MSHA dangered off and will not be cleaned up,didn't affect any ventillation or travelways.

Rock Fall 5 belt Bk-31 between 3&4 entry.Fall is 5'highx40'longx18'w fell between the #3and 4 entry dangered off will not be cleaned up. MSHA and OMSL was notified of the Rock Fall on 2/13/2010.

Member cut band on bundle of bolts and plate's, band flew back and struck his right arm. This required stitches to close his wound.

Fall was found my MSHA Inspector and weekly examiner on the 4th South West Mains between No. 5 and 6 crosscut. Area was dangered off and will not be cleaned up. Fall was approx. 100' long X 12' high X 18' wide.

EE was using slate bar to hold rail over, he had placed his hand under the rail tie to hold it up against the track. The slate bar slipped allowing the rail to move which caused a laceration to his finger requiring stitches.

EE was cleaning on tailpiece on his knees out of sight of foreman. EE told foreman to throw him a crib block when he threw the crib block it struck ee in the lower back. EE wasn't scheduled to return back to rockhouse, accident occured on 7/31/09 he was to return back to solid his next scheduled shift. He suffered broken vertebra tips and hasn't returned to work.

Employee was putting up the outside bolt in the 2 left cross cut. As he swung his drill pot to install the inside bolt a piece of drawrock fell scraping his left leg and foot. He went to a specialist and was told to remain away from work. He was diagnosed with strained ligaments.

Employee was picking up a timber in the #4 heading when he felt pain in lower back. He was given off work until 4-26-09 because of a back strain.

EE and another co-worker were moving the miner cable out of the roadway at Bk 11 on 8 belt in the track entry when coal rib rolled off, striking him on the left ankle, causing a bruning sensation. He was taken to the hospital where no fractures were found and he was given light duty work for 10 days. He had no laceration or markings on his ankle.

Roof fall was found by JD Adams it is on the North east mains at Bk-48 and is appr. 6 high and goes from number 1 entry to number 7 it is in aircourse that is required to be checked weekly. It's not close to any working sections.

Injury aggravated a old injury that he had prior to being employee at Rockhouse he had suffered a herniated disc at another time. He had been seeking treatment since 2005 for back problems. He started missing work on 6/2/09 after doing an investigation of this incident we feel that His continued back problems is why he became unable to work. cause him to start missing work.

The roof fall was dicovered by MSHA inspector during his weekly exam the fall was a continuation of an existing fall that had occured some time ago. The fall will be re-supported and cleaned up to extablish a travel way. The area around the fall will be dangered off to prevent access to the fall area 7 head at spad no 30063.

The roof fall was discovered by MSHA inspector during his weekly check the fall was a continuation of an old fall that had occured when the longwall panel was mined. The area will be dangered off and not be cleaned up. Headgate spanel at cross cut 19 thru.

Employee was finishing up the 6 right cross-cut. He was standing apprx. 6' from the 5 right cross-cut when a piece of drawrock fell knocking him backwards and then striking him above the left ankle causing compound fractures.

Employee was pulling roof bolter cable when his back started hurting, EE pulled muscle in his back.

Fall behind the return fanshaft in the number 2 entry area was cleaned and resupported. The material disturbed and caused some spot bolting that needed to be done. The area was checked by MSHA and the OMSL.

Member was letting the miner cable down when he dropped the s-hook on the cable hanger on his right hand causing lacerations and requiring stitches to close the wound.

Member was trying to remove rock off miner when he lost his balance falling into rock cutting 2 fingers on Rt hand requiring stitches to repair the laceration.

Making exam traveling track. Came upon timber in roadway, removing it EE's back started to hurt. Has had two previous back injuries and being treated prior to this incident. This injury occurred due to repeated trama over course of his mining career.

Brookeville mantrips bumped together causing Employment mining hat to come off and allowing his head to strike a sharp object requiring stitches to close wound.

Employee was traveling the Rockhouse No 1 track when a small piece of rock fell striking him above his upper lip requiring stitches to close a small laceration.

Member states he was helping to build a stoppings when he injured his back. He never reported the accident to the foreman nor were any of his coworkers told that he had hurt himself. He left the mine without filling out an accident report. I'm reporting this to prevent a citation. I don't think he was injured at this mine after the investigation was completed.

On April 22nd due to occupational illness, as substantiated by medical records and reports, ee became unable to work because of the ailments described in the medical reports. This is not due to one single event at the mine, but repeated trauma over an extended period of time.

Member was dropping his shuttle car cable, when he went to drop the cable, the hook on the rope hanger caught his nose and caused a laceration that required stitches to close the wound.

Member was inserting a pin into hitch on the rockduster, when his index finger was fractured and lacerated, requiring sutures and splint to treat.

Member stated there was a burr or something on his drill steel that caught his left glove and jerked his left hand and thumb causing a fracture to his thumb. Member returned on his next regular shift.

Member was removing belt structure from above the new 9 head installation when the discharge boom moved and landed on his foot fracturing his little toe. Member was working off of the number 8 tail piece when the discharge boom moved.

Employee was helping to pull the 350 cable along with two other members when he felt a pain in his groin area. He had continued to work until 7/7/08 when surgery was performed to repair the injured area.

Roof fall was in the No. # entry at spad no. 8172. Fall area has been dangered off and will not be cleaned up at this time. Roof fall is 20' w x 20'l x 5' high.

Fall will not be cleaned at this time. It is 30' long x 20' wide x 4' thick. it is at x-cut 9 in the main intake. The escapeway is clear for travel and the fall has be checked by MSHA and OMSL personel

Roff fall is appr 18'X30'X6'high on the 2nd west sub mains at x-cut 34 spad number 3134. Fall will not be cleaned up at this time. It was found by weekly examiner and OMSL.

Rock fall was found by weekly examiner and personel from OMSL. The roof fall was 19' x 30' x 5.5' high. Fall will not be cleaned up. It has been dangered off. It was located on 4th NW Mains at x-cut 12.

Fall was found by the preshift examiner on the 3rd shift it was located two cross cuts inby survey station 12770 in the number five intersection catching the head of the continous miner. Fall is approx. 20'x20'x6' high it was investigated by MSHA and OMSL. Fall will not be cleaned at this time the section will be moved outby this bad area.

Member was operating the forklift as he was going to pick up the section water pump he ran over the end of a piece of belt structure this caused the structure to come up and hit him in the back of his head this required staples to close the wound.

Member was loading a shuttle car when a rock measuring 21"x15"x8" slid off the fender of the shuttle car landing on top of his left foot causing a fracture to a bone that works one of his toes.

Member was operating a dbt scoop when he ran over a rock that had fallen off the coal rib this caused him to be thrown into the scoop canopy which jammed his neck.

Member was roof bolting in the 4 right cross-cut when a piece of drawrock fell striking him on the right thumb causing a chipped bone to his thumb.

Employee was cutting the bands on a pallet of 48" top belt structure. When he cut the last metal band six pieces of structure fell over hitting his right foot behind the metatarsal guard on the leather boot. This caused fractures to the bones in the top of his foot.

Operating RB in #4 entry on 003 section. Drilling hole, steel broke, debris struck EE on lower lip causing injury. Taken to E.R. stitches used to close wound.

Employee was moving the fork over on the forklift when his back started hurting. He was taken to the hospital and told to remain away from work because of a muscle strain.

Ee was loading coal out of the #1 heading, when the right rib fell over, striking him on the right foot and pinned his foot against the mine floor. He has a fracture or chipped bone to this foot. The rock was 23" wide by 15" thick.

EE was loading the operators side tray on the roof ranger bolter in the 2 left X-cut, when the rib rolled off, striking him on the back of his leg. This caused contusions and abrasions to the lower calf area. He was instructed to remain away from work for two days.

EE was operating the offside of the RRII bolter when the bolts broke that holds the valve mount assembly. When this fell, it struck EE knocking him into the rib. He injured his left knee when this occurred.

Member was hanging the roof bolter cable in the #5 x-cut when a piece of drawrock fell striking him on the back of his foot causing a fracture to his left foot.

Member was cutting the metal strap on a bundle of roof bolt plates. When he cut the strap, it contacted his right forearm causing a laceration requiring stitches.

Roof fall occured int he track entry at x-cut 8 at spad no. 40 on thr rh1 trackway. Fall was approx. 4' thick x 20x30. Area will be resupported and cleaned up.

Loading supplies onto bolter. Turned with plates, hit left hand on tram lever, injured left little finger. Taken to e.r. for treatment, stitches used.

Installing bolts, bent bolt to put into hole, when straightening bolt, injured left shoulder.

Member had been cutting strings off of bottom hanger brackets when a belt splice caught his glove pulling his left arm into the belt. He has continued to work but did get a skin graft on 3-9-07 and is now unable to work because of the risk of infection to these areas.

Setting timbers 001 pilar section. Timber fell, struck left thumb. Completed shift, went to ER, X-ray positive.

Knocking pins from boom of H.D. Draw rock fell onto head causing laceration. Stitches used to close wound, x-ray negative.

Roof fall is approx 30' x 60' x 4' high. It fell into the intersection of No.4 entry blocking the primary escapeway. Fall area has been examined by MSHA and State. The primary escapeway has been re-routed and is now travelable. The area will be cleaned and resupported.

Employee was cutting the 4 right cross-cut when a piece of draw rock fell striking him in the back area and legs. He was transported to the hospital and diagnosed with a sacrum and pubic fracture. Physical therapy is recommended for these fractures.

Employee was cutting the bands of a pallet when the knife slipped cutting his left hand requiring stitches.

EE was moving the mule cable out of the roadway, when he stepped in a hole, twisting his right knee. His 1st day away from work will be 1/2/07. He had surgery to repair a torn ACL in his knee on 12/22/06 and was on a scheduled vacation until 1/2/07. He was unable to return to work after the surgery.

Member was setting bits on the tail drum of the shear when some rock fell striking him on the neck and back area. He was taken to the hospital and released. He continued to work until 11-19-06. He was taken off work due to fractured ribs.

Employee was cleaning under the headgate shields throwing rocks into the panline when he experienced pain in his lower back. He worked his next shift and went to the emergency room where he was given three days away from work for a muscle strain.

EE was hanging the roof bolter cable when he received an electricla shock, MSHA and OMSL were notified and investigated accident. Member was taken to hospital and evaluated and received a pain shot. No further treatment was needed.

Employee was cutting insert from CM. Hot slag burnt through pant leg, right side, fell down to top of boot causing burn on side of leg. Did not see Dr. until 10-2-06 and began missing work.

Fall occurred appr. 800' from the portal at the 1 st overcase underground on the rock house track way. Area was dangered off and cleaned up and resupported MSHA and state were notified and they did an investigation.

Removing rock from pan-line, shear was being pulled. He was standing on it, got his foot caught between relay bar and pontoon.

While loading supplies onto bolter, rob roll struck left side, injuring neck and back area.

Employee was putting up 6' torque tension bolt in #3 heading. He was unaware the bolt was bent. As the bolt began to spin, it contacted the left arm causing injury. He was taken to the ER for treatment.

EE was helping load a tail roller. As he lifted a 6" sheive wheel to hang on bolt to pull the roller with, he developed pain in his low back. He was taken to the Williamson A.R.H. hospital for treatment.

Cutting old fire hose from, knife slipped, injured right leg. Taken to ER for treatment. Stitches used to close wound.

Removing rock from top of CM. Left forearm contacted sharp edge of rock causing laceration. Taken to e.r. for treatment, stitches used to close wound.

Employee was drilling hole for bolt. Applying up pressure, steel broke, kicked back, struck right jaw and rear of right shoulder. He was taken to ER for treatment. Sutures were used to close wound on jaw.

Area has been mined over and under the rockhouse coal seam. Rock fall is in the no. 6 entry & the no. 7 entry in the intersections. Fall area will be cleaned and the mine roof re-supported. MSHA AND OMSL have been notified and MSHA has looked at the roof fall.

Employee, moving broken waterline, contacted line with left forefinger, causing laceration. He was taken to E.R. for treatment; stitches used to close wound. Injury happened on Saturday.

Preparing to cut lug from drum of shear, face rock fell against drum, catching left hand between rock and drum. EE taken to ER for treatment, transfer to Louisville for surgery.

Removing canopy from shuttle car, it tilted, pinching finger between canopy and mine roof. E.R. used stitches to close wound.

EE was loading mandoor onto a pallet of block. No gloves on, cut right thumb on sharp corner. Taken to ER, stitches used to close wound. Location was #2 entry on 001 section.

Roof Fall is appr 30' in length and 20' wide by 4' thick in the number 4 entry in the intake 2nd crosscut underground. The escapeway has been re-routed thru another crosscut and entries dangered off going into the fall area.

EE was in mine office preparing for work and fell landed on right elbow. Taken to ER due to pain x-ray revealed chipped bone.

EE stepped on a crib block causing him to fall backwards landing on his back causing back pain.

EE was lifting a roll of fly pad when he experienced pain in neck area causing him to miss work. Strain his neck area.

EE was at break #85 on #5 track helping load old structure onto flat car, he dropped structure onto his left hand. He was taken to the ER for treatment and stitches were used to close the wound.

Employee was using a cinder block to knock down other blocks. The block busted, causing him to injure his left hand and fingers. He was taken to the ER where stitches were used to close the wound.

Employee was putting up bolts in #7 entry, steel broke, struck left arm causing laceration. He was taken to e.r. for treatment, stitched used to close wound.

EE was preparing to move a anchor for a shuttle car when he received an electrical shock. MSHA and OMSL were notified and investigated the accident. He was admited to hospital for observation he was kept until 12-15-05 at which time he was seen by a specialist and released to return back to his normal job.

Employee was getting an oxygen bottle when he dropped it on his right foot. It caused a bruise and contusion to his foot. He started missing the next day.

Employee was walking through warehouse, left hand struck support beam, causing fracture to his left pinky knuckle.

Helping remove planetary from continuous miner using hydraulic jack and slate bar, planetary slipped from jack causing bar to strike employee on forehead causing laceration. Employee taken to hospital for treatment; stitches used to close wound.

WHILE WALKING BESIDE THE 008 SECTION BELT, EE FELL INTO BELT CAUSING HIS RT HAND AND SHOULDER TO BECOME INJURED. HE HAS CONTINUED TO WORK. HIS FIRST DAY MISSED WILL BE 12/19/05.

Walking past shield #88, tripped, fell against jack, causing injury to his back and rib area. *Employee began to lose time 10/25/05 due to back injury.

EE was replacing belt chain, draw rock fell striking left hand causing laceration. Taken to Williamson Hospital for treatment. Stitches used to close the wound.

While loading supplies, employee was stung by a bee. He was taken the the hospital due to being allergic to bee stings.

Area has been mined under by another mine which could have caused the roof fall. Fall is app. 50' to 60' in length by 20' wide by 6' to 8' high. Area will be cleaned up & resupported. Fall is on no. 5 belt bk 17 in the number 3 entry.

EE MOVED SCOOP. WHEN EXITING THE SCOOP, A LUMP OF COAL POPPED OFF COAL RIB, STRIKING HIS RIGHT HAND AND CAUSING LACERATION. HE WORKS ON THE LONGWALL. HE WAS SCHEDULED OFF ON 8/25/05. HE RETURNED ON 8/26/05.

ROOF FALL ON LF#1 BELT LINE. FALL IS 8' HIGH BY 22' WIDE BY APPROX 50' LONG. AREA WILL BE CLEANED UP AND RESUPPORTED.

In the tailgate entry on the longwall panel a roof fall 75' to 90' and 6' thick by 20' wide. Contacted federal inspector on 5-31-05 about roof fall.

Roof fall was found by beltman on 5/1/05. Area will be cleaned & bolted. It was at No. 5 Head on the A-Frame of the Head Drive. Fall is approx. 4' thick x 16' long x 18' wide. MSHA was notified of fall when found.

ROCK FALL IN #4 ENTRY AT BK6 ON D TRACK. FALL WAS APPROX 6' IN HEIGHT, 20' WIDE AND 20' LONG. FALL WILL NOT BE CLEANED UP. AREA IS OUT BY WORKING SECTION

HAD A HOSE BUST ON SHIELD, ALLOWING EMULSION TO SPRAY ON EE'S FACE AND IN EYES. EE WENT TO DOCTOR WHERE HE WAS PRESCRIBED EYE DROPS FOR EYES.

Employee was carrying a 5 gallon hydoil can when he experienced pain in his low back and groin area. He has continued to work until 2/11/05 at which time he had surgery to repair a tear in his groin area.

WHILE TRYING TO TURN A TRAM MOTOR FOR A 10SC SHUTTLE CAR, MEMBER HURT BACK AND LEFT LEG.

At break 61 on the 008 - 009 mmu, a rock fall occurred on the belt and track. The fall was approximately 8' to 10'. It fell from the no. 2 entry to the no. 3 entry then up 1 break in the no. 3 entry. Area has been mined out under the 008 - 009 mmu.

EE WAS MOVING A WHEEL FOR A 15 TON MOTOR, ATTACHED TO THE HOIST. THIS IS A MANUAL HOIST THAT WILL ONLY LIFT OBJECTS. IT HAS TO BE MOVED BY PULLING ON A STEEL CABLE THAT IS ATTACHED TO THE HOIST. EE WILL HAVE TO HAVE THE HERNIA REPAIRED BY SURGERY.

WHILE USING A 4" DISC GRINDER SOMETIME DURING THE SHIFT MEMBER GOT A PIECE OF METAL IN HIS RIGHT EYE. DOCTOR REMOVED OBJECT FROM RIGHT EYE AND GAVE HIM PRESCRIPTION EYE DROPS.

WHILE MOVING APPROX 20 PIZZA PANS, HE CUT HIS LITTLE FINGER ON LEFT HAND, RECIEVING STITCHES TO LITTLE FINGER.

EE was removing a piece of drawrock from the top of the sheer when his left hand slipped allowing it to go into the drawrock causing a small laceration to palm of hand.

HE WAS USING A 2 LB. HAMMER AND CHISEL TRYING TO CUT A 1/2" BOLT OFF OF A BELT RAIL ON THE NEW INSTALLATION OF THE LONGWALL BELT WHEN A PIECE OF METAL PENETRATED HIS RIGHT EYE. HE WAS TAKEN BY AMBULANCE TO THE HOSPITAL WERE THE METAL WAS REMOVED.

#4 ENTRY ON 5 BELT IN MAIN INTAKE. THE ESCAPEWAY WAS BLOCKED. MEN WERE PULLED OUTBY FALL AREA. FALL WAS FOUND BY FEDERAL INSPECTOR WHILE MAKING THE WEEKLY EXAMINATIONS WITH OUR FIREBOSS. AREA WAS REBOLTED AND WILL BE CLEANED UP AFTER BOLTING IS COMPLETED. THE FALL WAS LOCATED AT BK 33 - BK 37 IN NO. 4 ENTRY APPROX. 4' THICK BY 20' WIDE BY 30' LONG.

FALL AREA WAS INVESTIGATED BY FEDERAL AND STATE ROOF CONTROL AGENTS. AREA WAS DANGERED OFF AND THEN MINED THROUGH. WE ARE OUT OF FALL AREA NOW.

EE WAS PULLING A PIECE OF ROCK WITH A SLATE BAR WHEN HE DID, IT LANDED ON THE BOTTOM THEN BOUNCED UP STRIKING HIM IN THE LEFT LEG CAUSING A SMALL LACERATION.

EE WAS WORKING ON FIRE SUPPRESSION ON SHEAR. HE WAS CLEANING SHEAR OFF WITH A WASH DOWN HOSE WHEN HE SLIPPED ON A ROCK CAUSING HIM TO FALL ALLOWING WASH DOWN HOSE TO GET LOOSE FROM HIM, STRIKING HIM IN THE RIGHT HAND. REQUIRED SUTURES.

WHILE UNKOOKING A ROPE HANGER THE MINER CABLE FELL CAUSING HIS LEFT ARM TO BE PULLED DOWN, HURTING EE SHOULDER. EE HAD CONTINUED TO WORK UNTIL 11/01/2004 AT WHICH TIME EE WAS UNABLE TO WORK.

EE WAS USING A CHAIN RATCHET TO REMOVE A CABLE TRAY OFF OF OLD STAGE LOADER IN OLD PULLOUT AREA WHEN CHAIN RATCHET CAME LOOSE STRIKING HIM IN THE MOUTH, CAUSING A LACERATION REQUIRING 4 SUTURES. THIS WAS EE'S LAST SHIFT BEFORE STARTING HIS ALREADY-SCHEDULED VACATION. HE RETURNED TO WORK ON NEXT SCHEDULED SHIFT.

PIECE OF ROCK FELL BETWEEN SHIELD # 95 & 96 STRIKING HIM ON LT. HAND CAUSING A FRACTURE TO CARPAL BONE. MEMBER FINISHED HIS SHIFT. HE WAS WEARING GLOVES AT TIME OF THE ACCIDENT.

No. 5 TRACKWAY 47 X-CUT. ROCK FLIPPED UP INTO OPERATOR DECK ON 12 TON MOTOR CAUSING A LACERATION AND FRACTURE TO RING FINGER ON LEFT HAND. ROCK MEASURED 6" X 12" X 2.5" THICK. THIS WAS HIS LAST SHIFT BEFORE HE STARTED HIS VACATION ON 7/3/04. HE RETURN TO WORK FROM VACATION ON 7/10/04 AND CAME BACK TO WORK.

EE was removing a rock out of the britby tray. other was assisting when other raised the rock from his side if flipped over. EE's hand was in the other it pinched his ring finger on his left hand. he had a fllow up with a specialist to correct his finger or 6/23/04. he returned to work on 6/24/04.

A SMALL ROCK FELL FROM THE ROOF HITTING THE DRILL POT AS IT WAS TURNING, CAUSING IT TO STRIKE EE'S GLASSES. RESULTED IN A LACERATION TO EE'S CHEEKBONE.

EE WAS REMOVING THE TOP OF A HYD. OIL CAN THAT HAD BEEN PARTIALLY CUT WITH A UTILITY KNIFE. HE WAS USING CHANNELLOCKS TO PULL ON LOOSE LID WHEN IT CAME OFF. IT HIT HIS LEFT HAND ON TOP OF THUMB, CAUSING A LACERATION.

HOSE THAT IS USED TO LET SHIELDS DOWN WITH CAME UP AND HIT EE UNDER CHIN CAUSING A SMALL LACERATION TO CHIN.

MEMBER STEPPED ON A NAIL THAT HAD BEEN IN A PALLET. WENT TO HOSPITAL AFTER WORK AND RECEIVED A TETNUS SHOT.

EMPLOYEE WAS STRUCK IN THE RT. FOOT WITH A HYD. HOSE THAT HAS APPROX. 320 PSI ON IT. THE FITTING FAILED THAT CAUSED THE HYD. HOSE TO STRIKE HIM IN THE RT.FOOT.

EE WAS HELPING INSTALL ROOF BOLTS ACROSS LONGWALL FACE WHEN A PIECE OF ROCK FELL, HITTING HIM IN HIS LEFT FOOT. HE RECEIVED TWO TARSAL BONE FRACTURES AND A LACERATION TO HIS FOOT.

ROOF FALL IN NO. 9 ENTRY ON THE 008-009 SECTION. FALLIS 4' TO 5' THICK, 60' IN LENGTH. MINING IN THIS AREA HAS STOPPED. FALL IS DANGERED OFF AND WILL NOT BE CLEANED UP.

DROPPED A 8" X 8" TIMBER ON TIP OF MIDDLE FINGER ON LEFT HAND. RECEIVED STITCHES TO TIP OF FINGER. EE WAS SCHEDULED OFF FOR 3 DAYS FOLLOWING THIS. HE RETURNED TO WORK ON 5/3/04, THE LONGWALL CREW WORK A 4 ON AND 3 OFF SCHEDULE.

EE WAS HOLDING STRUTS TO BE WELDED WHEN HE RECEIVED FLASH BURNS TO HIS EYES.

Employee was using a railroad jack & a jack bar to help put a flat car back on the track that had been wrecked. When he went to release the jack the bar slipped out of his hand & hit him over the left eye resulting in a laceration for which he received stitches.

Employee was building a brattice, he was removing a block off of a pallet when another block fell over striking him on his right thumb, resulting in a fractured thumb.

MEMBER WAS RAISING A LID ON THE SHEAR TO CHECK THE TRANSHAFT WHEN HIS HAND SLIPPED OFF THE LID CUTTING IT ON A CARTER KEE THAT WAS HOLDING A PIN. MEMBER REQUIRED STITCHES IN RIGHT HAND.

ROOF FALL WAS FOUND BY MINE INSPECTOR WHILE TRAVELING THE INTAKE AIRCOURSE. IT IS IN BREAK 56 IN NO. 5 ENTRY AND BREAK 55 IN NO. 4 ENTRY. FALL IS APPROXIMATELY 4' THICK AND 40' LONG. THE FALL WILL BE CLEANED AND REBOLTED. THE PRIMARY ESCAPEWAY IS IN THE NO. 1 ENTRY.

ROOF FALL ON NO.5 BELT BK-31. APPROX.SIZE OF ROOF FALL 4'THICK X 35'LONG. EE WAS CHECKING NO.5 BELT & FOUND THE ROOF FALL. AREA WILL BE DANGERED OFF.

PULLING BELT WITH CHANNEL LOCKS WHEN THEY SLIPPED OFF BELT CAUSING HIM TO LOSE HIS BALANCE AND FALL AGAINST TAILPIECE HURTING HIS BACK.

ROOF FALL ON NO 5 TRUCK UP. OVERLAY AND UNDERLAY MAPS SHOW AREAS HAVE BEEN MINED ABOVE AND BELOW THE CEDAR GROVE SEAM. AREA HAD BEEN INVESTIGATED BY STATES AND FEDERAL AGENCIES FALL WAS FOUND BY EE ON 2ND SHIFT.

EE was setting shield on longwall face when his foot became lodged between relay bar & pontoon. He twisted his knee.

A rock flew from the rotating shearer drum onto the panline, then glancing off panline onto his legs knocking him down and landing on his hand.

EE WAS CHANGING INSERTS ON THE RCM WHEN A PIECE OF METAL WENT INTO INDEX FINGER RT HAND. HAD TO BE REMOVED & STITCHES PUT IN.

EE WAS WALKING AROUND ROOF BOLTER WHEN HE TRIPPED & FELL LANDING ON HIS BACK CAUSING A SPRAIN TO BACK. EE WAS NOT ALLOWED TO WORK THE FOLLOWING DAY BECAUSE OF FALSIFICATION OF RECORDS. I AM ONLY FILING THIS TO SATISFY THIS CITATION THAT WAS ISSUED. I DON'T FEEL THAT THIS WAS A LOST TIME ACCIDENT.

EE WAS USING A RAILROAD JACK TO RAISE THE OUTBY END OF TAILPIECE TO RESET A CRIB BLOCK UNDER THE LEG WHEN THE JACK SLIPPED OUT CAUSING HIM TO FALL ON RT HAND & ARM. EE HAS CONTINUED TO WORK U NTIL 12/11/03 WHEN HE HAD TO HAVE A TENDON IN HIS RT ELBOW REPAIRED.

ROCK FALL BK 32 ON NO.5 BELT AREA HAS BEEN MINED UNDER & OVER TOP ACCORDING TO OVERLAY MAP'S & UNDERLAY MAPS. EXTRA SUPPORT HAS BEEN ADDED. THE FALL WAS APP 4' THICK & 120' IN LENGTH. INVESTIG ATED BY STATE & FEDERAL.

EMPLOYEE HAD A LACERATION THAT REQUIRED STITCHESAT HOSPITAL. EE WAS USING A CHISEL AND 4 LB HAMMER WHEN THE CHISEL OR A PIECE OF THE HAMMER SPLINTERED OFF AND WENT INTO LEFT LEG.

EE WAS ROOF BOLTING WHEN A PIECE OF ROCK FELL AND STRUCK HIM ON THE NECK CAUSING A LACERATION.

EE WAS ROOF BOLTING WHEN DRILL STEEL BECAME STUCK AND TRIED TO FREE IT, HE HIT HIMSELF IN HEAD, CAUSING LACERATIONS.

AREA HAS BEEN MINED OUT UNDER & OVER TOP OF US. FALL IS ON THE NO. 4 TRACK BETWEEN OVERCAST & 5 HEAD. ROCK FALL IS APPROX 4' IN HEIGHT X 30' LONG X 20' WIDE. HAS BEEN CHECKED BY FEDERAL INSPEC TORS.

OVERLAY AND UNDERLAY MAPS SHOW THAT THIS AREA AT BK 3 ON LF4 BELT HAS BEEN MINED OUT OVER & UNDER THUS CAUSING PRESSURE ON THE MINE ROOF. THE FALL IS ON THE TRACK ENTRY AND IS APPROX 6'IN HEIG HT X 70'IN LENGTH. AREA WILL BE CLEANED UP. HAS BEEN INVESTIGATED BY MSHA INSPECTORS.

MEMBER WAS USING A SLATE BAR TO GET ROCK OFF TOPTHE NO.2 CM MACHINE. WHEN THE SLATE BAR SLIPPED HE WAS THROWN ONTO THE PIECE OF DRAWROCK. HE HAS CONTINUED TO WORK UNTIL 9/24/03 WHEN THE DOCTOR HE WAS SEEING REMOVED HIM FROM WORK UNTIL SURGERY COULD BE PERFORMED ON HIS RIGHT SHOULDER.

ON JULY 14, 2003, DUE TO AN OCCUPATIONAL ILLNESS, AS SUBSTANTIATED BY MEDICAL RECORDS & REPORTS, EE BECAME UNABLE TO WORK BECAUSE OF AILMENTS DESCRIBED IN THE MEDICAL REPORTS. THIS WAS NOT DUE TO ONE SINGEL INCIDENT AT THIS MINE BUT REPEATED TRAUMA OVER AN EXTENDED PERIOD OF TIME OF 30 YEARS.

INSTALLING PIGTAILS EE REACHED FOR A STICK OF GLUE AND ACCIDENTLY TURNED UP PRESSURE SWITCH ON CAUSING DRILL TO RAISE PINCHING EE'S HAND BETWEENMONO-RAIL. HAD 4 STITCHES TO 1 FINGER ON LEFT HA ND.

THEY WERE LINEAR LINES IN THIS AREA. FALL WAS 180' LONG RANGING FROM 5' TO 8' IN HEIGHT. EXTRA SUPPORT WILL BE ADDED TO THESE AREAS ON NEXT HEADGATE III PANEL. IT WAS IN THE NO. 3 ENTRY AT BK 108 TO 110.

FALL IS AT BK 42 ON NO 6 BELT BETWEEN THE NO 4R ENTRY NO 5 ENTRY IS APPLIED IN LENGTH & 4 1/2 TO 5" IN HEIGHT. AREA WILL BE CLEANED UP & REBOLTED. FALL HAS BEEN CHECKED BY FEDERAL INS & STATE INS.

EE WAS ASSISTING THE SECTION FOREMAN MEASURE UP A HEADING WHEN STUMBLED ON A RIB THAT HAD FALLEN OFF CAUSING HIM TO TWIST HIS ANKLE. HE WAS TRANSPORTED TO HOSPITAL WHERE X-RAYS SHOW A BROKEN R T ANKLE.

FALL IS IN THE NEW LONGWALL TAILGATE ENTRY AT CHECK SPAD NO. 52 + 15. GOES TO 53 + 50. IS APPR. 6' TO 8' IN HEIGHT. IT IS UNKNOWN AT THIS TIME OF WHAT WE ARE GOING TO DO WITH ROCK FALL.

EE WAS MOVING ROOF BOLTS ON TOP OF THE ROOF BOLTER. AS HE WAS HE STUCK A METAL BAND IN HIS WRIST CUTTING IT. HE WENT TO HOSPITAL WHERE HE RECEIVED STITCHES TO HIS LEFT WRIST.

EE A FEDERAL MINE INSPECTOR FOUND THE ROCK FALL IN THE NO. 1 ENTRY ON THE LONGWALL SET-UP FALL IS APPR 6' TO 8' IN HEIGHT AND APPR 300' IN LENGTH. FALL STARTED AT BK S8-GOING TO BK 61. WE HAVE NO PLANS AT THIS TIME TO CLEAN ROCK FALL NO EQ INVOLVED OR PERSONS.

EE WAS BUILDING A CRIB WHERE THEY WERE PULLING SHIELDS ON THE TAILGATE PULL OUT AREA. HE WAS FINISHING THE CRIB WHEN HE HEARD A NOISE, HE TURNED TO GET OUT OF THE WAY AN OBJECT STRUCK HIM IN T HE BACK. THE OBJECT CAME UNDER TO SHIELD TIP WHERE HE WAS WORKING. HE TOOK HIMSELF TO WILLIAMSON MEMORIAL HOSPITAL AFTER FINISHING HIS SHIFT.

EE STATED HE WAS INJURED AT QUACKER COAL IN THE PAST. HIS BACK HAS CONT'D TO HURT SINCE. EE STARTED MISSING WORK ON 3/27/03 SOUGHT MEDICAL ATTENTION FOR HIS BACK. HE TOLD 3 SUPERVISORS THAT HI S BACK PROBLEMS BEGAN FROM AN INJURY THAT HAPPENED AT QUAKER COAL COMPANY

EE WAS OPERATING A CAT BOLTER BOLTING THE LONGWALL FACE WHEN A PIECE OF DRAW ROCK FELL CAUSING A LACERATION TO RT WRIST. EE FINISHED HIS SHIFT & THEN WENT TO HOSPITAL WHERE HE RECEIVED STITCHE S.

EE WAS WALKING UNDER A 4" WATERLINE AT THE MULE TRAIN ON THE LONGWALL WHEN HE RAISED UP HIS HEADIT STRUCK A BELT HANGER, CRACKING HIS HARD HAT & CAUSING A CONTUSION TO HIS HEAD.

EE WAS GOING TO LF #2 SWITCH ON THE TRACK OPERATING A MANTRIP WHEN SUPPLY MOTOR CAME INTO THE # 2 SWITCH THEY HIT FLATCAR HIT MANTRIP CAUSING EELT. WRIST TO BE THROWN ONTO THE SIDE OF MANTRIP.

EE WAS MOVING HIS SHUTTLE CAR ANCHOR & REHANGING IT WHEN HE FELT PAIN IN HIS LOWER BACK.

EE WAS RUNNING PHONE LINE UP TO THE SLOPE BELT ON THE ROCKHOUSE SLOPE, HE WAS TYING THE PHONE LINE TO THE 2" AQUA MINE WATER-LINE. HE WAS CUTTING THE PLASTIC TIE WITH A UTILITY KNIFE WHEN THE 2" WATER LINE BURST SPRAYING HIM IN THE EYES CAUSING HIM TO JUMP BACK. WHEN HE LANDED HE BROKE HIS RT ANKLE.

EE WAS CRAWLING AROUND SHIELD 100 TO PREPARE HIMSELF TO MOVE ANOTHER SHIELD WHEN A PIECE OF ROCK FELL BETWEEN THE SHIELDS STRIKING HIM ON LT HAND. MIDDLE FINGER. REQUIRING 5 STITCHES. EE WAS O N HIS SCHEDULED DAYS OFF WHEN THIS HAPPENED, HE WASN'T TO RETURN UNTIL 1/9/03.

EE WAS STRIKING CHAIN GUIDE IN THE PAN LINE WITHA 10LB SLEDGE HAMMER WHEN A PIECE OF THE CHAIN GUIDE BROKE OFF GOING INTO HIS LEFT KNEE. HE WASTAKEN TO HOSPITAL WHERE THE METAL HAD TO SURGICA LLY REMOVED FROM HIS KNEE. NOTE EE WAS SCHEDULEDOFF UNTIL THE 1-5-03 HE WAS WORKING ONE OF HIS DAYS OFF AT THE TIME OF ACCIDENT.

ROCK MEASURING 11" WIDE X 13" LONG X 3" THICK FELL FROM BETWEEN THE SHIELDS AND STRUCK THE PANLINE GLANCING OFF THE PANLINE AND STRUCK EE ON THE RIGHT LOWER LEG.

BREAKING BAND FROM APLLET OF CRIB BLOCKS. THE METAL BAND BROKE SLICING THRU HIS GLOVE AND CUTTING FINGER ACROSS KNUCKLE REQUIRING 4 STITICHES. MEMBER DID NOT NOT TEASE WORK.

EMPLOYEE FOUND FALL IN NO. 4 AND NO. 5 ENTRY. FALL IS APPROX 4 FT HIGH. FALL IS AT BK 34 ON NO. 5 BELT IN THE MAIN INTAKE. AREA WILL BE CLEANED UP AND BOLTED. AIR IS STILL PASSING THROUGH AND OVER ROOF FALL. ESCAPEWAY IS STILL TAVELABLE. AREA HAS BEEN CHECKED BY FEDERAL AND STATE INSPECTOR.

WORKING BELTLINE, MUD BUILT UP ON GUARDS. THEY WENT UP TO CLEAN MUD OFF GUARDS, LOWERED GUARD USED HOE, MADICK, SHOVEL, AND HAMMER TO BREAK THE FROZEN MUD FROM GUARD. WHEN EE REACHED IN WITH THE HAMMER TO BREAK LOOSE MUD, THE ROLLER CAUGHTJACKET BETWEEN ROLLER AND BELT AND PULLED LEFT ARM THROUGH.

ROCK FALL AT THE HEADGATE 001 SECTION AT BK. 68. FALL IS IN THE TRACK & BELT ENTRY. IS ABOUT 6' HIGH & 20' TO 25' IN LENGHT. FALL WILL BE CLEANED UP. EXTRA SUPPORT WILL BE ADDED IN AREA.

ROOF FALL AT BK. 15 ON THE NO. 5 BELT IN THE NO. 4 ENTRY ON THE INTAKE. FALL IS 3' TO 4' THICK, 15' TO 20' LONG. NOT HINDERING ESCAPEWAY OR VENTILATION. WE HAVE THE NO. 5 ENTRY THAT CAN BE TRA VELED. FALL WILL NOT BE CLEANED UP AT THIS TIME.

SECTION FOREMAN FOUND THE ROCKFALL ON THE 005MMU SECTION AS HE WAS PRESHIFTING IT FELL ON THE STAGE LOADER. ROOF FALL WILL BE CLEANED UP. AREA IS BEING GLUED & CRIBBED & TIMBERED +BEAMS ARE BEING INSTALLED AS NEEDED. FALL IS APPROX. 65'L X 8'HIGH. AREA HAS BEEN CHECKED BY FEDERAL AND STATE ROOF CONTROL.
