Employee was bent over, cleaning rock dust from the rib with a hammer when a rock fell striking him on the top of the head, jamming his neck. The Maxxim Shared Services employee began receiving therapy on 8/29/2014.

Claim filed for pneumoconiosis. Termination date of 4/8/2013. X-ray read date of 5/5/2014.

While in the process of hand loading 6" waterline, employee was pulling waterline from the stack when his hands slipped off causing him to fall backwards striking his back. Compression fracture to T12 vertebrae.

Employee was on his knees bolting and had to bend the bolt to install it. As he installed the bolt and pushed on the bolt to straighten it back out he felt his left knee pop. He went to the doctor that day, continued to work and began missing work on 10/22/13. (Strain to the left knee).

Claim filed for pneumoconiosis. Made aware of claim 12/16/2013.

Claim filed for carpal tunnel syndrome (right wrist).

While in supply yard loading pipe, employee stepped on wet pipe and fell, hurting his left leg (knee sprain).

While changing a bottom roller, employee struck a fouled slate bar which swung around and hit him above the left eye causing a laceration requiring stitches to close.

Employee was removing trash from scoop and his right index finger contacted a jagged edge on an oil bucket and caused a laceration. The wound required 7 stitches to close.

While pre-shifting the P-1 seals he got off his four wheeler and started walking around it a piece of drawrock fell and hit the top of his right foot.

Employee was taking down a roof bolter cable and when it came down it struck him on the chin causing him to fall backwards, landing on his rescuer / tools.

Roof fall occurred on #5 belt, break #14.

Employee was putting a roller back into the bracket on a piece of belt structure and hit it with the side of his right foot.

Employee was using a socket and ratchet to remove a lid from a continuous miner and the ratchet slipped. Employee's hand struck a nail that was lodged in the recessed area of the lid handle and was pushed into his hand between the third and fourth knuckle.

Employee was tramming a Joy 10SC shuttle car up #6 heading and when he rounded the corner he hit a hole making the shuttle car bounce, and jamming his head into the canopy.

Employee was installing a fire suppression system and when he lifted the alarm for the system he felt a pain in his right knee and heard a pop. Employee was released to return to work 3/8/12 with no restrictions, but continued to experience pain. He was transferred to mine 1519097 on 6/8/12 and then was scheduled for surgery on 6/12/12.

Employee was riding mantrip when foreign material got into his right eye. He was given prescription medication.

An unplanned roof fall occurred approximately 2500 ft off the section in the #6 entry at spad #4489. The fall was approximately 20'W X 40'L X 6'H and has been breakered off at all entrances to the fall.

Employee was installing T-channel in #2 heading. The channel was laying on ATRS and while he was putting glue into bolt hole, the channel fell striking his finger against the drill pot causing a laceration that required stitches. Started missing work on 12-14-11.

Employee was loading rope bolts onto roof bolter when the bolts got fouled and as he freed the bolts up his wrist twisted causing a knot to appear. He was given a brace at the hospital.

Employee was dismounting the shuttle car at the feeder, to check on the belt, when he felt a pain in his right shoulder and neck.

Employee was driving a spad nail into the rib to hang a cable when the top of the rib rolled out striking him on the right ankle resulting in a fracture. Rock was approximately 0-6" thick by 39" long by 8" wide.

Employee was loading track rails onto a flatcar when a piece of rail slid around catching his left index finger between the rails causing a laceration that reqiured stitches.

Employee was removing a rope bolt from the ripper head of the continuous miner. As he started to move the bolt, it sprung loose striking him on his safety glasses knocking them into his face fracturing his left cheek.

EE was operating a four-wheeler on the surface, and when he turned to put it in reverse he hit his elbow on a reciprocating saw blade that was on the back of the ride, resulting in a laceration, requiring 4 stitches to close the wound.

Employee was walking between a parked scoop and rib when he slipped twisting his right ankle.

Employee was dropping a MRS cathead that was hanging from the roof with an 18" rope hanger. As he was unhooking the hanger the cathead slipped, causing the hanger to flip around and the hook caught him in the nose, resulting in a laceration requiring stitches.

Employee was using a utility knife to cut end of belt when the knife slipped, striking him in right thigh, causing a laceration resulting in employee receiving sutures.

A unplanned roof fall occurred approximately 2000' from active in the #2 entry left return at spad #4645. The fall was approximately 90'x20'x5-8', no injuries or equpment were involved in the fall.

Appx. 11 feet outby the face methane was detected coming from a drill hole that had been drilled to install primary roof support.

A roof fall appx. 20'x60'x4.5' occurred in the #1 entry, which is the return entry, of the 002 section at spad # 47072 at 7:30 PM. No equipment or persons were involved.

A roof fall approximately 20'x100'x5', 1300' outby active face occurred at 1:52 PM. There were no injuries or equipment involved in the fall. The area will be rehabilitated.

A roof fall occurred appx. 3100' outby the active face at break 15 on #5 belt. The fall was appx. 18'x50'x8'. The roof in this area is mixed with no coal rider seam present. All entries leading to the fall have been timbered off and will not be rehabilitated.

Roof fall occurred appx. 2200' outby 2 section in the #3 entry at spad #4480. The roof fall was appx. 20'x50'x4'. The fall was investigated by MSHA inspector.

While pulling track rail, a rail rolled over on a slate bar, causing the slate bar to flip up and struck member in the ear causing a laceration.

While pre-shifting glory hole, the cage was stopped at approximately 160' level. While stopped some unobserved material that had been stuck to the wall of the shaft broke loose and struck EE on top of head and shoulder area, knocking his hard hat off and pushing him into the side of the cage, causing a laceration to his forehead.

Member was making a belt splice outby 001 MMU feeder using a driver rod and a five pound sledge hammer. The hammer slipped while striking the driver and struck the members knee.

A roof fall occurred in the #2 entry of the 001 section. This is a bleeder line for the pillar section. The size of the fall is approximately 19'x30'x6'.

Inby spad #10787 in the #1 entry of 001-0 MMU a roof fall occurred in the bleeder of the section. The fall was approximatley 19'x45x6'. The strata is mixed & wet.

While EE and another employee were transporting a head drive boom, EE boarded the flat car the boom was on to watch for clearance while passing under a conveyor belt. As rail car moved the boom twisted and cut his right leg around the calf area lacerating it.

Fall occured 60 ft inby spad # 10489 in the bleeder system on the North West U submains. This fall is in the process of being sealed. MSHA inspector investigated this fall.

EE was starting inside the mine on a Johnson 4-wheeler and was going around a parked mantrip on the right side. As he passed the unit, the right front wheel of the ride hit the rib, causing the steering wheel to spin. The knob on the steering wheel struck the back of his right hand.

Shuttle car went down, operator exited car looking for troubleshooter. Walking between #8 & #9 heading, step in rut, twist left knee. Taken to E.R. for treatment, x-ray negative; Dr. told him stay off 2 days, if swelling does not go down he could see specialist if needed.

Prior to shift beginning, employee was getting ready for work, putting on mining belt, had knife on light pouch and extra blade which was sticking out of pouch. Left hand came across blade causing laceration. Employee went to E.R. for treatment, received stitches in palm of hand.

Opening air-lock door, not holding door securely, intake pressure caused door to slam. Employee tried to catch door, mashed left ring finger between door and frame. Taken to ER, stitches were used to close wound.

Employee 1 break across from 3-D belt head helping start the forklift. When he tried to boost the battery it exploded causing injury to his eyes. Taken to E.R. for treatment, Dr took him off work until follow-up 11/24/08.

Fall was discovered # 9 entry 2 brakes outby spad # 578 in the intake. Ventilation was not affected, passage of people not affected and equipment not involved. Fall was 19 ft x 60 ft x 4-5 ft high. Fall area was breakered off. Both MSHA and OMSL were notified.

EE was pulling slack off the roof bolter reel and slipped and fell to the ground. He landed on his light battery, he finished bolting the area and then stopped work and stated he felt pain in his left side and rib cage area.

Employee shoveling #4 tailpiece, raised up & struck head against crosswalk. Employee continued to work; 9-3-08 saw Dr. due to pain. 9-4-08 employee called to report that he had been taken off work. He has entered therapy and work hardening program.

End of shift, employee stopped SC in #5 entry. As he was exiting SC, he stepped into mud and gob, causing him to twist his right knee. He visited his doctor, who refered him to an orthopedic surgeon. The Ortho recommended a MRI. May 23 he had MRI, and has been scheduled for surgery.

Employee was getting supplies for the bolter. He was exiting the forklift, holding the overhead handles, his left foot caught under the seat, his right leg slipped under the forklift, twisting the left knee. He was taken to E.R. x-ray negative, referred to a specalist. Saw specalist on 4-8-08 taken off work until 4-22 when MRI is scheduled.

Using a block hammer to cut wire, missed wire, struck left hand causing injury. Taken to E.R. for treatment, stitches used to close wound.

In #2 entry, cut band on bundle of bolts. Band snapped back, struck left arm causing injury.

Setting timber on pillar section. Put timber up, turned to get hammer, timber fell and struck his right rib area. Went to the ER after work, X-ray positive, failed to report; discovered when hospital sought payment.

Employee operating man-bus. As he passed #4 head drive a piece of metal was loose from guard, it caught his left forearm, causing injury. He completed his shift, then went to see a dr who put stitches in to aid with the healing process.

Starting inside bolt, 3rd row. Level chain broke and struck employee under left eye causing injury. Employee was taken to ER for treatment. Stitches were used to close wound.

Carrying broken chain ratchet up steps, tripped, right thumb caught between chain ratchet and hand rail causing injury. Taken to ER for treatment, stitches used to close wound.

Employee preparing cable to re-enter CM, knife slipped, causing laceration to left thumb. Taken to E.R. for treatment, stitches used to close wound.

Fly pad securely nailed to curtain board. Employee trying to pull down with channel locks. Channel lock slipped, struck mouth, broke tooth.

Employee stated he was carrying a stand for the #4 beltline & as he stepped across the track he stepped into a small hole & twisted his ankle.

Putting wreck man trip on track. Jacked up unit, cribbed it; reset jack, put on header, it split, allowed unit to fall onto cribs. Caused jack to twist, handle hit EE on left cheek, causing hair-line fracture. (5 Breaks inby #3 B H)

Employee was installing hopper jack on feeder. Jack slipped, caught left ring finger between jack and frame causing injury. Employee was taken to E.R. for treatment; stitches were used to close wound.

EE was on mantrip coming outside after completing shift. A 15-ton motor collided with the mantrip. EE was taken to the Williamson Memorial Hospital for treatment.

EE REPORTED HE TWISTED HIS LEFT ANKLE WHILE WALKING THROUGH INTERSECTION. X-RAYS AT ER WERE NEGATIVE. 1/25/06: ORTHOPEDIC SPECIALIST DIAGNOSED A HAIRLINE FRACTURE. EE STATED HE INJURED THIS ANKLE ALL THE TIME.

EMPLOYEE WAS CLEANING AN A BELT LINE AND FELL INTO THE ROLLER, HURTING HIS LT. SHOULDER.

Employee was marking a roller that needed to be changed with a reflector. When he went to put reflector on bracket his hand slipped & hit roller causing pain in rt. hand.

Employee was on operator side of bolter. off side man swung boom causing machine to hit wrist. x-ray showed hair line fracture at left wrist.

#3 belt, #38 break, employee was trimming 4" water line with utility knife. The knife slipped causing him to cut his left index finger at the base. He was taken to the hospital E.R. for treatement. Stitches were used to close the wound.

EMPLOYEE WAS ASSISTING SPLICE OF CABLE. HE WAS HOLDING CABLE WITH LEFT HAND, PUSHING KNIFE WITH RIGHT HAND. KNIFE SLIPPED, CUT ACROSS BASE OF LEFT THUMB. ER USED STITCHES TO CLOSE WOUND

EE PLACED THE S & S SCOOP LOADED WITH ROOF BOLTS CLOSE TO HIS ROOF BOLTER AND WAS STANDING BETWEEN THE SCOOP AND BOLTER.

EE WAS HELPING HANG MINER CABLE WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. HE COMPLETED HIS SHIFT AND CONTINUED TO WORK UNTIL MARCH 30TH WHEN DUE TO NUMBNESS IN HIS LEFT LEG HE SOUGHT MEDICAL ATTENTION. HE WAS REMOVED FROM WORK PENDING MRI RESULTS.
