Strip Coal

Employee was bolting an open break between #6 and #7 entry. Employee went to bend a bolt and felt a sharp pain in his lower back and tail bone. Employee stated that the pain went down to his left leg.

A non-injury roof fall accident occurred on active 003 & 004 mmu in the last open cross cut between #2 and #4 entries above the anchorage zone of 6 feet. The fall was approx 20 feet wide by 150 feet long and approx 7 feet thick.

The principle was putting a roof bolt in the hole when a piece of draw rock broke loose from the roof striking his right forearm. The rock caused lacerations across his arm, was taken for treatment and received 9 sutures.

A non injury roof fall has occurred in the main return on old #19 panel 60 feet inby spad #5747 in the #8 and #9 entries. The was 120 feet long 19 feet wide and height unknown.

He claimed that he got finished cutting the place he was in and got up to tram the continuous miner, then his knee popped. During the time his knee popped he stumbled then twisted his lower back. This we believe should be a medical issue from a previous old injury and should not be a MSHA reportable injury.

EE was getting a cleanup car, when a rock came out of the conveyor chain of a continuous miner and struck him in the right jaw and behind the right ear. He was x-rayed and determined to have no fractures, but he did have a laceration that required 4 sutures.

He was checking for what side of the beltline was the thick side. The knife he was using slid off the beltline as he was cutting it. The knife came in contact with the palm area next to his thumb on his left hand resulting in a laceration.

Was putting up a pogo stick to hold down a ventilation curtian when he felt pain in his left hip and leg.

He was trying to pull cable off of roof bolter, when he felt pain in his lower back. He did not start missing work until 9/2/09 because of incident.

While loading roof bolter with supplies a metal strap on a bundle of rope bolts was broke and sticking out as he turned to put supplies on bolter left arm caught the broke strap cutting his arm.

While throwing Miner Cable out of roadway for roof bolter to cross over, felt something give in lower back.

EE was straightening a 5' bolt after it was inserted in the top. The bolt turned to the side and he then felt something pull in the lower part of his back.

Mine Foreman and MSHA Inspector were traveling in Right Return & found roof fall. 1300 Feet from face on a different panel at spad #3107 between #4 and #5 entry. Approximate size of fall 6' height, 19' width, 25' length.

Making Weekly examination of Right Return and at 2:30 pm found roof fall at spad # 2980. Fall was 3200 feet from the Face and approximate size of fall was 10' height 20' width 30' length.

While backing little two man Johnson buggy up, ran over loose coal and got head in the top and stove neck up. Did not have any medical treatment til 5/5/09 and started missing work on 5/6/09.

Doing repair work on electric roll up door at mine portal when the wrench slipped off a bolt hitting his left hand on the edge of a metal enclosure causing a cut on the back side of left hand. This required 4 stitches to the left hand.

Bolter Operator was starting drill steel, and it bent out and hit him on his left wrist.

He got a roof bolt hung up in the hole and was trying to bend it to break it off. The roof bolt flew over against his right arm.

A roof fall occurred 2800' outby face at spad 2334 in #1 entry. It measured 18' wide x 50' long x 10' thick.

He was trying to knock a bearing off of an axel with a hammer and punch. A piece of metal chipped off and went in his left arm, just above his wrist. Had to make a incision to remove metal.

A roof fall occurred next to spad 2450 between #3 entry and #2 entry. It was approximately 20 feet long, 19 feet wide & 3 feet to 6 feet thick.

Standing between roof bolter and 2 man buggy in break between #6#7 entry. A ram car came around block in #6 entry going to Continuous miner hitting 2 man buggy and pushing it into the roof bolter, catching his right leg between the two pieces of equipment. Started missing work on 9/12/08.

Employee was operating a roof bolter, when he got his middle finger on his left hand caught between the boom and the frame, resulting in a laceration that required several stitches to close.

Employee was operating a continuous miner in the #6 face, where the mine floor was wet and muddy. He had finished loading a coal hauler and as the hauler was pulling away, it began to spin which allowed the rear of the hauler to slide over, striking EE's right ankle.

Employee was using a utility knife to cut a piece of conveyor belt, when he accidentally cut his left arm with the knife. The cut required 13 stitches to close and he was away from work 10 days.

Employee was operating a roof bolter when a small piece of draw slate fell striking his right hand. This resulted in a fracture to his index finger.

While putting up a rope bolt about 6" from top, bolt started to bend. He tried to spin it the rest of the way up when the head of the bolt broke off & plate flew off the end of the bolt striking him in the face leaving a 2" cut over right eye.

Employee was operating the off side of a dual boom bolter in the #8 crosscut when a piece of draw slate measuring 2' by 2' by 3 inches fell striking his left ankle. He continued to work until 03/07/07 at which time he began loosing time due to the sprain in his ankle.

A roof fall measuring 5 feet by 18 feet by 240 feet occurred in the 001-0 section return air course. It did not interupt ventilation.
