Employee was replacing the cotter pin in the master link of the leveling chain on a Fletcher RRII roof bolter, and the drilling pot rolled back and pinched his right ring finger between the sprocket and leveling chain.

Employee was walking and stepped into a hole and felt a sharp pain in his left knee. Accident was not reported, and employee continued to work until 11/02/2015 at which time employee experienced another incident with his knee while attempting to enter the deck of an underground forklift. His first lost day was 11/03/2015.

Employee had finished his shift and opened the tailgate on his personal truck, when it became unhinged. While attempting to realign the tailgate he caught his right middle finger between the tailgate and the bed of the truck.

Employee was operating the continuous miner when a rib rolled off pushing employee against shuttle car that was being loaded resulting in a fracture to his lower right leg.

Employee was leaning against a scoop when a piece of draw rock fell and struck his left pinky. Surgery was performed to amputate part of the first joint of the left pinky.

A non-injury roof fall occurred at break 11 on the #2 belt line, #3 "track entry". The fall measured approximately 20' long x 45' wide x 10' high.

Claim filed for pneumoconiosis. Diagnosis date 05/29/2015. Employer received notification 09/15/2015.

Employee strained his back while lifting a battery to change it out on a 4-wheeler in the underground shop. Employee continued to work but started physical therapy on 10/6/15.

Employee was in the process of cutting a conveyor chain link on a Joy shuttle car that had become fouled up. He cut one side and when he had the second side cut almost halfway through the conveyor chain broke, (due to it having tension on it), and flew back striking employee on his left hand causing a laceration requiring stitches to close.

Employee was attempting to cut a cable tie from a roof bolt plate with a utility knife. The utility knife got stuck and as employee was trying to free the knife it slipped cutting his left hand, which required sutures to close the wound.

Employee stated that while he was examining belts he struck his head against the roof. He continued to work and then struck his head yet another time on an overcast beam. He was then brought to the surface and transported to the hospital and received a full release. He continued to work until 8-19-2015 when he returned to the Dr. and was taken off until further notice.

A non-injury roof fall occurred on the #3 belt, break #31, intake #5 and #6 entries. The fall measured approximately 110' long x 16' high x 20' long.

A roof fall, approximately 20'x 30'x 8', occurred at break 34 on the 1st Northwest Mains in the #7 entry.

Employee was replacing the hose reel on the maintenance scoop and the lid of the reel compartment fell and struck him on his wrists, resulting in bruising to the hands and wrists. X-rays were negative, but a splint was applied because of the swelling.

Claim for pneumoconiosis. Termination date of 4/16/2015. X-ray read date of 6/19/2015.

While attempting to pull 1000' of 48" conveyor belt, the chain being used, broke and struck employee in the nose and mouth resulting in a simple fracture to his nose.

While moving a rock out of haulage roadway, employee's finger was caught between the rock and the coal rib. The injury resulted in an open wound to his right index finger.

Employee had been receiving treatment for non-work related degenerative disc disease. On May 4, 2015 the employee was walking when he struck his hard hat against a roof bolt. The following day the employee sought medical treatment. Physician did not attribute the incident to a new injury; the medical notes indicate aggravation to the existing cervical degenerative disc disease.

While employee was drilling a hole to install a roof bolt, a piece of draw rock became dislodged, rotated down the steel, broke apart and was projected toward employee striking him in the nose, causing a laceration requiring stitches to close.

Employee was standing alongside the roof bolter when a piece of draw rock fell striking him in the back, pushing him into the bolting machine. The rock measured approximately 3'long x 12" wide x 0-12" thick.

Claim for pneumoconiosis. X-ray was read 11/24/14. Employee was terminated 11/26/13

Another employee was tramming bolter, the cable jerked and pulled a rope hanger down. Injured employee stated the cable pulled sideways and struck him in the right knee. (Bruise to right knee)

The employee was attempting to boost the battery on a forklift and had lifted the lid to access the battery compartment and as he was attaching the booster cables the lid fell on to his left hand. This caused a minor fracture to his left thumb. He was wearing gloves at the time.

Roof fall occurred on 2 belt at break 41 in the #6 entry. The approximate size of the fall 20' x 20' x 10'.

Claim filed for pneumoconiosis. Termination date of 7/9/14. X-ray date of 9/9/14.

Roof fall occurred at break #33 on #3 belt #6 entry. Approximate size of fall 22' x 22' x 10'.

Claim filed for pneumoconiosis. Termination date of 3/19/2014. X-ray date of 8/22/2014.

While attempting to pull 25' of 48" belt from between belt rails, with the help of another employee, he felt pain in his groin area. Employee began missing 7/31/2014.

Claim filed for pneumoconiosis. Termination date of 8/7/2012. X-ray date of 6/13/14.

Claim filed for hearing loss.

A roof fall occurred in the intake entry at spad 279. Fall was approximately 20' by 50' by 6' high.

Employee was pulling a metal band strap from under a pallet containing belt rails with a pair of channel locks. When the strap came loose it cut his arm. (Sutures required).

Employee was operating miner when the rib rolled off of the corner striking him on the right foot. Fracture to the right foot.

Contract employee was picking a timber up out of the roadway and felt his shoulder pop. (Strain to right shoulder)

The truck was pulling under the coal hopper when the hopper gate opened prematurely causing the coal to fall between the cab of the truck and the front of the trailer. The driver stated that when he got out of the truck he slipped in the coal, causing injury to his neck, back and leg.

Employee was hand loading left bolter when utility knife came open and cut his right leg.

While drilling the roof to install strap in the #1 heading the middleman steel became stuck. While trying to free up, the steel broke loose catching his right index finger between the finisher and middleman steel. Laceration requiring stitches.

Employee was attempting to help with suction on the operators side of the bolter. He had his hand on the arm of the bolter trying to pull the suction hose off of the pot when the operator raised the pot, pinching his fingers between the arm and the frame. Simple fracture to pinky finger on left hand.

EE was walking to slope ride when he slipped and fell. He tried to catch his fall using his right hand. Fracture to right hand.

Employee had parked his mantrip when another mantrip entered the parking area and slid into his. Pain in neck and shoulder.

Employee was bringing drill steel out of the hole when steel came out of the drill chuck, striking him in the nose. (Fracture and laceration to the nose requiring stitches).

Employee was carrying bags of rock dust from track switch to head drive and hurt his back. Employee was taken to ER and released, but on a follow-up visit a chronic pars fracture to his cervical spine was found and attributed to the accident. The fracture was unknown to the operator until it was discovered on a Part 50 audit. (Citation #8300822)

Employee was lifting the lid on a 15 ton motor and felt a pain in his left shoulder/neck. Employee went to his family doctor and received a full release to work. During a part 50 audit the operator was informed that the employee had received Physical Therapy until August 2013. (Citation #8300823).

Employee was walking and stepped into a depression in the mine floor and twisted his right knee.

Claim filed for hearing loss. Employee's termination date 01/23/2013.

While using a cutting torch a piece of hot slag fell onto the acetylene hose and burned a hole through it causing it to blow out. Employee used his right hand to push the hose away from him causing him to receive a burn on his right middle finger and hand.

Roof fall occured in the cross cut of #3, #7 entry on #6 belt.

Roof fall occurred in break #1 from #4 entry to #5 entry and into 5 right cross-cut.

Employee was swinging his drill pot out to install a bolt and had his hand resting on the supply tray of the bolter and his finger was mashed between a bolt and the side of the tray. He was taken to ER and received stitches to close the wound and a splint was applied.

A small rock 2"x3" flew off the drill pot and hit employee's safety glasses.

EE was installing conveyor belt structure and when he lifted the structure above his head felt a pop in his shoulder. He began to miss work on 7/28/2012.

Employee was walking back from putting up bolts to move roof bolter to next row when a piece of draw rock 3.5'x 3' x 2" fell striking him on the back of the head and neck.

Employee was helping to remove a conveyor chain from a shuttle car and while another employee was hitting the chain with a hammer a piece of metal chipped off and hit him in the right side of his face(upper cheek).

Employee was hanging a piece of ventilation curtain and a piece of draw rock fell and hit him on the shoulder and back.

While standing on the off-side of the roof bolter waiting for operator to move bolter up a piece of draw rock slid off of the canopy and flipped onto employee's ankle and foot.

While lifting a spool of tracking cable employee felt a sharp pain in his shoulder.

Was loading shuttle car when a piece of rock fell striking his right foot.

Employee was setting up bolter for first row of bolts using inch tram when a rock fell causing him to bite his lip.

Employee was moving cribs block through a man door as another employee was tossing crib blocks towards the man door. As the injured employee was getting a crib block his middle finger of his right hand was smashed between two of the crib blocks resulting in a fracture to his finger.

Employee was in the #7 entry, standing behind the miner boom, when a piece of rock (approximately 2 1/2' X 3' X 2 1/2") slid off the shuttle car striking his left foot resulting in a fracture.

Employee was hanging the cable to the roof bolter, in #3 entry, when he received an electrical shock. He stated that he felt an electrical shock while hanging the cable and next thing he remembered was lying on the ground. He returned to work his next scheduled shift.

Employee was attempting to remove a rock between the shaft and guard on the belt drive when his ring and middle fingers, of his left hand, got caught between the shaft and guard resulting in the tip of his middle finger being amputated.

Employee was stepping off the tailpiece, while using his left hand to steady himself, when his foot slipped causing him to catch his weight on his ring finger resulting in his finger being amputated at the middle joint.

Employee was trimming a link off of a belt splice when the knife slipped cutting his right leg above his ankle requiring stitches.

An unplanned roof fall occurred between 9/26/11 and 10/3/11 in the #1 return entry of #1 section approximately 5,000 ft off the section. Fall was approximately 80'L X 18'W X 6' to 8'H and has been breakered off and is 3 breaks outby spad # 815.

Employee was changing hose on roof bolter when a piece of rock fell onto his back, knocking him into the bolter. He continued to work his scheduled shifts until 2/3/12 when he started missing work.

Employee was repositioning the roof bolter to install another set of chain hangers when a piece of rock (approximately 8"L X 5"W and 0 to 1/2"T) fell hitting him on his right elbow causing a laceration that required stitches.

Employee was cutting highline cable with cutters to make a splice, when cutter handles went together his thumb on his left hand was caught between the handles. Resulting in a laceration that required stitches.

EE61 was laying track in muddy conditions, and when he started to move his foot was stuck causing him to fall sideways twisting his knee.

Employee was removing belt off old 3B belt, when a piece of structure fouled. As employee was removing structure, belt rail came loose striking employee in right wrist/hand fracturing his pinky finger of his right hand.

Employee was roof bolting in 2-Left installing the outside bolt of the last row. As he was tensioning the bolt a piece of rock, approximately 24" by 16" by 1", fell striking him on his right elbow causing a laceration that required stitches.

While spraying degreaser onto roof bolter, with a pump up pressure can, the hose blew off can connection spraying degreaser onto the right side of face and underneath his glasses.

EE was pulling a cable with the cathead over his shoulder. He slipped and fell forward striking his chest on the cathead. He was taken to ER and was diagnosed with a fractured sternum.

EE was dropping the miner cable, which had been hung with a rope type hanger, and the hook on the hanger caught his hand between his thumb and index finger causing a laceration. He fininshed his shift and went to ER after work and received eight stitches to close the wound. He received a full release to return to work on next shift.

EE aggravated old injury he had prior to being at Process Energy. He'd suffered a herniated disc at another time. He'd been seeking treatment since 05 for back problems. Started missing work 9/30/10. After an investigation of this incident we feel his continued back problems are why he became unable to work. This single event didn't cause his previous inj

Employee was installing roof bolts in the #2 heading and was preparing to install the inside bolt when a piece of drawrock approximately 5' x 29" x 3" thick fell and hit his left ankle and foot. He was diagnosed with fractures to his foot.

Employee was stacking concrete blocks to build a stopping, and when he got to the last block on a row he caught his little finger between the concrete block and the rib.

An unplanned roof fall occured In the #6 entry of #2 conveyor belt at spad # 350. The fall was approximately 20'x18'x7'. No injuries or equipment was involved in the fall.

An unplanned roof fall occurred in the #3 entry appx. 3100' outby 001 section. The fall was appx. 20'x18'x9'. No injuries or equipment was involved in the roof fall.

EE was attempting to remove a chain from a pile of rail with the help of another member using a forklift. As he reached for the chain some of the rails slipped and caught his hand between them.

Member received burns when he came in contact with energized electrical components while performing preventive maintenance on electrical installation.

Unintentional roof falls was discovered by MSHA Inspector. (1) Near spad #224 in the return 5500 ft from the face between #7 & #8 entrys, measuring 25 ft long X 18 ft wide X 8 ft high. (2) Near spad #293 in the intake 3600 ft from the face in the #5 entry, measuring 20 ft long X 18 ft wide X 8 ft high. This is to abate citation # 8737577.

While climbing into scoop injury was grasping the hand holds to assist in getting into the scoop. While his hands were in the holds a piece of draw rock fell from the roof and struck his left hand. The appx. size of the draw rock was 1'x2'x2.5". The rock hit his ring and index finger causing fractures.

Employee ran over a rock with the shuttle car he was operating after leaving the feeder area, and caused him to hit his head against the canopy. He was given a restricted duty release.

Working on overcast, broke band on beams, beam slid against hand, mashed index finger of right hand. Employee taken to ER for treatment, stitches used to close wound.

Drinking soda while operating shuttle car. Hit a hole, bounced, struck teeth with soda can. He chipped a tooth and loosened another. Employee went to a dentist to have the tooth repaired.

Moving Rb cable, EE was holding the cable and EE releaed cable hanger, hanger hit EE on the nouth area and boke his tooth.

Employee was pushing forks of ug forklift closer togather, EE got left middle finger mashed in the process. Stitches were needed to close the wound.

Employee was re-entering miner cable when a piece of draw rock fell, striking him on his left lower leg. He was taken to E.R. for treatment & X-ray indicated a fracture.

Employee was using a utility knife to cut tape from a cable. The knife slipped, causing injury to the right thumb. He was taken to the Hospital E.R. for treatment, and they used stitches to close the wound. Employee returned to work next shift.

Setting pump at face, abrasive material in water, walking around wet until 3:00 P.M. Went to E.R. due to injury, admitted for treatment.

Employee was raising belt structure with chain ratchet. Chain ratchet slipped while pulling through the spreader. His left middle finger was caught between the spreader and belt chain causing injury. He was taken to ER for treatment, stitches were used to close wound and he returned to work.
