Employee was on 8 beltline at brk.21 repairing a brattice, employee was on his knee's when he reach over to get a block, a block from the brattice fell out striking him in the back fracturing 3 vertebrae's in back. Employee became a lost time due to this injury.

A roof fall has occurred at Transition Mains, at break 33 in the #1 entry intersection over to the #2 entry. Area will be crib, Heimans beams will be installed and 16' rope bolts will be installed to prevent future falls.

A roof fall has occurred at Transition Mains at break 35 in the intersection of #3 entry. No ventilation has been effected and no equipment is involved. Area will be cleaned and additional roof support of 5' and 10' rope bolts will be installed.

Employee was bolting top when a piece of draw rock fell striking right little finger, sutures required and employee has been place on restricted duty due to this injury.

Employee was shoveling plaster out of a wheel barrel onto a brattice when plaster splattered into left eye causing irritation. Safety talk was given to employee on the importance of wearing safety glasses.

Employee was changing idler out on continuous miner when rope securing idler slipped causing idler to fall on right middle finger, employee received a fracture to distal end of finger and a laceration requiring 3 sutures. Employee has returned to regular duty.

Employee was plastering a brattice when he tripped and fell to the ground receiving a cut to his left arm, employee received 5 sutures and returned to regular duty.

Employee was on 7 main track at brk.16 checking a pump, employee lifted pump straining stomach, employee continue to work, on 7/2/2012 employee was placed on light duty due to a hernia from this injury.

Employee was putting a new shaft in feeder when it slid striking left knee, employee has been placed on restricted duty on 2/16/2012.

Employee is a scoop operator that was loading a bundle of roof bolts that were binded together with a metal strap, the strap broke loose striking employee on right forearm, cutting arm, employee received 6 sutures and returned to work with no restrictions.

A roof fall has occurred at 8 Mains in the old works at break 20, between 3 and 4 entry no ventilation or equipment effected, the fall is approx. 60' long and 8' to 10' thick. The area is dangered off and operator has proposed to seal off entire area.

Employee was working on miner track driving keeper in track pin when a piece of metal broke off lodging into left hand. Employee had metal removed requiring 1 suture and returned to regular duty.

A roof fall had occurred on 001 section at 10:00 am in the #4 entry at break 5 outby last open break, the size of the fall was approx. 18'W x 20'L x7'H no ventilation effected, no equipment effected, At 4:07 am 9/14/2011 a fall occurred in #4 entry connecting to previous fall, above the last open break, the fall was approx.18'W x 60'L x 6'H.

A roof fall has occurred at 8 mains in the #1 entry at brk. 15, the size of the fall was approx. 20'W x 20'L x 7'-8'H, no equipment effected or ventilation effected.

Supply man was straightening up a load track rails he just loaded onto supply car when he cut his left middle finger requiring sutures, employee has returned to regular duty.

Employee was making a splice on shuttle car cable when utility knife slipped cutting top of left hand, 4 sutures required, employee has returned to regular with no restrictions.

Employee was working on track putting track tie in when chain that was holding up track fell striking left hand, employee received a cut to left wrist and fracture to left thumb. Employee has been placed on light duty.

A rock fall has occurred in the #3 entry on 8 beltline at brk. 19, the approx. size of the fall was 10'H x 20' W x 28' L. No equipment or ventilation affected, the area will be cleaned and bolted with 6' glue bolts and 10' cable bolts.

A roof fall has occurred in the return bleeder panel in the #2 entry at brk.11. The size of the fall is approx. 19'W x 50'L x 6" thick. No ventilation was affected, no equipment affected. The area has been timbered and dangered off.

A roof fall was found in the #4 track entry, between break 17 & 18 the size of the fall was approx. 18' x 22' x 6'. The primary escapeway has been re-routed to #3 belt entry, the effected area will be cleaned and bolted with 10' & 12' rope bolts and 4' glue bolts.

Employee was cutting off wear runners under miner, when side lid fell down hitting head, sutures required. Employee has returned to reg. duty with no restrictions.

A roof fall was found at break 35 on the # 9 beltline, it was approximately 40' by 20' 10' thick.

A roof fall was found off 8 track at brk. 31 in #5 entry, extends thru #2 entry to the right, the size of the fall is approx. 60' long x 20' wide x 8'to 10' high. No ventilation, or escapeway affected. The area has been cribbed and dangered off.

Employee was passenger in mantrip when mantrip spinned out on track hill causing mantrip to roll backwards, mantrip hit 20 diesel motor that was on track. Employee became lost time due to head injury.

Employee was walking to mantrip when stepped on rock slipped and fell on left knee. Employee was placed on light duty.

A rockfall occurred in the #3 entry at brk.3, this is the #10 beltline, the size of the fall was approx. 25'L x 30'W x 8'-10'Thick. No ventilation or machinery was affected. Area is being cleaned and additional support is being added.

A roof fall was found in 9 mains at brk. 31 in #5 entry, this is located in old works 5 brks. inby section. No ventilation or machinery effected, the area has been crib off on both sides of entry. The fall was approx. 20'W x 40'L x 5'thick.

Employee was stepping out of railrunner when he twisted his right ankle. Employee receive a fracture to his right ankle. Due to this injury he became a lost time on 9/14/2010.

A roof fall was found at 4 seam mains at break 10 in the #1 entry, the fall was approx. 7'Thick x 30'long x 16' Wide. No ventilation or machinery effected. The area has been timbered and roped off.

A roof fall was found in 8 main track entry at break 18. The fall was approx. 6'x24'x16' wide. No ventilation was affected, the fall was on top of the roof support arches that were in placed prior to this fall.

While operating roof bolter, employee was drilling hole into roof when drill steel broke, cutting left index finger. Employee received 11 sutures and been placed on light duty.

Employee was traveling on track to the outside when a piece of drawrock fell, hitting mouth, 7 sutures required. Employee has returned to regular duty with no restrictions.

Employee was grinding on lo lo structure when something went into his left eye. Employee went to doctor on 5/19/10, object was removed from eye. Employee returned to work with no restrictions, employee was wearing safety glasses, a guard was on the grinder at time of accident.

A roof fall was found in Old 9 Mains, in the #4 entry between brk. #23 thru brk.# 26. The fall was approx. 5 'thick, 20' wide, 180' long. The area has been dangered off. No ventilation or machinery effected.

Employee cut left leg shin while using grinder working on lo lo structure, requiring 4 sutures. Employee has returned to reg. duty with no restrictions.

A roof fall was found Gasswell Mains, located in old 9 mains, in 3 left, in the # 3 heading. The size of the fall was approx. 16'L x 20'W x 6' thick. The area has been timbered and dangered off.

A roof fall was found in #3 heading at break 17 in the intersection, ventilation was not affected no equipment or persons involved, the fall size was apporox. 20' wide x 9' thick x 30' long. The area has been cribbed and additional 16' rope bolts have been installed.

Employee was standing at roof bolting machine when scoop backed out of right break into #4 entry. Bucket of scoop caught corner of rib and broke rib loose causing rib to roll onto EE and roof bolting machine. Employee received a fractured collar bone and 6 left ribs were fractured, employee became lost time on 12-10-09.

A roof fall was found at 11:45 am by MSHA inspector and carried out and reported to safety dept. at 1:45pm. The fall was reported to MSHA hotline at 1:50 pm. The fall was located in the East Main at Break 23-24, in#4 entry, the fall was approx. 18'wide,20'Long,10'thick.The area was timber and dangered off.

A rock fall was found in the East Mains #5 entry brk. 22 intersection off the section, no ventilation, or machinery involved, no miners hurt. The fall was approx. 5' thick, 20'Long, 15' wide. The affected area has been dangered off.

A roof fall was found in the East Mains at brk. 22 in the #1 entry, the fall was approx. 15'W, 20'L, 6' thick and Brk. 23 in #2 entry, the fall was approx. 15'W, 50'L, 6' thick. Ventilation was not affected, no equipment or persons involved. The area has been timbered and dangered off.

On 7//21/09 employee was plastering a stopping when ladder fell spraining his left ankle, employee had returned to regular duty,it came to my attention recently that the employee had returned to work on 7/22/09 but not to regular duty, light duty. Employee was on restricted duty for 1 day.

Employee was bolting top in when drill steel fell hitting right middle finger fracturing it. Employee returned to reg. duty with no restrictions, just received x-ray report 6-11-09.

Employee was operating scoop with left hand outside of deck, left hand got caught between bumper stop and deck, causing injury to his left index finger. Employee became lost time on 4/08/09, left index finger tip removed due to this injury on 4/7/09.

While operating roof bolter, wrench used on drill steel broke, hitting employee left hand. Employee received a fracture to his 4th metarsal, employee became a lost time on 10-09-08.

Employee was gettting belt with scoop, belt hung on scoop then broke loose from scoop hitting him in right ear. 8 sutures required, employee has returned to regular duty.

While cleaning torch tips, employee accidentally bumped oxygen handle, causing oxygen to blow debris into right eye. Employee has returned to reg. duty, with no restrictions.

Employee was trying to replace top roller using a bar for leverage. Jerked on bar and felt sharp pain down right shoulder and middle upper back.

employee was bolting top in #4 heading when a piece of drawrock fell off canopy striking left knee, until has been place on light duty.

Employee was working at #1 tailpiece making a spill board; employee said when picking up a piece of sheet metal he strained his upper back. This injury occurred on 4-25-08; employee continued to work. On 7-1-08 employee became a lost time due to this injury.

Employee had slipped on a rock falling into side of feeder hitting stomach, employee had been place on restricted duty on 04-04-08, employee will be a lost time on 04-18-08 for surgey, due to injury employee has a hernia.

Employee was bolting top when turned to get supplies and cut left cheek on hanging strap, sutures required, returned to reg. duty.
