The employee was changing bits on a continuous miner. The employee struck the tip of left little finger with a hammer causing a fracture to the tip end.

The employee attempted to step off of a spare belt tailpiece when EE put EE's foot on the ground, EE twisted EE's right knee. The incident occurred 10/14/2019 but did not start missing time until 02/03/2020

The employee bent over to pick up a concrete block and felt pain in lower back. The employee did not start missing time until 9/26/19.

the employee was behind the bolter reaching for a piece of tie wire with right hand, had left hand on top of the operators deck canopy the bolter slightly moved, it hit a high spot on the bottom, causing it to raise up mashing left index finger between the canopy and the top causing a fracture.

Employee was opening the forks on the #3 lo trac. They lost grip on the forks causing the left ring and middle finger to get pinched.

An unplanned roof fall occurred at X-cut 17+00 outby #2 unit in primary escapeway, 2nd main south entry #7

Operator reeled up bolter cable while tramming, EE was standing on the wrong side the cable when the cable was torn down from the roof striking EE in the jaw. EE was shoved against the rib causing EE to strike head causing a laceration to head resulting in 3 staples.

The employee was raising the drill pot to the roof EE was attempting to put the drill steel in the pot, the steel got caught on the chuck, when it went into the chuck it had too much up pressure the steel caught EE's glove and struck EE's hand. The employee did not start missing work until 8-3-2019.

The employee was cutting a metal band that was around a pad of concrete blocks, the band broke loose cutting the employee in their right arm, resulting in to receive sutures.

The employee was pulling the miner water hose through the belt regulator when EE became tangled in the hose then EE felt a pop in EE's right knee

The employee was cutting a piece of belt with a utility knife, the knife slipped and cut their upper right leg and they required sutures

The employee was hooking up a cathead to a generator, employee was getting up off of the ground, their hand was on the edge of the hitch, their hand slipped on the side of the hitch it slid over a sharp burr causing a cut resulting in sutures

EE was picking up a 6"X20' section of waterline when EE felt a sharp pain in EE's lower back. EE was released to work on 03/12/2019, EE didn't begin losing time until 04/05/2018.

The contract employee had finished loading supplies on a man-trip when the employee bent down to lift lunch box. When employee lifted lunch box employee felt a pain in their back. The following were not a factor: rules, regulations, mining systems or equipment, training, protective clothing or devices. Attitude may have been a factor. Did not start to lose time until 3/2/2019.

EE was servicing man trips for the start of third shift, EE had just finished one and parked it in front of the bath house, when EE got out of the man trip and stepped on the gravel parking lot and twisted lower back. EE didn't start losing time until 03/18/2019.

The miner was riding on the 14 man Damacus man trip, the man trip ran over a skid located in the rides path. The ride came to an abrupt stop causing the miner to strike knee on the back of the seat in front of EE. The miner went to the ER and was released, the miner didn't start losing time until 01-04-2019.

EE was loading 5'resin in ee scoop bucket when ee felt a pain in left shoulder. EE had already loaded 20 bundles of glue before the incident.

The employee was bolting, had a bottom steel hung in the roof. The employee had 1' wrench in the drill pot and was raising it up to connect it with the steel, and caught left hand between the two, causing a laceration requiring sutures

The employee was operating the unit scoop. EE got out of the scoop for some unknown reason, the scoop started to roll. While trying to get back in the scoop, EE's right foot was caught between the coal rib and the scoop deck, resulting in a partial amputation of EE's right foot.

Our mantrip was coming off #5 road and was turning on to #4 road the mantrip did not make the turn, the operator slowed down to re adjust the turn, another mantrip came from behind and hit the back of our mantrip. The employee did not start missing work until 10/08/2018

Employee put too much pressure on the pinner steel causing it to bend, when the steel was rotated the steel hit the operator in the left eye. The employee was fully released to duty, and didn't start losing time until 9-24-2018. The employee suffered a hand injury at home after the eye injury and was off from 09-17-2018 until 9-21-2018.

There was an unplanned roof fall in the 1st east panel #1 on the #1 unit in the #1 entry at spad # 2+00

A battery scoop was being extracted from the 8-r crosscut with a battery ramcar. The employee was standing in the intersection and had turned to walk away and was struck with the ramcar, knocking the employee down. The employee did not start losing time until 8/31/2018

The employee was using a utility knife to cut some belt, when EE lost EE's balance and cut EE's leg

The employee was holding up a miner cable and waterline. The employee untied the waterline and it hit EE above the eye.

The employee was getting out of the ram car and felt some pain in left knee.

Pinner Steel was stuck in the pinner chuck, EE was trying to remove it when EE accidently bumped the rotation lever and the steel spun and hit EE's wrist

The miner cut left arm with a belt knife while cutting open a bulk bag of rock dust. The miner received 3 stitches to the left forearm.

Rock fall at spad 1700 1B supply road Approximately 70' by 20' by 8'.

The miner was loading the roof bolter when miner felt a sharp pain in right shoulder.

The miner received 1st and 2nd degree burns when ee removed a battery lift jack after the bottom pin was cut out with a cutting torch. The miner placed the jack on the mine floor and the hot oil sprayed on the miners hand. The miner was wearing gloves at the time of the accident.

While Roof bolting employee swung the boom with the canopy pressurized to the roof causing a rock to be dislodged and striking the operator in the left cheek (face). Resulting in 8 sutures to the affected area.

Was moving forks on lo-trac broke tip end of left ring finger and received 8 sutures

The employee smashed finger, received 7 sutures

Employee strained EE's back while operating a roof bolter

Employee was unloading belt structure and pulled a muscle in their back.

The employee pinched EE's right ring finger while operating the supply tractor

The employee smashed left pinky finger between a steering jack and the mine roof. The finger was fractured and employee received 10 sutures.

EE hand was on the side of piece of equipment. A rock fell from the top breaking right ring finger and also received 4 stitches

A rock fell out the roof striking employee's right forearm, resulting a laceration were employee received eight stitches.

The employee was pushing a roof bolt up in the hole with the bolter and a 1' wrench and had the tip of a finger between the head of the bolt and the wrench pinching the finger.

EE was building an intake brattice, when EE felt EE's shoulder pop.

Employee was straitening bolt back to install in roof when they felt a strain in the left shoulder and elbow.

Employee was kneeling down and was cribbing under the continuous miner when a rock fell, hitting EE in the top of head and slid down EE's right shoulder.

While closing mandoor, employee's right middle finger was caught between the door and door frame, pinching, resulting in sutures.

Employee was pulling drill steel out of roof, when they felt a pain in their back and twisted their knee. Employee did not miss work until 05/08/2017.

Employee was driving a scoop and ran over a rock, jamming employee head into the canopy of the scoop injuring neck.

The employee stated: that they were tearing down old concrete block airlock on the 4-a belt. Employee was picking up a block, when a block that was stuck to the roof with plaster, suddenly fell hitting employee on the left hand

The employee was shoveling rockdust into a bantam duster with back turned away from an oncoming examiner that was traveling toward the employee in low visibility. The examiner bumped the employee's leg and pushed it into the rockduster.

Employee was cribbing up the bumper of miner. The miner sit down on the end of the crib block, causing the other end of the crib to strike employee in the chin, causing a laceration.

Employee was between #2 & #3 entries, employee was attempting to break a hose loose and when it did break loose employee fell backwards tripping over the bolter tray landing on employee's elbow, jamming employee's left shoulder.

The Employee stated: Employee was using a utility knife to cut a piece of ventilation curtain and cut left hand

The Employee stated: that EE was bending a roof bolt on a flat surface, the roof bolt twisted around while EE was bending it and EE felt a strain on the right side of EE's lower back.

Employee stated; was unloading 20ft sections of 4 inch fresh water line on #2 unit. Employee was working alone when they stated that they felt pain in their lower back on the left side.

3 individuals were in the process of replacing the lid on the power center. The injured person was on one side of the lid and the other two were on the opposite side of the lid. As they were placing the lid in place two of them accidentally dropped their end. The injured employees hand was underneath allowing the lid to land on left index finger.

Employee Stated: EE was building a brattice. EE turned around in a twisting motion and bent over, picked up a block and raised up turned around and felt a pain in lower back on the right side.

EE States: EE was walking across a pile of gob in front of the feeder, and rolled ankle

Employee states working on #2 unit #10 entry while placing starter steel in the chuck, a piece of draw rock fell striking on top of left hand resulting in 2 stitches.

EE states got off ram car to remove flagging and to make sure it was safe to travel through cross cut and while getting back in car felt pain in lower back.

Employee states while trouble shooting loss of suction on roof bolter on #2 Unit, placed right hand on boom (pot) area and raised boom pinching index finger on right hand resulting in six stitches.

Employee states while bending 5 foot roof bolt in hole EE's body was not square and bolt shifted causing pain to right shoulder.

Employee states while stacking block brattice, EE didn't cap and while turning to get cap boards the brattice fell striking EE on foot and lower leg on right side.

Employee states while swinging in with roof bolter canopy EE pinched EE's finger between canopy and slide out extentions of canopy causing fracture to tip of finger (Index).

Emloyee states after pinning an entry started loading bolter. While loading pie pans for skin control 20-30 slid back cutting forearm resulting in stitches.

Employee states while loading plaster for brattice on #3 Unit outby unit. Employee states tripped over pallet and twisted back.

Employee states while cleaning Cont. miner off with wash down hose in cross cut between 4 and 5 entry's EE tangeled feet in water hose falling on left knee. Employee stated didn't report to supervisor during accident, but reported the next day.

Employee was traveling and examining return air course off old #1 unit and EE's ride was stuck in mud. Attempting to pull ride out with come-along it broke striking EE in mouth. Employee went to dentist and a crown on EE's tooth was broken. 4-15-2016 it was brought to my attention that a crown was broken and replaced on EE's teeth after seeing a dentist.

EE traveling out on damascus mac 10 mantrip employee failed to maintain EE's entire leg inside mantrip while traveling through airlock door and was struck by door causing seven stitches to left leg (shin).

Employee states was pulling a dumpster with Dapco EE had a chain get caught under skid and attempted to jerk the chain out and felt burning pain middle lower back

Employee states while moving miner back out of cut dropped his head walking through a wing curtain and tripped over a pile of spilled coal or gob losing his balance, caught his self and didn't fall to mine floor, but twisted his back. Taken to emergency room. Employee also stated he has an ongoing back issue with disk deterioration.

Employee stated he was closing airlock door at the mouth of # 1 unit and timber worked loose and door fell striking him on his left hand on ring finger cutting at first joint of finger. Took to Ohio co. ER and received four stitches

Employee states on the 24th of October, dispatcher was getting out of her chair in the office and twisted her knee. Employee reported to safety department on the 29th of October and went to hospital from work and returned that date with no release until the 2nd of November. Employee states felt major pain and burning to knee.

Employee was on #3 unit entry #7 working on the pre-dump of double boom fletcher roof bolter. Employee told the operator to try the pr-dump. As employee was getting up off the mine floor he became off balance and grabbed the pr-dump with his left hand and the pre-dump closed on it resulting stitches to his middle finger and index finger.

Employee stated: bolting in #4 entry on #4 unit and had pinner wrench on bolt when boom was lowered, the wrench came out of the chuck striking his mouth resulting in a fracture to top left tooth.

Employee stated: was moving power on #2 unit and was in #6 entry moving power cables. There was mud and water in this entry and was dragging cables thru this area resulting in straining right side of lower back.

Employee stated: was on #1 unit entry #7 bolting the face. The steel was placed into the chuck when the glove was caught in between the base of the steel and the chuck. The rotation lever was pushed resulting fracture to the right wrist.

Employee stated: Installing top Roller belt framing on the #4 unit entry #4 when thumb on the left hand became caught between the framing and the rope resulting a fracture to the end of the left thumb.

Employee stated: was moving belt air lock on #3 unit entry #4 while bending over to pick up timber felt lower back knot up and could not straighten up.

Employee was cutting the plastic top off of a belt chain bucket with a belt knife. The knife slipped resulting in a cut on lower right leg. The cut did not require stitches, the cut became infected and went to the Dr. and received antibiotic to treat the infection.

Employee was in the shop on the surface bending a piece of metal in the bending press. While bending a piece of metal, the metal broke and struck right index finger resulting in a laceration in the bend of the finger. Employee received five stitches.

Employee was on #2 unit in the #3 entry working on a breaker panel lid. Employee was trying to remove pin from the breaker panel when a piece of a chisel struck and entered the top of his left hand.

Employee was on #4 unit entry #5 working on the Feeder. He put pin in the cat assembly and removed come along off the cat assembly. They lowered the cat assembly with the hydraulics of the feeder and as the assembly was coming down the cat caught the tip of the hook and slip off striking him in the mouth and busted one tooth.

Employee was on #4 unit in the #5 entry walking and stepped in a hole and felt right knee pop.

EE was bolting on the #1 unit entry #8 right. While putting steel in roof accidentally hit the rotation and steel stuck right hand resulting in fracture to the index finger.

EE was rock dusting on the #6 belt x cut #57. EE was getting out of the deck of the scoop. When all the body weight was placed on the left leg the left knee twisted.

EE was on #3 unit supply road loading old pallets onto a trailer. EE picked pallet up and twisted to throw pallet onto the trailer which resulted in a strain to the lower left side of his back.

#1 unit #7 entry in the last open cross cut. EE was loading glue on back of the roof bolter. While turning back around to put glue on the bolter, right side of head struck damaged pie pan on roof bolt. Piece of pie pan went under the side of hard hat causing laceration three inches above right ear resulting in two staples.

Was bolting on #3 unit entry #9 at the face. Made roof bolt up and walked to front of the bolter and turned to get bolt. When turning back around right shoulder struck pie pan resulting in laceration. Received 9 stitches.

A roof fall occurred on the #1 unit working section in the face of #5 entry measuring 5 feet high by 25 foot long by 16 foot wide. This area was bolted with 5 foot fully grouted roof bolts and 9 foot cable bolts.

EE was bolting on #3 unit entry # 0 at the face of entry. After making roof bolts up to install in the mine roof EE turned to walk to middle of bolter and twisted right knee.

Employee states while riding in on a mantrip to the unit they hit a bump jarring employees lower back

EE states: Joy miner seemed hung up, went to look operator side cat and miner caught traction on one side, end of tail hit the operator around shoulder area on right shoulder, left shoulder hit coal rib. Resulted in a fracture to the collar bone.

EE states: was throwing concrete block and strained left shoulder. This accident occurred 2/24/2014 as reported only. I was just notified yesterday that he had surgery thru Human Resources.

EE. States: taking spill board down on belt line lost balance fractured left ankle.

A roof fall occurred on the #4 working section in the #10 entry at spad # 0+60 20x20x9 feet high.

EE. Stated: was bolting 9 left and received 6 stitches.

EE. Stated: was leading strap on ram car battery and cell sprayed acid into eyes.

EE states: was putting up keyhole plates and hit head above left eye and received 6 stitches.

EE states: installing roof bolt and turned pie pan, cut index finger on pie pan. 6 stitches

EE states: he woke up and lower back was hurting.

EE stated: was roof bolting and pulled left shoulder out of socket.

Picked up concrete block and turned to throw block in scoop bucket and twisted left knee.

Hooking up motor leads on header motor and back started bothering him.

Employee reached down to pick up miner cable and felt pop in lower back

Employ states while loading 5 foot pins in #9 entry a rock fell striking side of left leg between knee and ankle.

A roof fall occurred on the #2 working section at the face of #1 entry in by spad #14+50 measuring 27x19x5 feet high.

EE states he was working on rock duster and pulled the lid off to work on breaker and strained his back.

EE states he was loading bolter and picked up a arm load of pins, He states turned while loading and felt a pain in groin area.

Employee states while crossing Miner cable, the roof bolter while tramming over pads protecting cable the bolter went over jarring lower back. This became a lost time on 9-10-2013.

Working on suction hose on ram car and knife blade broke causing his forearm to hit broken blade cutting arm causing seven stitches.

EE and several employees picking up trash just outby the unit, EE was throwing trash in container and another employee threw a timber from the other side hitting EE in ring finger causing seven stitches and fracture to finger.

Employee was cleaning feeder and slipped on wet rock around feeder causing possible strain to left knee.

Empoyee notified the company on 11-18-2013 that he had strained his right shouder on 9-18-2013 while tightening up a bolt on a continuous miner. This has now become a Lost Time Accident.

Employee was taking belt rollers in mine and roller struck an arch turning it and striking him in back.

Employee states working setting new header 4A and stepped in mud and twisted knee, this was reported Aug. 5, 2013 no work missed and went back to regular job. Employee re-injured same knee 1-14-2014. May require surgery and now become's a lost time on 1-15-2014.

EE states he was cleaning the back side of belt, breaking a large rock with hammer and a small rock hit him in right eye. while talking to EE stated that he was not wearing safety glasses.

Employee states drove over something in road causing him to strike head on top of Ram car canopy. Employee was off with medical issues I didn't know he was off concerning the report he filed in June of 2013. Had surgery in Dec. 2013 concerning neck injury!

EE stated was getting in Ram Car when left ankle popped.

EE was operating ram car going thru curtain and cable bolt struck ear.

EE stated he was getting out of mantrip, left leg popped with pain.

EE was operating diesel hauler, was making a turn and hit rib. Received stitches on the right side of head.

Employee came to me march 11, 2013 and reported a possible injury to his shoulder while building brattice. This was a report only, he missed no work his shoulder got worse and went to dr. and received surgery on sept 13, 2013. No days lost until day of surgery!

While flushing water line, the water hit him in left eye with mud and water.

while putting in belt with another employee, we lost our footing and I fell striking head on something. causing four stitches.

EMPLOYEE STATES WHILE BOLTING IN #8 ENTRY ON #2 UNIT HAND WAS PINCHED BETWEEN FIRST AND SECOND STEEL.

EE WAS PULLING ON BELT WITH A STEEL ROPE AND HE SAID THE ROPE WAS SLIPPING OFF ROLLER SO HE KICKED IT OVER WITH RIGHT FOOT. FOOT WAS CAUGHT AND TWISTED.

EE STATED THAT HE WAS WORKING ON #2 UNIT BUILDING A BRATTICE. AFTER GETTING OUTSIDE AT THE END OF THE SHIFT NOTICED THAT HIS BACK WAS HURTING. NEXT DAY REPORTED TO ME STATING NEEDS TO GO TO DOCTOR.

WHILE WORKING ON #4 SECTION INSTALLING BELT, EE WAS THROWING 6X6 TIMBER UNDER THE FEEDER AND CAUGHT RIGHT THUMB BETWEEN FEEDER AND TIMBER. EE WAS TAKEN TO EMERGENCY ROOM AND RECEIVED SEVEN STITCHES.

WHILE WORKING IN #4 ENTRY ON #3 UNIT, STOOD UP AND TURNED AND TWISTED RIGHT KNEE.

EMPLOYEE TRAVELING DOWN # 5 ROAD ON GOLF CART STRUCK BY ROCK HITTING ON HAND CUTTING FINGER AROUND KNUCKLE AREA, RECEIVING SEVEN STITCHES AROUND KNUCKLE OF RIGHT HAND MIDDLE FINGER.

EE WAS BUILDING A RIGHT RETURN BRATTICE AND FAILED TO PUT WARNING SIGNAL UP AND A RAM CAR DROVE INTO BRATTICE ON EE WHILE WORKING ON OPPOSITE SIDE. THE BLOCK FELL STRIKING HIM ON RIGHT LEG.

WHILE ROOF BOLTING, INSTALLING MIDDLE PIN STEEL OF THREE DROPPED TOP STEEL AND SMASHED INDEX FINGER BETWEEN STEELS WHILE SITTING IN DRILL POT.

EE WORKING ON #2 SECTION AS MECHANIC, WHILE FIXING A DUST SKIRT ON RIGHT BOLTER CUT HIS RIGHT LEG WITH BELT KNIFE. AFTER TREATMENT NEEDED THREE STITCHES TO RIGHT LEG.

while working in worked out area, re-claiming belt structure and belt ropes, EE states he loosened rope clamps and walked behind structure and was struck in face with rope that slipped.

EE WAS CUTTING BAND ON ROOF BOLTING SUPPLIES AND CUT HIS MIDDLE FINGER ON LEFT HAND WITH BELT KNIFE, HE CUT HIS FINGER THREW LEATHER GLOVE. OHIO COUNTY HOSPITAL GAVE HIM SIX STITCHES.

EE stated he cut 9 face and back up to cut 9 right. While cutting 9 right piece of rock fell out between pins striking left leg.

EE WAS LEAVING #3 UNIT AFTER STAYING IN TO ROOF BOLT AND ANOTHER EMPLOYEE WALKED UP FROM BEHIND AND GRABBED HIM AROUND NECK (PLAYING A JOKE) AND THEY FELL AND EE TWISTED KNEE.

EMPLOYEE WORKING AS UNIT MECHANIC ON #4 UNIT WENT BACK TO TOOL STORAGE AREA TO PICK UP TOOLS, WHILE LOADING TOOLS ON GOLF CART EMPLOYEE PICKED UP A SMALL TOOL BAG AND HE FELT PAIN IN CENTER OF LOWER BACK.

THE THIRD SHIFT MINE FOREMAN DISCOVERED ROOF FALL ON SUPPLY ROAD IN THE FIRST SOUTH PANEL APPROX. 1000' FROM # 1 UNIT.

while pre-shifting #3 unit walking across stepped in hole causing pain to lower back.

Employee was inby utility on # 2 unit, after walking unit fixing ventilation controls he sat down against the rib in # 11 entry and a rock fell from area above head striking on left knee.

While roof bolting on #1 unit a rock fell from top striking employee on back causing injury that resulted in four stitches to lower back.

while changing ram car batteries on # 2 unit operator pulled car away from batteries and still had pressure on lift arms causing car to drop while pulling away. when car dropped operator hit head and shoulder on top of car

WHILE PRE-SHIFTING #1 UNIT' EXAMINER FOUND ROCK FALL IN AREA OF 9+40 INTAKE AIR COURSE.

WHILE WORKING ON # 1 UNIT DRIVING A RAM CAR PARKED CAR AND EXITED TO ASSISTED IN MOVING THE MINER, STEPPED ON ROCK AND TURNED ANKLE. TRANSPORTED TO OHIO COUNTY HOSPITAL LATER DETERMINED FRACTURED ANKLE.

EE WAS ROOF BOLTING ON # 3 SECTION AND ROCK FELL FROM OVER HEAD AND STRUCK ON BACK OF HEAD AND LEFT HAND. EE WAS TAKEN TO EMERGENCY ROOM IN DAVIS COUNTY AND RECEIVED FIVE STITCHES IN PALM OF HAND

Employee was working in battery charging station and cut his left ear on fire suppression system hanging from the Mine roof. Employee finished shift and went to local hospital after work and received five stitches inside ear.

while bolting on #2 section stood up from kneeling position and slipped on rock causing twisting of right knee.

UTILITY PERSON BUILDING BLOCK STOPPING ON RIGHT SIDE OF #2 UNIT, RAM CAR OPERATOR WAS NOT AWARE OF SITUATION AND SOUNDED BELL AND ATTEMPTED TO TRAVEL THRU CURTAIN STRIKING BLOCK WALL PUSHING IT OVER ON UTILITY PERSON ON OTHER SIDE. THE BLOCK FELL ON RIGHT LEG.

EE was loading eight inch water couplings on diesel ride and smashed his small finger on his right hand between couplings and steel post on ride. Transported to Ohio county received four stitches. After leaving hospital released to return to work same date.

Employee was pinning in #12 entry on #1 unit installing middle bolt (off side) in a six wide pin pattern and after setting ATRS to support the top he was standing and turned to his left and a large rock approx. 3'x 3'x 6" broke and fell from the inby side of ATRS and fell down and rolled back on his right leg between knee and ankle.

There was no witness to the accident, ee was struck in the face by the metal concrete pipe to pour concrete on the slope floor. Were cleaning out the excess concrete in the pipe at the time of the accident which includes inserting 2 foam balls into the top and blowing them down the pipe. When the pressure relapsed it appears to have kicked back.

Employee was standing on crane when machine overturned resulting in contusions to multiple parts of the body.

A crane was overturned during a lift causing bruising to the left upper arm.
