Arcosa Erwinville Plant #1671 Metal/Non-Metal

While pulling the 8 gauge shotgun stand out of the #2 hood after shooting a ring, one of the wheels of the shotgun stand came apart and made it more difficult for the employee to pull the gun. The employee continued pulling the shotgun stand and twisted left knee.

Employee used a 10 foot ladder to climb the bowl mill structure to inspect the top of the bowl mill. As employee was climbing down, accidentally placed left hand near an unguarded drive belt and pulley located approximately 15ft off the ground. Employee received a serious injury to left pinky.

Employee was pulling a water hose in a man-lift. EE was attempting to wash the number #2 scrubber tank. As EE tried to lift the water hose, EE felt a pain in lower back. Employee went to the doctor on 1-23-17. The doctor released EE to regular duty without restrictions on 1-23-17. EE went to EE's own doctor on 2/3/2017 and was put on restricted duty.

Employee strained his shoulder while pushing a guard back in place. Employee did not report injury until the following Monday. After his first visit to the doctor we was NOT given restrictions. He went to is own doctor and was put on restrictions weeks later.

The grinding machine that employee was operating stopped functioning. He was trying to check the tension on the drive belt when the pulley engaged and his left index finger ran through the pulley degloving the tip of his finger.

Employee was tapping a hose on the ground to dislodge a clog when the material released and hit him in the eye

EE WAS USING A WENCH TO OPEN DOORS ON THE BOWL MILL, THE SAFETY LATCH DID NOT FULLY ENGAGE AND THE WENCH HANDLE BACK SPUN AND STRUCK THE EMPLOYEE'S RIGHT INDEX FINGER CAUSING A FRACTURE

EE was checking Off Spec Door on # 4 Grizzly on the # 4 Kiln. The door was stuck in the open position and the EE pulled on the door to get it back on the hinges. He felt a pull in his lower back. Conditions and work conditions were normal, no Slip, Trip or Fall Hazards and Poor Housekeeping was NOT noted.

Employee (Bowl Mill Attendent), was attending the clay feed belt, noticed large piece of clay, he stopped the belt and attempted to remove clay without assistance. He felt pain in his lower back, notified Supervisor. He was taken to local hospital - Diagnosed w/ back strain - light duty release to work 14 days.

Employee (mech) installing cylinder on scraper. After getting out from under it, felt a catch in his back. Occupational Doctor on 7/25/06 ? diagnosed w/ minor lower left back strain. Also given 10lb lifting restriction - later clarified on 8/11 to be because of back strain not an unrelated personal hlth issue.

Employee was unjamming a lightweight aggregate log in #3 cooler that was formed by an upset in the kiln. He was using an iron bar (10' long, 10lbs) to pry the log in the cooler. He was standing outside the cooler on a platform & when the log loosened/broke free, his momentum carried his left hand down onto the handrail resulting in left finger tip fracture & dissolvable stitches.

On 9/21/04 empl. reports back pain attributed to a work accident on 3/9/03. That day lifted 10lb can, strained back. 3/10/03 and 9/27/04 Dr visits status: perform normal job. Visits on 10/6 & 10/8/04 Dr noted no change, refer to specialist exam 10/12 & MRI 16th. MRI & diagnosis was Lumbar Spondylosis. Empl. could contin. to work w/limits on lifting, bending, etc.

EQUIPMENT REPAIRS, EMPLOYEE HAD TWO FINGERS CUT.

EMPLOYEE WAS ASSISTING WITH THE SET UP OF A CRANE FOR DRAGLINE. WHILE STANDING ON THE TRACK HELPING TO GUIDE THE MAIN HOIST INTO POSITION IN ORDER TO PUT GANTRY IN PLACE, CABLES THAT WERE ATTACHED TO THE GANTRY WERE PULLED CAUSING THE GANTRY TO TIP & FALL. PART OF THE GANTRY HIT HIS LEG RESULTING IN A GROSS CONTUSION & SPRAINED RT. FOOT.

After loading the clay bin, EE entered maintenance shop. He placed gloves on his hands to begin check of 938 Caterpillar. Moments later, he felt the glove on his right hand tightening. EE removed the glove on his right hand and noticed swelling in a centralized spot on his wrist. He also noticed a black dot in the center of swelled knot on his wrist.

We had a coal bin fire and were in the process of dumping the coal into loader buckets for disposal. The injured party was opening a dump valve when a small piece of floor grating shifted and allowed the employee to fall through to the ground below. The employee sustained a laceration to his head approximately 2 inches long requiring 10 stitches.

AN EE OF AN OUTSIDE VENDOR WAS SECURING AN OLD MOTOR GRADER TO A LOW BOY TRAILER FOR TRANSPORT OFF SITE. AN EE OF ANOTHER CONTRACTOR RAN INTO THE AFOREMENTIONED EE WITH A 400 SERIES CATERPIL LAR TRUCK. BOTH INVOLVED PEOPLE NOT EMPLOYED BY BIG-RIVER INDUSTRIES.

EE WAS STANDING ON THE TRACKS OF A BULLDOZER AND SLIPPED. HE FELL TO THE GROUND FROM THE TRACKS.

EE WORKED NORMAL SHIFT AS A CLAY FEED ATTENDANT & WENT HOME. HE NOTICED SWELLING AROUND HIS RIGHT ELBOW. THE DOCTOR SAID HIS SWELLING IS FROM BURSITIS FROM AN OLD INJURY 4/25/00.

EE WAS WALKING DOWN CATWALK WHEN HE FELT SOMETHING CATCH IN HIS RIGHT KNEE, TAKEN TO CLINIC STRAINED KNEE.

EMPLOYEE WAS REMOVING A LOG FROM #4COOLER WHEN PRY BAR SLIPPED AND JAMMED FINGER AGAINST HAND- RAIL. RESULTING IN LACERATION TO FINGER. STITCH-ES REQUIRED.

EMPLOYEE WAS SUPERVISING A PROJECT WITH FITTING AND WELDING TAKING PLACE. EMPLOYEE RECEIVED FLASH BURN TO BOTH EYES.

EMPLOYEE WAS RUSHING DOWN STEPS AND TRIPPED LANDING ON HIS KNEES. THE MEPLOYEE BRUISED HIS LEFT KNEE.

EE ALLEGES THAT THE SEAT ON THE 613C CATERPILLERSCRAPER CAUSED AN INJURY TO HIS LOWER BACK

EE WAS PUSHING DIRT CLODS ON GRILL BELT WHEN A LARGE PIECE OF DIRT CAME ON BELT. WHEN EE WENT TO PUSH DIRT OFF BELT, HE PULLED A MUSCLE IN HIS BACK.

EE WAS GOING UP #3 CATWALK TO CLEAN A CHOKE ON #3 DUST CHAMBER--EE SLIPPED ON CATWALK--CATWALK AND EE'S BOOTS WERE MUDDY.

EE WAS REMOVING A JAMMED CLINKER FROM THE CLINKER CHUTE OF COOLER WITH A PRYBAR. THE EE PULLED A MUSCLE IN HIS UPPER BACK.

3 MEN WERE WORKING ON A DUMP TRUCK WHEN ONE WENT TO PUT A BOLT IN A SPRING INSULATOR AND THE TRUCK FELL. THE TRUCK WAS HELD UP ON BLOCKS AND 3 JACKS. THE TRUCK FELL ON CO-WORKER'S HAND AND HI T INJURED ON THE RIGHT LEG.

3 MEN WERE WORKING ON A DUMP TRK, WHEN EE WENT TO PUT A BOLT IN TO A SPRING IN INSULATOR ON THE TRK FELL THE TRK WAS HELP UP BY BLOCKS & JACKS THE TRK FELL ON EE'S RIGH HAND, HE RECEIVED A PO INTER CUT THAT REQUIRED 4 STITCHES ON TOP SIDE OF HAND BETWEEN THUMB & FINGER

EE WAS PREPARING TO LOAD A TRUCK-WENT TO REDIRECT FLAP TO DIRECT MATERIAL TO TRUCK-ROPE ON FLAP SLIPPED WHEN FLAP MOVED AND HOOK ATTACHED TO ROPE PINCHED THUMB BETWEEN HOOK AND SIDE WALL.

EE WAS PERFORMING HIS REGULAR DUTIES AS A SCRAPER OPERATOR. HE HAD NOT EATEN ALL DAY, AND ONLY HAD SOME WATER FOR FLUID INTAKE. HE DID NOT TAKE ADVANTAGE OF THE GATORADE PROVIDED TO EES. HE BV ECAME OVERHEATED DUE TO THE ABOVE CAUSES AND HIGH TEMPERATURES.

EE WAS ATTEMPTING TO COOL A LOG WITH WATER HOSE AT BACK DOOR OF #4 COOLER. WHEN WATER SPRAYED ON HOT MATERIAL, STEAM WAS CREATED AND BLEW BACK, BURNING HIS RIGHT ARM.

EE WAS TIGHTENING ROPE ON ROLLER ON REVERSIBLE BELT ROPE BROKE AND EE JAMMED HIS FINGER BETWEEN ROLLERS AND ANGLE IRON.

EE WAS SHOVELING BEHID #2 KILN BELT AND PULLED A MUSCLE IN HIS UPPER BACK.

EE WAS THROWING A PIECE OF CLAY OFF THE BELT, WHEN THE CLAY BROKE IN EE'S HAND. EE TRIED TO GRAB PIECE OF CLAY AND HIT HIS ELBOW.

EE WAS CLEANING OUT THE CLAYFEED HOPPERS USING ASHOVEL WHEN HE SLIPPED AND HIT HIS HEAD ON THE HANDRAIL.

EMPLOYEE WAS SWINGING A 16 LB MAUL TO A SMALL CONTACT POINT (PINCH BAR). EMPLOYEE MISSED CONTACT POINT ON 5TH STRIKE AND STRUCK CONTACT POINT WITH FINGER AND HANDLE, CUTTING LEFT INDEX FINGERT IP AND FRACTURED TIP. NAIL BED WAS ALSO DAMAGED.
