Employee strained ankle walking into work.
Miner was working in man lift when EE accidently switched the lift into high gear. The lift hit a large, hidden rock and jolted several times. The miner's lanyard shortened with each jolt and EE could not dislodge EE's foot from the dead man's switch. EE sprained EE's right ankle.
Pinched finger between rope and handrail, pulling finger apart requiring 11 stitches in finger. Return to work next day on light duty at office.
An operator was descending down a set of stairs located at the back of the warehouse. The operator missed/ slipped on the last two steps resulting in a fall. The operator claimed to have no injuries but was sent to the local medical facility for a proper diagnosis. The employee indicated that they were using the handrail at the time of the incident & there were no witnesses.
Employee was laying down a water pump that weighed 86 lbs. As they were laying the pump down their right hand ring finger was smashed fracturing a bone in the finger.
Employee was assisting in removing material from the "short" belt. While doing so, employee felt a pain in lower left leg. Employee reported the injury the following day. Diagnosis strain on 6/25, no treatment, and returned to full duty. Recheck on 6/26/19, prescribed physical therapy which classifies it as medical treatment.
Employee was cleaning the area around 4 mill when EE decided to move a few 55# grinding media bags off of a platform about 42" above the floor without assistance. EE grabbed a bag in each hand and as EE swung the left handed bag to the floor, EE felt a pain and pull in EE's abdomen. Diagnosis hernia and placed on restricted duty.
A recent MSHA inspection noted that an employee failed to complete a follow up hearing test. As a result, initial testing indicates that the employee has experienced a reportable threshold hearing shift after adjusting for age.
Two employees were moving a diesel generator to another level. When they were connecting the 8" flexible hoses, one of the employees strained side (front wall of thorax) while trying to get the hose positioned in order to close the cam-locks.
