Employee hit a bump while driving a haul truck. As a result, the employee was jolted (was wearing seat belt) and felt pain in upper back.
Moving a 4'x4' metal plate of off truck bed and plate slid, catching EE's finger between plate and weld equipment on truck. Fractured tip of left 1st finger.

Employee was cutting bars out of the grizzly with a torch. Employee sent another employee for a tape measure. When employee couldn't find tape measure in the truck, he got in truck and drove to get tape measure. Employee forgot that employee was using the torch that was attached to the truck. Torch jerked and twisted and struck the left wrist of employee that was using it.

Employee was walking down steps at the sand tanks when he slipped on the ice covered steps. He went to catch himself with the hand rail and that was also ice covered which caused his hand to slip on rail. He bent his right index finger backward and fractured his finger.
Employee was using a hammer to dislodge rock from the grizzly when he developed discomfort in his left elbow. Diagnosed with an elbow strain.
Employee was climbing off counterweight and strained his right upper arm and shoulder. Employee was wearing fall protection at time of occurence. Employee was diagnosed with strained right arm and placed on restricted work activity.

EE WAS WORKING ON CONVEYOR AND HAD TO LIFT THE TAIL WHEEL ONTO WELDING TRUCK. HE HAD ANOTHER HELPING HIM LIFT THE APPROX. 100# TAIL DRUM. WHILE LIFTING HE FELT SOMETHING GIVE IN HIS LOWER BACK . DIAGNOSED WITH A LOWER BACK STRAIN.
EE WAS SHOVELING SAND OUT OF SAND STORAGE TANK WHEN HE SLIPPED AND FELL BACK AGAINST THE EDGE OF THE TANK AND BRUISED HIS LOWER BACK.
EE WAS MOVING A SPEED REDUCER FROM ONE SIDE OF SHOP TO THE OTHER USING THE LIFT & SLIDE METHOD. WHEN HE LIFTED TO SLIDE HE FELT SOMETHING POP IN HIS RIGHT GROIN AREA. HE WAS ALSO COMPLAINING O F PAIN IN HIS LOWER CENTER BACK.
OPERATOR SLIPED OFF STEP EXITING CAB OF MACHINE FALLING ONTO A HYDRAULIC CYLINDER CONTACTING LOWER LEFT SIDE OF BACK.
OPERATOR STEPPED OUT OF EXCAVATOR CAB ONTO TRACK STARTED TO STEP OFF OF TRACK ONTO THE NEX STEP EXITING THE MACHINE. HIS FOOT SLIPPED OFF OF STEP AND HE FELL TO THE GROUND LANDING ON BUTTOCKS ABOUT 3 FEET. WENT TO DOCTOR 4/15/02. FIRST DAY OFF 4/16/02.
