Stepping of a step ladder, EE stepped backwards and lost EE's foot, fell backwards catching self on EE's wrist
Changing a tail pulley and the tail pulley slipped smashing the finger
At approx. 2:00pm employee noticed a irratation in his right eye,while crusher was in operation.
Employee was tightening a bolt on a loader when the socket head slipped off the nut, hitting him in the left cheek, causing a small cut under left eye.
