Employee cut the tip of his finger off when trying to load a pipe into the back of a pick up truck.

Employee was struck in mouth with a pad eye. Employee received six stitches to his upper lip.

Employee felt discomfort to the right shoulder and bicep area when lifting & disposing of a wear cap weighing 79 lbs.

Employee received a laceration to his left forearm while drilling on a piece of stainless steel.

Employee fractured right leg while deboarding a shovel.

After hooking up the cable to the channel the employee fell and struck his head.

Employee chipped right front tooth. On 10/10/2012 we were made aware the employee had the tooth filled with resin.

Employee was carrying a box and the box began to slip. As the employee grabbed the box, he felt a pull in his upper right arm. Restrictions began 08/02/2012.

Employee was a degenerative disc condition as well as bad knees with three knee replacements. These conditions make him prone to back and knee injuries. Employee was moving a box of rain jackets and felt pain in his lower back. He stated the pain was similar to the incident on 02/27/2012. This is being submitted under protest.

Employee got his finger caught in a pinch point while putting a chain on a sprocket.

Employee slipped on a mine road on 4-16-11 and strained his shoulder. Although his condition improved, he did not completely heal. Physical therapy began on 5-25-11. Safety was notified on 5-31-11.

While employee was removing a hydraulic oil tank drain plug, he felt soreness in his neck.

Employee pinched his finger between his wrench and a drill mast motor when his wrench slipped.

Employee slipped on ice after exiting his vehicle and hit his head against the ice.

While an employee was removing the shovel hoist drum locking lever, the lever pulled out from the drum faster than expected. He tried pulling back on the lever and it stretched out his left arm, causing pain in his left bicep and elbow.

Employee reported to work the day after he removed a pump at the utility plant.

Employee was opertating a hand held grinder when the grinder kicked back, hitting him in the chin with its handle. This caused his mouth to close quickly, chipping an upper front tooth. Employee received a crown on 7/21/10.

A natural gas odor was present in warehouse at the start of the shift. The doors were opened and the odor cleared. Late in the shift, an employee felt ill and went home due to the odorant. Her work location was changed due to the exasperation of a pre-existing condition.

Employee received small sliver in his leg while attempting to remove a lip shroud using both a hardened steel hammer and a brass hammer.

While grinding slag from the edges of a floor liner plate, the grinder came in contact with the employee's leg.

While exiting a vehicle, employee slipped and his hand was cut on the door of the truck.

Employee was walking towards his truck and slipped on the snow covered ground. Initially this was a no treatment case, but has been changed to a Restricted Work Case as of 1/12/2008. **Due to surgery, this case is changed to a Days Away case as of 3/6/2008.**

While employee was removing bolts from a grinder, dust blew in his eye causing irritation.

While exiting a dipper, EE slipped on shank, resulting in EE falling on his right hip and shoulder. Due to an unresolved back pain, the doctor recommended physical therapy as of 12/17/07.

While installing torque tool onto wheel nut, the torque tool slipped off of the nut causing the left 3rd finger to be pinched between the reaction arm of the tool and the rim of the tire. Employee changed jobs & has restrictions in his new position. However, the restrictions were short lived & did not affect his job.

Employee's finger was crushed while repairing shovel pad.

Employee's finger was trapped between the guard and grinding wheel of a portable 5" right angle grinder, cutting his left index finger.

While dumping scrap from a hopper into a dumpster, employee cut finger.

While removing welding plug from welding recptical, felt pain in shoulder. EE will just be starting physical therapy, 2-7-06, thus becoming a medical treatment case.

While removing engine oil pan from sanding truck, struck right hand with hammer.

While cutting rubber hose with knife, employee cut his right index finger.

While moving a support piece for a disassembled shelving unit, the employee injured his abdomen.

While using an impact wrench to remove a nut and wear plate-retaining bolt from a cat loader, the EE lacerated his left middle finger.

An accumulation of acetylene in a storage cabinet ignited and exploded injuring an EE's right heel.

While pushing a cart, the employee began to turn the cart around when he felt pain in his knee.

While walking across parking lot on the way to work, employee slipped on ice injuring his arm. Became medical treatment case 12/16/05.

WHILE BENDING DOWN TO PLACE A CLAMP UNDER SOME PLATE, EMPLOYEE FELT PAIN IN HIS KNEE. ACCEPTED AS A CASE 12/2/05.

While replacing lip shrouds on shovel dipper, a foreign body became embedded in his leg.

While operating a shear, the employee lost balance causing his hand to slide under the guard, resulting in the tip of his right fifth finger being crushed by the hold down ram. **Employee received additional treatment resulting in lost time. Employee returned to work 4-18-06 with 13 days lost.**

While positioning a boring jig, the jig fell pinching the employee's finger.

While grinding, a small particle became embedded in his eye.

While polishing a bearing surface with emery cloth and a pneumatic air grinder, the cloth slid off cutting the back of his hand.

While installing lower liner plates on a shovel, one of the upper liners fell through its slot, bounced and struck the employee's arm.

While attempting to remove a two piece coupling from a damaged hydraulic hose, ee struck his hand with a hammer.

WHILE ROLLING OVER A BEAM TO PAINT, HE INJURED HIS BACK. BECAME AWARE OF PHYSICAL THERAPY 7/1/05

While walking to a welding truck in the mine, wind blew dust into the injured's eye.

On 10-8-04, the injured was walking on a shovel roof when he slipped. On 12-21-04 he began losing time.

While dumping a fuel filter, the miner cut his finger.

THE MINER WAS PUSHING ON A LINING BAR AND FELT A PAIN. BECAME AWARE OF CHIROPRACTIC CARE ON 8/27/04.

The miner slipped while cleaning a tire in the steam room.

On 3-26-04 the miner was in a man basket using an impact wrench to tighten bolts and felt a pain. Became aware of chiropractic treatment on 4-16-04.

WHILE POUNDING ON A LIP SHROUD THE INJURED WAS STRUCK BY A PIECE OF METAL.

WHILE SHARPENING A PUTTY KNIFE ON A GRINDER, THE INJURED'S FINGER WAS PULLED INTO THE GRINDING WHEEL.

THE INJURED WAS A MEMBER OF A CREW WHO WAS REMOVING ANGLE IRON FROM THE FLOOR WITH AN OVERHEAD CRANE. THE HOOK PULLED THROUGH THE ANGLE IRON & STRUCK THE INJURED ON THE HEAD.

ON 3-20-03 THE INJURED WAS CLOSING A HYDRANT, HEHAD LOOSEN A 2 1/2" CAP TO RELIEVE THE PRESSURE THE CAP CAME OFF & STRUCK HIM & THE FLOW OF WATER THREW HIM BACK AGAINST A PARKED TRUCK. ON 3-26-03 HE BEGAN LOOSING TIME.

ON 1-14-03 THE MINER WAS PULLING & LIFTING A PLATE & FELT A PAIN. ON 2-24-03 HE BEGAN PHYSICAL THERAPY.

THE INJURED HAD HIS FOOT PINNED TO THE GROUND AS HE WAS A MEMBER OF A CREW WHICH WERE LOWERING A SUBSTATION.

ON 11/7/02 THE MINER LIFTED A SPROCKET AND FELT A PAIN. ON 1/24/03 HE UNDERWENT SURGERY.

WHILE HAMMERING ON A LIP SHROUD THE INJURED GOT A PIECE OF STEEL IN THE BACK OF HIS LEG.

ON 7/25/02 THE MINER PUT A GARBAGE BAG INTO A DUMPSTER & FELT A PAIN. ON 11/4/02 HE BEGAN LOSING TIME.

WHILE REMOVING A FOUR-LEGGED CHAIN FROM A LIFT, ONE LEG GOT CAUGHT ON A D-RING. THE EYEBOLT BROKE & THE MINER WAS STRUCK BY THE CHAIN.

ON 5-9-02 THE INJURED WAS POUNDING ON A SHOVEL BUCKET SHROUD WITH A HAMMER AND WAS STRUCK BY A PIECE OF STEEL. ON 5-20-02 HE RECEIVED MEDICAL TREATMENT.

WHILE TORQUING NUTS ON TIRE INJURED CAUGHT HIS FINGER BETWEEN THE TIRE RIM AND REACTION BAR.

THE MINER REACHED OUT TO CATCH A CONTAINER AND FELT A PAIN.

AFTER TAPPING A KEY INTO A KEYWAY ON A SHAFT THE INJURED RAN HIS HAND ALONG THE KEY & CUT HIS FINGER. SUTURES.

WHILE OPERATING A LATHE THE INJURED CUT HIS FINGER ON A STEEL SHAVING--SUTURES.

ON 10/30/01 THE INJURED CAUGHT HIS FINGER BETWEEN A TORQUE WRENCH & TRACTOR PAD. ON 1/2/02 BECAME AWARE OF PHYSICAL THERAPY ON 12/16/01.

WHILE WALKING DOWN STAIRS THE MINER SLIPPED AND TWISTED HIS KNEE.

WHILE HAMMERING ON A SHANK TO POSITION IT ON A SHOVEL BUCKET THE MINER WAS STRUCK ON THE LIP BYA PIECE OF METAL.

THE MINER WAS PUSHING A DOOR INTO A SHOVELDIPPER & HE FELT A PAIN. ON 9-6 HE RECEIVED PHYSICAL THERAPY.

ON 7/17/01 THE MINER TRIPPED ON A FAN. ON 10/26/01 HE BEGAN RECEIVING PHYSICAL THERAPY.

WHILE STRIKING A LIP SHROUD ON A SHOVEL BUCKET WITH A HAMMER A PIECE OF STEEL STRUCK THE INJURED.

WHILE WALKING TO WORK BEFORE SHIFT STARTED THE MINER SLIPPED ON ICE & FELL.

WHILE REMOVING A WHEEL HUB ON A PRODUCTION TRUCK THE INJURED CUT HIS FINGER ON THE SHIMS.

WHILE CLIMBING DOWN A LADDER ON 4-5-01 THE MINER MISSED THE BOTTOM TWO STEPS AND TWISTED HIS KNEE, ON 5-29-01 HE BEGAN RW.

THE MINER WAS USING AN AIR GRINDER & HE GOT DUSTIN HIS EYE. HE WENT TO THE CLINIC ON 4/6/01.

THE MINER STEPPED ON A ROCK WHILE GETTING OFF THE BED OF A CAR CARRIER.

WHILE REMOVING A SHIM FROM A SHOVEL DIPPERDOOR, THE INJURED CAUGHT HIS THUMB BETWEEN TWO SHIMS.

ON MARCH 2, 2001 EE WAS USING AN ECCENTRIC WRENCH. THE WRENCH SLIPPED AND HE FELT PAIN IN HIS BACK. BECAME AWARE OF PHYSICAL THERAPY ON MARCH 29, 2001.

WHILE LIFTING A DECK LID ON A PRODUCTION TRUCK THE MINER FELT A PAIN.

WHILE ROLLING A PENDULUM ON A BENCH THE MINER FELT A PAIN.

THE MINER WAS BLOWING SLAG FROM WELD AND GOT SOMETHING IN HIS EYE.

WHILE LIFTING A SHEET OF METAL THE MINER FELT A PAIN.

THE MINER STRUCK HIS FINGER WITH A HAMMER.

THE MINER WAS REMOVING CHAINS FROM A CRANE HOOK.BECAME AWARE OF PHYSICAL THERAPY ON 2/28/01.

ON 10-4-00 THE MINER WAS LIFTING A HOOD ON A PRODUCTION TRUCK AND FELT A PAIN. BECAME AWARE OF CHIROPRACTIC TREATMENT ON 10-18-00.

WHILE PUSHING A CART WITH AN AIR DUCT ON IT, THE DUCT SLID BACK OFF CART AND STRUCK THE MINER ON THE HAND.

WHILE INSTALLING A BUSHING THE MAN STRUCK HIS THUMB WITH A HAMMER.

ON 7-10-00 THE MINER WAS WORKING ON AN ALTERNATOR AND HE BUMPED HIS TOOTH, BECAME AWARE OF DENTAL TREATMENT ON 7-25-00.

WHILE CUTTING A PLATE THE MAN BURNED HIS SHOULDER

WHILE PULLING ON A BEAM THE INJURED FELT A PAIN IN HIS SHOULDER.

THE INJURED WAS HAMMERING ON A LIP SHROUND ON A SHOVEL BUCKET AND WAS STRUCK BY A PIECE OF METAL

AFTER TIGHTENING LUGS ON A TIRE THE MINER FELT A BURNING IN HIS EYE.

ON 4-11 THE MINER WAS USING A TORQUE WRENCH TO LOOSEN A BOLT AND THE RATCHET SLIPPED CAUSING THE HANDLE TO STRIKE HIM ON THE CHIN, BECAME AWARE OF DENTAL TREATMENT ON 5-8-00.

WHILE OPERATING A LATHE A CHIP OF METAL FLEW UNDER THE INJURED'S SAFETY GLASSES.

WHILE GRINDING ON A LADDER, THE LADDER FELL OFF A TABLE STRIKING THE MINER ON THE FOOT.

ON 1-29-00 THE MINER WAS WALKING IN A TRUCK BOX AND FELL, BECAME AWARE OF MEDICAL TREATMENT 5-8-00.

WHILE CLIMBING DOWN THE LADDER ON THE #33 SHOVEL THE INJURED'S FOOT SLIPPED & HE CAUGHT HIMSELF FROM FALLING.

WHILE CLOSING THE DOOR ON A DRILL THE MAN CAUGHT HIS FINGER.

WHILE REMOVING A MIRROR FROM A WALL THE MAN CUT HIS FINGER.

THE MAN STRUCK HIMSELF ON THE MOUTH WITH A SPLICE PIPE WHILE PLACING IT IN A TRUCK BOX. BECAME AWARE OF DENTAL WORK ON 1-24-00.

ON 1/10/00 THE MAN WAS LIFTING A BEARING CAGE AND FELT A PAIN. BECAME AWARE OF MEDICAL TREATMENT ON 7/20/00.
