CHESAPEAKE QUARRY Metal/Non-Metal

Plant operator fell from height of 12-13 ft while clearing ice/snow from a storage bin & suffered a broken hip requiring surgical repair with metal plate and screws, anticipated 12 weeks of missed work recovery. Expect full recovery with no disability. EE disregarded foreman's instructions not to enter bin, used a rope not a harness, had no second person, ice/snow slip hazard.

Technician was making repairs to the rear axle of a CAT 769C truck. While carrying the rear axle from the rear to front of the machine, lost balance of axle and footing, stepping forward; right foot (toes) came down on the edge of the 6x6 wood blocking, resulting in closed fractures of metatarsals 3-5.

Worker noticed lights left on haul truck. Climbed up 2 steps to reach inside and turn off lights. Stepping back down foot slipped on bottom rung which had mud build-up slipped and fell to ground. (approx. 2') Tried to break fall with hand resulting in injury to wrist.

Front end was building safety berm, stopped to load customers. Trucks continued the effort by hauling & dumping material to build berm for edges of stockpile. EE was dumping load of material & positioned too close to edge. Surface did not support & truck went over backwards, slid & fell 25 feet & landed on top. Impact caused his head to strike metal cooling fan mounted in cab.

INJURED EE WAS WORKING ON SOME PLANT REPAIRS WITH OTHER EMPLOYEES. TO REMOVE BLOWBARS FROM A CRUSHER, THE BLOWBAR SHOE MUST BE STRUCK WITH A HAMMER IN A WAY TO LOOSEN THE SHOE TO ALLOW REMOVAL OF THE HAMMER. EE POINTED TO A LOCATION ON THE SHOE TO BE STRUCK & PLACED HIS FINGER ON THE PATHOF THE HAMMER WHEN AN EE ACCIDENTLY HIT HIS FINGER W/HAMMER. STITCHES WERE REQUIRED TO CLOSE WO
