Employee was walking along frame of rock crusher, slipped off and fell to ground causing elbow injury.

Employee operating loader, inadvertantly loader bucket struck rock debris pile, causing loader to abruptly stop. Operator was gripping steering wheel with both hands, causing a slight impact affecting wrists of operator; wearing seatbelt. Operator remained in full duty work as per doctor. As of early April, employee received minor physical therapy for wrist discomfort.

EE came to work this morning and notified other ee that he did not feel good he was having chest pains. Other ee told mgr ee what was going on. MGr said take ee to the hospital. Other EE took him to the hospital and came back to work. He had test ran and the hospital found he had 2 blocked arteries and they needed to do open heart surgery.

Employee was working on impact crusher belt when his foot was caught between the impactor blow bar and the wear plate.

Employee reported back injury on 8-11-04, but did not seek treatment at that time. Employee reported aggravation on 9-13-04 and was sent to the doctor. He was diagnosed with lumbar strain and was placed on restrictions, making this event recordable.

EMPLOYEE SLIPPED OFF LADDER WHILE EXITING HAUL TRUCK AND ARM STRUCK EXHAUST BRACKET DURING FALL. CURRENTLY ON LIGHT DUTY - NO LOST TIME INVOLVED.

OPERATOR WAS CLEANING WINDOWN ON LOADER, SLIPPED & HIT THE CATWALK ON LEFT BACK.
