Employee was bar scaling in the back of a room when a piece of rock from behind where he was working came down and struck him on the backside of his head. His hard hat did not cover this portion of his head, it struck him about two inches above the hairline on his neck. Resulting in a laceration to the back of his head.

While working on a crusher a rock became lodged in the impactor. The employee attempted to remove it and it slipped, causing the burst laceration to his left middle finger.

While repairing a water line, employee used a utility knife to cut a cable. The knife slipped causing a laceration to the left forearm.

Employee was shoveling rocks off of a conveyor when she experienced pain in the neck and shoulders.
Employee has had ongoing pain in his elbow since 5/11/06 and no medical attention. ON 6/14/06, he reported lifting metal and pain increasing, at which time he had medical treatment.
Employee was dumping trash in the dumpster when a large raccoon came out of the dumpster, startled him, he jumped back.

The bolts came loose on the yoke of an ANFO pot. The yoke came off of the tank and hit the employee in the right knee. Employee was placed on restricted duty.
