Employee was attempting to lift elevator door on man elevator and door stuck which caused EE to strain EE's back. Was still working at full duty, became a recordable on August 20, 2025. Prescribed medications and also limitations. Able to work with limitations.

Injured was climbing down the ladder of a 980M, left foot was on the ground, the right foot got caught on the rung, causing the injured to fall backwards into a parked pickup truck. Suffered a shoulder sprain.

EE was working on removing a grizzly grates in our underground crusher area. As EE attempted to pry out grate a small piece broke free and struck EE in right shoulder blade area.

EE was cleaning mill building, after a few hours employee went to cool off and take a break, after removing goggles something went into left eye.

Employee was climbing into pick up truck, foot slipped on running boards and employee fell to the ground causing a sprain to the left ankle. Employee was placed into a brace.

EE was switching cement lines as gate was stuck, as EE was switching, gate got stuck halfway open which caused cement to spray out and into EE facial area. Causing cement to enter EE's eyes.

EE was removing plug from a silo chute, as EE struck the chute with pipe EE's right hand came off pipe and struck the flange at the chute. This caused some lacerations, which required stitches.

EE was replacing drive chain on bucket elevator. As EE was attempting to pull chain together to pin, EE's hand slipped and caused EE to hit bottom of guard causing a small laceration on left hand ring finger second knuckle. Requiring 5 stitches.

Contractor received an injury while power washing the inside of a railcar. While repositioning self the contractor slipped and accidentally squeezed the trigger on the power washer wand. The wand was aimed just behind the steel toe of EE's boot. The water jet cut through the boot and pierced the skin of the individual.

While moving a punch top drum, Employee received a cut on their right thumb from a sharp edge. After the thumb was cut, Employee's thumb was glued for closure of the wound, hand and arm were placed in a splint to stabilize and keep the thumb from moving. Employee was prescribed Hydrocodone, Acetaminophen, and Cephalexin, they also received a Tetanus shot and T-Dap Vaccine.

Employee was working on guzzler truck removing back suction hose, when employee loosened clamp hose fell and struck employee in left foot area, just behind steel toe area.

Employee was talking with another employee in welding shop and other workers were trying to free a steel liner from the Finish mill door. As they struck liner with sledge hammer, a small piece of scrap metal shot off and hit employee in right leg below knee cap. Injured employee was 8 feet from other workers.

Employee works in Quarry area and in direct sun light during working hours, as EE did this at end of day (shift) EE felt a little light headed and pale. EE was taken by personnel vehicle to ER where EE was found to be dehydrated.

While hand scaling a rib, a rock fell striking the employee. Lacerations to the forehead and right shoulder needed stitches.

Employee was lifting door holding bar from finish mill when EE felt pain in EE's lower back.

Employee was inspecting air slide and rotary feeder, when material became loose and blew out and underneath protective eye wear.

Employee was noticing wrist issues and pain, diagnosed with carpal tunnel in both wrists.

Employee was using a pry bar to remove a screw conveyor, when the screw became loose it made the pry bar slip and caused the employee to fall back into screw housing.

Employee was replacing a guard when it slipped through employee's hands and dropped on left foot, just behind the steel toe protection.

Employee was climbing under trailer, notice pain in lower back area.

Employee was working on dozer, when EE noticed a socket under side hatch door. EE attempted to remove socket and when EE did it fell on EE's left hand index finger area.

Employee noticing pain and discomfort while welding in one position for long periods of time.

Employee was walking in Finish Mill area and turned and ran into chute, jarring neck. EE was seeing a doctor for this on 4-29-2021. They prescribed Physical Therapy. EE is under no restrictions or limitations.

Affected employee was assisting in the removal of scaffolding from the Pre-heater tower via pallet jack. Whilst loading the scaffolding on to the elevator, another employee slipped on snow/ice. This sudden movement resulted in the shift of material thus trapping the EE's left hand between the material and elevator wall, smashing the fifth digit (pinky finger) on the left hand.

Employee was exiting tower elevator when door became stuck, EE attempted to pull up and push down with no luck. EE than put a foot on door half and opened rest of way. EE noticed a pain in a shoulder.

Employer was informed that following the employees annual audio-metric testing, a significant standard threshold shift was identified.

Swinging a 20 lb sledge hammer to open a finish mill door.

Employees were cleaning material in the preheater tower. Employee was burned by hot material when inspecting a port for blockage.

At 12:44 there was an explosion in the quad shredder. One employee was injured and significant damage the facility occurred. The employee received 3 staples for a laceration on EE's head and was given pain medication and antibiotics for two holes in EE's eardrum

The employees began to open the inspection port when pressurized cement unexpectedly began flowing outward. One employee attempted to quickly reinstall the port, and as EE pushed the port inward, cement began to spray into the employees' eyes. The employee was struck by cement in both eyes and was placed on restrictions and prescription medication.

Continental Cement employees had left a jackhammer bit on a raised platform and left the area. A contractor was working below when the jackhammer bit fell through the raised platform and hit the employee in the side.

EE was rotating the motor manually of the fan pulling the belt and the pulley when cotton glove was caught between the belt. The momentum pulled the glove and cliff pulled hand back. When EE pulled hand away, it removed the hand from the glove; left thumb had been lacerated a portion of the thumb was lost.

A miner was provided prescription medication for a back strain.

On 9/4/2019, employee was seen by OccMed doctor and complained of repetitive hand/elbow injury; cause of injury unknown and assumed to have occurred over time. Surgery occurred on 12/6/19, return to work with restrictions on 12/7/19.

Employee stated was experiencing soreness in left wrist. Initial treatment was first aid, on May 20 doctor visit employee was given a wrist brace to use while at work.

Employee had lowered a man lift, gotten out, and was leaning over the man lift to pick up approximately a bucket of tools weighting 25-30lbs and strained EE's back.

Employee was carrying an approx. 50-lb ladder guard on shoulders across an unpaved lot when employee slipped & hurt back. Employee has been placed on restrictions.

Employees were inside the coal mill replacing the rubber sleeve. The employee's left leg was standing on a lip and right leg was kneeling on the roll in order to repair the rubber sleeve. When the employee lifted the sleeve, EE's left knee "popped". Employee diagnosed with a lateral collateral ligament sprain and had surgery on June 13, 2019.

The employee stated they stepped forward off a platform with their right foot onto a 4-in drop onto a lower platform, felt their weight shift, and reported pain in their back & hips on January 11. On January 14, OccMed placed the employee on limitations and prescribed 800mg of ibuprofen.

An employee was walking from the storehouse area to the maintenance breakroom/bathroom. Employee rounded the corner of a designated walkway so that they were outside the designated walkway and hit their left shoulder against the ladder rack holder.

On 6/11/2018 ee was performing mobile equipment inspection on the cat 262C skidsteer. EE knelt down to check the cab filter when Left knee popped and ee felt a slight pain. EE was seen by a physician who recommended ice and OTC meds for pain. On 9/4/2018 ee noticed pain in the same knee and was seen by a physician who ordered an MRI. Surgery occurred on 10/4/2018.

The employee was pulling themselves up to get onto loader and felt something pull below the right shoulder blade. On September 26, the employee requested medical attention. The employee was prescribed a muscle relaxer and was not placed on restrictions.

The employee was tightening nuts with right hand felt a pull in middle lower abdomen in the groin area. Medical attention was requested on August 29. The employee has been diagnosed with an abdominal/lumbar strain. EE has been assigned restrictions and prescription medications.

Employee was changing a sight glass on a drive. When on the east side of the platform on the VSK bearing pedestal, EE hooked their left foot on an RTD Cable going to the outboard bearing, catching the wire with a foot and falling forward, landing on the left knee. Employee was prescribed 3 weeks of physical therapy by Ortho Surgeon on 10/2/2018.

Employee stated that as ee was installing roof bolts, ee felt a pain in elbow.

EE was feeding the system when EE was signaled by co-worker to come upstairs. EE stopped the skid-steer, set the parking brake and exited the equipment. The bucket was full of material. When EE stepped down from the skid-steer, right foot came down on a piece of material. EE came down on it in such a way that EE's weight shifted and EE rolled ankle.

Employee was climbing inside of the crusher and hit right knee on the grizzly finger. Employee requested medical attention in May when employee realized the injury wasn't healing. Employee received surgery on August 30 and is on restricted duty.

A Utility Swingman was using a sledgehammer to break up the syn-gyp that had frozen along the transition area of the syn-gyp belts. On one of the swings, the employee hit right hand thumb against a part of the transition.

The employee placed small metal cylinder onto a vice grip and was using a handheld grinder to cut a notch into the cylinder. The grinder became wedged in the cut of the cylinder, jumped out, and cut the employee on the left hand. The employee bandaged the wound themselves and called the Safety Department. The employee has received six stitches.

EE was operating the hydropulper and obtained a laceration on left middle finger. A 55 gallon drum fell over while being rolled on a hydraulic lift. EE used a shovel to catch the empty drum and it fell again. After a couple attempts at grabbing the drum with the shovel, ee instinctively reached for the drum and cut finger on the sharp edge of the drum.

An employee was walking a 5-ft ledge of loose gravel to check the delays on a shot line when the employee lost footing. The employee fell against right forearm, which jarred shoulder and back. The employee requested medical care and cannot complete regular duties. The employee will be reviewing MRI results with a doctor shortly.

On July 19, an employee was walking down a belt ramp when EE stubbed boot toe on the metal grating and twisted right foot/knee outwards. The employee did not require medical care. On July 31, the employee stated knee had not reduced swelling and requested medical attention. An MRI has been scheduled. The employee cannot climb and squat.

On June 27, the employee stated right eye became irritated, the employee flushed it out, & continued on the task. Did not report the incident to supervisors. On June 28, the employee's eye became inflamed & irritated. The employee reported for work on June 29 & requested medical attention. The employee was diagnosed with a chemically burned eye & prescribed eye drops.

The employee noted seeing a 'fire ball' come up from the bottom of the system. The employee jumped from the forklift and attempted to fight the fire with a hand held fire extinguisher and eventually called crew for assistance.

Two employees moved the wagon through the narrow pathway with a 500 lb tail pulley. The bottom of the wagon struck a bracket protruding from the floor. While pushing the wagon to free it, the tail-pulley rolled off wagon, Employee 1 fell backwards into the roll press door, and pinned Employee 1's left ankle between the pulley and the removed roll press door.

The employee was grinding on cracks underneath the water truck. The employee was using a 6-ft ladder placed near the back tire to reach the undercarriage of the truck. The employee stepped off the ladder onto the berm area and rolled the left ankle. The employee sought medical attention and was given a rigid boot and restricted duty for one week.

EE was maneuvering a piece of sheet metal to install on the side of a cone when the metal caught on EE's glove and when EE got it loose it scraped across EE's right wrist cutting it. Dr. stated it was a surface laceration only and nothing vital was cut. EE received stitches and was released back to work regular duty.

Employee had cut a 2 15/16 shaft 36 1/2 long and was placing it in the 4th rack approx. 49" from the floor on top of two other pieces of pipe when the employee set the shaft down the two pieces of pipe separated allowing the 36 1/2 long shaft to fall through and strike the employee's left foot. The employee's left foot had an impact tear on the outside bottom of the foot.

Employee picked up high pressure water wand by the nozzle end, and trigger was pulled when it caught on a bracket. Water pump was off, but system was pressurized. Employee sustained HP water injection injury to right ring finger.

Employee was pulling open manually operated doors on freight elevator and sustained a back strain. Injury did not become reportable until placed on restricted duty on 9/11/2016

As per 30 CFR 62.175 (b), the audiogram shows that a miner has incurred a reportable hearing loss as defined in this part.

Employee was walking and slipped in mud. In efforts to stop EE's fall, EE grabbed a nearby handrail, and EE's shoulder twisted during the fall. Injury did not become reportable until 10/19/2016 when surgery was performed to repair partial rotator cuff tear

Employee was walking and was tripped by a guard that was being moved by a coworker. Employee landed with elbow underneath body, resulting in injury to shoulder. Injury did not become reportable until 08/04/2016.

Employee was scrapping belt conveyor frame, was leaning on frame when cutting with Ox/Acetyl torch. Frame shifted, and employee was off balance, but never fell. Back strain from becoming off balance. Did not become reportable until 6/22/2016 when restrictions were written.

Employee was using jackhammer, and chunk hit the bit, pinching the employee's left index finger between the jackhammer and a door frame. Laceration required stitches.

Employee climbed ladder to look inside truck bed, and as EE was climbing down, the treads of the step caught EE's wedding ring, causing a laceration that required stitches.

Employee right pinky finger was caught in pinch point of roller and rail, sustaining avulsion of a portion of the pad of the finger. Medical procedure was required to enclose wound with remaining tissue.

Employee was moving a lid, approximate size of 12" by 18", and felt a pop in shoulder. Injury did not become reportable until 5/03/2016 when employee had surgery to repair a SLAP tear in the shoulder joint.

He was kneeling in the cab of the scaler working, and when he stood up he felt his knee pop followed by intense pain.

The employee was loading 50 pound anfo bags and felt pain in his back.

Cyst on wrist developed from repetitive motion of work. Injury did not become reportable until 12/01/2015 when miner had cyst removed and was placed on restricted duty.

Employee dropped a screw, bent down to pick it up, and experienced increasing pain the next few days. Employee was given work restrictions to limit knee movement. Full diagnosis is pending. Injury did not become reportable until June 24, when restrictions were issued.

Employee was inspecting the new chest style explosives magazine that was just delivered to store Cardox supplies. As the lid was being lowered by hand, employee's left ring finger was caught between the lid and the lock cover. The left ring finger had tissue severed, and ultimately needed surgical amputation of the tip of the finger.

Employee was attempting to clear blockage in sampler used to gather cement samples for quality control. Employee stuck fingers in plunger pipe to dislodge material, and plunder actuated, cutting the employee's finger. Injury required stitches.

Employee was using a breaker bar and a great amount of physical force to remove a stubborn bolt. Employee felt pain and tingling in lower back.

The employee was lifting a piece of belting and felt a pop in his back.

Employee claims to have stepped off ladder of wheel loader and twisted his knee. Injury did not become reportable until 01/15/2015, when surgery was performed to repair damaged meniscus.

Employee was exiting skid steer and twisted ankle as the foot made contact with slippery material that had accumulated on barrel grabber attachment. Employee suffered a sprained right ankle.

The employee was working on the quick release system for the skid steer bucket and dropped the bucket on his foot. On 8-14-14 the employee was prescribed Physical Therapy, making it a reportable incident.

An empty, unsecured 250 gallon tote fell from an elevation of approximately 10' and struck employee on the back.

Employee was inside material chute to replace wear liners. While leaning the new liner against the wall of the chute, the right middle finger was caught between the unfastened liner and the chute wall. Injury required stitches.

Customer truck driver suffered some sort of health related issue while sitting in his truck with the doors closed. Driver was discovered by plant personnel and CPR was performed until professional medical personnel arrived. He passed away at the Hannibal Regional Hospital

Employee was handling the suction hose on a vac truck, when the hose sucked against the wall. This caused the hose to coil with vac pressure. Employee lost grip and hands were pulled up and back toward the top of the forearms by the hose. Employee was diagnosed with a sprain to the right hand/wrist.

Witness describes they (3) were lowering in a man basket when it suddenly and rapidly descended. Upon braking, EE hit his chin on the basket side rail, biting his lip. Crane operator immediately returned personnel to the surface and EE was taken off-site for medical treatment.

Injured employee was in a crouched position, pulling torque wrench toward self. Torque wrench slipped, and employee fell against wall behind him. Incident did not become reportable until surgery was performed on 11/27/2013.

The employee stated he felt numbness in his arms. The employee underwent surgery on his left arm on 7-22-2013 to relieve the symptoms.

The employee was moving a rock by hand and strained his back.

Employee was walking around the machine,slipped and fell.

The Employee was operating a Roof Bolter, stretched reaching backwards and inadvertently stuck his fingers into the A/C vent, contacting the squirrel cage fan.

Employee was working at the face drilling lifter holes, he was clearing the fines from the hole, when a piece of rock fell from the face and struck him.

Injury occurred on 24 April, 2012 when employee hurt back attempting to close tower elevator door. Employee reopened date on 30 August, 2012 . Employee had surgery on 24 March, 2017 and went on "lost time" on 27 March, 2017.
