Operator was getting off a railcar after setting the hand brake and slipped off of the knuckle of the railcar, hitting is thigh area on the way down. Operator requested to see doctor on 11/25 and provided restrictions on 12/2.

Employee was closing bottom hatches on a hopper car. One hatch was hard to close and couldn't get it with the small bar. Employee had to get a bigger to help get it close. Afterwards employee felt muscle/groin pain on left side. This was reported as an ache and pain, employee went to see doctor on 6/7/2024.

While putting a full grease cartridge in automatic grease gun employee felt a pop in their right hand. Employee reported incident to management on 2/1 as an ache and pain. Employee asked to see a doctor on 3/20 and referred to a specialist on 3/26.

While spinning the shaft on the turbulizer during an inspection two of the employee's fingers got caught between a paddle and the side wall.

On 1/19/2023 A Robo 3 operator got a sample out of a 50 lb bag, then pushed the bag on the belt and made sure it was straight. Operator felt a shift between EE's shoulders/upper back in the process of adjusting the bag. Employee originally reported as an ache and pain. On 2/2/2023 the employee informed management that EE wants to see a chiropractor.

While working on hopper and belt B485 the employee's elbow popped while pulling on a wrench. 12/21/2022 employee reported the incident to management as an ache and pain. 1/4/2023 employee went to see the doctor and was provided restrictions.

On 8/29/2022 The employee team lifted the shaft and carried it down the stairs and team lifted the new shaft back up the stairs. EE felt pain in EE's shoulder. On 9/8/2022 the employee informed safety that the pain had not subsided and wanted to see a doctor.

Employee was heading down the stairs in robo 2. The employee overstepped the last stair causing EE's foot to contact the ground with 14" step down. The employee felt a pop in EE's right knee

Employee was in Stone packaging when they felt something go into their eye. EE tried to irrigate eye to no relief and was taken to the doctor where employee was prescribed prescription eye drops.

One employee squatted down to pick up the backstop, which was in a 12"x12" box and weighed close to 100lbs. Upon squatting down the employee had to adjust position due to being uncomfortable in the coveralls. Once the employee adjusted position, EE went to lift the box containing the backstop and felt a small tug in lower back.

Using a 12 foot long ice pick to dislodge material inside hopper to fall. EE got a sharp and a warm sensation in that area, came and left? End of shift sore shoulder but felt ok.

EE was replacing hanger bearings below Stone Packaging on knees for a few hours on 4/8/21 and was sore the next few days. As time went on the knee felt better. On 5/18/21 EE had to replace the springs on the pellet mill. The next week the knee felt worse, EE had to limp and could feel fluid building on knee. On 5/28/21 the employee decided to get fluid drained making it reportable.

Operator was loading trucks steadily throughout the day when EE starting getting unbearable neck/back pain. Suspected cause was twisting and jarring from forklift.

While going down the stairs in Robo 2 the operator lost footing and rolled ankle.

Employee was enlarging a 9/16" hole in a stainless steel transition for a slide gate when the drill bit, bit on the edge of the hole and the drill twisted in the employees hand, twisting EE's wrist. Employee was fine but woke up the next morning with minor swelling.

Two employees thought they had a piece of pipe secured when the pipe started to roll off of the compressor. Once employee reached out to stop the pipe from rolling and did keep it from falling off of the compressor. The next day the employee woke up with a sore shoulder and suspected that EE had aggravated an old injury.

While standing at the counter in the lunch room the employee turned and felt EE's knee pop. The employee called a few hours later to report the pain.

Employee was prying bearing housing off with pry bar on a rotary and was exerting a lot of force when pry bar slipped and they felt their right wrist pop and there was an immediate sharp pain.

When loosening bolt socket came off and thumb hit sidewall of housing between ratchet handle and the side wall. Thumb is sore and swelled some ice and Ibuprofen was used at home. Thumb is sore and has some limited range from stiffness and swelling but has a limited range of motion at this time.

Operator sustained a fracture to right big toe when a Skidsteer Loader Bucket slipped while the employee was in the process of training coworker on the bucket change out procedure. The bucket which dislodged from the hydraulic lift assembly, dropped approximately 6 inches striking the employee across the steel toe of work boot.

A Third Party finished product truck driver for trucking company sustained sunburns to both eyes as a result of locked out workers deflecting sunlight with a hand held mirror into the truck drivers eyes.

Employee reports lung sensitivity to chemical and requested Dr visit. Initially seen and released to full duty 6/15/18. EE sought full work restriction 6/18/18 which was granted by Physicians Assistant for 5 days 6/18 - 6/22. Employee resigned/retired effective 6/20/18.

Employee was assisting two other maintenance employees in guiding the tail end of a bucket elevator conveyor belt through the shop by sliding it while the forklift pulled the opposite end of the belt to position it for a crane lift. When IP bent over to guide the tail end of the belt around a workbench by sliding it on the floor, IP felt a tearing sensation in left groin.

Utility operator strained right on 3/16/2018 using a sledge hammer and placing rail car vibrators. Operator initially denied medical treatment but did seek medical attention on 5/10/18 followed by acupuncture on 5/10/18. Treatment continues without restrictions

Employee slipped on ice exiting a front end loader. The employee reported the event but did not seek medical attention until 11/7/17. The employee has had 2 chiropractic adjustments with no time loss or work restrictions.

Employee was manually handling mill parts when employee strained employee's Left Biceps muscle

Employee strained right biceps muscle lifting a 10 HP Motor on 2/17/16. The employee sought medical attention and received an MRI on 2/22/16 at which time EE was diagnosed with a muscle tear. On 2/29 the employee was seen by an orthopedist who determined that surgery was not required. The employee is restricted from climbing and issued 6 weeks of physical therapy

Employee strained right shoulder while assisting in demolition and reconstruction of a packaging line palletizing unit.

Employee strained intercostal muscle of his left chest area while attempting to lift the cover of a product transfer conveyor

Lifting siding sheet with pulley and rope. Was almost to top. EE had his hand over the edge. His finger got caught between the sheet and a piece of steel.

Employee reports backing into a stationary roof support pole while operating a forklift. The employee reported neck and shoulder pain but denied medical attention at the time of the event. The employee request a medical evaluation on 7/8/14 as the pain persisted. The employee was diagnosed with a right shoulder (rotator cuff) strain and prescribed physical therapy.

Employee slipped on a flat surface, grabbed loader ladder to break fall, strained right shoulder. No medical treatment sought until 4/21/14, Physical Therapy Prescribed following 4/21/14 medical examination.

Employee had a standard threshold shift of 26 in left ear and 41 in right ear detected in yearly hearing tests.

Employee was lifting box of files and twisted as she placed them on a shelf. She had some discomfort, but failed to report the incident until 9/8/2010. She was sent to Three Rivers Clinic for evaluation on 9/8/2010. No medical treatment until second visit to the Clinic on 9/14, the doctor ordered Physical Therapy.

The injured employee was trouble shooting the CB system. He disconnected a stainless steel line to check for flow. As the chemical began to flow, it struck another nearby line and a portion splashed into the employee's left eye. The employee's eye was flushed and then he was transported to Urgent Care. The employee took one dose of pain medication that night.

An AFFCO employee (contractor) was removing materials from the roof of the PT-9 construction area. While entering an elevator with scaffolding legs, a bucket of material, and a power tool; the elevator's exterior door closed on the injury's right hand resulting in a laceration to one finger. He was taken to Three Rivers Clinic for treatment - 8 sutures. 3 stitch

Employee slipped and fell on unseen ice in walkay, sticking back of head on concrete. Employee transported to local clinic, laceration to back of head required four sutures.

Contract truck driver entered property to pick up load. Pulled onto truck scale for light weight. Got out of truck, entered scale shack, light weight taken, exited scale shack to reenter truck. Came around front end of truck and slipped on a patch of ice and rolled left ankle. Got in truck and pulled over to loading door. Reported slip to forklift operator.

EMPLOYEE WAS WORKING INSIDE A SCISSOR LIFT. HE BENT DOWN TO PICK-UP A PIECE OF PIPE ON THE FLOOR OF THE BASKET & EXPERIENCED A PAIN IN HIS LOWER BACK. THE EE SOUGHT TREATMENT WITH HIS CHIROPRACTOR & RETURNED TO WORK FOR HIS NEXT SHIFT.

Employee was attempting to close the sliding rail car door when finger became pinched between door and railcar. Suturable laceration to right ring finger.

Employee was lifting 50lb bags of talc onto a converyor stystem when employee felt pain in groin. On September 7, 2005 employee was evaluated by doctor as having sustained an left abdominal inginial hernia. Employee continued to work until September 20 at which time employee had surgery to repair hernia. On October 3rd the company determined that the incident was work related.

The employee was lifting and moving a wood pallet, which had a 50lb blower on it, when it slipped. The right middle finger of employee was caught in between two wood pallets. The employee sustained a (R) long finger crush injury with suturable lacerationand open distal phalanz fracture.

POTENTIAL WORK RELATED HEARING LOSS GREATER THAN 25DB FROM BASELINE CONDUCTED IN 1997. RIGHT EAR 4000 HZ-25DB SHIFT. LEFT EAR 3000 HZ-47DB; 4000 HZ-25DB SHIFT. EE HAS WORKED AS A MILL HAND, WORKING AROUND MILLING EQUIPMENT. HEARING PROTECTION PROVIDED. EE IS ENROLLED IN OUR HEARING CONSERVATION PROGRAM.

While ducking under a conveyor employee bumped his head on conveyor frame. The employee continued to work. On January 25, 2005 x-rays taken and negative. On February 22, 2005 employee was seen by a neurologist with a MRI completed, which was negative. On June 8th employee started to receive Myofascial - Physical Therapy employee was returned to work at full capacity.

WHILE EMPLOYEE WAS WELDING, HELMUT SHIFTED CAUSING A FLASH BURN TO HIS RIGHT EYE. (PRESCRIPTION WAS GIVEN AND FILLED.)

WHILE WORKING ON A CRUSHER REPAIR, THE EE BUMPEDHIS HEAD ON AN OVERHEAD DUST COLLECTION PIPE. EESUSTAINED A STRAINED NECK.EE REPORTED INCIDENT ON 1/22/04 WORKED ENTIRE SHIFT. NEXT SHIFT STATED HIS NECK WAS SORE--TREATMENT AT CLINIC 1/23/04 RET'D TO WORK ON 1/23/04 NO RESTRICTIONS

EE LACERATED HIS LEFT MIDDLE FINGER WITH A SAFETY UTILITY KNIFE WHILE ATTEMPTING TO STRIP THE INSULATION FROM A SERIES OF RIGID 500 MCM WIRES DURING THE REPLACEMENT OF A MAIN DRIVE MOTOR FOR A N AIR CLASSIFYING MILL. THE LACERATIONS REQUIRED 6 SUTURES. THE EE RETURNED TO WORK SAME SHIFT WITH NO RESTRICTIONS.

POTENTIAL WORK RELATED HEARING LOSS GREATER THEN 25 DB FROM BASELINE CONDUCTED IN 1987. LEFT EAR 2000 HZ, 46 DB SHIFT, 3000 HZ, 57 DB SHIFT. EMPLOYEE HAS WORKED AS A MECHAINIC AND WELDER FOR O VER 28 YEARS. EXPOSED TO NOISE WHILE WORKING AROUND MILLS AND WELDERS OVER THE YEARS.

AN AGE-ADJUSTED AUDIOMETRIC LOSS COMPARING BASELINE (1987) AND MOST RECENT MEASUREMENTS (2003) HAS BEEN ESTABLISHED, RESULTING IN A HEARING LOSS IN THE EMPLOYEE'S RIGHT EAR.

POTENTIAL CARPAL TUNNEL SYNDROME RIGHT WRIST. ON 1-17-02 THE COMPANY ACCEPTED THE EE'S CLAIM OF CARPAL TUNNEL SYNDROME IN RIGHT WRIST. ALTHOUGH EE FIRS REPORTED PAIN IN RIGHT WRIST WHILE OPERA TING A FORKLIFT ON 12-10-02 AND SOUGHT MEDICAL ATTENTION AND REMOVED FROM SERVICE. IT WAS NOT DETERMINED UNTIL 01-17-02 FROM AN INDEPENDENT MEDICAL REVIEW TO BEW WORK RELATED.

EMPLOYEE WAS STANDING ON BOTTOM SHELF OF STORAGE RACK, SECURING THE BOARDS ON THE MIDDLE SHELF. THE SHELF SUPPORT BOARDS THE EE WAS STANDING ON WERE NOT SECURED TO FRAME AND PULLED AWAY FROM FRAME CAUSING THE EE TO FALL APPROX FOUR FT TO GROUND LEVEL. DURING THE FALL THE EE'S RIBS STRUCK A METAL GEAR, THUS CAUSING A FRACTURE TO RIGHT RIBS.

WHILE CUTTING WITH A TORCH UNDER #1 DRYER, A HOT NUT LANDED ON EE'S LEG. THE HOT NUT BURNED THROUGH EE S COVERALLS, PANTS, AND BURNED LEG/KNEE. THE EE WAS WEARING KNEE PADS. THE HOT NUT BECOME CAUGHT BETWEEN KNEE PAD AND LEY/KNEE. THIS RESULTED IN THREE. 3RD DEGREE BURNS.

WHILE USING AN AIR HAMMER TO REMOVE HARDENED TALE FROM A COATING TURBOLIZER, A PIECE OF TALE FLEW OFF & UNDER FACE SHIELD STRIKING EE'S LEFT EYE.

WHILE WALKING A PORTABLE OXYGEN-ACETYLENE TORCH DOWN FOUR STEPS, EMPLOYEE THOUGHT HE HAD PLACED THE TORCH SET ON THE WALKWAY BELOW STEPS, WHEN IN FACT, THE EMPLOYEE PLACED TORCH SET ON THE BOT TOM STEP. AS THE EMPLOYEE REMOVED HANDS FROM TORCH SET, THE TORCH SET FELL. AS TI FELL THE HANDLE BECAME CAUGHT IN THE EE'S WELDING JACKET, CAUSING HIS HEAD TO BE PULLED FORWARD ONTO TORCH SE

EE WAS CARRYING A WATER COOLER IN HANDS WHEN HI S LEFT FOOT CAUGHT THE EDGE OF CONCRETE BARRIER. AS A RESULT THE EE STUMBLED FORWARD AND FALL TO GROUND. METAL EDGE OF WATER COOLER CUT PALM OF RIGHT HAND WHEN HAND AND COOLER MADE CONTACT WITH GROUND DURING FALL.

EE SUSTAINED AN ABRASION TO HIS RIGHT EYE WHEN APIECE OF SLAG POPPED WHILE STAINLESS STEEL WELD WAS COOLING EE WAS WEARING SAFETY GLASSES AT THETIME OF THE INCIDENT
