While an employee was working on changing a tire on a haul truck the employee suddenly lost control and feeling in their right arm. Also felt fatigued and nauseas.

Employee was in Haul Truck #28 being loaded by 559 Shovel on the Phase 6 6000 Bench. While being loaded a large boulder rolled off the bucket of 559 shovel, the boulder landed in the bed of the truck. The impact jarred the truck operator.

The employee was entering the plant heading to the plant break room. When the employee stepped on the top stair to descend to the break room, EE slipped causing EE to fall awkwardly down the stairs. Employee is experiencing shoulder pain post fall.
Audiometric testing performed on 8/23/2023 showed that the employee had hearing loss. Audiometric retesting on 9/18 confirmed that the employee had hearing loss.

The employee was working on a light vehicle and was standing on the front bumper/cattle guard. When they went to dismount the vehicle, they slipped causing them the strike their ribs/upper torso against the cattle guard.

Employee was operating haul truck near the new pond when a light vehicle pulled out in front of EE causing EE to lock up the brakes resulting in aggravation of the lower back.
The operator was pushing material and constructing a ditch with a dozer in order to convey water away from the dig face. At the end of shift the operator reported to the pit lead that neck was sore when they tried turning head. Employee could not pinpoint a single event or time that caused the soreness.

Lab Technician slipped and fell while walking up to/reaching to open the door of the Ambulance Barn. Diagnosed with a fracture to the left wrist.
Employee had descended into a trench (approximately 3 feet deep X 6 feet wide) at the water pond area of the mine and was climbing up the other side when they felt a pop in their right calf muscle. The employee was later diagnosed with a partial tear to the calf muscle.

Employee alleges that employee strained left shoulder on 3/21/18 barring wet material from feed system. Incident was not reported to the company until 04/09/18. Employee provided doctor's note on 4/09/18 stating need for light duty.

Employee was performing weed spraying on a slight grade, felt minor stinging sensation in left knee. Later in the shift while entering a pick-up truck the employee reports a pop in the back of his left knee. The employee worked until he was seen by a physician on 7/26 and diagnosed with a torn ACL. Employee was issued work restrictions and will likely require surgery.

Walking from loader to travel on concrete, which was wet from the snow, making the film of tail slick. He slipped and fell hurting his left wrist. X-ray's were taken the day of the injury showing a fracture. He will be seeing another Dr for a 2nd opinion and treatment.

The employee was closing the tailgate of his pickup truck and caught the little finger of his left hand between the tailgate and truck bed. The employee was transported to the Ennis Clinic for treatment. The employee was diagnosed with a lateral fracture and receive 4 stitches to close the wound. Antibiotics were prescribed and taken for infection control.

An age adjusted audiometric loss comparing baseline (1987) and most recent measurements (2005) has been established, resulting in an average loss of 29 dB in left ear at the 2000Hz range. This illness was confirmed on 4/11/2006 by 3rd party administrator.
While loading drill blast holes with anful (ammonia nitrate and diesel fuel) a wind gust blew dust into employee's right eye. The employee sustained a chemical burn from ammonia nitrate dust on cornia of eye.

Haul Truck driver was proceeding to dump site, just after having a break. Truck immediately turned almost 90% from the straight haul path, He hit the berm squarely with both tires and slid down the hill approx. 35 feet. He stated that he saw the truck was turning and heading for the berm, but he could not move his legs or arms to stop the truck.
The employee reported a problem with "trigger thumb". Employee is a QA Lab Technician that works with hands for multiple years. Dr. determined employee sustained work related "trigger thumb" to left thumb.

EE WAS EXITING EE TRANSPORT VAN AT START OF SHIFT, PUT LEFT FOOT ON GROUND. FOOT SLIPPED ON ICE CAUSING EE S LEFT ANKLE TO TWIST AS EE FALL TO GROUND. LEFT ANKLE FRACTURE.

HOWEVER, THE TREATMENT WAS FOR ARTRITIS AND POSSIBLE CTS.

SHE LEFT HER STATION AND WENT AROUND THE BIN TO RETRIEVE THE TALC, STUMBLED OVER THE WOOD BLOCKS STAC K ONE UPON THE OTHER, FELL AND HIT HER SHOULDER ON THE SECOND SORTING BIN. THE BLOCKS ARE USED AS FOOT RESTS FOR THE SORTERS DURING THEIR SHIFTS.
