Employee stated as he turned to sit down in his CAT excavator he felt a pain in his lower back - right side

Employee was hit in the right ear by an air gun that flew out of a co-workers grasp causing laceration behind right ear requiring 5 stitches.

Employee was checking for a leak from a drill rig hydraulic hose. As he moved hoses around, pressurized fluid shot out & caused a puncture wound to his left middle finger tip.

Employee was climbing out of a chute under #4 shaker when he felt a "Pop" in his right knee. Diagnosis: Right Knee Strain - possible Medial Meniscal Tear.

Employee was carrying a 4' x 8' shaker screen with a co-worker when he felt pain in his lower back.

While changing screens, employee reached under screen to retrieve a bolt while screen was being lowered. Screen edge hit employee on shoulder& back of neck causing bruise / contusion.

Employee was loosening nuts with a rachet & socket when he received a lumbar strain.

As employee was stepping off a haul truck ladder, his left foot landed on a piece of core stone causing an ankle sprain.

While replacing a haul truck suspension cylinder the employee got his left index finger piched between the cylinder & the truck when the cylinder shifted causing a fracture to finger.

Employee was walking from the parking lot to the time clock when he slipped and fell on ice causing a left shoulder sprain.

Employee got his fingers caught between a steel cable and a pulley snatch block causing fracture of left ring & middle finger.

Employee ducking under an over-head beam. He thought he was clear and he started to stand up straight. He hit the back of his neck on the beam causing a contusion to his cervical spine.

Employee was shoveling under shaker cleaning up spilled stone when wind blew dust/dirt into his left eye causing a corneal abrasion.

EMPLOYEE WAS USING A TORCH TO LOOSEN A GREASE FITTING THAT HAD SEIZED UP. THE FITTING EXPLODED, SHOOTING OUT A BALL BEARING THAT BECAME IMBEDDED IN EMPLOPYEE'S LEFT FOREARM.

Employee was pulling on a bungee cord to connect it between a back hoe door and a handrail when he felt a pull in his back.

Employee received 6 stitches & a tetanus shot when he caught his finger between a screen deck hole and a plant rail. he was lowering the deck to another employee when it slipped.

EE WAS USING A WIRE WHEEL GRINDER TO CLEAN THREADS ON A CRUSHER FRAME. THE GRINDER JUMPED BACK AND THE WIRE WHEEL BRUSHED EE'S FACE, CUASING A LACERATION TO HIS LEFT CHEEK.

EE STATED THAT HE HIT A POT HOLE WHILE DRIVING A HAUL TRUCK AND EXPERIENCED PAIN IN HIS LOWER BACK AND RIGHT LEG. DIAGNOSIS: DEGENERATED DISC.

EMPLOYEE WAS USING A UTILITY KNIFE TO CUT A PIECE OF RUBBER CONVEYOR BWLT WHEN HE CUT HIS LEFT INDEX FINGER TOP - 8 STITCHES.

EE WAS WALKING DOWN STEPS WHEN HE SLIPPED ON MUD AND FELL CAUSING LOWER BACK STRAIN.

EE WAS SITTING AND WELDING A CHUTE LINER. WHEN HE TURNED TO WELD THE OTHER SIDE, HE FELT PAIN IN HIS BACK.

EE WAS POKING CLOGGED MATERIAL WITH A METAL BAR THAT WAS CAKED IN A CHUTE. THE BAR SLIPPED, SMASHING HIS FINGER BETWEEN THE BAR AND A HAND RAIL.

WHILE ASSISTING A WELDER CUT A PIECE OF STEEL WEAR LINER, THE STACK OF STEEL SLID DOWN STRIKING THE EMPLOYEE'S LEGS.

WHILE LIFTING A DOOR TO CLOSE OFF THE CORE STONEA HOOK SLIPPED CAUSING EMPLOYEE TO BE PULLED INTO A HAND RAIL.

EMPLOYEE WAS REMOVING SHAKER SCREEN WHICH WAS HOOKED TO A CRANE. THE SCREEN SHIFTED AND PINCH-ED EMPLOYEE'S FINGER BETWEEN SCREEN AND RAIL.

EMPLOYEE WAS WEED WACKING WHEN HE HIT A YELLOW JACKET NEST RESULTING IN STINGS.

EE WAS LYING ON HIS BACK USING A HAND GRINDER ONA STEEL TRACTOR BOLT WHEN A PIECE OF STEEL WENT UNDER HIS SAFETY GLASSES AND ENTERED HIS RIGHT EYE.

WHILE CLIMBING DOWN STEPS ON A 992 LOADER, EE SLIPPED AND CUT HIS SHIN ON FLOOR BOLT.

EMPLOYEE BENT OVER TO PICK UP TWO 5 GALLON OIL BUCKETS WHEN HE FELT A SHARP PAIN IN BACK.

EMPLOYEE STEPPED OFF A LADDER AND HIS FOOT TWISTED.
