EE was installing a bucket linkage arm on a loader. The pin turned during this and caused EE's arm to slide off and hit their left-hand index finger which caused a fracture.

Working on a WA500, changing cutting edge that was elevated around 24" off ground. EE was standing in bucket and stood up to move and felt strain in lower back.

EE was removing steel out of a hull truck body when EE felt a strain in lower abdomen.

Employee was welding underneath truck with welding helmet and safety glasses on when buildup dirt fell from the bottom of the frame and dropped down onto the shield and scratched their left cornea.

Employee was moving a steel plate with another worker when left ring finger got pinched, fracturing the finger.

Employee was holding the head of a bolt while turning the nut with an impact gun causing the bolt head to lacerate his left palm, requiring 40 sutures.

Employee completed scale check and was putting tools back into truck. Employee closed tailgate and caught co-worker's finger between gate and truck.

Miner was grinding steel and F.B. entered Lt eye under safety glasses-required follow up treatment after continued irritation.

Reaching between the seat for a dash board knob. Right index finger slid along sheet metal.

EE was removing bolts from cutting edge of cat loader horn bolt head spun while removing and cut through 2 pairs of gloves cutting left index finger.

Employee was standing just below screen deck. A rock roller off of deck and struck employee over left eye. Laceration over left eyebrow.

Emp. using a pipe wrench to open a coupling. Wrench slipped of nut and hand caught between pipe stem & wrench.

Employee picked up box of boosters and felt pull in back, it hurt and became tight.

Employee was removing & replacing cover guard (wt. approx 5 lbs) on hydraulic control @ the primary breaker - twisted & aggravated pre-existing injury to lower back.

EE was installing "teeth" on loader bucket using sledge hammer - piece of steel broke off and struck left forearm.

Caught left forefinger between pin & loader bucket while installing.

EE WAS LIFTING BAGS OF SALT INTO SPREADER & FELTPAIN IN BACK

EMP WAS REMOVING WORN BOLTS FROM CUTTING EDGE OF FRONT END LOADER. BOLT SLIPPED OFF HAMMER HEAD BEING USED TO APPLY PRESSURE & EDGE OF BOLT HEAD LACERATED FINGER.

EE WAS STANDING ON BUCKET ARM OF LOADER TIGHTNING NUT. HIS FOOT SLIPPED ON GREASE AND HE FELL AGAINST BUCKET BRUING BACK AND RIBS.
