No accident at work occurred ee went to Dr. w/back pain and said it may be from lifting on tables and chairs at an off hours/off premises Company function which ee voluntarily attended. Dr. claims injury could be from repetitive motion of ee's job. Not from a specific incident. Company disputes this claim and will seek further evidence of work related cause.

Employee was getting into his work truck, went to sit down and felt alot of pain in his back.

On 10/24/12 EE strained his rgt. side back and hip area while installing a water line. EE declined medical attention at that time. EE reports that on 11/28/12 he further aggravated the injury while performing the same task. He worked through the day. Worked 11/29/12 then sought medical treatment that evening in the emergency room and was placed out of work.

Employee was kneeling on the grip strut catwalk, on a glove, while installing a return idler guard on under side of conveyor.

Employee states his finger was pinched between a large wrench and the exposed threads of the wedge bolt. The large wrench was being struck with a sledgehammer to loosen the nut when his finger slipped into the pinch zone.
