Employee was inside the cowl of the Poclain, bleeding the hydraulic pump. He leaned over the PTO and his clothing was caught, immediately pulling employee in.
Employee was piling stone. He lifted heavy stone, and turned his body to pile on pallet. When he did so, he felt a pulling in his back
Employee was moving large stone from path of clamp truck when he felt pull and pain in lower right quadrant. He sat down for a few minutes because of the seriousness of the pain. Because the pain did not subside, he notified the quarry manager that he was seeking medical attention.
Employee was on first day of training. Sledge hammer slipped off chisel and smashed finger against rock.
Employee was turning stone on edge to split when foot slipped in wet depression causing employee to twist. He felt a pop and pain in his lower back.
Employee was moving large stone to split when the stone slipped, catching his ring finger on his left hand, between 2 stones.
Employee was lifting a large stone when he felt and heard a popping in his lower back.
Employee was splitting stone when another employee lost control of the stone he was splitting. The stone rolled down hill landing on first employee's foot cause the breaking of 3 toes.
Employee was turning a garden path stone to put on pallet when a fork lift behind him made a loud noise, startling the employee. He turned and when he did so, stone fell on his arm. He did not seek medical attention at that time. On June 13, 2006, employee was carrying a stone to throw on pallet when he felt his arm pop, causing him to fall to ground.
3 EES WERE REPAIRING EUCLID TIRE WHEN THE LOCK RING GAVE OUT CAUSING THE TIRE TO JUMPE OFF THE GROUND CAUSING INJURY TO ALL 3 EES.

EE WAS LIFTING STONE ONTO PALLET. FELT STABBING PAIN IN LOWER BACK.
EMPLOYEE WAS PICKING UP STONES TO PILE ON PALLET. WHEN HE BENT TO PICK UP STONE AND STARTED TO LIFT IT, HE FELT A POPPING AND PAIN IN LOWER BACK.
EMPLOYEE WAS CARRYING LARGE STONE. CORNER OF STONE BROKE AWAY. EMPLOYEE LOST HOLD & STONE CAME DOWN OF FOOT.
EE WAS SPLITTING STONE & BLACKED OUT. FELL & HIT HIS HEAD.
