Employee was moving large stone from path of clamp truck when he felt pull and pain in lower right quadrant. He sat down for a few minutes because of the seriousness of the pain. Because the pain did not subside, he notified the quarry manager that he was seeking medical attention.

Employee was splitting stone when another employee lost control of the stone he was splitting. The stone rolled down hill landing on first employee's foot cause the breaking of 3 toes.
Employee was turning a garden path stone to put on pallet when a fork lift behind him made a loud noise, startling the employee. He turned and when he did so, stone fell on his arm. He did not seek medical attention at that time. On June 13, 2006, employee was carrying a stone to throw on pallet when he felt his arm pop, causing him to fall to ground.

EMPLOYEE WAS STACKING SLAB STONE WITH 2 OTHER EMPLOYEES. WHILE SLIDING STONE ONTO OTHER STONE ALREADY ON PALLET, STONE SLIPPED PINCHING FINGER.
EE WAS SUPPORTING LARGE STONE WHILE SECOND EE REACHED FOR STONE WEDGE. EE STEPPED BACK TO BRACE HIMSELF SLIPPED AND WAS OVERPOWERED BY THE WEIGHT OF THE STONE. HE FELL TO THE GROUND WITH STONE LANDING ON LEG.
