Drowning Creek Dredge & Plant Metal/Non-Metal

A MECHANIC WAS INFLATING A TRUCK TIRE WITH A STANDOFF DEVICE INSIDE THE MAINTENANCE SHOP. THE SIDEWALL OF THE TIRE BLEW OUT AT 60-70 PSI, KNOWN AS A ZIPPER FAILURE. DEBRIS STRUCK THE MINER IN THE FACE. WE TRANSPORTED EE TO THE ER WHERE EE WAS RELEASED AND ASKED TO RETURN THE NEXT DAY. EE RECEIVED SURGERY ON 11FEB2022.

EE was getting out of truck and there was some ice on the step of the truck and he stepped down on step, foot slipped, EE fell on left knee. His left knee was bruised.

EE was changing screen on motor on classifer, when coming down steps, foot slipped because he had mud on shoes. EE slid down 5 steps and right arm got caught or hung on stair rail, causing him to pull his right shoulder. He was also holding his tool box in right arm at time of fall. He however, landed on his feet.

EE was loading sand on out dump truck , when he was getting out of the truck, he jumped from the drivers compartment to the ground, spraining his lower back.

The lifting lug was only welded in one spot of the 6' X 12' X 1/4" plate steel. This caused the plate steel to be too weak and unbalanced, therefore, it fell on EE when the forklift tried to move it. EE should have welded both sides of the plate steel. The steel plate weighed approx 500-700 lbs, causing EE lower back trauma.

He was hauling mud to reclamation site, ran over nerm and off into ditch full of water and mud. The tractor part stuck in mud, the trailer was standing upright and rolled over in mud also.

EMPLOYEE WAS SHOVELING ROCK BACK INTO GRAVEL WASHER, EE WAS BENDING OVER AND STRAINED LOWER MIDDLE BACK.

EE WAS WORKING ON DUMP TRUCK OUTSIDE & SOME SAND FLEW INTO HIS LEFT EYE BECAUSE OF STRONG, HEAVY WINDS. HIS EYE BECAME INFECTED AFTER GOING TO URGENT CARE ON 2/6/02. ON 2/14/02 HE WENT TO CARO LINA EYE FOR MINOR SURGERY ON HIS UPPER & LOWER LID ON THE LEFT EYE.

EE PULLED THREE DRAWERS OF TOOL BOX OPENED AT SAME TIME CAUSING AN OUT OF BALANCE SITUATION. TOOL BOX WEIGHS 1000 LB.
