A miner was injured when EE's right hand was struck by a piece of metal causing a small laceration and became embedded, requiring medical removal.

A miner was pulling a belt onto a conveyor frame using a Cat 226 Skid steer when the mechanical splice failed, causing the rope and attached metal splice to snap back and strike the miner in the leg, causing a laceration.

A miner was using a pneumatic die-grinder to prepare the steel surface on a dump truck for welding when the miner felt something go into eye. The miner flushed eyes several times & finished shift. The next day the miner experienced discomfort in the eye & sought medical attention. A foreign object was removed from eye & miner was released to work with no restrictions.

A miner was using a pry bar when the miner felt a sudden and sharp pain in lower back and hip area

Miner was positioning a 55-gallon drum of Hydraulic Oil when EE's hand slipped off and EE caught it on the lip of the drum, causing EE's left thumb to bend back and resulting in a sprain.

Employee was utilizing pliers to remove shims from shim-kit when the pliers slipped and the shim made contact with the employees right index finger, resulting in a laceration.

The maintenance crew was moving and placing a piece of HDPE pipe. In the process of setting the pipe, which had apparently caught on something, the pipe released, which catapulted the pipe into the employee, striking employee in the chest and pinning employee against a conveyor frame. Employee was diagnosed with fractured left clavicle/shoulder.

Miner was standing outside of the heavy equipment shop going through task training on a front end loader and was drinking a soda. A bee stung the employee on the lower lip. The miner had an adverse reaction to the bee sting and was transported to a local clinic where employee was treated for an allergic reaction to the sting.

Employee pulled a transmission which had been lowered and was resting on a jack with casters out from underneath an on-highway dump truck. Employee woke the next day stating EE was sore and sneezed at which time EE felt a "pop" in the left side of EE's abdomen. Employee sought medical attention and has a strain / possible minor rib fracture.

Employee was grinding on a conveyor belt splice. Employee finished EE's shift, and the next day reported pain in EE's right eye. Subsequently the employee was taken to the doctor, and a piece of metal shaving was removed from EE's right eye. Employee was wearing safety glasses, but was not wearing a face shield.

Employee was striking a guard with a sledge hammer and felt pain in his lower abdomen. Employee was diagnosed with a permumbical hernia. No medical treatment was given, and employee was released to full duty with the doctor noting the hernia would not need surgery.

Employee was repairing belt on LC1, employee cutting belt with box cutter and his finishing cut came though the belt and struck employee left leg above the knee . Employee received stiches

Employee working in load out control room watching sand conveyor. Opened window and sand blew into his left eye.

Employee was being shown area he was to be working in for the repairs of crushing plant, employee slipped on wet slab with crusher dust on it. Employee suffered a sprain to the left knee

Employee was throwing away trash from service truck. Employee stepped up onto ladder of dumpster and put right hand behind top rung of ladder. Employee threw away trash and stepped down with hand still behind top rung. Employee felt a pop in hand. Upon medical examination employee fractured ulna styloid and metacarpal on right hand.

Employee has noise-induced hearing loss in his right ear.

Employee helping with screen change on wash tower, when placing new screen in position, employee did not move his finger and it was caught between the screen and side rail.

EE was dismounting a CAT D9 Dozer after fueling and felt a burning in right shoulder.

EE was installing a gear on a motor shaft for a crusher, while tapping the gear with a hammer to align gear with keyway on shaft, a 3 millimeter fragment of the gear chipped off and hit EE in neck. EE received puncture wound to neck.

EE reported 10/31/13 that a personal injury was a work related injury. EE believed while helping lift 4X8 wire screen on LS1 screen that he strained his back/neck. The EE cannot attribute any instance during this task which caused the injury. EE stated he was sore the next morning, however he did not attribute his soreness to his current injury which has worsened.

The injury to the employee is a muscle strain in the right arm. The exact cause of the strain is unknown. The employee believes that it may be from operating the steering wheel on the Cat D-400 haul truck. The employee has been diagnosed with a muscle strain.

The incident took place in the pit at the dragline. Employees were upending drag cable. employee was setting socket by pulling cable through the beckets. While he was pulling he felt a pop in his right arm. Employee suffered torn right bicep.

Employee was pulling a 2" water hose up onto a screen deck approximately 17 feet off the ground utilizing a tag line. As the employee was pulling the last 10 feet of hose up ee felt pain in his lower back which was diagnosed as a lumbar strain.

Stringing a conveyor belt onto a conveyor for vulcanization. Reached end of belt and while attempting to remove belting from the wood core the belt recoiled which caused a nail that was hammered through the belting to cut employee's forearm. Affected area was right forearm below the elbow 5" cut.

It is not entirely clear how employee hurt his left knee. Several hours after working underneath the dragline, employee says his knee began to hurt and didn't think much about it. Two days later the employee reported to his supervisor that his knee was swollen and sore and wanted to seek medical attention.

EE was placing wear liner on conveyor belt and pinched the tip of his Rt. hand ring finger between the wear liner and troughing roller, which caused a small laceration and fracture on the tip of EE's finger.

EE pinched his rt. middle finger in between the top of a center pull bearing and the top of its slide frame while installing new bearing. Appropriate PPE was being worn including gloves. No safety rules were violated, modified duty was in place for two days (no forceful repetitive gripping, maintain bandages). Slight fracture to finger tip, and subungual hematoma.

The mobile crane had set the empty spreader bar on the trailer-on its edge. The crane operator was then given the sign to lower the hook, in order to disconnect the top rigging. As the hook was being lowered the spreader fell over and hit the right ankle - soft tissue injury (Note: incident occurred during the de-mobilization of the crane)

While unrolling a discharge hose, a retainer wire struck the worker's finger causing a laceration and bleeding.

Delivery dump driver was in the process of unhooking and dropping off the rear trailer near the office and in the process of lowering the jack on the trailer's tongue EE strained his lower back.

Turned on blue pump about 1/2 throttle, then turned on water valve about 3/4 pressure. Hosed for about 10 minutes when nozzle separated from hose; the band on the hose slid down employee's hand cutting left hand between thumb and index finger.

Contusion to left knee climbing down into the engine well. Foot slipped and hit knee on metal part in well. Fell about 1 foot.

Standing on loose material so lost footing when pressure from water hose caused loss of balance. Dropped hose on foot hitting above steel toe of rubber boot.

Working alone.

Employee was installing screens in a confined area and struck his left arm with a 3# hammer.

Employee was trying to bend a 3/8" piece of plate that had been heated in the middle by jumping up and down on it. The next morning the employee had some pain in his leg/knee but thought it would go away.

EE was replacing a return roller. When he lifted the roller he hit his head on the conveyor frame. He was not aware he was so close to the conveyor frame.

While repairing a guard on a conveyor, the socket slipped off the bolt and the ratchet struck employee's upper lip pushing tooth back.

14' piece angle iron - fabricating steel structure overhead. Poor body position, over exertion - strain low back.

INSTALLING TAKE UP FOR NEW CONVEYOR. TAKE UP WAS TOP HEAVY & TIPPED OVER ON LEFT HAND CRUSHING OR PINCHING LEFT INDEX FINGER, LACERATING MIDDLE MEATY PORTION, 6 SUTURES.

EMPLOYEE WORKING ON T-ROLLS IN TUNNELL, IN A HURRY, TRIPPED IN TUNNEL FALLING INTO A PIECE OF STEEL, LAC RT LITTLE FINGER KNUCKEL.

INSTALLING NEW SCREENS ON S2 - CUTTING TABS OFF SCREEN TO INSTALL - KNIFE SLIPPED CUTTING LEG JUST ABOVE KNEE CAP.

TAKING OUT AUGER SHAFT-USING CRANE & CHAIN RIGGING. RIGGED CHAIN AROUND SHAFT ON 1 END, OTHER END HAD CHAIN AROUND AUGER FLIGHT. EE HAD HIS HAND WRAPPED AROUND CHAIN, CHAIN SLIPPED OFF FLIGHT PINNING LT HAND AGAINST AUGER HOUSING & CHAIN. THE CRANE HAD TENSION ON SO WHEN IT SLIPPED IT PINNED EE'S 2 FINGERS. THEN CRANE PULLED UP ON CHAIN LACERATING FINGERS.

EE WAS HAMMERING FLOXCOS OUT OF A BELT WHEN PIECES OF HAMMER IMPEDED INTO THE TOP OF MY LEFT HAND.

2 EMPLOYEES WERE MOVING A MOTOR ASSISTING WITH PRY BAR. PRY BAR SLIPPED HITTING EMPLOYEE ON CHIN, BREAKING TOOTH AT GUM LINE.

UNEVEN GROUND, LIGHTING, HURRYING TO SHUT CONVEYORS OFF.40 STITCHES UNDERSIDE. 20 STAPLES TOP SIDE OF SCALP.

AN EE WAS DRIVING A 988B LOADER ON UNEVEN GROUND WITH BUCKET RAISED CAUSING LOADER TO LAY OVER ON LEFT SIDE (ACCIDENT ONLY-NO INJURY).

EE WAS JUMP STARTING A LOADER BATTERY. WHILE DOING THIS THE BATTERY EXPLODED AND ACID SPRAYED EE'S FACE. BATTERY CAP HIT EE'S FACE.

EE WAS CLEARING/HOSING TAIL PULLEY IN LOADOUT TUNNEL C17 WHEN A WELD GAVE WAY, AT THE HOLD CAME AROUND & HIT EE IN THE FACE

EMPLOYEE WAS ATTEMPTING TO LOOSEN BOLTS ON A BEARING SEAL PLATE WHEN THE BOLT BROKE LOOSE, SMASHING FINGER AGAINST STEEL, CUTTING FINGER BONES.

EE WAS IN THE PROCESS OF REPAIRING A SCREEN ON CRUSHER. HE WENT DOWN A SET OF STAIRS TO CHECK THE OIL AND AS HE PROCEEDED BACK UP THE STAIRS HE SLIPPED AND FELL INJURING HIS KNEE/ANKLE. EE STA TED THAT HE HAD EXCESS GREASE ON HIS SHOES WHICHCONTRIBUTED.

EE WAS ATTEMPTING TO BLEED THE OMNI CRUSHER WHEN HE OPENED TH PLUG THE CRUSHER STILL HAD PRESSURE AND BLEW HYDRAULIC FLUID INTO HIS FACE AFFECTING HIS EYES.

EE WAS ATTEMPTING TO LIFT AND REMOVE CHUNKS OF SCRAP METAL STEEL FOR PURPOSE OF "CLEAN UP" WHEN HE INJURED HIS BACK.

WHILE EMPLOYEE WAS REMOVING THE TAIL PULLEY ASSEMBLY BY DRAGGING IT ALONG THE FLOOR OF THE TUNNEL, HE STRAINED HIS BACK AND ELBOW.

WHILE ASSISTING ANOTHER EMPLOYEE TO WELD AND CLEAN CLAY FROM UNDER A 4X4 CRUSHER, EMPLOYEE GOT DEBRIS IN HIS EYES.

LIFTING STEEL PLATE - CLEANING METAL OFF DECKS

EE WAS REPAIRING 4 1/4 CRUSHER WIUTH A CUTTING TORCH WHEN A HOT PIECE OF SLAG TOUCHED EE'S COAT CAUSING A BURN ON HIS ARM.

EE WAS CLEARING ROCK AWAY FROM HAD PULLEYS AND TAKING TO STOCK PILES WITH A LOADER, WHEN HE HURT HIS NECK. INJURY WAS ALLEGEDLY CAUSED BY A BUMP IN HIS PATH.

EE WAS TIGHTENING DOWN RETAINER BOLTS ON SCREENS & RATCHET BROKE WHICH CAUSED EE TO SLIP AND OUTSIDE OF RIGHT ELBOW CAME DOWN ON TOP OF ANOTHER BOLT. ELBOW TENDONITIS NOT REPORTED TO EMPLOYER UNTIL 5-18.

DRILLING BOLT WHEN METAL FRAGMENT ENERTERED EYE CAUSING IRRATATION. EE WAS WEARING SAFETLY GLASSES AT THE TIME

EE WAS WALKING DOWN STEPS FROM INSPECTING A CRUSHER WHEN HE SLIPPED AND CAUGHT FOOT, ROLLED TO THE BOTTOM. THE STEPS WERE WET HANDRAILS WERE IN PLACE BUT NOT USED. IT WAS RAINING. EE TWISTED HIS LEG AND DAMAGED HIS KNEE]

EE WAS PERFORMING A PRESHIFT INSPECTION OF A BACKHOE, WHEN A ROCK SHOT OUT OF 5/2 CRUSHER & STRUCK HIS EYE. THE EE WAS NOT WEARING SAFETY GLASSES AT THE TIME. HE SUFFERED FROM A LARGE ABRASION ON HIS LEFT EYE.

THE EMPLOYEE REPORTED ON 4-29-98 THAT IN AUG OF 97 HE HAD A PROBLEM WITH THE HEATER IN THE CAT 988 LOADER LEAKING COOLANT AND BLOWING ON AN ALREADY IRRITATED WOUND, CAUSING A RASH ON RIGHT ANK LE.

EE WAS ATTEMPTING TO HOOD A CABLE AROUND A CONVEYOR WHEN THE CABLE SLIPPED & CAME BACK AROUND HITTING THE EE IN THE MOUTH. HE BROKE A TOOTH AS A RESULT.

THE EE WAS ATTEMPTING TO CHANGE SCREENS ON A WASH TOWER & REMOVE BOLTS. EE STRUCK HIS KNEE ON A STEEL STRUCTURE CAUSING SWELLING IN HIS KNEE.

THE EMPLOYEE WAS ATTEMPTING TO CUT A BOLT OFF WITH A TORCH WHEN HOT METAL ENTERED EMPLLOYEES LEFT EYE. THE EMPLOYEE SUFFERED FORM A SCRATCH ON HIS LEFT EYE AND EYE IRRITATION.

EMPLOYEE WAS ATTEMPTING TO CUT A THICK BELT WITH A RAZOR KNIFE WHEN HE CUT HIS THIGH.

THE EMLOYEE WAS ATTEMTPING TO FREE UP A WIRE COME-A-LONG WITH A CHAIN COME-A-LONG WHEN THE CLAMP BROKE CAUSING THE COME-A-LONG TO HIT THE EMPLOYEE IN THE FACE. THE EMPLLOYEE SUFFERED FROM CUT S TO HIS HEAD AND NOSE AND CONCUSSION.

EMPLOYEE WAS OEPRATING A WIRE COME-A-LONG WHEN HE TWISTED HIS BACK CAUSING BACK STRAIN.

EE WAS DIGGING OFF BACK WALL IN PIT B WHEN THE WALL SLUFFED OFF HITTING THE LOADER & BROKE WINDSHIELD.

LIFTING FEED PLATE. IT SLIPPED, FELL AND CUT LITTLE FINGER RT HAND.

WAS KNOCKING RACE FROM HUB AND STRUCK FLOOR CUTTING THUMB.

EE WAS TRYING TO CLEAR UKP A BLOCK. THE LINES RENDERED & THE BLOCK HIT HIM IN THE LEFT EYE CAUSING A CUT THAT NEEDED STITCHES.

HYDRO RAM WAS NOT FULLY RETRACKED SO HE WAS ADJUSTING HEAD AND IT KICKED BACK CAUSING THE LEFT HAND UNDER THUMB AREA TO BE PINCHED.

EMPLOYEE WAS KNEELING AND CUTTING RUBBER WITH A UTILITY KNOFE. THE KNIFE SLIPPED AND CUT HIS THIGH.

EMPLOYEE WAS GETTING INTO TOOL BOX WHEN CHUNK OF STEEL FELL OUT & FELL ONTO FOOT

WHILE CHANGING TEETH ON THE DREDGE BUCKET, A PIECE OF METAL BROKE OFF AND HIT EE IN THE LEFT EYE.

EE STATES HE WAS FLIPPING GATE AND CUT LEFT CALF MUSCLE.

OPERATING 988B 40-50 HOURS A WEEK. RIGHT ARM HASLOST STRENGTH AND HAS PAIN FREQUENTLY. RIGHT HAND HAS LOST FEELING. RIGHT ARM PULLS HYDRAULICCONTROLS CONSTANTLY.

EMPLOYEE WAS WORKING OUTSIDE WITH PORT POWER ANDWAS STRUCK WITH A BEAM CAUSING HIM TO FALL ON RIGHT LEG.

EE RELEASED EXCESS HAULBACK LINE AND TO GET OUT OF THE WASY, HE MOVED HIS HEAD QUICKLY AND HIT HEAD HARD ON MACHINE.

EMPLOYEE WAS LIFTING GUARD WHEN HE TWISTED AND TORE MUSCLES IN LEFT RIB AREA.

LOWER BACK PAIN FELT WHILE DRIVING LOADER OVER ROUGH ROAD.

EE WAS PUSHING WIRES INTO STARTER BOX AND SLIPPED ON BOARD AND INJURED BACK.

EMPLOYEE STEPPED THROUGH HOLE THE TOP PLATE HAD BEEN REMOVED FOR MAINTENANCE AND NOT REPLACED PRIOR TO EMPLOYEE WALKING DECK.

WHILE OUTSIDE CHECKING ISC CHUTE, A ROCK APPROX. 3" FELL FROM #1 TOWER. THE ROCK STRUCK EMPLOYEE IN BACK AND SHOULDER CAUSING HIM TO FALL FORWARD.

WHILE WORKING ON CRUSHER PARTS, HAND SLIPPED OFF PRY BAR, SLID ACROSS SHARP RIM AND CUT INTO FINGRS.

EE TOLD EE HE TOLD EE TO TELL THE PIT OPERATOR TO START THE BELT EE THEN WENT FOR THE RADIO & EE TRIED TO CROSS THE BELT BEFORE BELT STARTED EE WAS NOT OFF BELT JUMPED & HIT HARD GOUND

EMPLOYEE WAS PULLING YARDER CABLE,IT TWISTEDUPA ND CAUGHT HIS GLOVE TWISTING IT AROUND THIS ARM.

CARPEL TUNNEL ON ELFT HAND DUE TO EXCESSIVE USE SUCH AS SHOVELING AND RELATED DUTIES AT LOADOUT SYSTEM AND PLANT AREA.

WHILE STANDING ON THE PLATE THAT HOLDS THE TROUGHING ROLL EE REMOVED THE BOLTS HOLDING THE TROUGHING ROLL, TIPPED IT OVER & PROCEED TO PULL THE ROLL OUT FROM UNDER THE BELT, IT HUNG UP AND EE GVE IT A BIG JERK, THE TROUGHING ROLL HOOKED HIS GLOVE AND PULLED HIM OVER THE SIDE OF THE BELT, EE THEN HIT THE GROUND APPROX. 10 FEET DOWN.

EMPLOYEE SLIPPED WHILE GOING DOWN EMBANKMENT. ALL WEIGHT WENT ON LEFT FOOT BREAKING LEFT ANKLE ON BOTH SIDES. AREA WAS ICY.

EE PASSED OUT AND WALKED OR FELL INTO POND.

WHILE EMPLOYEE WAS CUTTING A PIECE OF PAPER, HIS KNIFE SLIPPED AND CUT LEFT UPPER LEG.

EMPLOYEE WAS WORKING ON FEEDER HEAD PULLEY WHEN HIS THUMB CAUGHT BETWEEN THE SHAFT ON GEAR BOX & PULLEY

STRAINED LOWER BACK WHILE SHOVELING IN TUNNEL. ALSO WAS IN UNLIGHTED AREA AND HE WALKED INTO PIPE DRIVING HARD HAT DOWNWARD.

PRIOR LOWER BACK PROBLEM REOCCURRED 7-27-91 AFTER RETURNING TO WORK FULL TIME AND REGULAR DURY 7-15-91.

HE WAS LIFTING AN OXYGEN BOTTLE WHEN HE TWISTED HIS LOWER BACK.

EMPLOYEE DISCOVERED SWELLING ON LEFT LOWER GROIN AND THE PAIN PROGRESSIVELY GOT WORSE.

BACK WAS SORE AFTER DOING MAINTENANCE ON SCREEN AT CRUSHING PLANT. TWO DAYS LATER HE REPORTED TO DR. AND WAS TOLD TO WORK LIGHT DUTY FOR ONE WEEK. INSTEAD OF LIGHT DUTY, HE CHOSE TO MISS WORK FOR REMAINDER OF WEEK.

EMPLOYEE WAS PULLING ON A HOSE WHEN HE TWISTED HIS RIGHT KNEE.

EE BENT OVER AND STRAINED LOWER BACK.

EMPLOYEE SLIPPED ON ICE AND INJURED RIGHT KNEE.

EMPLOYEE WAS SHOVELING IN TUNNEL WHEN HE STRAINED LOWER BACK AND FELT EXCESSIVE PAIN.

EMPLOYEE HIT ARM ON GUARD RAIL WHILE GOING UPSTAIRS.

EMPL SLIPPED AND FELL ON BARGE DECK.

EMPLOYEE WAS WIRE BRUSHING PIPE WHEN A PIECE OF SAND GOT INTO THE LEFT EYE.

WAS WORKING ON CONVEYOR BELT INSHOP WHEN HE GOT UP HIS HIP AND BACK WERE STIFF

WORKER WAS CUTTING RUBBER CONVEYOR BELT WITH LEFT AHND WHEN RAZOR CUT MIDDLE FINGER.

DUE TO EXCESSIVE USE OF RIGHT HAND ON 10-KEY CALCULATORS & COMPUTER CARPEL TUNNEL SYNDROME HAS DEVELOPED

WAS CLEANING GREASE OFF SOCKET RING AND RAN FINGER OVER SHARP EDGE.

IN JUNE 1989 EMPLOYEE'S HAND STARTED ACHING AND GOING TO SLEEP. THEN UPPER ARM AND IT HAS GOTTEN PROGRESSIVELY WORSE.

EMPL GRINDING OUT CRACK ON PUMP HOUSING TO WELD.OBJECT FLEW FROM GRINDER AND GOT INTO EYE.

HIT ELBOW ON FLOOR OF LOADER HOOKING CHAINS.

EMPLOYEE WAS PUSING ROUND STOCK AROUND PIPE AT TROMMEL TO WELD IN PLACE WHEN RIGHT LEG SNAPPED WHICH WAS A PINCHED NERVE.
