EE stepped on loose rock, lost balance and cut right knee when EE fell.

Employee was using a torch to remove rusted bolts on the middle screen deck when hot slag entered left ear causing a perforated ear drum. Employee did NOT seek medical treatment until April 10, 2019.

Employee was removing an operators seat and overextended pulling a hamstring

Employee was on knees working on a water pump at the wash-out. When they stood up, the right knee popped. Employee continued working thru the day but the next day it was swollen. The doctor said the ligaments were OK and gave employee a Cortisone injection.

Employee was swinging a 3lb hammer overhead to loosen the stone box feeding the syntron on the tunnel belt at plant #1 and strained a muscle in EE's right side.

Employee was putting new drive belts onto syntron. While rolling it on, left pinky was pinched under the belt, causing a laceration that required sutures.

Employee was on ground when a hoist began lifting a screen cloth up to the screen. The cloth became detached from the lifting hook and bounced around when it struck the ground. It hit the employee's hand. It barely made a mark and the employee worked the rest of the week. Over the weekend EE went to the ER and was told EE had a broken bone in left hand.

After servicing a skid steer, the mechanic shut the engine lid. A cloud of dust was thrown by the fan into his face. He rubbed his eyes, but later his left eye was bothering him. He rinsed it but that didn't help the irritation. He went to the doctor who discovered a piece of metal embedded in his eye which he removed using tweezers and a brush.

THE EMPLOYEE WAS TIGHTENING BOLTS THAT WERE OVERHEAD AND A SMALL FLECK OF DIRT OR METAL FELL INTO HIS RIGHT EYE AND BECAME LODGED.

EMPLOYEE WAS CARRYING A BELT CUTTER ON HIS SHOULDER WHILE WALKING UP A CONVEYOR BELT CATWALK. THE CUTTER SLID OFF HIS RIGHT SHOULDER, AND CUT HIM.

After parking his tri-axle, the employee was checking his tires. He went to kick one of them and his other foot slipped on the ice and it went out from under him. He fell on his back, hurting it and his left leg.

EMPLOYEE WAS OPERATING A HAND GRINDER WHEN THE CUTTING WHEEL SHATTERED. THE GRINDER WAS KICKED OUT OF HIS HAND, CUTTING THE 4TH FINGER OF HIS LEFT HAND. THREE SUTURES WERE REQUIRED TO CLOSE LACERATION.

EMPLOYEE WAS USING A KNIFE TO CUT ZIP TIES ON A TAIL PULLEY GUARD SO IT COULD BE REMOVED TO INSTALL NEW ROLLER AND SKIRT RUBBER. THE KNIFE BLADE SLIPPED AND HE CUT HIS RIGHT INDEX FINGER DEEP ENOUGH TO REQUIRE SUTURES.

While installing belt rollers, the employee was pulling hard on a wrench to tighten the bolts and strained his right shoulder. At a later date, it was determined he had torn his rotator cuff.

While installing teeth on loader bucket, employee used hammer to adjust tooth position. A sliver of metal came off the tooth and struck his left arm. It penetrated the skin and was lodged there.

WHILE EMPLOYEE WAS PREPPING HIS TRUCK FOR SERVICE, ANOTHER EMPLOYEE WHO HAD A CONFLICT WITH HIM, APPROACHED AND STRUCK HIM IN THE HEAD WITH HIS LUNCH BOX CAUSING A SEVERE LACERATION ON THE RIGHT SIDE OF HIS HEAD.

While opening the hood of his International triaxle, he threw his weight back to get it started. Later that day discomfort turned to pain in his left shoulder and neck area.

Employee was installing skirting along a conveyor belt, when he ran a wicker of steel into his left forefinger.

employee was using ladder on side of tri-axle truck to check inside the bed when he lost his footing and fell to the ground. he landed on his feet and rolled back-wards, injuring his back.

Employee was in the process of greasing 988F loader. As he walked around the loader, he stepped on a rock that rolled under his foot. He sustained a sprained right knee and it would not allow him to walk.

While running the bulldozer, the employee developed chest pains which ultimately were diagnosed as a heart attack.

Employee was pounding a pin into position on a trackhoe and a fleck of metal flew up under his safety glasses, striking him in the left eye. He thought it just bounced off and continued working. It was a few days later that his eye became irritated and he discovered that the fleck was lodged up under his eyelid.

After wiping mirror clean on truck, the employee stepped to the ground. His foot came down on a stone that rolled under his weight, causing a sprain to his left ankle.

Hurt back by lifting up door of truck.

Employee was changing a truck tire and sprained right lower back.

He was trying to open metal doors and one flew open and he flew back a few feet, he struck the hand rails thats what caused the injury.

MASSIVE HEART ATTACK

TAKING A TOOTH OFF A BUCKET WAS HITTING THE TOOTH WITH A HAMMER. THE SPAW FELL OFF HIT HIM IN THE RIGHT LEG AND A PIECE OF METAL WENT INTO HIS LEG.

Tightening a spring bolt and was using an air wrench, slipped and cut some of his pinky finger off.

mechanic started acetylene/oxygen blow torch, heard loud popping sound, turned it off, it "blew up" near hose. his left palm, side and back of hand was burnt. He performed first aid on himself ,we sent him to Tyrone Hospital where they prescribed him pain relievers, antibiotics and wrapped his hand. torch was rebuilt approx. 2 years ago by National Valley Gases.

EE was working on Plant 4 screen and said he "Bent his knee wrong" felt like he twisted it Saturday April 24th. On April 27th an accident report was filed. EE went to see a physician from our Workmens Comp panelists who referred him to an Orthopedic specialist who recommends surgery scheduled for June 22nd 2004.

TWO EMPLOYEE'S WERE MOVING A PORTABLE LIGHT PLANT WITH A COMPANY PICK-UP TRUCK. WHEN THEY WERE OUT OF THE VEHICLE THE LIGHT PLANT STARTED ROLLING FORWARD INTO THE PICK-UP TRUCK AS THEY WERE TR YING TO SWITCH THE LIGHT PLANT. WHEN IT STARTED ROLLING FORWARD EE REACHED OUT TO STOP IT AND GOT HIS LEFT INDEX FINGER CAUGHT IN BETWEEN THE HITCH & THE TRUCK. RECEIVED 6-7 STITCHES, ANTIBIO

WHILE WORKING ON COLLECOT DUST AT BAGHOUSE HIS RIGHT ARM HOLDING WRENCH SLIPPED AND BANGED AGAINST AN UNKNOWN OBJECT RIGHT FOREARM WAS PUNCTURED.

EE WAS LIFTING A SCREEN WITH USE OF A PIECE OF STEEL AS A PRY BAR WHEN IT SLIPPED OUT FROM UNDERNEATH THE SCREEN AND HE HIT HIMSELF IN THE RIGHT EYE WITH THE STEEL BAR. HE WAS TOLD TO GO TO TH E HOSPITAL ONE WEEK LATER BY HIS EE DUE TO THE SWELLING OF EYELID. DR RX ON ANTIBIOTIC AND GAVE HIM A TETANUS SHOT.

WHILE CUTTING A STEEL PLATE WITH A TORCH, A PIECE OF THE BURNING STEEL FLEW UP FROM UNDER EE'S SAFETY GLASSES CAUSING A POSSIBLE ABRASION TO HIS RT. CORNEA OF HIS EYE AND ALSO BURNING TO HIS RT. EYELID.

EE'S BACK LOCKED UP ON HIM WHEN STANDING UPRIGHTFROM A KNEELING POSITION WHEN BENDING OVER BELT.POSSIBLE RE-OCCURRING PROBLEM FROM 1ST QTR INJURY. DOCTOR DIAGNOSED PAIN AS MUSCLE SPASMS.

EE STRAINED HIS BACK WHILE DRAGGING WASH HOSE FROM UNDERNEATH A CONVEYOR.

EE DIAGNOSED IN OCTOBER AS HAVING A PINCHED NERVE IN NECK CAUSED BY A REPETITIVE MOTION WHILE OPERATING A DOZER, CAUSED BY LOOKING OVER RIGHT SHOULDER.

EMPLOYEE FELL AND BROKE HIS RIGHT WRIST WHEN THE LADDER HE WAS STANDING ON CAME OUT FROM UNDER HIM. DUE TO SPACE LIMITATIONS LADDER WAS BRACED AGAINST CONVEYER AND ALSO THE GROUND WAS WET FROM RAIN.

SLIPPED ON SNOW COVERED ICE WHEN GETTING OUT OF HIS VEHICLE.

PERFORMING MAINTENANCE ON PLANT THREE CONVEYOR. EE BLEW DUST OFF OF A BOLT AND DUST/DEBRIS BLEW INTO HIS RIGHT EYE CAUSING IT TO BE SCRATCHED. CONVEYED TO HOSPITAL. DR. PRESCRIBE AN OINTMENT A ND GAVE HIM AN EYE PATCH TO BE WORN FOR 24 HRS.

WENT TO CLEAN OUT OBSTRUCTION FROM TROUGHING ROLLERS IN PLANT 5 BECAUSE THEY HAD STOPPED SPINNING. HE PLACED HIS HAND BETWEEN ROLLERS AND THEY STARTED OPERATING, RESULTING IN INJURY TO HIS LEF T HAND WHICH WAS CRUSHED LEAVING HIM ON RESTRICTED DUTY. NO USE OF LEFT HAND.

WHILE TRYING TO TIGHTEN A TAIL-PULLEY EE FELT SOCKET SLIP OFF HIS RIGHT HAND HIT THE METAL ONT HE TAIL-PULLEY COVER CAUSING HIS RIGHT THUMB TO BE BENT BACKWARDS. EE WAS PLACED ON RESTRICTED DU TY AS OF JUNE 30TH 2000.

EUCLID DRIVER WAS INSTRUCTED BY MECHANIC TO CHECK ANTI-FREEZE TEMPERATURE OF HIS EUC E-16 WITH A HAND-HELD HEAT GUN. EE UNFAMILIAR WITH USING A HEAT GUN LOOSENED CAP AND WAITED FOR HISSING TO STOP AND RESUMED LOOSENING CAP, PRESSURE BLEW OFF CAP AND BLEW ANTI-FREEZE ALL OVER HIM.
