While employee was tearing down Hatachi 1200 Hoe , He was using a wrench and his right arm locked into a bent position. He used his left arm and straighten out the right arm. Finished out the day working. Worked out the balance of the week and went to Occupational Health that Friday 11/02/2012

The Hoe Operator was loading a large rock onto a Euclid when the rock slipped from the hoe bucket and fell onto the bed of the Euclid, jolting the Euclid driver causing strain to his neck, shoulders and lower back.

As EE was tightening the bolts on the pay loader bucket, the 1" air wrench slipped off one of the bolts striking his right hand between the air wrench and pay loader bucket. Bruising the tendon of the second finger in the right hand .

AS A WELDER WAS WELDING AND REPAIRING PL-74. HE WAS GRINDING A WELD WHEN THE 4" GRINDER KICKED BACK AND MADE A LACERATION TO HIS RIGHT CHEEK ON HIS FACE.

As EE was cutting a rubber wiper for a side wiper the knife slipped off the cut and cut into thumb of right hand.

Jumped off screen deck to floor by stairway he sprained left ankle.

As welders injured EE and another EE were attemting to get a piece of steel channel in deep snow when EE pulled his groin causing a hernia.

Stringing cable to pull the big belt back together-pulled left groin causing a strain.

Working on a crusher, he overextended his right arm with elbow while using a sledge hammer.

Fractured finger while scraping a chute.

He slipped on a rock and sprained his ankle walking beside the plant.

Laceration - right arm cut when trying to remove a filter. Filter slipped and cut arm.

Twisted right knee climbing out of trackhoe.

Employee did not have eye protection on and suffered welder's flash.

ACCUMULATED EFFECTS FROM USING THE BREAKER MACHINE (EXCAVATOR) THE SEAT HAS A HARD BOTTOM AND THERE IS NOT MUCH GIVE WITH THE CONTINUAL JARRING.

Workers were stripping off wood forms. Wood slipped and hit angle iron. Piece of angle iron fell and cut EE's left thigh.

LEFT HAND LACERATION, HOLDING A 3/4" WRENCH WHEN IT SLIPPED OFF NUT & PINCHED HIS HAND BETWEEN GUARD AND WRENCH.

Burning garbage in a fire barrel, he went to dump barrel. Fire came out and burned his face.

GETTING OFF OF MACHINE AND SLIPPED ON TRACK, BRUISING RIGHT ANKLE.

SLIPPED ON CATWALK - HIT ELBOW AND FOREARM.

WORKING ON A DOOR ON CRUSHER. DOOR SLIPPED ON HINGE CATCHING FINGER ON DOOR. CAUSED LACERATION TO LEFT RING FINGER AND HAD STITCHES.

GETTING OFF OF SKID LOADER (PL74), TWISTED ANKLE ON LIP OF BUCKET CAUSING A GRADE 1 SPRAIN.
