Employee bumped right knee on metal steps while climbing up getting into truck.
Employee lifted blocking.
Employee was cleaning out a rock jam in feeder hopper and a rock came out and hit him on his left knee.
Employee hurt back and neck operating loader.
Employee was cleaning under a screen using a long tee bar and accidently touched the driver hub which caused back or reverse pressure and injured his second and third fingers.
Employee slipped and fell backwards while opening hood on dump truck.
While operating a front end loader the bouncing aggravated a prior illness in his neck area.
EMPLOYEE WAS USING A 3/4" RATCHET AND SOCKET TO TIGHTEN WHEEL LUGS ON A DUMP TRUCK AND THE SOCKET WRENCH SLIPPED LOOSE AND HE PINCHED HIS FINGER BETWEEN THE HANDLE BAR AND THE GROUND.
WHILE REMOVING SAND FROM UNDER CONVEYOR STRUCTURE UPRIGHTS, THE WEIGHT OF FROZEN SAND CAUSED ONEOF THE LEG STANDS TO BEND DOWN WHICH IN TURN PULLED PART OF THE CONVEYOR DOWN ALSO. NO CONTACT W ITH FRONT END LOADER.
EE PULLED LEVER ON 1B BIN CLAM GATE AND WRENCHED HIS RIGHT SHOULDER. HE CONTINUED WORKING UNTIL 4/25/03. HE IS NOW ON WORKMAN'S COMPENSATION.
EMPLOYEE WALKED INTO HANGING ROCK CHUTE AND CUT HIS HEAD.
