EMPLOYEE WAS USING A 3/4" RATCHET AND SOCKET TO TIGHTEN WHEEL LUGS ON A DUMP TRUCK AND THE SOCKET WRENCH SLIPPED LOOSE AND HE PINCHED HIS FINGER BETWEEN THE HANDLE BAR AND THE GROUND.
WHILE REMOVING SAND FROM UNDER CONVEYOR STRUCTURE UPRIGHTS, THE WEIGHT OF FROZEN SAND CAUSED ONEOF THE LEG STANDS TO BEND DOWN WHICH IN TURN PULLED PART OF THE CONVEYOR DOWN ALSO. NO CONTACT W ITH FRONT END LOADER.

WAS WORKING UNDER A MACHINE AND HAD TO MOVE SOME STONES THAT BECAME JAMMED IN THE AREA. HE FELT A TWINGE IN HIS BACK AND THE FOLLOWING DAY HAD PAIN BUT SHOWED UP FOR WORK. THEN HE WENT TO THE HOSPITAL.
THE EMPLOYEE WAS CLIMBING DOWN OUT OF HIS TRUCK, HIS FOOT SLIPPED. HE HAD A GRIP ON THE STEERING WHEEL. THE WEIGHT OF HIS BODY SLAMMED INTO THE RUNNING BOARD, INJURING HIS RIGHT RIB CAGE.
WORKING ON A FRONT END LOADER-EMPLOYEE WAS STANDING ON THE BATTERY BOX, PULLING ON A WRENCH, THE NUTS LEFT LOOSE CAUSING HIM TO FALL.
