Employee was attempting to remove a bucket tooth that was stuck in a jaw crusher. Employee hit tooth with a sledge hammer. Tooth shot out of crusher making contact with employee and causing a left leg fracture.

Employee was getting on the dozer at the beginning of the shift. Their foot slipped. They smacked their left knee off the blade.

Employee was completing pre-work inspection on equipment when employee slipped into a piece of metal striking the side of right foot.

A CAT 777D, co#6, backed into the refuse discharge tower causing minor structural damage.

Belt scraper got turned upside down near tailwheel, employee reached in with a pry bar to dislodge it and got caught in belt.

operator error caused 988H loader to be toppled on its left side.

A co-worker swung door open, door struck EE in the rear of head.

EE was on a ladder thawing the tail wheel skirting on the spiral plant. When EE was climbing down, the ladder slipped and EE fell approx. 4 feet landing on EE's right side. EE did not seek medical attention until 03/18/19.

Employee was entering pump house, EE slipped & went down on the left knee.

Conveyor that moves coal from the dryer to silo caught fire. The job site was idle & there were no witnesses.

employee on street sweeper was on side of road facing east on SR4030 (Gilberton Road). Blue Mercedes behind was almost stopped. Tan buick approached & hit into blue mercedes and pushed it into street sweeper. Local ambulance, paramedic's & police were on site.

Employee lost control of truck while descending a ramp and rolled over.

Employee heard a "whining" noise from D65PX, thought a hose may have blown or had a leak. Employee got out of cab & was standing on R side track to look at hydraulic sight glass. Employee slipped off the track and landed on feet on the ground (approx. 3 ft). Employee twisted a knee & jarred their neck.

on catwalk that accesses conveyor line; slipped; put out hands to break fall; came in contact with conveyor line.

Employee was digging out a blocked tail wheel when EE hurt something in right arm above elbow (bicep).

EE was pulling himself up into the truck using handrail and ladder rung when be felt a sharp pain in right bicep.

Employee was operating a JLG lift and while repositioning the lift, he had his arm on the handrail and raised it up into the bottom of a chute.

Hooking cables from crane to screen, lower brace shifted causing screen to pivot, EE slipped twisting his left knee.

While driving a 777 Haulpak on the haul road, EE fell asleep and ran into a 345 excavator parked on the left side of the haul road.

EE was replacing skirting on a screening plant and was bent over on his knees, when he got on his feet he turned still bent over he felt his right knee pop.

EE was fueling dozer, he tripped over frozen material and fell straining the area behind his right knee.

EE was walking behind garage in the snow and stepped on a nail in a board.

Employee was walking up stairs, he slipped on a step and hit his right knee on the corner of the steps.

EE slipped off of the fuel tank ladder while filling the fuel truck. He stopped himself with his right arm on the the ladder hand rail, the sudden jerk hurt his right shoulder.

Employee was changing a shive; he dropped it into position to slide it out, then rested it on the hook on the vibrator. That screen slipped off and the hook came down on his fingers.

Two employees were lifting a rock approx. 100 lbs off the pusher table when one tripped over a rock on the ground, fell and the rock fell on his leg.

Cleaning mud from under pusher with bar and pinched gloved hand between roller and frame. Bar slipped off and pinched finger.

Banging frozen mud of portec screen with a hammer, pinched ring finger of left hand between hammer handle and screen.

EE FELL IN CRUSHER AND HURT BACK, EE WAS ADVISED BY IMMEDIATE SUPERVISOR TO GO TO HOSPITAL, EE SAID HE'D BE ALL RIGHT AND WENT HOME. HE HAS NOT RETURNED TO WORK, ANSWERED OR RETURNED PHONE CAL LS, HAS NOT GONE THROUGH PROPER CARE PROCEDURES.

EE'S EYES WERE FLASHED WHILE WELDING.

EE WAS CLEANING OUT #6 BAY, GOT MATERIAL INTO EYES WHICH BECAME INFECTED.

PICKING JUMPS OFF A CONVEYOR.

EE WAS EMPTYING A WHEELBARROW. HE STEPPED IN A HOLE AND TWISTED HIS ANKLE.

EE WAS MOVING DRAIN OIL BARRELS.

EE WAS LEAVING DOZER, THE DOOR CLOSED ON HIS RIGHT HAND.

EE WAS CLEANING TUNNEL BAY WITH A LONG SCRAPER. SCRAPER SLIPPED DOWN AND HER SHOULDER JERKED FORWARD.

EE WAS CLEANING OUT THE BAY OF THE #1 SCALPER WHEN MATERIAL (FEED) LET LOOSE.

EE WAS FIXING HAND RAIL ON HAULPAK WHEN HE FELT PAIN & A POPPING SENSATION IN LEFT CALF

EE WAS REMOVING A GEAR FROM THE 450C SHOVEL, WHILE ROLLING THE GEAR OUT HE GOT HIS FINGER PINCHED BETWEEN THE GEAR AND SIDE OF THE MACHINE.

EE WAS GETTING DOWN OFF THE WA-800 LOADER, WHEN HE SLIPPED/FELL FROM THE 3RD STEP TO THE GROUND.

EE WAS LIFTING CHANNEL IRON TO PUT IN PUSHER WHEN HE STRAINED HIS BACK.

EE WAS MOVING BATTERIES WHEN HE FELT A POP IN HIS LEFT SHOULDER.
