Worker received laceration to his right thigh while cutting conveyor belt with utility knife. Injury required sutures.

The right long-finger of a loader operator was struck by the cab door, resulting in a smashed finger and broken bone in the finger tip. The operator had just opened the cab door and beginning to exit the cab, when a gust of wind blew the door shut. The steel frame on the cab side of the door struck the operator's finger.

Worker was using an air wrench to loosen a lock nut from a 1"x7 1/2" bolt located on jaw crusher. His middle fingertip got smashed and broken by the bolt head.

Worker (Blaster)was preparing bottom rows of blast holes in a face for loading of explosives. The face and work area had been previously scaled and work area inspected prior to the work. A rock fell from the face and struck the worker in his right forearm, causing a laceration. The fallen rock had scaling marks.

Worker on blasting crew was at mine face loading explosives when rock fell approximately 10 feet before striking him on his hardhat and left arm. Rock measured aproximately 1-inch thick by 15-inches by 6-feet. The work area had been scaled mechanically and by hand.

EE was inside cab of machine scaling when rock from the mine roof fell & struck the cab of the scaler. The rock that hit the equipment jolted the worker inside the cab. 1 or 2 small rock pieces had fallen in the area just before the last rock fell. The engine on the scaler turned off after the cab was struck so the worker exited the cab through rear window to flee the work area

Worker was dismounting Front End Loader at end of his shift. As worker stepped onto the ground, while dismounting equipment, he felt a muscle pull/strain in his lower back. The ground was firm, level and free from debris. Day of incident was worker's second day on the job following a 2-month layoff.

Rock fell from roof of mine and landed on the basket of the anfo loader. The operator was in the cab and suffered a back sprain. The employee had restrictions placed on his work by a doctor causing the injury to be recordable.

An unknown condition started a fire in the engine compartment of a Fletcher scaler while it was in use back-scaling. The operator activated the fire suppression system and initiated the mine evac system. All personnel exited the mine safely and MSHA was notified immediately.

Employee lifted sample bucket from truck bed and felt pain in right elbow.

A small rock fell from the back and hit the employee's hard hat, shoulder, and arm.

Employee accidentally dropped a bucket of aggregate on her foot causing a fracture. Treatment was application of a cast.

LIFTED AND CARRIED POWER CABLE FOR VENTILATION FAN AND STEPPED IN A HOLE.

WHILE TIGHTENING BOLT, WRENCH SLIPPED CAUSING EE TO PROPEL BACKWARDS TRIPPING ON WATER HOSE AND FALLING ON CORNER OF CRIBBING USED UNDER CRANE OUTRIGGING.

EE WAS SCALING IN THE BENCH AREA WHEN A LARGE ROCK HIT THE BOOM OF THE SCALER RESULTING IN HIS BACK BEING SPRAINED.

ADDING ENGINE OIL TO LOADER IN UNDERGROUND SERVICE AREA WHEN STRUCK BY A PIECE OF FALLING ROCK.

EMPLOYEE HAMMERING BIT OFF OF END OF DRILL STEEL. A SMALL PIECE OF THE HAMMER HEAD BROKE OFF WHEN HE STRUCK BIT AND PROPELLED UPWARD STRIKINGHIM IN THE CHEST.

EE WAS HAND SCALING FACE AREA WHEN HE ENCOUNTERED LARGE PIECE OF LOOSE ROCK. IT WAS DETERMINED THAT A SCALING MACHINE WOULD BE NEEDED. AS HE AND ANOTHER EE WERE PULLING AWAY THE ROCK CAME LOOS E AND FELL, HITTING THE MANLIFT. THE FORCE THREW 2ND EE INTO 1ST, PUSHING HIM INTO THE SIDE RAIL OF THE BASKET BRUISING LEFT SIDE OF HIS RIBS.

EE WAS HAND SCALING FACE AREA WHEN HE ENCOUNTERED A LARGE PIECE OF LOOSE ROCK. AFTER EXAMINATION IT WAS DETERMINED THAT THE SCALING MACHINE WOULD BE NEEDED. AS HE WAS PULLING BACK THE ROCK POP PED LOOSE AND CAME DOWN HITTING THE MANLIFT. THE FORCE THREW HIM INTO ANOTHER EE WHO WAS PUSHED INTO RAIL OF BASKET. FIRST EE WAS HIT ON ELBOW & HAND.

EE WAS LOADING LIFTER HOLES IN WATER AND CUT HIS FINGER ON SHARP ROCK.

TRIPPED OVER GEAR SHIFT WHEN ENTERING DRILL WHILE TRYING TO LOOK AT BOOM AT SAME TIME

CHECKING OIL IN WATER TRUCK, WAS ON RIGHT FENDER CHECKING OIL, HE MISSED THE STEP AS HE WAS CLIMBING DOWN FROM THE TRUCK.

WAS ON THE BACK OF XL5110 SCALER. WAS IN PROCESS OF ADDING OIL TO THE HYDRAULIC TANK AND WAS ATTEMPTING TO REMOVE THE PLUG WHEN WRENCH SLIPPED OFF PLUG INJURING FINGER.
