Jeddo No 8 Preparation Plant Coal

While cleaning up after a welding repair in sizing screen, the Miner transferred their weight onto their left arm and rotated to swing their legs over side of sizing screen to exit. This caused the Miner to strain their left shoulder.

While changing work shirt at the end of the shift, foreign matter that was on the Miner's shirt got into EE's eye which was removed by an eye care specialist the following day. The Miner missed the following day due to discomfort in their eye.

Employee was working on the Skid Steer Loader. Upon entering the cab of the loader, employee slipped on the wet smooth surface of the loader bucket and injured their left leg and ankle. Employee reported they did manage to maintain three points of contact with their two hands on the grab bars and one foot on the inside of the bucket.

Employee was walking down slope next to Off Road Diesel Fuel Tank with Fuel Hose in hand. Employee 'lost footing' falling on right arm and shoulder. Employee stated cannot lift right arm without extreme pain. Dry Soil Conditions, Dry Weather, Sunny with few clouds, Hot Temperature 80 to 90 degrees Fahrenheit.

Lifting bales of hay from pickup truck box. Off work since 4-9-21.

Repairing vibrator screen - leaned on screen, sliver of screen caused superficial puncture wound to left forearm.

Placing electrical conduit - Twisted right leg. Incident occurred on 10/20/2017 - put on restrictions on 11-21-17.

Dumping load - Malfunction on hydraulic system - Box came down - rocking truck causing injury to employee's back.

Injury occurred 3/15/16 - unable to work beginning 6-27-16

Trying to move wheelbarrow, wheelbarrow was frozen to ground, employee forced wheelbarrow out of frozen material, when it broke free employee fell backward striking EE's lower back on a board.

Material fell from floor above, foreign material entered right eye.

While using a hammer and chisel, a piece of steel broke and entered left wrist, requiring two sutures.

Walking on snow cover ground - slipped fell injuring occurred on 12/11/14 did not seek medical attention until 12/15/14 - off work since 12/15/14.

Installing vibrator deck frame - frame shifted pinning leg between vibrator and fine coal beater

Did not report accident till 8/4/14. Walked into vibrator - bumping left side of chest

Pulling on wrench-wrench slipped-put arm down to break fall-sprained wrist

Carrying a 5 gal. pail of refuse material claimed injury occurred on 10/25/13 did not report injury until 10/29/13 off work starting 10/30/13.

Disconnecting power screen - finger pinched between frame and truck wheel guard - requiring sutures.

Welding - foreign material in eye.

Working off 2nd rung of ladder, slipped & fell into door latch, pulling abdomen. Did not seek medical attention until 7-17-13 off work starting 7/18/13

Replacing belt on vibrator - pinched hand between belt & pulley

Changing belts on crusher finger caught between belt and sheave. Avulsion requiring sutures.

Diagnosis w/carpal tunnel both hands on 7-31-12 did not seek treatment until 3/4/13

Working on spray bar w/pipe wrench, wrench slipped pinching finger between wrench & metal plate.

Insect Bite

Moving 55 gal barrel, felt burn in lower back into left leg. Injury occurred on 1/11/11, sought medical treatment on 1/27/11. Doctor has him off work till further notice.

Lifted kerosene heater - felt sharp pain in left side and groin area.

Moving 55 gal barrel. Injury occurred 12/21/10. Rechecked on 3/16/11 - off work since 3/16/11.

Standing on ladder, EE slipped and fell to the floor. Landed on buttock. Acc. occured on 10/21/10, did not seek medical attention until 11/12/10. EE has been off work since 11/12/10.

Cutting pipe w/torch overhead, dropped torch and burning tip entered glove, causing burn.

EE experienced chest pain while shoveling material. Pain seems to be heart related as no injuries were reported. Further testing as outpatient is scheduled for heart condition. Tests showed no blockage but irregularities to the vessels were noted. Also drinking power drinks may have brought on symptoms.

Replacing screen on sieve. Weld broke on rail, fell to floor, injuring back.

Stepped on pipe clamp - Twisted left foot

EE was cutting pipe with plasma cutter while cleaning piece of slag from tip, inadvertently compressed trigger and this caused arc to strike finger tip.

Climbing front end loader ladder, he slipped and fell to the ground.

While repairing water pipe, moved to avoid water, twisted causing pain to lower back. Accident occurred 8/19/08. Did not seek medical attention until 8-25-08. Off starting 8/25/08.

Repairing coal crusher and was removing his burning goggles, EE had a foreign body in his eye. Did not seek medical treatment attention until 6/6/08.

Wiping face-foreign material into eye

Climbing onto truck, foot slipped off step. Whie holding on with right hand, wrenched right shoulder.

Climbing loader ladder, felt pain in right forearm.

Bee sting to lower left arm. Taken to hospital for preventive treatment.

Walking - twisted knee.

Lifting & replacing beltline rollers.

Swinging sledge hammer - felt pain

Descending truck steps - foot slipped off last step - landed full weight on left foot.

Shoveling material-claimed numbness in right hand-continued working @ regular job-rechecked by orthopedic surgeon-determined carpal tunnel syndrome of right hand. Off work since 3/14/07 for surgery.

Cutting piece of metal w/cutting torch - hot slag fell onto pant leg causing burn to right calf.

Standing on second rung of ladder, slipped and fell to floor, striking elbow on floor. He fell approx 2'.

Chipping rust off plate, a particle of rust entered his left eye behind glasses.

Replacing screen on Siene Bend-while replacing screen strained left lower back-worked until 9/18/07

Removing spare parts from trailer, he felt pain. The insect bite became abcessed.

Removing door from crusher - hit in chest with door handle.

Removing welding goggles/hood - foreign material entered eye.

Pulling material out of jammed jig crusher. Strain to left rib cage area.

Repeatitive motion injury - use of hands as a plant operator. Reported to company on 1/28/06. Did not seek medical treatment until 6/21/06.

Employee was leaning on pickup truck front fender. Other employees were utilizing pickup truck for pump repairs, caused pickup to rock. Employee claimed rocking motion of pickup cuased him to bump rib area on fender. Pickup was not in operation at time of incident. Did not seek treatment until 11-6-05. Reported to company on 11/7/05.

Grinding on metal with 4" angle grinder - foreign material entered left eye. Accident occurred on 9/2/05 - did not seek medical attention until 9-6-05.

Lifting pump casing-casing struck right knee-had pain and swelling no time lost and no restriction-rechecked because renewed pain-mri showed a tear in muscle went off work on 9-22-05 to have tear repaired.

SHOVELING EXCESS MATERIAL AT RAIL LOADING SITE. DID NOT SEEK MEDICAL ATTENTION UNTIL 03/03/2005.

EE WAS LOADING STEEL GRADING AND PINCHED HIS RIGHT HAND BETWEEN PIECES. LACERATION TO HIS MIDDLE FINGER, RIGHT HAND - REQUIRED SUTURES.

CONTRACTOR DELIVERING PREPARED FINE COGEN MATERIAL TO SITE WITH TRACTOR-TRAILER, WHILE LIFTING BOX ON TRAILER TO DUMP LOAD, TRACTOR-TRAILER FELL OVER ONTO LEFT SIDE. OPERATOR SUSTAINED FRACTUR ED RIBS TO LEFT RIB CAGE.

MOUNTING CRANK ARM ON SHAKER SCREEN; THE RIGGING SLIPPED & CRANK ARM STRUCK EE ON THE HEAD, DIS-LODGING HIS HARD HAT AND CUTTING HIS SCALP.

SLIPPED ON ICE OUTSIDE SCALE HOUSE, FELL ON KNEE.

WALKING TO DO REPAIRS ON BELTLINE. STATED HE SLIPPED ON ICE, TWISTING BODY. DID NOT REPORT ACCIDENT TO THE COMPANY UNTIL 1/23/03.

REPAIRING AT CRUSHER - PULLING ON COME-ALONG, EXPERIENCED PAIN IN CHEST AREA. DID NOT REPORT ACCIDENT UNTIL 11/21/02. DIAGNOSED AS BLOCKAGE IN AN ARTERY IN THE HEART.

EMPLOYEE EXPERIENCED A THRESHOLD SHIFT IN LEFT EAR EVIDENT ON HIS ANNUAL AUDIOGRAM, A RETEST AUDIOGRAM CONFIRMED A THRESHOLD SHIFT IN THE LEFT EAR OF 30 DBA. AT THIS TIME THE COMPANY WANTED TO MAKE AN APPOINTMENT FOR FURTHUR OTOLOGICAL TESTING WHICH HE REFUSED. DELAY IN FILING THE 7000-1 REPORT WAS LOST IN MAIL & CONSULTATION WITH MSHA WAITING FOR REPLY. BELIEVE LOSS IS NOT JOB REL

EE WAS WORKING ON A LADDER REMOVING CONCRETE FORMS AND FELL APPROX. 6' LANDING ON HIS FEET. ONE FOOT LANDED ON ROCK, RESULTING IN A FRACTURED HEEL ON THAT FOOT.

GREASING FRONT END LOADER BUCKET, SLIPPED, FELL APP 2 1/2 TO 3 FT, LANDED ON FEET, JAMMED RIGHT HAND ON BUCKET. ACCIDENT OCCURRED 3-11-02, DID NOT SEEK MEDICAL ATTENTION UNTIL 3-20-02, DID NOT REPORT TO COMPANY DOCTOR OF HIS TREATMENT UNTIL4-12-02.

HE TWISTED LEFT KNEE WHILE STANDING ON STEP LADDER. FELL APPROX. 1 1/2' TO 2' TO THE FLOOR, LANDING ON RIGHT LOWER BACK.

CLIMBING ONTO RAILROAD CAR LADDER TO SEAL HOPPED DOORS. STRAIN TO LOWER BACK.
